Cloud computing, financial incentive and patient-centered care are the game changers that deepen EHR diffusion beyond breadth. Based on the innovation diffusion theory (IDT), technology-organization-environment (TOE) framework and alignment literature, this study examines how these changes shape business requirement, service value and society need that drive different phases of EHR diffusion in terms of planning, adoption, usage and upgrade. A longitudinal analysis with the USA National Ambulatory Medical Care Survey (NAMCS) reveals the impacts of different drivers on EHR diffusion. Background: Glycine is a proteogenic amino acid that is required for numerous metabolic pathways, including purine, creatine, heme and glutathione biosynthesis. Glycine formation from serine, catalyzed by serine hydroxy methyltransferase, is the major source of this amino acid in humans. Our previous studies in a mouse model have shown a crucial role for the10-formyltetrahydrofolate dehydrogenase (ALDH1L1) enzyme in serine to glycine conversion.

Objectives: To determine the genomic influence on serine-glycine ratio in 803 Hispanic children from 319 families of the Viva La Familia cohort.

In recent years, the U.S. Supreme Court has repeatedly said that the doctrine of Article III standing deprives the federal courts of jurisdiction over some lawsuits involving intangible injuries. The lower federal courts are carrying out the Supreme Court’s instructions, and privacy injuries have borne the brunt of the Court’s directive. This Article identifies two incoherencies in the Court’s recent intangible injury decisions and builds on the work of privacy scholars to fashion a solution.

Apoptosis is a naturally occurring cell death mechanism to remove the selective cell population. B-cell leukemia/lymphoma-2 (BCL-2) family protein plays a critical role in activating the upstream apoptosis signaling pathway, primarily the intrinsic apoptosis pathway. The BCL-2 family consists of both pro-and anti-apoptotic proteins, which are structurally and functionally similar, containing up to four BCL-2 homologies (BH) motifs (BH1-4). Defecting apoptosis along this signaling pathway can lead to various events, including malignant cell transformation, tumor metastasis, tissue fibrosis, and drug resistance. Structural studies on proteins provide valuable knowledge, from information about their fundamental nature, to laying a foundation for rational drug design. Here, we study two proteins, a human mitochondrial chaperonin heat shock protein 60 (Hsp60) and the main structural HIV-1 protein capsid (CA). We focus on the structural nature of both proteins to try to understand their role in disease and elucidate the mechanism of potential drug treatments. Hsp60 is a chaperonin found in human mitochondria that is responsible for assisting client proteins reach their native, functional, state. Tatton-Brown-Rahman syndrome (TBRS) is an overgrowth disorder caused by germline heterozygous mutations in the DNA methyltransferase DNMT3A. DNMT3A is a critical regulator of hematopoietic stem cell (HSC) differentiation and somatic DNMT3A mutations are frequent in hematologic malignancies and clonal hematopoiesis. Yet, the impact of constitutive DNMT3A mutation on hematopoiesis in TBRS is undefined. In order to establish how constitutive mutation of DNMT3A impacts blood development in TBRS we gathered clinical data and analyzed blood parameters in 18 individuals with TBRS. We also determined the distribution of major peripheral blood cell lineages by flow cytometric analyses. Our analyses revealed non-anemic macrocytosis, …

Drunken sex is common. Despite how common drunken sex is, we think
very uncritically about it. In this paper, I want to examine whether drunk individuals
can consent to sex. Specifically, I answer this question: suppose that an individual,
D, who is drunk but can still engage in reasoning and communication, agrees to have
sex with a sober individual, S; is D’s consent to sex with S morally valid? I will argue
that, within a certain range of intoxication, an individual who is drunk can give valid
consent to have sex with an individual who is sober.

BACKGROUND: Uncompensated vestibular hypofunction can result in symptoms of dizziness, imbalance, and/or oscillopsia, gaze and gait instability, and impaired navigation and spatial orientation; thus, may negatively impact an individual's quality of life, ability to perform activities of daily living, drive, and work. It is estimated that one-third of adults in the United States have vestibular dysfunction and the incidence increases with age. There is strong evidence supporting vestibular physical therapy for reducing symptoms, improving gaze and postural stability, and improving function in individuals with vestibular hypofunction. Tumor boards are multidisciplinary team meetings composed of providers from different specialties who collaborate to provide comprehensive cancer care. Although the term “tumor board” may be used commonly in practice, these meetings are often referred to as multidisciplinary cancer conferences (MCCs) within the literature and are referred to as such in this article. At this time, there is no current literature describing the role of physical therapists (PTs) and rehabilitation professionals as part of the MCC or multidisciplinary tumor boards. PURPOSE: Mendelian disease genomic research has undergone a massive transformation over the past decade. With increasing availability of exome and genome sequencing, the role of Mendelian research has expanded beyond data collection, sequencing, and analysis to worldwide data sharing and collaboration.

METHODS: Over the past 10 years, the National Institutes of Health-supported Centers for Mendelian Genomics (CMGs) have played a major role in this research and clinical evolution.

HIV-2 is distinguished from HIV-1 with low virulence, longer asymptomatic period, slower decline in CD4 count, low viral load, and low mortality from AIDS. While the kidney pathology associated with HIV-1 is well established, due to its low prevalence, kidney pathology in HIV-2 is not well documented. We report a case of HIV associated nephropathy (HIVAN) as the first presentation in a patient with HIV-2. This paper provides a review of using Interferometric Synthetic Aperture Radar (InSAR), a microwave remote sensing technique, for deformation monitoring of hydroelectric power projects, a critical infrastructure that requires consistent and reliable monitoring. Almost all major dams around the world were built for the generation of hydropower. InSAR can enhance dam safety by providing timely settlement measurements at high spatial-resolution. BACKGROUND: Hypothermia is neuroprotective in some ischemia-reperfusion injuries. Ischemia-reperfusion injury may occur with traumatic subdural hematoma (SDH). This study aimed to determine whether early induction and maintenance of hypothermia in patients with acute SDH would lead to decreased ischemia-reperfusion injury and improve global neurologic outcome.

METHODS: This international, multicenter randomized controlled trial enrolled adult patients with SDH requiring evacuation of hematoma within 6 h of injury. The intervention was controlled temperature management of hypothermia to 35 °C prior to dura opening followed by 33 °C for 48 h compared with normothermia (37 °C). Hyperphosphatemia is a common complication in dialysis-dependent patients with chronic kidney disease. Most dialysis-dependent patients need oral phosphate binder therapy to control serum phosphorus concentrations. Most phosphate binders have a high daily pill burden, which may reduce treatment adherence and impair phosphorus control. Sucroferric oxyhydroxide is a potent iron-based phosphate binder approved for use in dialysis-dependent patients in 2013. A randomized controlled trial of sucroferric oxyhydroxide demonstrated its efficacy for reduction of serum phosphorus with a lower pill burden than sevelamer carbonate. Clinical trials carefully select patients, monitor adherence, and routinely titrate medications to a protocol-defined goal. Consequently, trials may …

Research has established low preferences in undergraduate nursing students for working with older people. However, there is limited research on preferences for dementia care. STUDY OBJECTIVE: The objectives of this study were to describe the reach and adoption of Geriatric Emergency Department Accreditation (GEDA) program and care processes instituted at accredited geriatric emergency departments (EDs).

METHODS: We analyzed a cross-section of a cohort of US EDs that received GEDA from May 2018 to March 2021. We obtained data from the American College of Emergency Physicians and publicly available sources. Data included GEDA level, geographic location, urban/rural designation, and care processes instituted. Frequencies and proportions and median and interquartile ranges were used to summarize categorical and continuous data, respectively.

In response to some of the criticisms of previous research into the arts in criminal justice, this article presents findings from research with a music programme run by a Youth Offending team (YOT). Data were collected on the attendance of 42 participants at YOT appointments – matched against a comparison group – and measures of change over time in musical development, attitudes and behaviour and well-being. Participants who completed the music programme were statistically more likely to attend YOT appointments than a comparison group. The blue economy has roots in the international arena of sustainable development and sets out to unlock opportunities for economy and society whilst protecting and enhancing marine environments. To date there has been no analysis of how this overarching intention for sustainability has influenced the rapid development of blue economy policies at national and regional scales. In this article, we analyse the synergies and conflicts between blue economy policies from a diversity of national and regional policies and the UN Sustainable Development Goals. Airborne particulate matter (PM) with an aerodynamic size cutoff of 10 µm (PM10) has been collected using a high volume air sampler at two locations (urban and residential) in the city of Ahvaz, Iran, for sixteen 24-hour periods over four months (late summer to early winter). Microplastics (MPs) in the PM were isolated after sample digestion and were subsequently characterised by established techniques. All MPs sampled (n = 322) were of a fibrous nature, with polyethylene terephthalate, nylon and polypropylene being the dominant polymers and consistent with textiles and fabrics as the principal source. Nasal nitric oxide (nNO) measurements are used in the assessment of patients suspected of having primary ciliary dyskinesia (PCD), but recommendations for performing such measurements have not focused on children and do not include all current practices. To guide the development of a European Respiratory Society-supported technical standard for nNO measurement in children, an international online survey was conducted to better understand current measurement practices among providers involved in PCD diagnostics. 78 professionals responded, representing 65 centres across 18 countries, mainly in Europe and North America.
