Transdiaphragmatic Intercostal Hernia Repair In A Patient With Previous Thoracic Surgery, Lilyan T. Starkey, Ruth P. Gerola, David A. Denning
Marshall Journal of Medicine
A transdiaphragmatic intercostal hernia (TDIH) describes the herniation of abdominal contents through diaphragm and chest wall defects. It has been documented following traumatic injuries as well as after episodes of increased thoracoabdominal pressure. However, those resulting from iatrogenic cause remain uncommon and are less often recorded. We report an iatrogenic TDIH in a patient presenting 39 days after a complicated thoracic procedure, highlighting the surgical technique used for successful hernia reduction, diaphragmatic closure, and rib re-approximation. The case reinforces the possibility of a TDIH occurring as a post-surgical complication. Surgeons should anticipate the potential development of TDIH, particularly after thoracic …

Propofol Related Infusion Syndrome: A Subtle Adversary, Brandon Harris, Elizabeth C. Taylor, John W. Pickstone, Errington C. Thompson Md
Marshall Journal of Medicine
Propofol Related Infusion Syndrome (PRIS) was first described in 1998. It has a strange collection of symptoms, including marked bradycardia, persistent, recalcitrant metabolic acidosis, liver enlargement, rhabdomyolysis, and lipemic blood. We present two recent patients who appeared to have had PRIS. Creatinine kinase seems to be an early detector of PRIS. If PRIS is recognized early, this complex metabolic process seems to be completely reversible.

Robotic Hysterectomy: Surgical Approach And Outcomes Among A Large Institutional Cohort, Shirin Azadi, Sydney Graham, Stephen Bush, Rachel Lee, Nadim Bou Zgheib
Marshall Journal of Medicine
Introduction
The purpose of this report is to describe a technique for performing a robotic total laparoscopic hysterectomy (rTLH) with clinical outcomes on safety and efficiency. The rationale for our approach is based on a critical evaluation of the literature.
Methods
Data from all rTLH procedures performed on our gynecologic oncology service between January 2017 and December 2019 were retrospectively reviewed. Using this database, perioperative data including surgical times, intra- and postoperative complications, and length of hospital stays were evaluated. The steps used to perform the procedure were outlined and illustrated.
Results
826 cases of rTLH were performed during the …

A Taxonomy Of Applied Research Categories As An Aid To Research Pertaining To Aviation, David Trafimow
A Taxonomy Of Applied Research Categories As An Aid To Research Pertaining To Aviation, David Trafimow
International Journal of Aviation Sciences (IJAS)
Based on the notion that applied research can be theoretically grounded or not theoretically grounded, I suggest that there are roughly four categories within which quality work in this area can fall: basic research, auxiliary assumptions, theory combined with auxiliary assumptions, and research without theory. Although the importance of auxiliary assumptions in traversing the distance between theories and their applications cannot validly be denied, applied researchers, including those in the aviation area, often have ignored them. But auxiliary assumptions play an important role in understanding the four categories that I propose. Finally, I argue that much of the applied research …
Avionics Market Forecasting: Comparing The Performance Of Artificial Neural Networks Forecasting With Traditional Forecasting Techniques, Jason Alexander Hoop, Walter Rodriguez
Avionics Market Forecasting: Comparing The Performance Of Artificial Neural Networks Forecasting With Traditional Forecasting Techniques, Jason Alexander Hoop, Walter Rodriguez
International Journal of Aviation Sciences (IJAS)
Avionics can be thought of as the nervous system sustaining aircraft operations, and accurate forecasting is critical in predicting system demand. Modeled after their biological counterpart, artificial neural networks (ANNs) maintain the ability to recognize trends in chaotic and nonlinear systems. These models possess the capability to produce more accurate predictions than those of traditional forecasting methods. The purpose of this paper is to provide a comprehensive overview of ANNs and detail the use of these techniques as a tool for forecasting in the avionics field. Furthermore, it introduces and examines several commonly used types of predictive ANNs. The performance …
Grassland Productivity And Water Quality: A 21st Century Issue, David M. Nash, P. M. Haygarth
Grassland Productivity And Water Quality: A 21st Century Issue, David M. Nash, P. M. Haygarth
IGC Proceedings (1977-2023)
- Irrigation and other changes to the hydrological cycle can increase soil and water salinity.
- Primary salinisation is a natural process that affects much of Europe, Asia, Africa, the Americas and Australia. Secondary salinisation is caused by human activities such as irrigation and land clearing that mobilise salt stored in the soil.
- The critical water contaminants exported from grasslands are nitrogen, phosphorus, potential pathogens and sediment.
- The mechanisms responsible for diffuse pollution from grasslands and mitigation strategies are most effectively investigated using a ‘source-mobilisation-transport’ framework.
- There is a lack of coherent interaction across discipline boundaries that links pollutant sources to impact. …

Water Resources, Agriculture And Pasture: Implications Of Growing Demand And Increasing Scarcity, M. W. Rosegrant, R. A. Valmonte-Santos, S. A. Cline, C. Ringler, W. Li
IGC Proceedings (1977-2023)
- Water availability for irrigation is threatened in many regions by rapidly increasing demand for nonagricultural water uses in industry, households, and the environment. The scarcity of irrigation water will not only impact crop production, but also meat production, as much of the pasture used to feed livestock is irrigated.
- Grassland is caught between two countervailing forces: a requirement for increasing meat demand that boosts the need for additional pasture to support livestock production, and rapidly increasing water scarcity that makes pasture irrigation uneconomical.
- The most effective means of dealing with water scarcity is likely to be conserving water in existing …

Soil Microbial Community: Understanding The Belowground Network For Sustainable Grassland Management, Y. G. Zhu, W. D. Kong, B. D. Chen, Z. B. Nan, P. Christie
IGC Proceedings (1977-2023)
- In addition to the use of conventional methodologies in soil microbial research, molecular techniques are now being applied to gain insights into the soil microbial community;
- Plant diversity can exert impacts on soil microbial diversity (through root activities and plant litter etc.), but may in itself be significantly altered by soil properties;
- Soil microbial diversity largely determines the stability of soil ecosystems under biotic and abiotic perturbations.
- Management of soil microbial diversity can only be achieved through better understanding their structures and functions.
Grasslands1 For Production And The Environment, David R. Kemp, David L. Michalk
Grasslands1 For Production And The Environment, David R. Kemp, David L. Michalk
IGC Proceedings (1977-2023)
- To manage grasslands for production and enhanced environmental values requires a redefinition of the frameworks within which management decisions are made, and a tailoring of practices to suit the ways that farmers operate.
- Improving the perenniality and permanence of grasslands usually leads to better environmental and production outcomes.
- There is a case for a more conservative approach to utilising grasslands in order to sustain the functioning of local ecosystems and to improve water quality, nutrient and energy cycling and biodiversity.
- A landscape rather than paddock focus is more appropriate for meeting current grassland management objectives. Grasslands can be triaged to …

Evolution Of Integrated Crop-Livestock Production Systems, Martin H. Entz, William D. Bellotti, J. M. Powell, S. V. Angadi, W. Chen, K. H. Ominski, B. Boelt
IGC Proceedings (1977-2023)
- Many factors contribute to changes in the crop-livestock systems, but no logical end-point in the evolution process exists.
- While benefits of integrated crop-livestock systems over specialised crop and livestock systems are well documented, there has been a move to specialised crop and livestock production.
- Sustainability issues (manure nutrient concentration, soil quality maintenance, salinity, herbicide resistance, economic instability) have created a renewed interest in integrated crop-livestock systems.
- Farmer adaptability is as an important link in the evolution between ‘states of integration’.
Strategies To Mitigate Seasonality Of Production In Grassland-Based Systems, Claudio Porqueddu, S. Maltoni, J. G. Mcivor
Strategies To Mitigate Seasonality Of Production In Grassland-Based Systems, Claudio Porqueddu, S. Maltoni, J. G. Mcivor
IGC Proceedings (1977-2023)
- Fertilisation use and manipulation can cost-effectively alter species composition, increase seasonal herbage production and improve herbage quality.
- Choice of suitable grassland species, varieties and mixtures offers opportunity to mitigate limitations of seasonal grassland production.
- Special purpose fodder crops, cereals, shrubs and trees offer alternative or supplementary feed sources.
- Manipulation of stocking rates, grazing systems, transhumance and pasture management at various times of the season are significant advantageous options.
- Integration of different strategies is essential to mitigate seasonality in systems of animal production that must be inherently more sustainable over a longer time frame.

An Analytical And Comparative Study: Design Of Outdoor Shading Device Based On Fractal Geometry Strategies To Increase Thermal Comfort, Dalia Mohamed Ezzat
Journal of Art, Design and Music
Despite growing design demand, there has been little research on the design of shading devices for the outdoor environment. Heat-related illnesses are becoming a more pressing issue as temperatures rise due to climate change. Seeking shade is one effective way for humans to protect themselves. Blocking sun radiation is the main goal of shading. One of the most effective cooling strategies, found in nature, is the tree fractal strategy used to optimize shading. Similarly, Girih patterns are fractal in geometry. The purpose of this paper is to design a shading device based on the Girih fractal geometry inspired by tree …

Is There Room For More?: Considering The Need For A Decoloniality Community Psychology Core Competency, Jesica S. Fernández, Janelle M. Silva
Ethnic Studies
From a decolonizing standpoint, as proposed by Cruz and Sonn (2011), the current community psychology competencies seem insufficient because these often leave power structures intact. Consequently, we propose a decolonizing, decolonial and anti-colonial competency in community psychology practice to facilitate the practitioner’s process toward decoloniality, specifically decolonizing language, discourses, relationships and research processes with communities. A decolonial competency in community psychology practice is characterized by an iterative process of critical ethical reflexivity that aims to de-link community psychology practice from hegemonic Western Eurocentric perspectives in order to foster and center community voice, knowledge and power. Through an autoethnographic methodology we …

Geospatial research in archaeology often relies on datasets previously collected by other archaeologists or third-party groups, such as state or federal government entities. This article discusses our work with geospatial datasets for identifying, documenting, and evaluating prehistoric and historic water features in the western United States. As part of a project on water heritage and long-term views on water management, our research has involved aggregating spatial data from an array of open access and semi-open access sources. Here, we consider the challenges of working with such datasets, including outdated or disorganized information, and fragmentary data. Based on our experiences, we …

Background: The uptake of the human papillomavirus (HPV) vaccines has been controversial among religious parents due to beliefs that their children are expected to practice sexual purity and so do not need protection from a sex-related infection. Also, if at all they get infected in the future, God can protect them from sickness without a vaccine. Yet, most HPV vaccination messages are secular, lacking spiritual themes. This study compared the effectiveness of the Centers for Disease Control and Prevention (CDC) Vaccine Information Statement (VIS) on HPV with our intervention message- a scripture-embedded HPV vaccination message (using a randomized controlled …
