Death: The Great Antagonist In Poe’S “The Cask Of Amontillado”, Majed S. Al-Lehaibi
Death: The Great Antagonist In Poe’S “The Cask Of Amontillado”, Majed S. Al-Lehaibi
Journal of Arts and Social Sciences
Poe’s story “The Cask of Amontillado” has been read as a story of a premeditated crime, of revenge and the protagonist’s ultimate pride in his well-executed crime. Readers are repulsed by the sheer evil of Montresor, the main character of the story. However, this paper argues that this reading barely scratches the surface of the depth and complexity of this story. It argues that the story is told in the form of a confession. The narrator, an elderly Montresor, is full of remorse, and in telling the story, he is seeking absolution. In its deeper meaning, “The Cask of Amontillado” …

Colour Doppler-Guided Superthin Inferior Gluteal Artery Perforator Flap Elevation For Ischial Pressure Ulcer Reconstruction, Muhammed Said Aydin, Gökay Özler, Şükrü Kasap, Mustafa Nişanci
Sultan Qaboos University Medical Journal
Ischial pressure ulcer reconstruction requires durable coverage while preserving local donor options for possible recurrence. The inferior gluteal artery perforator (IGAP) flap is a muscle-sparing option, but superthin elevation requires precise knowledge of the superficial fascial plane and the subcutaneous course of the selected perforator. This technical note describes colour Doppler ultrasonography-guided superficial fascial plane mapping before superthin IGAP flap elevation in a 27-year-old paraplegic patient with a chronic left ischial pressure ulcer who presented in 2025 to a tertiary care university hospital in Muğla, Turkey. Preoperative high-frequency colour Doppler ultrasonography identified the dominant perforator, measured the relevant fascial intervals …

Granite Staters Disapprove Of Trump's Tariffs, Think They Will Increase Cost Of Living 9/3/2026, Unh Survey Center
All UNH Survey Center Polls
Pluralities of New Hampshire residents feel worse off financially than a year ago and expect to be doing about the same a year from now. More than three-quarters of Granite Staters say their cost of living has increased in the past year, especially gasoline, groceries, and energy. More than three-quarters expect tariffs to cause prices to increase and a majority disapprove of President Trump's use of them so far. Overall, Granite Staters are largely pessimistic about the wider national economy, with a majority expecting a recession within the next year, but are more ambivalent about the New Hampshire economy.

Somatic-type malignant transformation (STM) is a rare but aggressive complication of nonseminomatous germ cell tumors (NSGCTs), most commonly arising from the teratomatous component and frequently occurring in metastatic sites. We report the case of a man in his 20s with a history of mixed NSGCT (50% yolk sac tumor, 40% teratoma, and 10% embryonal carcinoma) treated with left orchiectomy and chemotherapy in 2023, who initially responded to treatment but was subsequently lost to follow-up. He presented with one week of progressive dyspnea and pleuritic left-sided chest pain. Serum alpha-fetoprotein was markedly elevated at 2,966.7 ng/mL. Computed tomography demonstrated a progressive …

Strategies That Black American Female Business Owners Use To Secure Government Contracts, Darrilyn Marie Brittian
Walden Dissertations and Doctoral Studies
Black American female business owners remain underrepresented in government contracting despite the availability of federal, state, and local procurement opportunities. Some Black American female business owners lack effective strategies to secure government contracts, which may limit business growth and profitability. Grounded in innovation and entrepreneurship theory, the purpose of this qualitative pragmatic inquiry was to identify and explore the strategies Black American female entrepreneurs use to secure government contracts. The participants were six Black American female entrepreneurs who had successfully secured government contracts and possessed experience navigating government procurement processes. Data were collected using semistructured interviews and organizational documents. Through …
Young Adults’ Perceptions Of Their Life Course Development After Their Parents Were Imprisoned For Violent Crimes, Selena Robinson
Young Adults’ Perceptions Of Their Life Course Development After Their Parents Were Imprisoned For Violent Crimes, Selena Robinson
Walden Dissertations and Doctoral Studies
Parental incarceration is an adverse childhood experience that can influence children’s emotional, educational, relational, and developmental outcomes into adulthood. Although previous research had examined the effects of parental incarceration on children, limited research had explored how young adults perceive their life course development when a parent was incarcerated for a violent crime. The purpose of this generic qualitative study was to explore the perceptions of young adults, age 18–25, regarding their life course development following parental incarceration for a violent crime. Elder’s life course development theory was the framework for this study. Data were collected through semistructured interviews with three …

BACKGROUND AND AIMS: Isolated perianal fistulizing Crohn's disease (IPFCD) is an uncommon presentation of Crohn's disease with an uncertain natural history. We aimed to characterize contemporary diagnostic strategies, management approaches, and long-term outcomes, including progression to luminal Crohn's disease.
METHODS: We conducted a retrospective multicenter cohort study across 25 centers within the ECCO-CONFER network. Adult patients with IPFCD and no evidence of luminal Crohn's disease at baseline were included. The primary outcome was progression to luminal Crohn's disease. Secondary outcomes included clinical and radiological fistula response and patterns of medical and surgical management. Time-to-event analyses were performed using Kaplan-Meier methods. …

Single-Session Robot-Assisted Removal Of A Retained Ureteral Stent With Complex Proximal And Distal Encrustation: A Case Report, Maximo Alvarez, Cole Freedman, Emma Romero, Paul Kim, Alexis Garza, Harras Zaid
School of Medicine Publications
Retained encrusted ureteral stents represent a complex urologic problem, particularly in patients with limited healthcare access. We present a 28-year-old undocumented female with a 10-month retained ureteral stent who developed massive right-sided encrustation, including a 3 cm proximal renal stone near the renal pelvis and a 5-6 cm bladder stone, classified as FECal Grade IV-V. A single-session combined approach consisting of robot-assisted cystolithotomy, intraoperative ureteroscopy with stent exchange, and robot-assisted pyelolithotomy was performed successfully. Intraoperative ureteroscopy between operative phases confirmed ureteral clearance and enabled immediate stent placement without repositioning delay. This case supports the use of combined robotic approaches for …
Social Interactions Between People Living With Dementia And Staff In Health And Social Care Settings: A Scoping Review, Stephanie Janka-Spurlock, Georgia Page, Gary Hodge, Helen Lloyd, Susie Pearce
Social Interactions Between People Living With Dementia And Staff In Health And Social Care Settings: A Scoping Review, Stephanie Janka-Spurlock, Georgia Page, Gary Hodge, Helen Lloyd, Susie Pearce
School of Nursing and Midwifery
Dementia is an umbrella term for progressive conditions affecting memory, thinking, communication and other cognitive functions. These changes can make social interaction challenging, particularly in unfamiliar or overstimulating environments. Although social interactions between staff and people living with dementia are recognised as an aspect of care, existing literature across health and social care settings largely focuses on general care needs and experiences rather than social interaction specifically. Consequently, there is limited understanding of the types of social interactions that occur and the contextual factors influencing them. This scoping review aims to address this gap by mapping existing research on social …

Background: Large language models (LLMs) are increasingly demonstrating the potential to reach human-level performance in generating clinical summaries from patient-clinician conversations. LLMs are usually evaluated against clinical summaries that focus mainly on patients’ biology and not on their biography (eg, preferences, values, wishes, and concerns). To achieve patient-centered care, artificial intelligence clinical summarization must incorporate patient-centered domains, implemented through patient-centered summaries (PCSs). Objective: This study aimed to develop a framework to generate PCS that capture patients’ values, preferences, and wishes while ensuring clinical utility for clinicians, and assess if current open-source LLMs can achieve human-level performance in generating PCS. Methods: …

Monogenic syndromes are associated with neurodevelopmental changes that result in cognitive impairments and neurobehavioral phenotypes, including autism and seizures. Limited studies and resources are available to make meaningful headway into the underlying molecular mechanisms that result in these symptoms. One such example is DeSanto-Shinawi Syndrome (DESSH), a rare disorder caused by pathogenic variants in the

Acquired brain injury (ABI) is the leading cause of adulthood injury-related death and disability worldwide. Despite ABI’s prevalence, little is known about the extent to which social workers are prepared for brain injury working and specifically pre-qualifying brain injury curricula. This study employed a nine-item survey regarding brain injury social work knowledge. The survey asked about curricula inclusion within social work pre-qualification via multiple education providers to understand the extent to which brain injury training is incorporated within UK courses. This included a series of open-ended questions. Data from 36 survey respondents identified that 1) there was an acknowledged gap …

Training For Social Prescribing Link Workers: A Scoping Review, S. Tierney, G. Hanley, H. Napierala, L. Burns, K. Husk
Peninsula Medical School
Background: Social prescribing involves connecting people to community-based resources to address non-medical determinants of health. Central to its delivery are link workers; their effectiveness depends, in part, on the availability and quality of training. Despite rapid expansion of the link worker role on an international scale, training provision for this workforce remains fragmented and poorly mapped. Aim: To identify and describe training programmes for link workers reported in the published and grey literature. Methods: We followed the Joanna Briggs Institute methodology on scoping reviews. Six electronic databases were searched alongside targeted grey literature searching. Sources were included if they described …

BACKGROUND: Bloodstream infections (BSI) are a significant contributor to morbidity and mortality among hospitalized patients. Rapid diagnosis of BSI is critical for timely and appropriate clinical management, but current diagnostic strategies have a prolonged turnaround time or only identify select pathogens with minimal prognostic information.
METHODS: We examined serial serologic samples from 77 intensive care unit (ICU) patients at time points before, during, and after diagnosis of presumptive BSI to identify host-derived protein biomarkers associated with specific organisms and with clinical outcomes in BSI. ICU patients with and without BSI were included. Two candidate biomarkers, tissue metalloproteinase 1 (TIMP-1) and …

BACKGROUND: Point-of-care testing (POCT) has expanded rapidly across healthcare systems, increasing both program scale and operational complexity. However, quantitative benchmarking data describing how POCT programs are structured and supported across institutions remain limited.
METHODS: We conducted a cross-sectional survey to characterize POCT program support, including program scope, staffing, operational practices, governance, and challenges. The survey was distributed electronically through professional networks, and 93 institutional responses were analyzed. Descriptive analyses were performed, and derived metrics, including sites per point-of-care testing coordinator (POCC), were used to evaluate the relationship between program scale and staffing.
RESULTS: POCT programs demonstrated substantial variability in scale, …

Healthcare System Barriers To Somali Refugees In Kenya, Zahra Sharif Hashim
Walden Dissertations and Doctoral Studies
Access to health care is a fundamental right of every refugee, yet gaps in implementing Kenya's Refugees Acts of 2006 and 2021 continue to limit access for Somali refugees in Dadaab camp. Existing research has documented resource constraints, movement restrictions, and other barriers to refugee health care, but how policy implementation gaps shape access in Dadaab has not been well explored. The purpose of this qualitative descriptive study was to understand barriers to health care access for Somali refugees resulting from gaps in implementing Kenya's refugee health policies. The narrative policy framework guided the study. Data from semistructured interviews with …

Strategies General Practitioners Use To Effectively Communicate Patient Cases To Medical Specialists In Saudi Arabia, Alshraefarana Alharthi
Walden Dissertations and Doctoral Studies
Effective communication between general practitioners (GPs) and medical specialists is essential for reducing healthcare costs and improving continuity of care. Healthcare leaders are concerned that ineffective communication between GPs and specialists may contribute to delays in care, fragmented services, and inefficient use of healthcare resources. Grounded in relational coordination theory, the purpose of this qualitative pragmatic inquiry was to explore the effective communication strategies used by six GPs in Saudi Arabia when communicating patient cases to medical specialists to reduce healthcare costs and improve the quality of patient care. The participants were six GPs in Saudi Arabia. Data were collected …
