Pediatric out-of-hospital cardiac arrest (OHCA) remains a high-mortality event, with survival to discharge consistently below 10%. Although extracorporeal cardiopulmonary resuscitation (E-CPR) improves outcomes for in-hospital cardiac arrest, its application in the pre-hospital environment is not well defined for pediatrics. Identifying which pediatric patients might benefit from OHCA ECPR activation is essential for developing feasible pre hospital pediatric E-CPR systems. Preliminary analysis of applying  criteria for adult OHCA ECPR activation can provide a crucial next step in establishing feasibility and applicability for the pediatric patient population.

Background: Current clinical practice guidelines support the long-term use of pharmacotherapy for obesity treatment. Historically, phentermine has been one of the most prescribed obesity medications (OMs), however, its long-term efficacy and safety have never been evaluated in a randomized trial as its market approval predates such requirements. Here we describe the design, rationale, and baseline characteristics for a 24-month, double-blind, randomized controlled trial evaluating the efficacy, cardiovascular risk, and safety of phentermine in adults with overweight or obesity.

The world has become more complex and interconnected, with challenges that span countries and disciplines. To navigate these challenges, individuals are needed who are equipped with skills and competencies to lead in cross-cultural, cross-disciplinary environments. These leadership skills and competencies can be developed through participation in experiential learning programs. However, the impact of international research-based programs remains unexplored. This is a qualitative, phenomenological study within a bounded case that explored the lived experiences of students who participated in an international graduate student research program from a leadership development perspective. This paper rethinks Marx and Engels’ comments on imprisonment and capitalism. While there has been a tendency to suggest the authors’ comments on crime and punishment were peripheral and secondary to their major projects, such as Marx’s Capital, this paper claims, in fact, their writings on incarceration were guided by their shared theoretical project which included historicizing systems of punishment as correlating with modes of production. Also, while more has been written on their ideas of criminality, instead, this paper excavates their thoughts on prisons, which have often been neglected. Social isolation and loneliness, which are experienced by many older adults, can negatively impact health and well-being. Although often used interchangeably, social isolation is defined as an objective lack of social ties (e.g., number of close family members or friends) and loneliness as an internal sense of disconnection (e.g., feeling alone even when surrounded by others). Because social relationships, health, and life circumstances change as we age, patterns of social isolation and loneliness may differ by age group. Therefore, this research brief asks, How do patterns of social isolation and loneliness differ across age groups among community-dwelling older adults?

Background: Acute cellularrejection (ACR) remains a major cause of morbidity afterhearttransplantation despite advances in immunosuppression. Whole genometranscriptomicprofiling offers a systems-based, unbiased approach to elucidate the molecular mechanisms underlying ACR. However, noninvasive,longitudinal biomarker assessments capable of capturing the temporal dynamics ofrejection biology remain scarce.

Methods: RNA sequencing ofperipheralblood fromheart transplant recipients before, during, and after ACR was compared with nonrejection controls. Pathway analysis was conducted using differentially expressed genes (DEGs), and a machine learning approach was applied to assessgene-based prediction of ACR.

Results: A total of 235 rejection-specific significant DEGs and 863 postrejection DEGs (false discovery rate < 0.05) were identified. During ACR, DEGs were enriched for T-cell activation/differentiation, apoptosis, and B-cell receptor signaling pathways. By combining the 2 sets of DEGs, a panel of 71 common genes was identified that reflected the significant, longitudinal transcriptomic dynamics of ACR. Light reprocessed by dust grains emitting in the infrared enables the study of the physics at play in dusty embedded regions, where ultraviolet and optical wavelengths are attenuated. Infrared telescopes such as JWST have made it possible to study the earliest feedback phases, when stars are shielded by cocoons of gas and dust. Comprehending this phase is crucial for unravelling the effects of feedback from young stars that leads to their emergence and the dispersal of their host molecular clouds. Here we show that the transition from the embedded to the exposed phase of star formation is short (< 4 Myr) and sometimes almost absent (< 1 Myr) across a sample of 37 nearby star-forming galaxies covering a wide range of morphologies, from massive barred spirals to irregular dwarfs. The short duration of the dust-clearing timescales suggests a predominant role of pre-supernova feedback mechanisms in revealing newborn stars, confirming previous results on smaller samples and allowing, for the first time, a statistical analysis of their dependencies. We find that the timescales associated with mid-infrared emission at 21 μm, tracing a dust-embedded feedback phase, are controlled by a complex interplay between giant molecular cloud properties (masses and velocity dispersions) and galaxy morphology. We report relatively longer durations of the embedded phase of star formation in barred spiral galaxies, while this phase is significantly reduced in low-mass irregular dwarf galaxies. We discuss tentative trends with gas-phase metallicity, which may favor faster cloud dispersal at low metallicities.

Two motives drive much of American health care. The first is the motive to heal the sick—the patient motive. The second is the motive to generate financial gains—the profit motive. This Article asks the following question: where do these two motives intersect and diverge? Over the past six years the United States has shifted from a relatively stable and laissez-faire privacy regime, anchored by a few, sector-specific federal statutes such as the Health Insurance Portability and Accountability Act, to an increasingly fragmented landscape dominated by numerous state-level consumer privacy laws. While these laws share commonalities, they also vary in important ways. This Article analyzes that patchwork legal regime and its impact on businesses through four points of friction: (i) statutory applicability and thresholds; (ii) notice-and-choice requirements; (iii) individual data-management rights; and (iv) controller–processor contracting obligations. Indigent appellate advocacy has long been confined to a narrow, technocratic model—one that prioritizes legal expertise over client autonomy and treats appellate cases solely as isolated legal battles rather than opportunities for systemic change. Unlike their trial-level counterparts, appellate attorneys representing indigent clients have received little scholarly attention, leaving critical questions about client participation, ethical representation, and social justice largely unexplored.

This Article challenges the status quo, arguing that appellate attorneys should rethink their role in ways that empower clients and confront the broader injustices shaping their cases. We introduce two alternative frameworks—client-centered appellate representation and participatory appellate representation—that draw …
