Journal of Social Work in the Global Community

This study evaluated a short-term individual development account for young workers (SIDAY) with low income in Gwangju Metropolitan City, South Korea. SIDAY used random sampling methods to form an experimental group (n = 191) and a control group (n = 106) out of 2,589 applications. The experimental group received a 100,000 Korean Won (KRW), which is the equivalent to an $85 USD monthly savings match for 10 months, as well as financial capability programs and services, such as online financial mentoring, spending plan consultations, and electronic financial newsletters. Upon completion of SIDAY, the experimental group on average had …

Adjunctive Probiotic Use During Antibiotic Exposure In Very Preterm Infants: A Narrative And Translational Review, Mustafa Törehan Aslan, Mustafa Altındiş, Fahri Ovalı Sep 2026

Adjunctive Probiotic Use During Antibiotic Exposure In Very Preterm Infants: A Narrative And Translational Review, Mustafa Törehan Aslan, Mustafa Altındiş, Fahri Ovalı

Journal of Pediatric Infectious Diseases

Infants born before 32 weeks of gestation are highly susceptible to late-onset sepsis because of immune immaturity, impaired intestinal barrier function, and frequent broad-spectrum antibiotic exposure. Whether probiotics should be continued or initiated during antibiotic therapy remains unresolved and is often conflated with routine prophylaxis in stable infants. This SANRA-guided structured narrative and translational review synthesized clinical, microbiome, resistome, mechanistic, and safety evidence identified in PubMed/MEDLINE, Scopus, and Web of Science through July 1, 2026, across three contexts: stable antibiotic overlap, suspected sepsis, and confirmed or unstable sepsis. Pooled prophylactic analyses suggest a small, heterogeneous reduction in late-onset infection (risk …

Paradoxical Reaction Presenting As Sacroiliitis In Brucellosis: A Case Report And Literature Review, Azer Karaman, Sümeyye Bayram, Hatice Gaber Kurt Sep 2026

Paradoxical Reaction Presenting As Sacroiliitis In Brucellosis: A Case Report And Literature Review, Azer Karaman, Sümeyye Bayram, Hatice Gaber Kurt

Journal of Pediatric Infectious Diseases

Although brucellosis is one of the most common zoonotic infections worldwide, the development of a paradoxical reaction during treatment hasrarely been reported in the literature. We present a 17-year-old patient who developed acute sacroiliitis in the second week of treatment for olecranon bursitis brucellosis, despite her initial clinical findings showing signs of improvement. After excluding other potential etiologies for sacroiliitis, clinical and radiological recovery was achieved by adding anti-inflammatory therapy without modifying the existing antimicrobial regimen. Consequently, the clinical picture was evaluated as a paradoxical reaction. This case emphasizes that new symptoms or clinical worsening during brucellosis treatment should not …

Journal of Pediatric Infectious Diseases

Objective: This study investigated the age-dependent clinical characteristics of Kawasaki disease (KD) in pediatric patients and assessed the influence of age on disease presentation and outcomes. Methods: A retrospective analysis was conducted of 480 children diagnosed with KD who were admitted to Hangzhou First People's Hospital between 2013 and 2022. Subjects were stratified into two cohorts based on age: <2 years and 2-14 years). Demographic data, clinical manifestations, laboratory parameters, comorbidities, and coronary artery abnormalities were comprehensively evaluated across the age groups. Results: Time from symptom onset to diagnosis was significantly prolonged in the older cohort (z=-2.70, P=0.007). The incidence of cervical lymphadenopathy was markedly higher in older children (χ²=15.12, P<0.001), while peripheral extremity changes were significantly less frequent (χ²=5.79, P=0.016). The younger cohort exhibited significantly elevated platelet counts, alanine aminotransferase, and NT-proBNP levels compared to older patients (z=-3.57, P<0.001; z=-2.39, P=0.02; z=-3.59, P<0.001, respectively). Coronary artery dilatation and aneurysm formation occurred more frequently in younger patients (z=7.70, P=0.006; z=14.63, P<0.001). However, coronary artery recovery rates following intravenous immunoglobulin therapy showed no significant inter-group differences (P>0.05). Conclusions: The clinical presentation of KD demonstrates significant age-dependent variation. Clinicians should maintain a high index of suspicion for KD in older children presenting with fever and cervical lymphadenopathy. In younger patients with KD, close monitoring of hepatic function, platelet …

Computational Design, Molecular Docking, Sar, And Admet Analysis Of Novel Jak1 Inhibitors As A Possible Treatment For Rheumatoid Arthritis, Sarah Ahmed Waheed

Al-Mustaqbal Journal of Pharmaceutical and Medical Sciences

Rheumatoid arthritis is an inflammatory autoimmune disease affecting the joints which eventually leads to systemic comorbidities. Its pathogenesis was attributed to disturbances in the Janus Kinase/Signal Transducer and Activator of Transcription signaling pathway. In this research, an innovative lead compound C6 was created and docked toward Janus Kinase1 (JAK1). The docking process was confirmed by re-docking of the co-crystallized ligand MI1 (Tofacitinib). C6 showed a predicted docking score of -9.17 kcal/mol, with enhancement of 1.26 kcal/mol over Tofacitinib. Evaluation of the interactions showed a framework of five hydrogen bonds within the binding site involving an important contact with Glu966 as …

Tamper-Proof Artificial Intelligence (Ai) Decision Chains, Raghunath Ramesh Kulkarni, Jaswanth D K, Pranetha Priya R

Defensive Publications Series

Autonomous artificial intelligence (AI) agents increasingly perform critical tasks, yet their lack of transparency makes it difficult to verify that a model was authentic and uncompromised when a decision was made and to maintain a trustworthy audit trail. The techniques presented herein implement a Cryptographic Agent Provenance Ledger (CAPL) security architecture that uses hardware-backed enclaves, privacy-preserving zero-knowledge proofs, hardware-rooted state continuity, and in-enclave cryptographic batching to create tamper-resistant, forensic-grade provenance for AI decisions. CAPL gates release of each inference output until its provenance record is created and ordered, while later hardware-backed notarization anchors batches of already-gated records. By cryptographically binding …

Sex-specific penetrance in autosomal-dominant Mendelian conditions is largely understudied. The neurodevelopmental disorder Pilarowski-Bjornsson syndrome (PILBOS) was initially described in females. Here, we describe the clinical and genetic characteristics of the largest PILBOS cohort to date, showing that both sexes can exhibit PILBOS features, although males are overrepresented. A mouse model carrying a human-derived Chd1 missense variant (Chd1R616Q/+) displays female-restricted phenotypes, including growth deficiency, anxiety, and hypotonia. Orchiectomy unmasks a growth-deficiency phenotype in male Chd1R616Q/+ mice, while testosterone rescues the phenotype in females, implicating androgens in phenotype modulation. In the gnomAD and UK Biobank databases, rare missense variants …

Death: The Great Antagonist In Poe’S “The Cask Of Amontillado”, Majed S. Al-Lehaibi

Journal of Arts and Social Sciences

Poe’s story “The Cask of Amontillado” has been read as a story of a premeditated crime, of revenge and the protagonist’s ultimate pride in his well-executed crime. Readers are repulsed by the sheer evil of Montresor, the main character of the story. However, this paper argues that this reading barely scratches the surface of the depth and complexity of this story. It argues that the story is told in the form of a confession. The narrator, an elderly Montresor, is full of remorse, and in telling the story, he is seeking absolution. In its deeper meaning, “The Cask of Amontillado” …

Colour Doppler-Guided Superthin Inferior Gluteal Artery Perforator Flap Elevation For Ischial Pressure Ulcer Reconstruction, Muhammed Said Aydin, Gökay Özler, Şükrü Kasap, Mustafa Nişanci

Sultan Qaboos University Medical Journal

Ischial pressure ulcer reconstruction requires durable coverage while preserving local donor options for possible recurrence. The inferior gluteal artery perforator (IGAP) flap is a muscle-sparing option, but superthin elevation requires precise knowledge of the superficial fascial plane and the subcutaneous course of the selected perforator. This technical note describes colour Doppler ultrasonography-guided superficial fascial plane mapping before superthin IGAP flap elevation in a 27-year-old paraplegic patient with a chronic left ischial pressure ulcer who presented in 2025 to a tertiary care university hospital in Muğla, Turkey. Preoperative high-frequency colour Doppler ultrasonography identified the dominant perforator, measured the relevant fascial intervals …
