Introduction: Mucinous neoplasm of the anus is an uncommon malignancy, representing less than 3% of all anal cancers. Even more rare is extra-appendiceal mucinous neoplasm. As a result, limited literature exists to define diagnostic strategies and optimal management. Our case report details the presentation and management of a patient with recurrent mucinous neoplasm of the anus. Methods: A retrospective chart review was performed which includes clinic documentation., operative notes, pathology reports. Results: The patient is a 76-year-old male with a history of T3N0M0 anal mucinous carcinoma diagnosed in 2015 and treated with neoadjuvant chemoradiation with complete pathological response. Postoperative delirium (POD) is a frequent and serious complication associated with increased morbidity and mortality. Although predictive models for POD exist, they are rarely implemented in routine clinical practice, partly due to their complexity and data requirements. Simple and interpretable tools based on routinely available data may improve clinical applicability. In this retrospective cohort study, we evaluated Fast-and-Frugal Trees (FFTrees) for predicting POD using routine electronic health record data from 61,150 surgical patients treated between 2017 and 2020 at a large German university hospital. Neonatal hypoxic ischemic encephalopathy (HIE) is a common birth complication that can cause death or lifelong disabling conditions like cerebral palsy, epilepsy, and autism. It is well established that maternal infection and inflammation are significant risk factors for HIE but reasons for this increase in neurological risk to the offspring remain unknown. BACKGROUND: Cystic hydatid infection is a parasitic disease caused by the larval stages of Echinococcus granulosus, most commonly involving the liver and lungs. Cardiac involvement is rare, accounting for less than 2% of cases, with the left ventricle being the most commonly affected site. Involvement of the right ventricle is extremely uncommon and may lead to life-threatening complications such as tricuspid valve obstruction and hemodynamic compromise.

CASE PRESENTATION: We report a rare case of a 42-year-old patient who presented to the emergency department with exertional dyspnea, hemoptysis, chest pain, syncope, peripheral edema, and signs of right-sided heart failure. The patient …

Statement of the Problem: School board members maintain influential power in public school systems, yet they face political turmoil, community backlash, and role ambiguity (Plough, 2014). While this stress has increased in recent years, little to no research has been conducted on them (Diem et al., 2015).

Purpose of the Study: The purpose of this study was to explore P-12 school board member burnout.

Sport for Development and Peace (SDP) has emerged as a global concept for advancing humanitarian, peacebuilding and social benefits, particularly in disadvantaged communities and conflict-affected regions of the world. Yet, its promise is often entangled with the broader contradictions of international aid, which in the Global South often shifts from solidarity to systems shaped by power asymmetries, geopolitics and self-interests. Drawing on critical aid theory and decolonial thought, foregrounded in the Palestine context, we argue that SDP offers potential benefits but remains constrained by the very systemic issues it purports to redress. Introduction: Throughout the pandemic, COVID-19 vaccination coverage was lower for the general population living in rural counties than in urban counties. In Tennessee (TN), COVID-19 vaccinations became available to pregnant women in March 2021. Little is known about rates of COVID-19 vaccination in pregnancy during the pandemic, or whether these rates changed or differed by urbanization level.

Introduction: Diabetes is a top ten Rural Healthy People 2030 priority for health improvement among rural Americans. Socioeconomic, environmental, geographic and political barriers contribute to the type 2 diabetes (T2D) prevalence and its disparate impact in rural communities. However, promoting proper self-care behaviors via self-management education can help people with diabetes better manage their condition.

Introduction: After making landfall on the Florida Gulf Coast in September 2024, Hurricane Helene impacted Western North Carolina (WNC) with historic flooding, landslides, and hurricane-force winds. More than 100 deaths in the region were attributed to the storm’s direct impacts.

Introduction: Infective endocarditis (IE) hospitalization rates have increased significantly in the last decade, primarily due to the opioid epidemic. In 2016, Kentucky (KY) ranked among the top ten states for opioid-related overdose deaths, but limited data exist on IE trends in KY.

Purpose: This study seeks to examine county-level spatial and temporal trends in IE hospitalization rates in KY.

Methods: Hospital inpatient discharge data from 2008–2018 for KY residents aged 18 and older were analyzed. IE admissions and drug use status were identified using ICD-9 & 10 diagnosis codes. U.S. Census data calculated yearly IE hospitalization rates per 100,000 residents. …

Introduction: Hurricane Helene brought widespread damage to Southern Appalachia, disrupting utilities, health care, and recreation. While natural disasters are linked to declines in mental and physical health, few studies have examined their effects on distinct types of physical activity (PA).

Appalachia’s opioid crisis reflects uneven burdens, especially in distressed coalfield counties, and it cannot be solved by symptom tracking alone. This commentary argues for a narrative shift, starting with story and cultural repair alongside clinical care. Narrative collapse is defined here as the loss of shared, dependable public meaning across groups, not the absence of storytelling in families, churches, and recovery circles. Trauma science suggests that chronic threat reshapes nervous systems across generations, making safety a prerequisite for renewed trust. Peter Singer’s preference utilitarian defense of selective infant replacement in cases of severe disability remains one of the most controversial arguments in contemporary bioethics. The core of Singer’s position relies on the claim that replacing a severely disabled infant with a non-disabled infant increases overall preference satisfaction and is therefore morally permissible. In this paper, I argue that Singer’s argument fails to justify this conclusion. Singer’s replacement premise requires a utilitarian calculus that depends on reliable predictions about future preference satisfaction. Language learning models (LLMs), using the practice of scraping, allow large systems to train using the copying of copyright protected work. Are the objective standards in medical malpractice law, including reasonableness and professional norms, neutral in a general manner or do they encode gendered assumptions that are disadvantageous to women in the system? Women are 66% more likely to encounter the disparity of misdiagnoses than men. Fentanyl overdose deaths are on the rise from both intentional use and accidental exposure. Our work focuses on identifying additional treatment options to Narcan, an opioid receptor antagonist which reverses overdose effects by receptor competition. One alternative method, that has been approved by the FDA, is the development of monoclonal antibodies (mAbs). This therapeutic method works by antibodies binding fentanyl in the bloodstream and preventing it from crossing the blood-brain barrier. Our group has begun identifying mAbs by vaccinating mice with a fentanyl hapten (F1) vaccine adjuvanted with a TLR 7/8 agonist. Vaccination leads to the production of antibodies that …

Trade-offs between offspring quantity and quality are fundamental to life-history variation among species and may lead to fitness variation within populations of organisms. The purpose of this research is to explore the relationship between clutch size and egg volume in black brant (Branta bernicla nigricans) geese while controlling for age and quality of the parents to accurately gauge this relationship. Pharmacogenetics To Enhance Personalized Proton Pump Inhibitor Therapy In Montana, Shivika Agrawal, Jon Graham, Shayna R. Killam

Purpose:

Cytochrome P450 2C19 (CYP2C19) is the primary drug-metabolizing enzyme for most proton pump inhibitors (PPIs), commonly prescribed for gastrointestinal disorders such as heartburn. Guidelines developed by the Clinical Pharmacogenetics Implementation Consortium (CPIC) indicate that genetic variants in CYP2C19 may influence PPI response and the likelihood of side effects, and they provide drug and dose recommendations. We systematically evaluated PPI prescribing patterns in Montana’s patient population to inform implementation strategies for pharmacogenetic (PGx)-guided PPI therapy.

Speech-language pathologists strive for patient-centered care, yet clinicians often face limitations when making collaborative decisions with children. Understanding a child’s unique emotional experience is vital for developing effective treatment plans for speech sound disorders; however, traditional assessments rely heavily on the perspectives of parents and teachers. This leaves the child’s internal world largely undiscovered. A Distributed IoT Cyber-Physical System for Non-Invasive Acoustic Mineral Extraction via Z-Scanning Phased Arrays

Abstract: Traditional placer mineral extraction relies on energy demanding mechanical mobilization and high-water usage. This research introduces a novel Cyber-Physical System (CPS) that achieves non-invasive vertical migration of high-density within non-Newtonian slurries using low power usage for targeted depths. Unlike conventional vibration which induces segregation (sinking), this system utilizes a floating, waterproof phased acoustic array to generate a Z-Scanning Helical Wavefront carrying Orbital Angular Momentum. Historians, anthropologists, and journalists have all claimed that Haitian dictator Francois Duvalier modeled his personal style after the Vodou spirit, or Loa, Baron Samedi. Guarding the crossroads between life and death, Baron Samedi is a member of the Gede family, a group of vulgar and mischievous spirits who mock authority. The Baron wears a top hat, dark suits, and sunglasses with one lens popped out. Duvalier’s signature look also included a black suit and hat which he paired with his thick lens glasses.
