Background: Despite widespread adoption of strategic planning in healthcare, empirical evidence linking strategic planning time horizons to hospital financial and quality performance outcomes remains limited. Healthcare executives invest substantial resources in strategic planning processes, yet lack evidence-based guidance on whether longer or shorter planning horizons yield enhanced financial and quality-related results.

 

Methods: This study employed an exploratory sequential mixed-methods design combining executive survey data with archival performance data. Evaluating U.S. Health Policy Levers Using Difference-In-Differences: Hospital Acquisitions, Global Budgets, And Income Policies, Kola Adegoke

Abstract

Background: U.S. health policy relies on multiple levers to influence system performance, financial stability, and population health. This dissertation uses a three-paper portfolio to evaluate organizational change (hospital acquisitions), payment reform (Maryland all-payer global budgets), and income-related state policies, applying difference-in-differences (DiD) designs with prespecified diagnostics to support causal interpretation.

Methods: Study 1 examined hospital change-of-ownership (CHOW) acquisitions using a national hospital-year panel from RAND HCRIS (2014–2023), linked to CHOW timing data from 2016–2023. Effects on revenue outcomes were estimated using a staggered difference-in-differences design implemented with the Callaway and Sant’Anna estimator, using doubly robust inverse probability weighting, hospital-level …

In the United States today, gender can be a hot button topic, seemingly controversial and assigned as political, and within conservative regions the topic of gender becomes more taboo. Unfortunately, this view can lead to the stigmatization and discrimination of individuals who fall outside of the socially constructed gender binary. In order to explore how living in the conservative landscape of the Midwest impacts individuals who reside outside of the gender binary, 11 semi-structured, qualitative interviews with Trans and/or Two-Spirit individuals were conducted in which they were asked about their lived experiences. Purpose: Daily anatomical changes can invalidate the geometric, dosimetric, and biological assumptions of reference radiotherapy plans, yet quantitative evidence describing fraction-level deviations across disease sites remains limited. This work developed and evaluated a metric-based framework for daily dose assessment to characterize temporal treatment behavior and inform adaptive decision-making in Head and Neck (H&N) radiotherapy and Prostate stereotactic body radiotherapy (SBRT). Methods: A web-based Dashboard was created to compute geometric, dosimetric, and radiobiological metrics from daily dose recalculations. Research suggests that people with chronic fatigue syndrome tend to suppress emotions more than healthy individuals. However, whether there are also changes in the neural substrates of emotional regulation in people with chronic fatigue syndrome remain unexplored. Specifically, it is unclear whether there is a neural delineation in how fatigue and anger-related memories are recalled or supressed in people with chronic fatigue syndrome. This study investigated this hypothesis using functional magnetic resonance imaging. Introduction:

Virtual reality (VR) systems have evolved significantly over the past decade, enabling immersive experiences with enhanced realism and interactivity. This has motivated an interest in socially oriented applications. As user proxies, Virtual Humans (VHs) play essential roles in such applications. However, despite technological advancements, achieving realistic, expressive, and socially responsive VHs continues to present design and implementation challenges. In this scoping review, we present the state-of-the-art of VR VHs, examining the impact of VHs on the user experience.

Aim: Sleep difficulties and elevated stress are common among individuals recovering from substance use disorders (SUDs), often persisting long after initial detoxification. Music-based interventions offer a low-risk, nonpharmacological option that may support sleep and stress regulation. This study examined whether a Structured Music Listening Protocol (SMLP) could improve sleep quality and reduce perceived stress within a residential SUD treatment program.

Methods: Fifty eligible patients were enrolled in the Intervention Group (SMLP – Structured Music Listening Protocol) and fifty in the Control Group (Standardized Therapy) from a residential treatment center for SUDs. Both groups received standard therapy from the center. The …

Aim This study aimed to assess the oral health status of individuals with schizophrenia and explore its association with behavioural and pharmacological factors, with particular focus on long-term antipsychotic treatment and cumulative anticholinergic burden. Methods A total of 153 adults with schizophrenia (18–65 years) from the Mental Health Network of Bizkaia (Spain), all under antipsychotic treatment for ≥12 months, were evaluated and compared with 153 controls from the general population. Introduction: Atrial fibrillation (AF) remains one of the leading dysrhythmias incurring hospitalization. We set out to determine the impact of socioeconomic status (SES) on in-hospital cardiac procedures and the readmission outcomes in patients with AF. Methods: The National Readmission Database (NRD) was searched for adults with a primary diagnosis of AF between 2010 and 2019. We determined unplanned AF-specific readmission within 30 days. Median neighborhood household income was assessed by patient zip code as quartiles: low income (Q1), middle income (Q2), upper-middle income (Q3), and high income (Q4). Platelet-derived extracellular vesicles (PEVs) play an active role in vascular protection and repair and are being explored as a viable alternative to platelet therapy. Because platelet function and stability are shaped by donor sex and storage conditions, these same factors are likely to influence the PEVs they release. Understanding these influences is key to developing PEVs into a safe and dependable therapeutic option. Optimizing the geographic allocation of health care providers has the potential to improve health care access and health outcomes for rural populations. To inform the design of a discrete choice experiment (DCE) survey to elicit health professions trainees' preferences for rural practice, this formative research aimed to: (1) elicit factors that influence job selection among health profession trainees, including medical residents physician assistant (PA) and nurse practitioner (NP) students, (2) identify how these trainees evaluate trade-offs for rural job placement and (3) inform attribute selection for inclusion into a future DCE to identify health professions trainees’ preferences for rural practice. …

Heating, ventilation, and air conditioning (HVAC) systems are major contributors to residential energy consumption. However, most homes continue to rely on single zone thermostat control, which regulates temperature based on a single sensor and cannot account for thermal variations across multiple rooms. This often leads to uneven thermal conditions and inefficient energy use in multi-zone residential buildings. This thesis presents a deep reinforcement learning based approach for improving HVAC zoning control through dynamic airflow distribution. Practice Problem: Patient satisfaction is a key quality indicator that reflects patients’ perceptions of care and the healthcare system’s performance. Patient satisfaction scores at the project site were significantly lower than the national benchmark, particularly on the adult medical-surgical unit.

This Article argues that the appointment of prominent wellness and antivaccine figures to senior federal health positions marks the culmination of a long-running evolution in American health fraud—from nineteenth-century traveling medicine shows to the institutional capture of government health agencies. Drawing a direct lineage from patent medicine marketing to contemporary wellness conspiracism, this Article demonstrates how tactics perfected by early medicine show entrepreneurs—emotional manipulation, conspiratorial framing, anti-establishment positioning, and the exploitation of information asymmetries—have persisted across regulatory and technological shifts, ultimately achieving unprecedented political legitimacy.
