Airway Management During Emergent Laparotomy In A Patient With Subglottic Stenosis And Unilateral True Vocal Cord Paralysis: A Case Report., Rayyan Bhutta, Ali Osman, Connor Henn, Hameed H Salah, Samir Patel
Surgery Articles
This case report describes the airway course of a 52-year-old male who required emergent exploratory laparotomy for suspected gastrointestinal perforation. The patient had underlying subglottic stenosis and unilateral vocal cord paralysis, as well as a history of significant cardiopulmonary disease. Attempts at intubation using standard tube sizes were unsuccessful. A 6.0 endotracheal tube was eventually passed beyond a point of resistance, allowing the operation to proceed. He remained intubated after surgery and was successfully extubated several days later. The report emphasizes the need for adaptable airway plans and highlights the constraints that may arise during urgent operative care.

Subclinical Hyperthyroidism Presenting As Urinary Frequency And Polyuria: A Case Report., Hameed H Salah, Elizabeth M Pines
Ambulatory and Primary Care Articles
Hyperthyroidism may present with a variety of symptoms including urinary frequency. This report discusses a case of subclinical hyperthyroidism presenting as generalized weakness, urinary frequency, polyuria, and intermittent diarrhea. However, this elderly patient had a complex past medical history including multinodular nontoxic thyroid goiter with tracheal deviation and multiple recent admissions for aspiration pneumonia, making the diagnosis of subclinical hyperthyroidism less obvious. The patient was managed with intravenous fluids and discharged on propranolol with recommendations to follow-up with endocrinology. This case demonstrates the importance of keeping hyperthyroidism on the differential for urinary frequency and polyuria without other clear etiologies.

Telogen Effluvium And Anesthesia Considerations: A Case Report., Elizabeth Johnson-Gray, Samantha A Currier, Hanen Fernandez, Matt Brown, Matthias Franzen
Ambulatory and Primary Care Articles
Telogen effluvium is a form of non-scarring hair loss characterized by premature shedding of hair. Pathophysiologically, telogen effluvium occurs when a significant number of hair follicles prematurely enter the telogen phase, resulting in excessive shedding. The exact mechanism is unclear, but it is believed to involve alterations in the hair follicle's growth cycle, possibly mediated by illness, infection, major surgery, low protein intake, or hormonal changes. Some medications can also be linked to this disease, such as beta blockers, carbamazepine, and vitamin A. When affected hairs are in the resting phase, noticeable hair shedding occurs. Our patient was a 61-year-old …

Cardiovascular Manifestations Of Pseudoxanthoma Elasticum., Aimee Willett, Vivek V Jasti, Sean M Muir, Jay Anderson
Ambulatory and Primary Care Articles
Pseudoxanthoma elasticum (PXE) is a rare systemic disorder of elastic fibers. The most common manifestations include characteristic papules and ocular angioid streaks. Cardiovascular complications include peripheral arterial disease, hypertension, premature coronary artery disease, and cerebrovascular disease. These associations are secondary to dystrophic elastic fibers, resulting in the calcification of small- to medium-sized arteries. We present the case of a 22-year-old otherwise healthy female patient diagnosed with PXE following a biopsy of a neck lesion. Ophthalmologic evaluation revealed characteristic angioid streaks. The patient also reported early satiety and leg cramping, which prompted referral to gastroenterology and cardiology. Further investigation raised concern …

Neuroscience Articles
Multiple sclerosis (MS) is a chronic neuroinflammatory and neurodegenerative disease that affects nearly 1 million adults in the United States. Owing to its unpredictable disease course, diverse phenotypes, and an array of currently available disease-modifying therapies, a personalized approach to MS management is required. There is an unmet need to identify, validate, and incorporate prognostic, monitoring, and predictive biomarkers into routine clinical practice for MS. A mounting body of evidence supports the use of blood biomarkers, such as neurofilament light chain (NfL), in predicting disease activity and monitoring treatment response. Previous hurdles for the widespread use of NfL in the …

[In the absence of an abstract, the opening of the editorial is presented.] There isn’t any other way to put it: libraries are being attacked by the current US presidential administration. The latest salvo is part of a broader culture war strategy to dismantle institutions supporting education, research, and knowledge sharing, all of which are core principles of the library’s mission. As we prepare this issue for publication in the spring of 2025, daily executive orders, directives, and announcements have led to unprecedented levels of uncertainty. Given the rapid deluge of attacks, the outlook could be markedly worse by the …

Background: Tandem lesions pose unique challenges in the endovascular treatment of acute ischemic stroke. We aimed to compare the clinical and procedural outcomes of patients with tandem lesions and extracranial internal carotid artery (ICA) complete occlusion versus those with moderate to severe stenosis.
Methods: This is a subanalysis of a multicenter cohort of patients with acute anterior circulation tandem lesions treated with intracranial mechanical thrombectomy and carotid artery stenting, between January 2015 and December 2020. The patients were categorized into 2 groups: extracranial ICA stenosis >70% to 99% and complete occlusion. Outcomes included successful and excellent recanalization, functional independence, symptomatic …

Jme4110: Hydro-Excavator Robot, Diane L. Evans, Nathan Fenton, Grant Cole, Nicholas Sipe
Washington University / UMSL Mechanical Engineering Design Project JME 4110
In an environmental situation wherein, diesel has leaked into soil surrounding high voltage lines in an underground confined space where workers cannot access physically and for safety reasons. To alleviate the situation, the project goal is to create a remotely controlled robot, with umbilical attachments. Due to the nature of the environment containing sensitive and electrically charged equipment, mechanical means of removing the soil are not feasible. For this reason, the robot is to remove contaminated soil by using a powerful water sprayer and vacuum combination. Other options for features of this robot are that it uses tracks for movement, …

Coastal communities have complex bodies of knowledge around environmental change, resilience, and sustainability, which extractive forms of research are not well suited to understand. This paper outlines an arts-based action research pilot project to understand social-ecological resilience in the Western Indian Ocean. Our project used visual, artistic, and participatory approaches to foster co-learning and action research on: (1) the arts-based methods themselves and (2) resilience building in coastal communities and ecosystems. This paper focuses on the former elements. We present our inherently phenomenological approach as a research assemblage that can be used to bring together different communities and stakeholders, disciplines, …

Two Cheers For The Revolving Door, Stephen Calkins, Erica Straus
Law Faculty Research Publications
Where do top US government antitrust lawyers come from – and go to? The so-called “revolving door” has long been a familiar part of the process. In recent years and especially during the Biden Administration, this custom has attracted increased attention and concern. What are the facts? This chapter sets out, in detailed tables, the background and subsequent employment of top government antitrust lawyers. Many enforcers came from major law firms (FTC commissioners much less frequently than others) and leave for major law firms. (Although studied in less detail, top state antitrust enforcers are starting to transition to major law …

Optimizing Oncology Urgent Care Services For Improved Outcomes And Reducing Hospitalizations, Jaime C. Champion
Optimizing Oncology Urgent Care Services For Improved Outcomes And Reducing Hospitalizations, Jaime C. Champion
MSHS Student Capstones
The complexity of treating patients with cancer is becoming an increasingly important priority for healthcare providers due to the risk associated with complications. Cancer patients may frequently visit an emergency department and put themselves at risk of acquiring other illnesses due to having an immunocompromised system. Unfortunately, emergency departments lack the resources and knowledge to care for patients who are undergoing cancer treatments, which can result in fragmentation of care. Consequently, Oncology Urgent Care clinics are becoming more prevalent in many health systems to bridge the gap between outpatient care and emergency room care. Using a mixed-methods approach, this study …

Soil Health In Nebraska: Exploring Practitioners' Needs And Insights From A Long-Term Organic Farm, Ingrids Isabel Mata Vigil
Department of Agronomy and Horticulture: Dissertations, Theses, and Student Research
Soil health is foundational to sustainable agriculture, environmental resilience, and long-term food security. Yet, the adoption of soil health assessments remains limited, often perceived as too technical, costly, or difficult to implement in real-world contexts. To better understand these challenges, the first chapter presents findings from a statewide survey of 41 soil health practitioners in Nebraska, including farmers, extension educators, NRCS staff, and conservation professionals. Respondents highlighted the need for simpler, more accessible, and context-specific tools for soil health assessment that help practitioners make informed decisions aligned with their management goals. Complementing these insights, the second chapter includes an independent, …
