A Pilot-Study Evaluating The Fidelity Of Chatgpt In Client Simulations, Eric Tod Beeson, Yusen Zhai, Russell Fulmer, Andrew M. Burck, Rakesh Maurya
Journal of Counselor Preparation and Supervision
Generative AI (genAI) is expanding in counselor education. This pilot study evaluated ChatGPT 4o's voice mode for simulating client scenarios during 53-minute mock-intake sessions with master’s-level counseling students. Analyzing eight domains (authenticity, consistency, emotional expression, empathy, cultural dynamics, self-awareness, goal setting, and role play confusion), ChatGPT’s responses showed good to excellent fidelity when compared to a typical client. Limitations such as formulaic and idealized responses raised authenticity concerns. ChatGPT offers a scalable, cost-effective tool for pre-practicum training, with potential to enhance skill development across the curriculum. Educators are encouraged to integrate genAI thoughtfully, prioritizing ethical considerations and iterative prompt refinement.

The integration of artificial intelligence (AI) into mental health care presents innovative opportunities for therapeutic interventions. This pilot study examined the use of a customized Artificial Intelligence Therapeutic Adjunct (AITA) as a supplementary counseling tool for adults with level one autism spectrum disorder who attended weekly counseling sessions. Using a Single Subject Design to review trends in mental health symptoms and perceptions of the AITA, participants engaged with the AITA over four weeks, completed weekly anxiety and depression assessments, and post-intervention and satisfaction surveys. Thematic Analysis identified common counseling concerns and the AITA’s use of counseling techniques. Although findings suggest …

The purpose of this manuscript was to introduce key concepts in artificial intelligence (AI) and machine learning (ML) to counselor educators and counseling researchers. As AI continues to play an increasingly significant role in various fields, counseling professionals need to develop a foundational understanding of AI, including ML. The first section of this manuscript outlined basic concepts in AI and ML, with study examples that utilize specific algorithms in mental health and counseling-related fields. Digital health technologies including artificial intelligence (AI) are transforming mental healthcare delivery, creating new opportunities and challenges for counseling practice. This article examines digital health applications in counseling, including telehealth, mobile apps, digital therapeutics, virtual reality, and AI-powered tools. We explore key implications for practice, including enhanced accessibility, impacts on therapeutic relationships, and ethical considerations. The article presents a six-domain competency framework covering privacy and security, technical proficiency, ethical literacy, clinical integration, therapeutic adaptability, and administrative management. A case study illustrates the practical application of hybrid digital care, while a discussion of counselor education highlights the need for curriculum updates. …

ABSTRACT
Purpose: The purpose of this article is to review the use of glucagon-like peptide 1 receptor agonists (GLP-1 RA) in heart failure (HF) treatment considering the HF phenotype and comorbidities, including type 2 diabetes mellitus (DM) and obesity.
Method: A PubMed and Google Scholar literature search was conducted with search terms, HF and GLP-1 RA. Seven pertinent articles were retrieved and served as the basis for this clinical review.
Results: The efficacy of GLP-1 RA therapy in patients with heart failure with reduced ejection fractions (HFrEF) is unclear, with one study showing increased adverse cardiac events including tachyarrhythmias, and …

This qualitative study investigates the impact of physician precepting on medical education and healthcare delivery, prompted by the need for consistency in precepting practices across various institutions and specialties. Physician preceptors, who bridge the gap between theoretical learning and practical application, play a critical role in shaping competent and compassionate healthcare professionals. However, inconsistencies in precepting quality and challenges such as time constraints, inadequate training, and balancing clinical duties with teaching responsibilities necessitate a comprehensive evaluation of precepting's effectiveness.

Technological innovation has had an enormous effect on safety in aviation. However, in Part 135 operations, where technology is not as ubiquitous, the accident rates have plateaued. Human error is constantly misrepresented as being responsible for 70–80 percent of accidents, when it is clearly only one of several contributing factors. In commercial aviation, all accidents should be considered organizational failures, because by definition all defensive layers have been breached. Monoamine oxidase A (MAO-A) has recently been identified as a candidate for cancer therapy due to its roles in tumor development and growth. Conventional approaches for the discovery of MAO-A inhibitors are both costly and time-consuming. The objective of this work was to apply in silico methods to predict natural isoquinoline alkaloids capable of acting as potent MAO-A inhibitors, and to establish this class of compounds as potential sources of a cost-effective anticancer therapy. This study aimed to investigate the computed tomography (CT) features of mast cell tumors (MCTs) and soft tissue sarcomas (STSs) in subcutaneous masses of dogs and to identify distinguishing characteristics between these two tumor types. This retrospective, multicenter study included 22 dogs with subcutaneous masses diagnosed as MCTs (n = 11) or STSs (n = 11) via fine needle aspiration or excisional biopsy. BACKGROUND: Adrenal insufficiency is a potentially life-threatening condition that often presents with nonspecific symptoms. While fatigue, hypotension, and electrolyte disturbances are common features, seizures and stress-induced cardiomyopathy are rare initial manifestations. This case is reported for its atypical presentation and to highlight the diagnostic challenge it posed in the absence of classic biochemical findings.
CASE PRESENTATION: We report a case of a 68-year-old Hispanic woman with diabetes, hypertension, dyslipidemia, and hypopituitarism secondary to Sheehan syndrome, who presented with new-onset seizures after abruptly discontinuing chronic steroid therapy. This literature review explores the influence Adverse Childhood Experiences (ACEs) can have on choosing a helping profession as well as their effect on law enforcement personnel and their ability to cope with occupational trauma. Law enforcement officers are routinely exposed to high-stress, high-risk environments which can exacerbate underlying trauma stemming from early life experiences. Officers with a history of ACEs may be more vulnerable to emotional dysregulation, hyper-vigilance, burnout, and substance abuse which can influence their decision making, relationships, and professional conduct potentially resulting in negative outcomes. Sleep disorders are prevalent in primary care settings and often remain underdiagnosed due to inconsistent assessment practices. This doctoral project was conducted to develop, implement, and evaluate whether an educational intervention for registered nurses would enhance their knowledge on using the Epworth Sleepiness Scale (ESS) for sleep assessment in primary care. Guided by adult learning theories and the analysis, design, development, implementation, and evaluation instructional design model, the aim of the project was to address the practice gap related to inadequate sleep evaluations, ultimately improving patient outcomes and promoting holistic care. This paper investigates the environmental and ecological toll of Israel’s current occupation and genocide of Palestine through ecological, historical, philosophical, and legal lenses. Chapter 1 explores the ecological impacts that Israel’s prolonged military operations have had on Gaza and the West Bank since October 7, 2023. Chapter 2 provides a comprehensive examination of the ideological motivations and historical context for Israel’s colonization of Palestine. The chapter also provides a pre-colonial history of Palestine’s indigenous agriculture and land management practices, highlighting its self-sufficiency and sustainability prior to the occupation. Background: Differentiating acute kidney injury (AKI) from chronic kidney disease (CKD) in children remains a critical unmet need due to the limitations of current clinical and biochemical markers. Conventional ultrasound lacks the sensitivity to discern subtle parenchymal alterations. The problem of interest is the current workforce crisis among the Radiologist Specialty in hospitals in Southern California. There has been a significant decline in the number of Radiologists in the workforce compared to the caseload of imaging studies. The purpose of this integrative review was to evaluate empirical and non-empirical literature for operational solutions to improve recruitment and retention of radiology providers. Black adults in the United States are nearly twice as likely to receive a diagnosis of type 2 diabetes mellitus (T2DM) and are more susceptible to severe diabetes-related complications. Effective flossing is essential for preventing periodontal disease. However, in a rural Appalachian community, adherence to flossing routines remains low, contributing to significant oral health disparities. The purpose of this study was to conceptualize intention of initiation and sustenance of flossing behaviors using the multi-theory model (MTM) of health behavior change. A cross-sectional survey was administered to individuals recruited from a shopping mall in rural Appalachia. The MTM constructs were measured and data were analyzed using multiple linear regression models. Of the 228 participants, 55% did not floss daily. Behavioral confidence (β = 0.571, p < 0.001) was a significant predictor for the intention of initiation of flossing among non-daily flossers, while emotional transformation (β = 0.377, p = 0.836) significantly explained sustenance among non-daily flossers. For daily flossers, changes in the physical environment (β = 0.432, p = 0.003) significantly predicted the intention of initiation of flossing, whereas emotional transformation (β = 0.344, p = 0.008) and practice for change (β = 0.594, p < 0.001) significantly predicted sustenance of flossing behavior. The MTM appears to be a robust framework for understanding and predicting flossing behaviors in rural Appalachian populations. Interventions designed to enhance behavioral confidence, physical environment changes, emotional transformation, and practice for change could improve flossing adherence. Further research should explore these constructs to develop targeted public health strategies.
