The Qualitative Report
Patient-centric digital infrastructure can potentially enhance the efficiency of healthcare systems. However, even in developed nations, evidence suggests low adoption rates for such infrastructure and lack of support from clinicians is considered as one of the most critical hindering factors. In this study, we examine physicians' perceptions of the proposed large-scale information technology initiative in India that aims to transform the health sector and provide universal health coverage to all residents of India. We employed the information ecology lens to understand the broader changes in the healthcare system that could result from the initiative. We use focus group discussion and …

The standard primary treatment for acute graft-versus-host disease (GVHD) requires prolonged, high-dose systemic corticosteroids (SCSs) that delay reconstitution of the immune system. We used validated clinical and biomarker staging criteria to identify a group of patients with low-risk (LR) GVHD that is very likely to respond to SCS. We hypothesized that itacitinib, a selective JAK1 inhibitor, would effectively treat LR GVHD without SCS. We treated 70 patients with LR GVHD in a multicenter, phase 2 trial (NCT03846479) with 28 days of itacitinib 200 mg/d (responders could receive a second 28-day cycle), and we compared their outcomes to those …

A 67-year-old male with metastatic lung cancer presented with acute shortness of breath and increasing oxygen requirements. He had a decreasing hemoglobin for which he required red blood cell transfusions. His chest x-ray showed near complete white-out of the left lung. Bedside ultrasound (Handheld Sonostar C4PL) showed a large pleural effusion with swirling echogenic material suggestive of plankton sign. The pleural effusion was aspirated and showed frank blood, after which a small-bore chest tube (SBCT) was inserted. A total of 3200ml of blood was drained with the SBCT. There was complete clearance of the pleural space, and no further blood …

European legislation for occupational safety and health (OSH) is based on the prevention of accidents and ill health. Apart from European directives, all Member States have national laws and strategies and expect enterprises to implement this preventative ethos, and thereby ensure adequate performance in OSH. Labour Inspectorates monitor and if necessary enforce these laws by deploying individual Labour Inspectors to chosen enterprises. If workplace safety and health conditions are found to be substandard, Labour Inspectors are expected to improve the situation using persuasion and, if necessary, their authority to compel enterprises through specific sanction powers. Depression is a common and disabling disorder in later life, particularly among people with poor physical health. There are many screening tools available that can be used to examine depressive symptoms; however, not all of them may be appropriate or accurate for older adults with cancer. This pilot study was designed to test the diagnostic performance of two screening tools and their short versions in a cohort of vulnerable (G8 score ≤ 14/17) older patients with cancer undergoing comprehensive geriatric assessment (CGA). Reformulating packaged foods has the potential to improve the nutrient density of the global diet. The present perspective illustrates The Kraft Heinz Company’s approach to product (re)formulation to develop healthier product lines that are lower in saturated fats, total sugars, and sodium, and contain health promoting components. Here we present the rationale for The Kraft Heinz Company’s global nutrition targets used for the global innovation and renovation of foods and beverages. Mathematics can be intellectually demanding, engaging, and fulfilling. Learning mathematical concepts adequately warrants an environment where students can err without penalty, shame, or hurtful consequences. Teaching mathematics efficaciously depends on the trusting relationship between the teacher and the students. We advocate blending mathematics teaching with kindness because it benefits the teacher, the students, and society. Kindness, niceness, caring, and benevolence are interrelated but not synonymous. We outline four progressive levels of kindness: conditional, superficial, optimal, and genuine. Blending mathematics teaching and kindness effectively requires the teacher to decenter from their own perspectives and adopt the student’s perspective as the student …

Inspired by Brent Davis' conceptualization of listening and conversation in his book Teaching Mathematics: Toward a Sound Alternative, we propose how we as a mathematics education community may move forward by continuing in the conversation that emerged from COVID. We encourage all involved to listen rather than assume a discussion-oriented stance. Using an enactivist lens, we look at the pandemic learning space, give an overview of the education conversation that emerged in Ontario, and offer a way to rethink Mathematics Education within the frame of a conversation. COVID-19 left a damaged path since its arrival in December 2019. Clinical Coordination in a Physician Assistant (PA) program is an important aspect of PA education. McKinney (2018) has argued that for the scholarship of teaching and learning (SoTL) to advance within a discipline, the integration of SoTL must be closely examined and opportunities for growth in SoTL must be recognized and discussed. Introduction: Transnasal flexible endoscopy (TNFE) is necessary for multiple assessments in speech-language pathology (SLP), but it is generally considered an advanced practice technique to be learned during clinical practice. As such, there is no standardized way that it is taught in training programs, leading to a substantial knowledge gap for new graduates. Though peer-to-peer practice has been discussed as an important step in training, it is not clear whether it confers additional benefits above and beyond simulation. Language sample analysis (LSA) is valuable clinical tool and an important component of comprehensive language assessment. However, LSA is underutilized in real-world practice. SLPs have identified time constraints, insufficient training, and lack of confidence in LSA skills as barriers to regular LSA implementation. Communication science and disorders (CSD) programs have opportunities to provide LSA instruction to address these barriers and prepare students to reliably, feasibly, and confidently use LSA in clinical practice. Evidence supports the use of simulation in educating speech pathology (SP) students, however most of the research has centered on low fidelity techniques as opposed to high fidelity and immersive technologies like virtual reality (VR). Whilst there has been research on the use of VR to teach clinical skills to students in other health disciplines (e.g., dentistry, nursing), use of VR in SP has focused mainly on its use in client intervention. Speech-language pathologists often lack preparation for working with patients near the end of life (EoL). Few academic training programs offer dedicated or sufficient content in the area of end-of-life care (EoLC). Furthermore, traditional knowledge-focused outcomes are not the most effective pedagogical strategy in teaching EoL and palliative care to students. The purpose of this study was to examine the psychometric properties of the Self-Efficacy Questionnaire (SEQ), a self-reported questionnaire using Rasch analysis, a one-parameter logistic probability model based on scientific measurement principles. Rasch analysis was completed using the rating scale model. The various analyses allow researchers to examine the item-level psychometric properties of the SEQ, which result in measures that provide evidence for validity, reliability, and sensitivity of the instrument. Rasch analysis demonstrated that the latent trait established by the SEQ, Perceived Self-efficacy (called self-efficacy from here on), was a unidimensional construct that could be measured on a linear scale. …

Although pedagogy focused on social justice, power, and privilege have been included in teacher preparatory programs for a number of years, such topics are much newer and relatively unexplored within the context of Communication Sciences and Disorders (CSD) preprofessional programs. This reflection provides information on use of a living document assignment to encourage undergraduate CSD students to grapple with these difficult topics (e.g., privilege, oppression, social justice, allyship) early in their professional careers resulting in benefits for the students, their future clients, and the field of CSD.

The purpose of this pilot study was to define what changes were experienced in the Fall 2020 semester by students in an undergraduate anatomy and physiology of speech and hearing course and highlight any differences in the experience of first-generation college students (FGCS). An online survey was administered to a single class of undergraduate students. Results indicated that students experienced a variety of challenges related to the pandemic and virtual learning. Differences in the experiences of FGCS compared to traditional students included increased hours worked and feelings of being overwhelmed and spending less time on course content each week. Future …

Coronary Flow And Unloading In Acute Myocardial Infarction Shock, Kiyo Ishikawa

In patients with cardiogenic shock that undergo successful coronary intervention, there are still factors complicating myocardial recovery. There is room for improvement in coronary flow using mechanical circulatory devices, specifically by left ventricular unloading. This idea was further explored in a research study using pigs. Results showed that subjects with acute myocardial infarction who have reduced cardiac contractility and/or high diastolic pressure would benefit from support strategies targeting left ventricular unloading.
