Aortic thrombosis is a rare but life-threatening condition. It typically presents with abdominal or lower-limb ischemia. Although acute kidney injury (AKI) can accompany aortic thrombosis, AKI as the sole clinical manifestation has not been previously reported. A 69-year-old man with extensive atherosclerotic disease and chronic kidney disease presented with severe oliguria. Initial evaluation, including non-contrast computed tomography (CT), did not reveal any aortic abnormalities. Renal Doppler ultrasonography findings and markedly elevated D-dimer levels prompted contrast-enhanced CT, which demonstrated extensive thrombus formation extending from the descending thoracic aorta to the bilateral iliac arteries, with complete occlusion of both renal arteries and …

Calcium oxalate stones comprise greater than 70% of all kidney stones. In the current conceptual framework, the initial stone nidus is thought to include the aggregation of inorganic crystallites, the formation of which is favored by elevated concentrations of dissolved constituents. Here, we show that this highly prevalent stone type comprises a form of organic-inorganic polycrystalline biocomposite with integrated bacterial biofilms. Evidence from electron microscopy and fluorescence microscopy reveal the unanticipated internal structure of kidney stones from human patients, where bacterial biofilms are intercalated between polycrystalline mineral layers, even in stones identified as "noninfectious" clinically, including those in patients without …

Ischemic limb disease continues to pose major clinical challenges. The effect of this disease causes restricted blood flow, which is followed by a limited ability to regenerate in the vascular tissues. The purpose of this study is to address the current limitations in producing large-scale generation of Smooth Muscle Cells (SMCs) derived from human induced pluripotent stem cells (hiPSCs) and then assessing their potential therapeutic efficiency individually and in combination with endothelial cells (ECs) for vascular repair. Purpose: Sexual and gender minority people (SGM) experience multiple cancer-related disparities, including higher rates of cancer risk factors, lower rates of cancer screening, higher lifetime risk of cancer, and unmet needs throughout cancer survivorship. Although many national organizations, including the National Cancer Institute (NCI) and the American Cancer Society (ACS), have emphasized the need for more cancer research among SGM communities, currently no consensus exists about the highest research priorities, promising research models, or mechanisms to support collaboration between geographically dispersed scientific teams.

Education Accountability Regarding Oral Contraceptives And Sugammadex, Hallianne Hobson Herndon, Terri M. Cahoon

Sugammadex is commonly used to antagonize neuromuscular blockade and has a clinically significant interaction with hormonal contraceptives by binding to plasma progesterone, reducing contraceptive efficacy and mimicking a missed dose. This case involved a healthy 21-year-old woman undergoing elective anterior cruciate ligament and medial collateral ligament repair whose only home medication was an oral contraceptive. She received midazolam prior to anesthesia assessment, limiting her ability to retain education, and counseling occurred at a suboptimal time without a support person present. Postoperative education was verbally relayed without standardized documentation or accountability. This project examined whether implementation of a standardized protocol regarding …

As archives continue to face threats from global climate disasters and increased institutional pressure to accomplish more with fewer resources, there is a need to evolve archival practice through a sustainability lens. While archival literature has frequently discussed sustainability practices such as eco-friendly building standards, how to reduce energy consumption, and other green initiatives, less focus has been placed on the underlying structures of unsustainability. This article proposes degrowth as one framework for considering archival sustainability. Degrowth is an ecological, economic, and political theory that advocates for the deliberate reduction of activities that rely on non-renewable resources. This article draws …

Background: Fenestrated and branched endovascular aneurysm repair can be complicated by branch vessel occlusion in the absence of structural stenosis. We hypothesized that computational flow simulation could identify adverse hemodynamic features associated with postfenestrated and branched endovascular aneurysm repair branch occlusion.
Methods: Patients undergoing 4-vessel fenestrated and branched endovascular aneurysm repair for Extent II to IV thoracoabdominal aortic aneurysms were retrospectively reviewed. Branches that occluded without identifiable kinking or stenosis on computed tomography were included, along with an equal cohort of anatomy-matched patent controls. Patient-specific pulsatile rigid-wall simulations were performed using SimVascular with individualized geometries and boundary conditions. Abnormal time-averaged …

Background: Alcohol, a known carcinogen since 1988, increases risk of cancers of the mouth, pharynx, larynx, esophagus, liver, breast, and colorectum. More than one half of US adults consume alcohol whereas about two thirds are unsure whether or unaware that alcohol intake is linked to cancer risk.
Methods: Our objective was to survey awareness, beliefs, intentions, policies, practices, and goals/perceived responsibilities related to establishing/promoting actions to reduce alcohol-related cancer risks from leaders of 72 NCI-Designated Cancer Centers.
Results: Sixty-one percent responded. Forty-three percent either believed in or were uncertain about alcohol's purported health benefits. Nevertheless, 86% considered it part of …

KBG syndrome is a rare autosomal dominant disorder characterised by developmental delay, characteristic facial features, macrodontia and skeletal anomalies, caused by mutations in the ANKRD11 gene. We report a 5.5-year-old Moroccan boy who presented in 2022 to a tertiary military teaching hospital in Rabat, Morocco, with psychomotor delay, autistic traits, epilepsy, bilateral hearing loss with chronic otomastoiditis and radiologically-detected macrodontia before clinical eruption, in whom artificial intelligence-assisted facial phenotyping suggested the diagnosis, subsequently confirmed by identification of a novel nonsense mutation (c.1977C>G; p.Tyr659Ter). Background: Alcohol use disorder (AUD) represents a highly prevalent, costly, and often untreated condition in the United States. Post-traumatic stress disorder (PTSD) represents a common comorbidity with AUD which worsens outcomes and decreases functional outcomes. Suvorexant (SUV) shows clear promise as a novel therapeutic candidate to treat AUD and PTSD.
Methods: This study features a promising compound (i.e., suvorexant), the application of a well-established human laboratory paradigm (i.e., alcohol cue reactivity), and a novel early efficacy laboratory model (i.e., practice quit attempt) to provide a cost/time-efficient evaluation of safety and initial efficacy of suvorexant for AUD with comorbid PTSD. Background: Stroke is a sexually dimorphic disease, with different risk factors, incidence, outcomes, and treatment responses in men and women. While sex differences have been documented in preclinical studies, these findings often come from single-site studies with small sample sizes and require validation across diverse research settings.
Methods: We used data from the SPAN (Stroke Preclinical Assessment Network), a randomized, placebo-controlled, blinded, multilaboratory trial, to determine if sex differences in neurological outcomes are present in preclinical stroke models. Many organisms rely on olfaction to navigate their environment in response to surrounding chemical cues. Within the olfactory system, first, external signals are transduced into neural activity patterns that drive a diverse array of behavioral responses. However, sensory processing alone does not fully account for this variability. Fluctuations in neuromodulator levels are equally critical in shaping how olfactory information is encoded and translated into behavior. The goal of this dissertation is to investigate how changes in the neurochemistry within the olfactory circuit influence neural signaling and, ultimately, behavioral responses. Stated Preferences For Public Provision Of Services: Experimental Evidence From Latin America, Hernán Bejarano, Matias Busso, Juan Francisco Santos

We study how individuals in six Latin American countries value public versus private provision of education and healthcare using a survey experiment. Respondents were randomly assigned to vignettes that vary income, service quality, and provider type. Reported service quality is the main driver of choices: the probability of selecting a private provider roughly doubles when reported quality of the public option falls from 80 to 20 percent, while income has a smaller effect. Higher institutional trust lowers the likelihood of switching to private providers but does not affect willingness to pay once individuals choose private provision.

This paper explores the sustainability potential of food and farming culture, which is largely overlooked by mainstream approaches for transitioning food systems. I present two cases where food and farming culture is alive and thriving—in the Maya-Achí territory of Guatemala and in Wales—illustrating how cultural practices and values may contribute to global sustainability challenges and resilience.

Background: Fenestrated and branched endovascular aneurysm repair can be complicated by branch vessel occlusion in the absence of structural stenosis. We hypothesized that computational flow simulation could identify adverse hemodynamic features associated with postfenestrated and branched endovascular aneurysm repair branch occlusion.
Methods: Patients undergoing 4-vessel fenestrated and branched endovascular aneurysm repair for Extent II to IV thoracoabdominal aortic aneurysms were retrospectively reviewed. Branches that occluded without identifiable kinking or stenosis on computed tomography were included, along with an equal cohort of anatomy-matched patent controls.
