Problem: The Cardiac Surgery Intensive Care Unit (CSICU) at an academic medical center is experiencing an increase in critical events. The 2022 Safe and Reliable Healthcare Safety, Communication, Operational Reliability, and Engagement (SCORE) Survey revealed that this unit scored in the lowest percentile for burnout climate and over seventy percent of the nursing staff left in a span of six months. The staff feels this has resulted in a lack of guidance and a chaotic environment. Incident reports have also shown an increase in near-miss events.

The Effectiveness Of Parent-Child Interaction Therapy: A Multilevel Meta-Analysis, Rana Hong, Jeong Ok Lee, Sanghee Lee, Shweta Singh

Objectives: Parent–child interaction therapy (PCIT) is an empirically supported treatment (EST) initially developed to help parents manage difficult behaviors in preschoolers aged 2–7 years. Given its proven effectiveness across diverse populations, this meta-analysis aims to evaluate its effectiveness for Koreans using studies from 1991 to 2022. Methods: A total of 11 studies (n = 201 children; 224 parents) met the criteria for this meta-analysis. Results: A robust pooled effect size (μ= –0.31, 95% CI [–0.44, –0.17], p < .001) for PCIT across all study designs were found. Three-level random-effects meta-regressions further confirmed a significant pooled effect (μ = –0.81, 95% CI [–1.12, –0.50], p < .001), indicating that PCIT effects were consistently significant across studies. Subgroup analyses revealed significant reductions in externalizing behavior, parenting stress, and parent depression. Discussion and implications: This study contributes to the growing body of evidence on the cultural applicability of PCIT to Korean families

Accurate Cancer Subtyping is a cornerstone of modern oncology essential for effective diagnosis and guiding personalized treatment. Histopathological Images (HIs) which capture the microscopic structure of tissues are widely used for cancer detection and subtyping. Even though deep learning has made significant advances, existing HI based subtyping methods often focus on specific cancer types, lacking a generic framework.

BACKGROUND: Anterior cruciate ligament (ACL) reconstruction (ACLR) using quadriceps tendon (QT) autograft is associated with lower rates of graft rupture and favorable functional outcomes compared with hamstring (HS) autograft in adolescents. The ACL-deficient knee is at increased risk of injury to secondary stabilizers of the knee, including the menisci, although studies have not reported differences in rates of subsequent meniscal tear between HS and QT groups.

PURPOSE: The goal of this study was to compare rates of graft rupture and subsequent meniscal tear after adolescent ACLR using HS or QT autograft.

INTRODUCTION: Relationship between changes in cardiac function, functional capacity, and patient-reported health status in heart failure (HF) remains incompletely defined, which may help inform endpoint selection and clarify how distinct clinical domains reflect treatment response.

METHODS: This post hoc analysis of the randomized cardiac microcurrent (C-MIC) II trial, which evaluated the efficacy and safety of C-MIC therapy in patients with chronic HF with reduced ejection fraction on optimal guideline-directed medical therapy, included 65 ambulatory patients with non-ischaemic dilated cardiomyopathy, New York Heart Association (NYHA) Class III-IV symptoms, and baseline left ventricular ejection fraction (LVEF) 25-35%. Background: Children affected by parental HIV/AIDS constitute a particularly vulnerable population, often facing heightened risks in psychosocial adaptation due to family disruption, social stigma, and resource deprivation. However, the characteristics of their executive functions, particularly the core component of conflict monitoring, remain unclear.

Methods: The present study compared the performance of children affected by parental HIV/AIDS (n=32) to a control group (n=32) on a cognitive conflict and emotional conflict face-word Stroop task.

Results: The results of the cognitive Stroop task demonstrated that both groups showed better performance in congruent stimuli compared to incongruent stimuli (86.20% vs.67.30%; 568.43ms vs.586.04ms). In the …

Background: Vamorolone is a dissociative glucocorticoid receptor agonist for treating Duchenne muscular dystrophy (DMD). The VBP15-006 study assessed vamorolone safety, tolerability, and pharmacokinetics in 2- to < 4 and 7- to < 18-year-old boys with DMD. Results for 7- to < 18-year-old boys, either corticosteroid (CS)-untreated or switching from prior CS treatment, are reported here. Exploratory objectives included efficacy and pharmacodynamic biomarkers related to safety. Methods: In this phase II, open-label, multiple-dose study, participants received 2 or 6 mg/kg/day vamorolone for 12 weeks, followed by treatment in an expanded access protocol (EAP Canada). Results: 34 participants (12 CS-untreated, 22 CS-treated) aged 7- to < 18 years were enrolled. Most treatment-emergent adverse events were mild. All 34 participants completed VBP15-006 and entered EAP Canada. During EAP Canada, CS-untreated participants maintained stable linear growth, while CS-treated participants exhibited catch-up growth consistent with serum bone biomarkers (median total vamorolone exposure 1.3 and 1.7 years, respectively). Some weight gain occurred, especially in CS-untreated participants receiving 6 mg/kg/day. Dose-dependent adrenal suppression occurred in CS-untreated and CS-treated participants; 2 individuals who switched from deflazacort to vamorolone 2 mg/kg/day had generalized weakness consistent with adrenal insufficiency. Vamorolone showed dose-dependent pharmacokinetics, rapid clearance, and no accumulation. There were no relevant efficacy changes in CS-treated and CS-untreated groups at either dose. Conclusions: Vamorolone demonstrated a consistent safety profile in 7- to < 18-year-old boys with DMD. Switching to 6 mg/kg/day vamorolone appeared to mitigate adrenal insufficiency risk. There was no negative effect on growth, and catch-up growth occurred in previously CS-treated individuals switching to vamorolone. Trial registration: ClinicalTrials.gov: NCT05185622, NCT03863119. Post-earthquake damage assessment is crucial to evaluate the safety and serviceability of bridges after an earthquake. Conventional screening methods rely on manual inspection, which is costly, time-consuming, and poses safety risks to inspectors. To address these challenges, this study presents a context-aware vision transformer framework for integrating structural element details into multi-defect identification through a late-fusion approach. The model is trained on a post-earthquake image dataset collected from three major earthquakes in Italy, consisting of 356 images. Background: A sense of belonging, feeling accepted, respected, and supported, is crucial for student success. Hybrid Doctor of Physical Therapist Educational Programs (PTEPs), which blend online and inperson learning, present unique challenges to fostering belonging. This study explored students' insights on belonging in hybrid PTEPs, identifying facilitators and barriers and the relationship with well-being, leading to program recommendations.

Methods: This cross-sectional survey study of 18 hybrid PTEPs, measured belonging, explored belonging factors, and linked well-being with belonging. Interpersonal violence extends beyond physical assault to include psychological and/or emotional abuse that can produce substantial physical and mental health consequences. This study examines how multiple forms of interpersonal violence relate to health outcomes among North Dakotan women, focusing on differences between rural and non rural residents. Acinetobacter baumannii is an opportunistic Gram-negative pathogen that poses a significant global health threat due to its remarkable ability to acquire multidrug (MDR) and extensively drug-resistant (XDR) phenotypes. Therapeutic failures are increasingly common as strains develop resistance to nearly all available antibiotics, including carbapenems, polymyxins, and tigecycline, leaving clinicians with limited treatment options. This study aimed to investigate combinatorial therapeutic approaches capable of revitalizing antibiotic efficacy against MDR/XDR A. baumannii clinical isolates. This research provides a detailed guideline for implementing and evaluating Proportional Integral Derivative (PID) control frameworks in lower limb rehabilitation exoskeleton robotics. It examines the role of control systems within rehabilitation robotics, outlines the principles of PID control, describes exoskeleton architecture, explores applications of PID control, reviews optimization strategies, presents experimental validations, and considers future developments in the field. The proposed control framework incorporates aspects of mechanical design, actuator and sensor selection, and PID-based control algorithms, thereby promoting safe, accurate, and individualized rehabilitation support. Recommendations and effective guidance for future work are also presented.

However, despite advances in medical science, digital technologies, health policies, and billions of dollars invested over the past 25 years, most healthcare providers are far from fully realizing the demonstrated benefits of today's digital technologies for improving patient care. Healthcare provider burnout and the capacity to address patients’ social determinants of health are critical issues in modern medical systems. This study investigated the relationships among providers’ personal social determinants of health (Prov-SDH) needs, burnout, and perceived efficacy in addressing patients’ social determinants of health (Pt-SDH) needs. Using a sample of 149 healthcare professionals, the study examined how individual Prov-SDH factors, professional degree, clinical setting, and years of practice influenced these outcomes. Statistical analyses revealed no significant correlation between providers’ current or lifetime Prov-SDH needs and their reported burnout symptoms or Pt-SDH-related efficacy. The present study examined the interplay between emotions and trust in the context of automation reliance. The study investigated whether specific emotions (happiness, interest, anger, sadness, and fear) systematically impacted users’ trust in an x-ray screening decision aid, their reliance on the aid, as well as their resilience to any errors made by the aid. Despite successfully inducing certain emotional states, there was no significant relationship between emotions and trust in / reliance on the automated aid. Key resiliency attributes—robustness, redundancy, resourcefulness, and rapidity—enhance infrastructure adaptability and safety. This study investigates significant factors and methods for improving resiliency, with a focus on equity and reliability. A new resiliency metric, incorporating equity as a core element, is developed using probabilistic approaches. Latino adults in the United States experience a greater burden of disability than non-Latino White adults. This brief summarizes findings from a study using data from the 2000-2016 American Community Surveys (ACS) linked to annual state policy data (1998-2014) on Latino adults ages 45-85 across 25 U.S. states (N=1,183,438). The authors describe how state labor policies (i.e., disability insurance, minimum wage protections, paid sick leave, unemployment compensation) predict disability among Latino adults. Purpose: To describe muscular fitness (MF) levels and percentile benchmarks among U.S. children and adolescents aged 6–15 years old across sociodemographic, physiological, and behavioral subgroups. Methods: Data were from the 2012 NHANES National Youth Fitness Survey (NNYFS). Social network analysis (SNA) offers an innovative approach to understand youths' complex social support systems. Growing work highlights links between youths' social networks and mental health. We used SNA to examine associations between social network characteristics and psychological empowerment, depression, anxiety, and post-traumatic stress among youth living in areas with concentrated disadvantage. We administered egocentric social network surveys in which youth (i.e., egos) enumerated individuals in their social network (i.e., alters), answered questions about each alter, and described how alters were connected.
