Objectives: Venous thromboembolism (VTE), especially pulmonary embolism (PE), can be treated in an outpatient setting after appropriate risk assessment. However, front-door physicians may be resistant to follow this practice. The possibility of cancer-associated VTE complicates the outpatient management of PE. This study aimed to assess the morbidity and mortality in patients with PE who were managed in an emergency/acute medicine-led outpatient clinic as per risk stratification as well as detecting diagnoses of new cancer sites (NCSs) in these patients. Methods: This retrospective study included all patients with confirmed PE managed in an acute medicine outpatient setting at Queens Hospital, …

Aim: Despite its frequency in tertiary headache centers, the International Classification of Headache Disorders, 3rd edition (ICHD-3) does not include refractory migraine. Multiple definitions have been proposed with a recent 2020 proposal for both refractory migraine and resistant migraine by the European Headache Federation (EHF). The aim is to reach an international consensus on the definition of refractory migraine.
Methods: This study is a Delphi consensus carried out by a group of international experts in headache medicine. Following a focus group, a panel of 20 experts and one facilitator reviewed the EHF proposed criteria to build upon their definitions. The …

Background/purpose
People with human immunodeficiency virus (PWH) are at increased risk of acquiring human papillomavirus (HPV) infection and subsequently developing HPV-related lesions. We aimed to determine the prevalence and strength of association between HPV-related oral lesions and HIV infection, using a large, real-world dataset.
Materials and methods
This retrospective cohort used 20 years of de-identified electronic health records from TriNetX (109 organizations, 24 countries). Adults ≥18 years with HIV (ICD-10 B20, LOINC antibodies 7917-8/7918-6, RNA 25835-0) were compared age and sex matched people without HIV. We quantified prevalence and associations for HPV-related benign lesions (papilloma, verruca, epithelial hyperplasia, condyloma) and …

TOEFL Prediction is widely used in Indonesian higher education as a preparatory tool, yet students’ perceptions of the TOEFL test remain rarely explored. This qualitative case study involved ten English Education students (batch 2021) at UIN Datokarama Palu. Data were collected through semi-structured interviews and document analysis, then analyzed using Miles and Huberman’s interactive model. Four indicators emerged: readiness and confidence, test-taking experience, motivation, and perceived fairness. Students valued TOEFL Prediction as a diagnostic tool, but faced challenges such as anxiety, technical issues, and limited vocabulary. TOEFL Prediction functions both as formative assessment and a motivational catalyst for learning.
Large-Scale Curve Time Series With Common Stochastic Trends, Degui Li, Yuning Li, Peter C. B. Phillips
Large-Scale Curve Time Series With Common Stochastic Trends, Degui Li, Yuning Li, Peter C. B. Phillips
Cowles Foundation Discussion Papers
This paper studies high-dimensional curve time series with common stochastic trends. A dual functional factor model structure is adopted with a high-dimensional factor model for the observed curve time series and a low-dimensional factor model for the latent curves with common trends. A functional PCA technique is applied to estimate the common stochastic trends and functional factor loadings. Under some regularity conditions we derive the mean square convergence and limit distribution theory for the developed estimates, allowing the dimension and sample size to jointly diverge to infinity. We propose an easy-to-implement criterion to consistently select the number of common stochastic …
Book Review Of The Art And Science Of Connection By Kasley Killam, Harit Agroia
Book Review Of The Art And Science Of Connection By Kasley Killam, Harit Agroia
Journal of Social, Behavioral, and Health Sciences
The Art and Science of Social Connection by Kasley Killam presents a case for better understanding individual health by ensuring that the social aspect is considered as a significant component of health in addition to the physical and mental aspects. Killam states that social health consists of an individual’s sense of belonging, the strength of relationships, and meaningful connections. By drawing from years of evidence building, Killam argues that social health is fundamental for one to truly flourish and prosper in their life as people who have stronger social health are shown to have higher quality of life, emotional resilience, …
The Psychological Impact Of Systemic Factors On Maternal Mental Health Outcomes, V Johnson
The Psychological Impact Of Systemic Factors On Maternal Mental Health Outcomes, V Johnson
Walden Dissertations and Doctoral Studies
Maternal mental health disparities in the United States continue to impact reproductive-age women, especially those from underserved populations. In this quantitative study, how healthcare accessibility and socioeconomic status (SES) influenced psychological stress and maternal mental health outcomes was examined. Bronfenbrenner’s socioecological model and critical race theory guided the theoretical framework. Secondary data from the 2019 Behavioral Risk Factor Surveillance System were used to analyze three research questions related to the predictive relationships between systemic factors and mental health outcomes, and differences in stress across age groups. Key variables included insurance coverage, access to a personal doctor, cost-related barriers, income, employment …

Staff Education About Cultural Awareness In A Healthcare Setting, Phylis Titty Tarawally
Walden Dissertations and Doctoral Studies
Summary This quality improvement project was carried out to address a practice gap in culturally responsive care among nursing staff working in a skilled nursing facility. African-American residents often face disparities in healthcare due to staff’s limited knowledge of cultural health beliefs and confidence in engaging with diverse populations. To improve this, an educational intervention was designed using the ADDIE instructional model and evaluated to see if it could enhance staff knowledge and confidence related to African-American cultural practices. The project was guided by the practice-focused question: Will a structured African-American cultural awareness program improve staff knowledge and confidence to …
The Progressive Changes In Posttraumatic Symptoms And Posttraumatic Growth Associated With Traumatic Incident Reduction, Joseph L. Mcclory
The Progressive Changes In Posttraumatic Symptoms And Posttraumatic Growth Associated With Traumatic Incident Reduction, Joseph L. Mcclory
Walden Dissertations and Doctoral Studies
Many millions of people are suffering from symptoms of posttraumatic stress disorder (PTSD). Just in the United States, 13 million Americans had a PTSD diagnosis as of 2020. Through the theoretical lens of catharsis, analysis of variance was used to examine whether traumatic incident reduction (TIR) is associated with changes in posttraumatic symptoms and posttraumatic growth by comparing pre, mid, and post-TIR PTSD Checklist for DSM-5 (PCL-5) and Posttraumatic Growth Inventory (PTGI) scores. Regression was also used to test whether post-PCL-5 scores accounted for variance in the post-PTGI scores. Ninety-seven TIR cases from the mental health agency between 2017 and …

This study aims to analyze the impact of maternal employment status and job type on children's literacy skills in Indonesia, using data from the 2023 National Socioeconomic Survey (SUSENAS) and a binary logistic regression approach. The results show that working mothers, particularly those in the formal sector, are positively and significantly correlated with children's literacy, especially in non-poor households. These findings highlight the important role of maternal employment contribution in strengthening educational investment within families. In contrast, among poor households, formal employment of mothers does not significantly influence children's literacy, indicating that economic constraints remain a major barrier. In addition, …

Water system centralization and decentralization have variously been promoted as key to achieving household water security and Sustainable Development Goal 6.1. We argue that the lack of specificity with which scholars and practitioners use the terms centralization and decentralization limits our understanding of different water system configurations and their impacts. In this Primer, we provide a framework for thinking about levels of (de)centralization across three linked system dimensions: infrastructure, governance, and operations and maintenance. We encourage those analyzing water systems to characterize (de)centralization with respect to these multiple dimensions, as well as the system's broader political-economic and hydro-climatic contexts. Emphasizing …

Early Definitive Fusion Versus Magnetic Growing Rods In "Tweeners": What Do Parents Prefer?, Sylvia Culpepper, Ravi Rajendra, Brett Lullo, Anthony A. Catanzano, Tyler C. Mcdonald, R Carter Clement
School of Medicine Faculty Publications
INTRODUCTION: In treating early-onset scoliosis (EOS), pediatric orthopaedic surgeons and families must often choose between growth-friendly interventions such as magnetically controlled growing rods (MCGR) and early definitive posterior spinal fusion (PSF). While each strategy has associated pros and cons, little is understood about the specific preferences of patients' families. Addressing potential nuances in these preferences is crucial for meaningful shared decision-making (SDM). To better understand the factors influencing family decisions, we conducted a choice-based conjoint (CBC) analysis to explore parents' preferences for PSF and MCGR as treatment options. METHODS: A survey was developed using Sawtooth Software (Lighthouse Studio version 9.2.0) …
A Retrospective Cohort Study Of Valproate And Infertility In Men With Epilepsy Or Bipolar Disorder Using International Health Data, Lance Watkins
A Retrospective Cohort Study Of Valproate And Infertility In Men With Epilepsy Or Bipolar Disorder Using International Health Data, Lance Watkins
Peninsula Medical School
Valproate is highly effective at treating epilepsy and bipolar disorder. It faces prescribing restrictions in men due to concerns it causes testicular dysfunction and infertility. These mostly stem from animal models – the human evidence is limited and conflicting. We report the largest ever retrospective cohort study of infertility in men with epilepsy or bipolar disorder, using real-world healthcare data from TriNetX. 91,917 of the men are exposed to valproate, and 535,803 unexposed. Cohorts are propensity score matched for a comprehensive set of baseline covariates, and survival analysis is undertaken using Cox-proportional hazards models. No significant difference is seen between …

Biophobia And Pet Ownership: Can Having A Pet Mitigate Our Fear And Aversion Towards Nature?, Alexia Barrable
The International Journal of Ecopsychology (IJE)
Biophobia, which we defined as fear, disgust, or aversion toward natural environments or organisms, has received little empirical attention when compared to biophilia, despite its implications for human and planetary wellbeing. This mixed-methods study explores the relationship between pet ownership, biophobia, and nature connectedness among 373 English- speaking adults. Quantitative findings reveal that (M = 26.4) exhibit significantly lower levels of biophobia than non-pet owners (M = 30.1; U = 5179, Z = -2.95, p = .002, R = 0.27), although no significant differences in nature connectedness were found across pet ownership (U = 7033, Z = 0.31, p = …

Misandry And Men’S Equity In Selected Novels By Female Authors, Omar Abbas Sallal
Alustath Journal for Human and Social Sciences
This study aims to shed light on the concept of misandry in selected novels by Octavia E Butler and Margaret Atwood. Several previous studies dealt with the concept of misogyny in detail and depth, so it became necessary to present a different point of view. This study reached the following results, analysing the female and male characters to accurately determine the concept of misandry. This study gives a different model of female novelists’ novels to inform the reader of the existence of two sides of female writers, and the novelist Suanne Laqueur was taken as a counter example to the …
