Background: Meningitis has significant clinical morbidity and mortality in children. Mortality in children, especially neonates, is high due to diagnostic difficulty. Diagnosis of pediatric meningitis is traditionally made using CSF parameters and cultures. The performance of cerebrospinal fluid (CSF) multiplex polymerase chain reaction (PCR) meningitis/encephalitis (ME) panel needs further evaluation in the pediatric population.
Methods: Medical records of children with a positive CSF ME panel (June 1, 2016 to August 31, 2018) were reviewed retrospectively. We extracted data about antimicrobials used, laboratory and culture results, duration of antibiotic treatment, hospital stay, and follow-up. Long term outcome was evaluated.

Immune-Mediated Necrotising Myopathy Following Semaglutide Treatment: A Contributing Factor?, Brian Lam, Eleni Tiniakou, Xinhai Robert Zhang, Shervin Assassi

This report describes a female patient in her 60s with an immune-mediated necrotising myopathy (IMNM) occurring after semaglutide use. She was found to have progressive dysphagia, dysphonia, proximal muscle weakness and creatine kinase elevation over a 4-month course. Her symptoms were preceded by a single dose of semaglutide. Her work-up revealed features consistent with necrotising myopathy with inflammatory infiltrate on biopsy, and she responded to immunosuppression with corticosteroids and intravenous immunoglobulin (IVIG). She gradually improved and successfully returned to her baseline functional status while off immunosuppression for several months and with continued avoidance of glucagon-like peptide-1 (GLP-1) receptor agonists. Objectives: Blended learning has gained increasing attention in higher education as a hybrid approach that integrates face-to-face and online modalities. This study investigates academics’ decision-making processes and challenges in implementing blended learning models to inform future educational practices.
Methods: An exploratory case study design was employed. Data were obtained from academics representing multiple faculties who participated in a professional development program on technological approaches and learning models. Participants completed the Blended Learning Sub-Model Preparation Form, which was analyzed to identify preferences and challenges in adopting blended learning.

Objective: In this study, we address the communication barriers between academics and international students using a phenomenological approach. The aim of the study is to examine in depth the communication barriers faced by these two groups in the education process and to develop solutions for these barriers.
Method: The qualitative data obtained from semi-structured interviews with 42 academics and 50 international students were analyzed using the MAXQDA 20 package program.

An increasing number of athletic trainers are transitioning from traditional settings, such as secondary schools, colleges, universities, and professional sports, to physician practice. Although research suggests athletic trainers may be changing settings in search of better working conditions, little is known about their motivations for and satisfaction with the setting transition. Athletic training students (ATS) may have limited exposure to emergencies during their clinical experiences, which can undermine their confidence. AT educators can improve ATS confidence and clinical reasoning using a simulation-based, learner-centered SNAPPS approach. Athletic training is entering a transformative period driven by the convergence of wearable technologies, artificial intelligence (AI), and advanced biomechanical modeling. Historically, wearable technology in sport was primarily used for performance tracking, including workload monitoring, heart rate assessment, and movement quantification. However, advances in sensor accuracy and data analytics have expanded the clinical relevance of wearable systems, allowing athletic trainers to monitor fatigue markers, movement asymmetry, and neuromuscular control patterns that inform injury risk and rehabilitation progression. Introduction: The timed single-leg step-down (SLSD) test is widely used in clinical and sports settings to assess lower extremity function and injury risk, yet the specific biomechanical determinants of performance on this test remain poorly understood. The purpose of this study is to investigate the relationship between performance on the timed SLSD test and hip and knee biomechanics captured by a markerless 3-D motion-analysis system. Methods: Twenty (20) healthy high school athletes with no recent history of lower extremity injury completed a 60-second timed single leg step down test. During the fall of 2023, Utah Women & Leadership Project (UWLP) researchers conducted a statewide study to establish a baseline for public perceptions related to the awareness, understanding, and attitudes about challenges that Utah women and girls face. In early 2024, we published 20 short research summaries1 that shared the results of this comprehensive study (see Nos. 9–28). Next, in the fall of 2024, we administered this survey, with slight changes, around the state of Utah for the second time. Biomedical sciences (BMS) are a common career pathway for U.S. Hispanic high school students interested in pursuing healthcare careers. Little was known about how middle school career exploration programs in science, technology, engineering, and mathematics motivate Hispanic students to continue pursuing BMS careers. The purpose of this basic qualitative study was to explore high school teachers’ perspectives of how middle school BMS career exploration programs can motivate Hispanic students’ interest in BMS careers. While de novo variants cause many Mendelian disorders, their detection currently requires sequencing of the proband and both biological parents. This is not feasible when only one parent is available, a limitation for millions of families. Here, we develop duoNovo, which identifies de novo variants from parent-proband duos using long-read sequencing followed by haplotype reconstruction and detection of identical-by-descent haplotype blocks. We sequenced 104 trios with PacBio HiFi sequencing and applied duoNovo to each of the 208 duos constructed by masking one parent, classifying over 55 million variants according to their de novo status. BackgroundThe presence of artificial structures in our marine environments is increasing rapidly, with negative impacts for biodiversity. Greening of grey infrastructure (GGI) - an eco-engineering method applied to the marine context - aims to increase the ecological value of traditional grey infrastructure, while still allowing it to perform its primary human-centric function. GGI is a rapidly increasing field of research, being tested and implemented worldwide by academics, private practitioners, governments, non-governmental organisations (NGOs), amongst others, using a variety of methods. Introduction: Patellar instability is a common knee pathology, particularly among adolescents and young adults, with reported incidence rates ranging from 7 to 77 per 100,000 person-years. Following an initial patellar dislocation, recurrence rates have been reported as high as 86%, particularly in patients with underlying anatomic risk factors. Identification of the multifactorial contributors to patellar instability is critical to guide appropriate management and prevent recurrent episodes. Common pathoanatomic factors associated with recurrent instability include trochlear dysplasia, patella alta, lateralized tibial tubercle, coronal or torsional malalignment, and abnormal patellar shift. Background: Sepsis is a life-threatening complication of infection with high mortality. A high-throughput analysis of circulating blood proteins may provide mechanistic insight and potent therapeutic targets for the prevention of sepsis.
Methods: We used multivariable Cox regression analysis to examine the association of 4955 plasma proteins, measured by SomaScan, with the risk of incident sepsis among 11 065 participants of the Atherosclerosis Risk in Communities (ARIC) Study (visit 3 in 1993 to 1995; mean age, 60.1 years, 54.4% female, 21.0% Black). Proteins (false discovery rate [FDR] of P < 0.05) discovered at visit 3 were replicated using data at visit 5 (n = 4869 in 2011 to 2013: mean age, 75.5 years) and in the Cardiovascular Health Study (CHS) (n = 3512 in 1992 to 1993; mean age, 74.5 years). Canonical pathways were identified by enrichment analyses.

Nardilysin (NRDC) plays a role in multiple cellular functions in diverse cellular compartments, including ectodomain shedding in the plasma membrane, as well as chaperoning a key Krebs cycle enzyme in mitochondria. We had previously reported limited clinical information from two individuals with homozygous frameshift variants in NRDC. With inclusion of previously published individuals, here we report 14 individuals (10 females, four males) from nine unrelated families carrying homozygous NRDC pathogenic variants. One of the most common interventions in the ED and ICU to restore physiologic homeostasis is fluid administration. It is essential for the intensivists and ED physicians to be aware of the physiological characteristics of different fluids and the potential harm associated with their use. In this article, we explore the physiology and pathophysiology of fluid administration in humans, emphasizing how fluid administration can alter acid-base balance, perfusion, osmotic and oncotic pressures, immunologic and inflammatory responses, and hemodynamics. Background and Objective:
While vaccination remains central to controlling the COVID-19 pandemic, the emergence of SARS-CoV-2 variants with partial resistance to immune responses has highlighted the need for complementary therapeutic strategies. Among these, antiviral agents that inhibit viral entry mechanisms are of particular interest. Animal venoms, especially scorpion venoms, are a rich source of bioactive peptides with potential antiviral properties. This study aimed to evaluate peptides derived from the Moroccan scorpion Androctonus mauretanicus as inhibitors of SARS-CoV-2 spike glycoprotein, which mediates virus entry into host cells via ACE2 receptor binding.
