Thyroid cancer is the most common endocrine malignancy with an estimated 43,800 new cases to be diagnosed in 2022 and representing the 7th most common cancer in women. While thyroid nodules are very common, being identified in over 60% of randomly selected adults, only 5-15% of thyroid nodules harbor thyroid malignancy. Therefore, it is incumbent upon physicians to detect and treat thyroid malignancies as is clinically appropriate and avoid unnecessary invasive procedures in patients with benign asymptomatic lesions. Background: Recent literature emphases that Diversity, Equity, & Inclusion (DE&I) efforts be data driven and transparent, often in dashboard format, to support the ability of GME programs and sponsoring institutions (SI) to clearly identify progress and concerns longitudinally. DE&I efforts have been a long-term focus in our GME programs and SI and are now further delineated in the ACGME's Common Program Requirements scheduled to take effect July 1, 2023. However, DE&I data sources may be limited, challenging to collect and/or in variable formats from multiple sources making it difficult to capture a longitudinal view of our efforts by program/SI.

Background and Objectives: Childhood obesity is a global public health concern with long-term and serious health implications. An important factor for childhood obesity is maternal gestational diabetes mellitus (GDM), which in turn impacts maternal and offspring long-term health. This study aimed to investigate the associations between maternal GDM and childhood weight status and multiple anthropometric and sociodemographic factors and perinatal outcomes. Materials and Methods: A total of 5348 children aged 2–5 years old and their paired mothers took part in the study. OBJECTIVE: To identify factors influencing sperm donor willingness to participate in direct-to-consumer genetic testing, comfort with sharing genetically identifiable data in commercial genetic testing databases, and likelihood to donate sperm again.

DESIGN: Cross-sectional online anonymous survey.

SETTING: Multicenter, 2 large American sperm banks from July 1, 2020 to July10, 2021.

PATIENTS: Sperm donors from 1980 to 2020.

INTERVENTIONS: None.

MAIN OUTCOME MEASURES: Associations between donor demographic characteristics, donation history, and attitudes toward direct-to-consumer genetic testing.

RESULTS: A total of 396 donors completed the survey. Most donations (61.5%) occurred from 2010 to 2020, and 34.3% were nonidentified donations. Nonidentified donors were …

Since the early 2010s, synthetic opioids have significantly contributed to overall opioid-related overdose mortalities. For point of reference, of the 68,630 opioid-related deaths recorded in 2020, 56,516 involved synthetic opioids. During much of this period, fentanyl has been the most commonly used synthetic opioid. This time when fentanyl was the most popular opioid has been called the “third wave” of the opioid crisis, partly because it led to a sharp rise in deaths from overdoses. Other synthetic opioids, such as carfentanil, protonitazene, and isotonitazene, have also become more widely diverted for nonmedical used. Carfentanil is an even more potent fentanyl …

The role of ribosome recycling factor (RRF) of E. coli was studied in vivo and in vitro. We used the translational coupling without the Shine-Dalgarno sequence of downstream ORF (d-ORF) as a model system of the RRF action in natural termination of protein synthesis. For the in vivo studies we used the translational coupling by the adjacent coat and lysis genes of RNA phage GA sharing the termination and initiation (UAAUG) and temperature sensitive RRF. The California Consumer Financial Protection Law (CCFPL) was passed in 2020. This piece of legislation rebranded the state’s financial services regulator as the “Department of Financial Protection and Innovation” (DFPI) and authorized the creation of an “Office of Financial Technology Innovation” (OFTI) within the DFPI. The CCFPL grants the DFPI expanded supervision over “fintech” providers—firms that leverage software-based technologies to deliver financial services to consumers via nontraditional conduits. Recognizing that fintech presents both potential benefits and risks for consumers, the California legislature took measures to ensure that the new regulatory scheme does not stifle innovation. In May 2021, in the case of LA Alliance v. Los Angeles, Judge David O. Carter of the Central District of California granted a sweeping order enjoining the city and county of Los Angeles to offer shelter to all unhoused persons living in Skid Row. The 109-page order identified structural racism and government indifference as the unconstitutional causes of homelessness in the region, and condemned California’s housing-first approach to addressing the issue. Although the Ninth Circuit swiftly vacated the preliminary injunction on procedural grounds, Judge Carter’s order begs the question: would universal shelter offers actually ameliorate the homelessness crisis? …

A majority of Mainers continue to approve of Janet Mills’ performance as Maine's Governor and she remains personally popular in the state. Majorities of Mainers support voter identification and family leave bills, as well as a bill which would allow abortion after 24 weeks if approved by a doctor. Most Mainers consider homelessness to be a serious problem in the state and the cost of living and housing are most often mentioned as the most important problems facing Maine.

Overprotected But Unrepresented: An Argument For Mandatory Appointment Of Counsel And Against Automatic General Conservatorships In California, Maria De Los Angeles Reyes Olmedo

Loyola of Los Angeles Law Review

Conservatorships are restrictive arrangements that must be reserved for people with severe intellectual and/or developmental disabilities. However, California probate courts unnecessarily impose conservatorships and forego less restrictive alternatives for the sake of administrative convenience. While AB 1194 will ameliorate California’s paternalistic conservatorship system, this Note advocates for requiring courts to appoint counsel to all conservatees and proposed conservatees in every conservatorship proceeding, and to enact conservatorships only after proving less restrictive methods are insufficient. Humans—regardless of their dis/ability—deserve to age with dignity in a conservatee-centric system that does not violate their constitutional, federal, and state rights.

Early response assessment is critical for personalizing cancer therapy. Emerging therapeutic regimens with encouraging results in the wild-type (WT) KRAS colorectal cancer (CRC) setting include inhibitors of epidermal growth factor receptor (EGFR) and glutaminolysis. Towards predicting clinical outcome, this preclinical study evaluated non-invasive positron emission tomography (PET) with (4S)-4-(3-[18F]fluoropropyl)-L-glutamic acid ([18F]FSPG) in treatment-sensitive and treatment-resistant WT KRAS CRC patient-derived xenografts (PDXs). Tumor-bearing mice were imaged with [18F]FSPG PET before and one week following the initiation of treatment with either EGFR-targeted monoclonal antibody (mAb) therapy, glutaminase inhibitor therapy, or the combination. Primary sclerosing cholangitis (PSC) is a rare autoimmune bile duct disease that is strongly associated with immune-mediated disorders. In this study, we implemented multitrait joint analyses to genome-wide association summary statistics of PSC and numerous clinical and epidemiological traits to estimate the genetic contribution of each trait and genetic correlations between traits and to identify new lead PSC risk-associated loci. We identified seven new loci that have not been previously reported and one new independent lead variant in the previously reported locus. Sepsis is a systemic inflammatory disorder that leads to the dysfunction of multiple organs. In the intestine, the deregulation of the epithelial barrier contributes to the development of sepsis by triggering continuous exposure to harmful factors. However, sepsis-induced epigenetic changes in gene-regulation networks within intestinal epithelial cells (IECs) remain unexplored. In this study, we analyzed the expression profile of microRNAs (miRNAs) in IECs isolated from a mouse model of sepsis generated via cecal slurry injection. Deformability and non-adherence are two fundamental functional properties of red blood cells (RBCs), which allow them to move unimpeded through the microvasculature. In sickle cell disease (SCD), polymerization of abnormal sickle hemoglobin leads to decreased deformability and increased membrane adhesiveness in RBCs. This contributes to abnormal blood vessel occlusion, which is central to the underlying pathophysiology. RBC deformability and adhesiveness are, therefore key hemorheological biomarkers in characterizing disease states as well as in evaluating novel therapeutics. In this Conversations in Urban Education, high school student leader Sofia Dí­az shares her experience with racial and gender inequity and justice while transitioning into high school in the 2020-2021 school year and attending school virtually and in-person during the continued pandemics of Covid-19 and racial injustice. Díaz provides insights on how educational leaders and staff can build relationships and reach out to students and families in order to support student well-being and safety.
