Childhood vasculitis encompasses a group of rare and heterogeneous diseases with systemic inflammation affecting various vessel sizes. This comprehensive review highlights recent advances in the pathogenesis, biomarkers, diagnosis, and treatment strategies for major pediatric vasculitides including IgA vasculitis, Kawasaki Disease, ANCA-associated vasculitis, Takayasu arteritis, and Polyarteritis Nodosa. Novel insights from genetic, immunologic, and imaging studies have paved the way for early diagnosis and individualized therapeutic approaches. Future directions emphasize the role of artificial intelligence, precision medicine, and international collaborative trials to optimize long-term outcomes and quality of life in affected children.

SARS-CoV-2 variants have demonstrated distinct epidemiological patterns and clinical presentations throughout the COVID-19 pandemic. Understanding variant-specific differences at the respiratory epithelium is crucial for understanding their pathogenesis. Here, we utilized human nasal organoid air-liquid interface (HNO-ALI) cell cultures to compare the viral replication kinetics, innate immune response, and epithelial damage of six different strains of SARS-CoV-2 (B.1.2, WA, Alpha, Beta, Delta, and Omicron). All variants replicated efficiently in HNO-ALIs, but with distinct replication kinetic patterns. The Delta variant exhibited delayed replication kinetics, achieving a steady state at 6 days post-infection compared to 3 days for other variants. Cytokine analysis revealed …

Hypothesis: Large language models (LLMs) like ChatGPT have increasingly been used as online resources for patients with orthopedic conditions. Yet there is a paucity of information assessing the ability of LLMs to accurately and completely answer patient questions. The present study comparatively assessed both ChatGPT 3.5 and GPT-4 responses to frequently asked questions on common elbow pathologies, scoring for accuracy and completeness. It was hypothesized that ChatGPT 3.5 and GPT-4 would demonstrate high levels of accuracy for the specific query asked, but some responses would lack completeness, and GPT-4 would yield more accurate and complete responses than ChatGPT 3.5.

The Genome Aggregation Database (gnomAD) is a foundational resource for allele frequency data, widely used in genomic research and clinical interpretation. However, traditional estimates rely on individual-level genetic ancestry groupings that may obscure variation in recently admixed populations. To improve resolution, we applied local ancestry inference (LAI) to over 27 million variants in two admixed groups: Admixed American (n = 7612) and African/African American (n = 20,250), deriving ancestry-specific allele frequencies. We show that 78.5% and 85.1% of variants in these groups, respectively, exhibit at least a twofold difference in ancestry-specific frequencies. BACKGROUND: Homelessness results from, and exacerbates, various social determinants of health, including poverty, racism, and inadequate healthcare access, which are further compounded by physical and mental health challenges. The street medicine movement seeks to address these disparities by providing direct medical care and harm reduction services to unsheltered homeless in low-barrier settings. The development of trust is critical to providing this form of care. Few studies have sought to understand the factors influencing trust development in street medicine encounters.

Me, Myself, And The Influencer: Examining How Parasocial Interaction And Self-Congruence With Social Media Influencers Affects News Media Trust, Ben Wasike

Social media influencers (SMIs) develop unique relationships with their followers, including high levels of trust and credibility. This study examined how these influencer–follower relationships affect news media trust with a focus on actual self-congruence, ideal self-congruence, parasocial interaction (PSI), and source and message credibility. Data showed that respectively, positive correlations exist between each of these variables and news media trust. However, ordinary least squares (OLS) regression only showed direct effects of actual self-congruence and PSI on trust, while message credibility had a negative effect. Structural equation modeling showed that actual self-congruence positively mediated the relationship between PSI and trust. Overall, …

Engagement By Design: Belongingness, Cultural Value Orientations, And Pathways Into Emerging Technologies, Daisuke Akiba, Michael Perrone, Caterina Almendral, Rebecca Garte

This theoretical article examines how belongingness, defined as the sense that one’s participation is legitimate and valued, interacts with cultural value orientations to help explain persistent disparities in U.S. technology engagement, including emerging technologies, across racial and ethnic groups. While structural barriers (e.g., racism, poverty, linguistic bias, etc.) remain essential to understanding such inequity, we argue that engagement patterns in technology also reflect how different cultural communities may define and experience belongingness in relation to digital domains. Drawing on Triandis and Gelfand’s framework, and focusing specifically on educational contexts, we propose the Belongingness through Cultural Value Alignment (BCVA) model, whereby …

In this interview, Breeding, a 75-year-old queer woman, explores questions of identity, culture, and activism. Breeding was born in New Hampshire, grew up in Ohio, and moved back to the East Coast as a teenager. She moved to Maine at 21, living in Ogunquit and Portland. Breeding discusses her family's background, including her parents' contrasting social classes, and the importance of a Unitarian Universalist group in creating early community. The Role Of Employee Relationship Orientation In The Value Co-Creation – Firm Performance Link, Perry L. Parke, Prachi Gala, Pramod Iyer, Stefan Sleep, Mona Sinha

Purpose – This paper links employees’ relationship orientation, or their sense that the organization respects their contribution, to value co-creation and overall firm performance. It also measures the effects on innovation and knowledge sharing.

Design/methodology/approach – A total of 301 US product and marketing managers participated in a survey. Measurement and structural models were evaluated using partial least squares structural equation modeling.

Findings – Main findings suggest that increased value co-creation (VCC) activity has a significant effect on employee relationship orientation. This study also finds that employee relationship orientation plays a mediating role in the relationship between VCC and firm …

OBJECTIVES: To evaluate temporal trends in the epidemiology of hip osteoarthritis (OA) in the USA from 1990 to 2019, with stratification by sex and geographic region.

DESIGN: Cross-sectional time-series analysis using secondary data from the Global Burden of Disease (GBD) study.

SETTING: US population-based analysis, stratified by the four US Census Bureau regions: Northeast, Midwest, South and West.

PARTICIPANTS: De-identified, aggregate population-level data representing all adults in the USA from 1990 to 2019, drawn from the GBD database.

The provision of residential and community‐based services for individuals with serious mental illness (SMI) has become increasingly important following the deinstitutionalization movement. Much of the existing research on supportive housing focuses on housing outcomes rather than exploring how the program helps its residents thrive in the broader community. This study draws upon data collected from 176 people with SMI residing in 16 supportive housing locations in Portland, Oregon. Analyses explore how housing staff support relates to residents' loneliness (interpersonal level), residential satisfaction (housing and neighborhood level), and sense of community (community level). Wearable technology (WT) has gained attention as a tool for supporting student health by promoting physical activity and stress management. However, most existing studies have focused on predominantly White or more affluent student populations, leaving a gap in understanding the role of WT among African American university students who face distinct socioeconomic and institutional barriers. Does the Fifth Amendment’s Takings Clause permit the state to destroy property without compensation whenever it declares an emergency? The long-standing doctrine of necessity empowers the state to do exactly that.

Courts dating back to common law England have permitted governments to claim a privilege of necessity to avoid paying for private property they destroy in times of crisis. What is a burden on interstate commerce? That’s an important question under the Dormant Commerce Clause’s Pike balancing test. But it’s a question whose answer has proven elusive. This shouldn’t be a surprise. After all, states disagree about what counts as a burden or a benefit, and how much weight each has. And there aren’t any obvious constitutional principles we can point to for resolving those disagreements. The war between Hamas and Israel has caused havoc in higher education. Amid student unrest, alumni pressure, congressional hearings, civil rights investigations, and student lawsuits, universities stand at a crossroads. The current situation, in which most private universities unevenly regulate student speech under ambiguous student codes, is not sustainable politically or legally. A tsunami of litigation and regulatory actions has already begun. One increasingly favored response is for private universities to more vigorously enforce existing codes or expand their scope. Background: The recent phase II randomized stereotactic ablative radiotherapy with and without immunotherapy (I-SABR) trial has shown improved event-free survival (EFS) when adding immunotherapy to stereotactic ablative radiotherapy (SABR) for early-stage inoperable non-small cell lung cancer (NSCLC). However, optimizing patient selection thereof is critical, because not every patient benefits from immunotherapy. Leveraging the powerful use of artificial intelligence, this secondary analysis of the I-SABR trial developed a modeling system (named "I-SABR-SELECT") based on clinical and radiomic factors to address which patients should receive additional immunotherapy.

Moving Beyond Traditional Legal Scholarship Impact Rankings, John Cannan

Legal academia is likely stuck with law faculty scholarly impact rankings—that is, using citation counts to judge a law faculty’s reputation and influence. But why can’t we produce better, more useful studies? Current rankings rely on outdated methods. Moreover, these rankings—touted by their creators as objective—suffer from long-standing flaws. They are also grounded in limited understandings of legal citations as sources of information. This paper calls on law librarians to advance legal citation research by adopting methodologies from other information sciences and developing legal citation taxonomies and theories.

Introduction: Effective airway management is critical for optimal patient outcomes in the emergency department (ED). Additionally, airway management is significantly influenced by the clinician’s selection of equipment, specifically the choice of intubating stylet. Also of note, the duration of intubation (time to intubate) impacts overall success. The choice of intubation device may influence first-pass success and intubation times. In this study we evaluated equipment trends for first-pass success and intubation duration. We collected data by reviewing a video database of recorded airways. Three commonly used intubating stylets were reviewed: the hyperangulated stylet; bougie (Eschmann stylet); and malleable stylet.
