Ai Mistakes: "Confabulation" And Abduction, Not "Hallucination", Stephen M. Mcjohn, Ian Mcjohn

"Hallucination" has become the common term for errors by AI systems, yet it implies a misleading analogy to human perception. LLM's process tokens. They do not have conscious experience or conscious perception. A hallucination is an experience, and (to our knowledge, to date) LLM's do not experience anything. This paper suggests that legal scholars follow the lead of a small number of AI researchers who have suggested that "confabulation" is a more accurate term, a metaphor grounded in psychology. Invasive plants have several harmful impacts on the agroecosystem including modifying habitats, affecting food web characteristics, and affecting the ecosystem processes and functioning. This study aims to evaluate the allelopathic potential of the invasive plant C. procera and to assess its effect on seed germination and seedling growth of two common crop plants (barley and alfalfa). In a complete randomized design with three replications, the effect of different concentrations of the aqueous extract of the C. procera on the germination and seedling growth of the two crop plants was investigated. Background: Malignancy is the second most common cause of pericardial effusion, typically occurring in advanced stages. Lymphomas account for a minority of malignant effusions, with symptomatic pericardial disease as their initial presentation being exceptionally rare. Case: A 64-year-old man with chronic obstructive pulmonary disease presented with two weeks of progressive dyspnea, orthopnea, and productive cough. Chest radiograph showed an enlarged cardiac silhouette and bibasilar opacities. Computed tomography of the chest revealed a complex circumferential pericardial effusion up to 3.6 cm, confirmed on transthoracic echocardiography (TTE) with diastolic underfilling but no tamponade physiology. The sustainability of global palm oil production faces significant threats from a range of diseases and infections affecting palm oil crops. This study presents a comprehensive bibliometric analysis to map the scientific landscape and evolution of research related to palm oil diseases over the past decade. Using data from the Scopus database, 366 relevant documents were analyzed using R and VOSviewer to uncover trends in publication volume, citation impact, prolific authors, country contributions, and keyword co-occurrence networks. This study employs the fuzzy analytic hierarchy process (FAHP) to identify the critical factors and their degree of relevance to the vehicle emission process. Its innovation lies in the potential to blend ambiguity and uncertainty with the established AHP. FAHP transforms information into a defuzzification state through signal-to-noise ratios, normalization, and pairwise comparison. The principal parameters considered are revenue, sold packing units, CAGR, packing materials, consumption, and CO2 emissions (A, B, C, D, E, and F, respectively). Purpose

This quality improvement (QI) project aimed to reduce uncaptured INR results by 5% over 12 weeks in an outpatient cardiovascular clinic by implementing a standardized tracking workflow.

Background

Suboptimal anticoagulation management in outpatient settings can lead to uncaptured International Normalized Ratio (INR) results, delayed therapy adjustments, and increased risk of bleeding or thromboembolic events.

Methodology

Guided by the Plan-Do-Study-Act (PDSA) framework, a pre–post intervention design was used. Interventions included an electronic medical record (EMR)-based tracking system using a reminders inbox for overdue results and nurse-led follow-up phone calls. Background: Trauma is ubiquitous within society; it is experienced by people who access healthcare, and their care providers, including nursing students who experience high rates of adverse childhood experiences (Gilroy et al., 2024). Although trauma-informed care (TIC) is an established approach that addresses the widespread health effects of trauma (Wathen et al., 2021), its systematic integration in nursing education curricula is lacking (Pfeiffer & Grabbe, 2022). In January 2025, over 230 employees were rapidly deployed from a Federally Qualified Health Center (FQHC) system to support the medical and social needs of Eaton wildfire evacuees at the Pasadena Convention Center (PCC). Of the thousands of Pasadena and Altadena residents displaced, many of the employees were directly impacted. As environmental disasters are becoming more frequent, organizations can implement measures to reduce the psychological impact by promoting coping strategies to improve mental wellbeing for nursing professionals.1,2

Background: Diabetes mellitus is characterized by impaired glucose regulation, predisposing patients to both hyperglycemia and hypoglycemia. Hypoglycemia, particularly frequent in insulin-treated individuals, remains a serious but underrecognized complication. Remote patient monitoring (RPM) technologies, such as continuous glucose monitors, smartphone apps, and hybrid closed-loop systems with remote monitoring capabilities, have emerged as promising tools to improve glycemic control and prevent hypoglycemia in nonclinical settings. In the aftermath of the Supreme Court’s Dobbs v. Jackson Women’s Health Organization (2022) overturning Roe V. Wade, states have taken divergent paths in regulating access to abortion services. In 2024, Nevada voters overwhelmingly passed Question 6 proposing to codify abortion rights into the state constitution. In November 2026, Question 6 will again be on the ballot for final confirmation. If it passes, these rights will be protected from future legislative action.

Despite Nevadans’ strong support for abortion rights, the state’s dearth of abortion providers limits access to these services. As this lecture explores, both providers and individuals seeking abortions, …

Colorectal cancer (CRC) remains the second deadliest and fourth most common cancer in the world. The metastatic form of CRC is especially devastating due to a lack of effective and safer therapeutic options. Another challenge includes an increasing number of younger patients. Among critical signaling pathways overactivated in CRC, mTORC2 plays a key role in the progression of metastatic CRC, yet there is no selective mTORC2 inhibitor in the clinic. Interestingly, a natural plant alkaloid from the Veratrum genus, Veratridine (VTD), shows promise as VTD can upregulate UBXN2A, a colon-specific tumor suppressor protein.
