Dysmetabolic states, such as type 2 diabetes (T2D), characterized by insulin resistance (IR), are associated with fatty liver, increased cardiovascular disease (CVD) risk, and decreased functional exercise capacity (FEC). Rosiglitazone (RO) improves exercise capacity and IR in T2D. However, the effects of RO on FEC and other markers of CVD risk in prediabetes are unknown. We hypothesized that insulin sensitization with RO would improve exercise capacity and markers of CVD risk in participants with impaired glucose tolerance (IGT). This study investigated cardiac stress and mitochondrial oxidative phosphorylation (OxPhos) in human donation after circulatory death (DCD) hearts regarding warm ischemic time (WIT) and subsequent cold storage and compared them with that of human brain death donor (DBD) hearts. A total of 24 human hearts were procured for the research study-6 in the DBD group and 18 in the DCD group. DCD group was divided into three groups (n = 6) based on different WITs (20, 40, and 60 min). All hearts received del Nido cardioplegia before being placed in normal saline cold storage for 6 h. Single-cell RNA-sequencing (scRNA-seq) has emerged as a powerful technique to identify novel cell markers, developmental trajectories, and transcriptional changes during cell differentiation and disease onset and progression. In this review, we highlight recent scRNA-seq studies of the gastric corpus in both human and murine systems that have provided insight into gastric organogenesis, identified novel markers for the various gastric lineages during development and in adults, and revealed transcriptional changes during regeneration and tumorigenesis. Overall, by elucidating transcriptional states and fluctuations at the cellular level in healthy and disease contexts, scRNA-seq may lead to better, more personalized clinical treatments for disease …

Spectrins function together with actin as obligatory subunits of the submembranous cytoskeleton. Spectrins maintain cell shape, resist mechanical forces, and stabilize ion channel and transporter protein complexes through binding to scaffolding proteins. Recently, pathogenic variants of SPTBN4 (β4 spectrin) were reported to cause both neuropathy and myopathy. Although β4 spectrin's role in neurons is mostly understood, its function in skeletal muscle, another excitable tissue subject to large forces, is unknown. Here, using a muscle specific β4 spectrin conditional knockout mouse, we show that β4 spectrin does not contribute to muscle function. In addition, we show β4 spectrin is not present …

Abdominal-based free flaps are the mainstay of autologous breast reconstruction; however, the region may not be ideal for patients with inadequate soft tissue or history of abdominal surgery. This case describes the use of a novel conjoined flap based on the profunda artery perforator and upper gracilis pedicles, named the perforator and upper gracilis (PUG) flap. This flap design aims to maximize medial thigh flap volume while ensuring robust tissue perforation. Here, we present our experience with the PUG flap in a breast cancer patient undergoing autologous reconstruction. Many factors affect patient outcome after congenital heart surgery, including the complexity of the heart disease, pre-operative status, patient specific factors (prematurity, nutritional status and/or presence of comorbid conditions or genetic syndromes), and post-operative residual lesions. The Residual Lesion Score is a novel tool for assessing whether specific residual cardiac lesions after surgery have a measurable impact on outcome. The goal is to understand which residual lesions can be tolerated and which should be addressed prior to leaving the operating room. Background: Artificial intelligence (AI) promises to become an important tool in the practice of laboratory medicine. AI programs are available online that can provide concise medical and laboratory information within seconds after a question is submitted.
Methods: We posed the following contemporary and relevant question for clinical chemists using an AI program: "Should labs eliminate CK-MB testing?" The results of this inquiry were critically reviewed by a cardiologist, emergency department physician, and clinical laboratorians for accuracy and appropriateness.
Results: An AI report answering this question was generated within 15 s. The experts felt that the information was inaccurate with the …

Background: Amyloid, presenting as a mass, is termed amyloidoma. Among the reported cases, fine-needle aspiration (FNA) of amyloid is often misinterpreted as acellular nondiagnostic material.
Methods: A computer search of all FNAs was performed and cases diagnosed as amyloidoma were identified.
Results: Among 11,956 cases and 20,634 FNAs, there were six cases and 12 FNAs of amyloidoma. One case with mucin/myxoid matrix was misinterpreted as amyloid, which on our review was Congo red negative. All five other cases of amyloidoma were adequate for evaluation. The smears showed most of the aspirated contents in the middle of the slide and it …

Chronic low-grade adipose tissue inflammation is associated with metabolic disorders. Inflammation results from the intertwined cross-talks of pro-inflammatory and anti-inflammatory pathways in the immune response of adipose tissue. In addition, adipose FABP4 levels and lipid droplet proteins are involved in systemic and tissue inflammation. Dysregulated adipocytes help infiltrate immune cells derived from bone marrow responsible for producing cytokines and chemokines. When adipose tissue expands in excess, adipocyte exhibits increased secretion of adipokines and is implicated in metabolic disturbances due to the release of free fatty acids. GrimAge acceleration has previously predicted age-related morbidities and mortality. In the current study, we sought to examine how GrimAge is associated with genetic predisposition for systemic inflammation and whether psychosocial factors moderate this association. Military veterans from the National Health and Resilience in Veterans study, which surveyed a nationally representative sample of European American male veterans, provided saliva samples for genotyping (N = 1135). We derived polygenic risk scores (PRS) from the UK Biobank as markers of genetic predisposition to inflammation. Results revealed that PRS for three inflammatory PRS markers—HDL (lower), apolipoprotein B (lower), and gamma-glutamyl transferase (higher)—were associated …

For decades, antimicrobial therapy for Helicobacter pylori infection has been given empirically, and the results of therapy (success or failure) have either not been confirmed or when confirmed have not been used to modify prescribing behavior. These practices coupled with increasing antibiotic resistance have resulted in low cure rates overall. Susceptibility testing for H pylori has increasingly become available, especially in the United States. Availability of susceptibility-based therapy has encouraged adoption of the principles of antimicrobial stewardship for H pylori infection (eg, limiting antibiotic choice to antibiotics for which the infection is susceptible given at optimal doses, formulations, frequency of …

The conventional design cycle in human–computer interaction faces significant challenges when applied to users in isolated settings, such as astronauts in extreme environments. Challenges include obtaining user feedback and effectively tracking human–software/human–human dynamics during system interactions. This study addresses these issues by exploring the potential of remote conversation analysis to validate the usability of collaborative technology, supplemented with a traditional post hoc survey approach. Specifically, we evaluate an integrated timeline software tool used in NASA’s Human Exploration Research Analog. Khalifa bin Salman Port (KBSP), a key pillar in Bahrain's maritime infrastructure, is the focal point of this study, highlighting the significant role of predictive analytics in optimizing port operations. This thesis analyzes container throughput data from 2017 to 2022, provided by Bahrain's Ministry of Transportation database. This data forms the basis for forecasting the 2023 throughput. This thesis investigates the USMEPCOM’s issue of modeling and engineering Business Intelligence data centralized around the MEPS of Excellence (MOE) program. MEPS around the US conduct military personnel in-processing and in doing so have a vested interest in the standardization and application of the data associated with such processes. There are currently 65 MEPS stations and one RPS that handle military personnel onboarding paperwork and make determinations for military eligibility. This topic is important due to the MEPS cloud data processing system modernization efforts requiring data processing adaptation to ensure relevant and meaningful usage of current data.

Federated Learning (FL) is a recent framework of machine learning implementation that trains models on a distributed network of clients as opposed to housing and analyzing this data centrally. This has data communication and practical data privacy advantages, the latter of which is particularly attractive to the medical community where patient privacy is closely safeguarded. We apply FL to a family of sparse linear integer models called Medical Scoring Systems (MSSs). This study utilizes survival analysis for examining the effect of Contractor Logistics Support (CLS) on the reliability of military aircraft before and after implementing CLS. The provider of CLS in this study is the original equipment manufacturer that designed and produced the target aircraft of this study, the Embraer A-29 Super Tucano.

This research examines the projected 2033 applicant processing scenario considering the digital modernization efforts of the United States Military Entrance Processing Command (USMEPCOM). The study evaluates the necessary modifications to current processes, with a particular focus on the influence of two key information technology systems, the MEPCOM Integrated Resource System (MIRS) 1.1 and the Military Health System (MHS) Genesis, on manpower at a Military Entrance Processing Station (MEPS). In doing so, the study establishes baseline processing metrics for assessing these impacts. This thesis explores the adoption of Value-Focused Thinking (VFT) in enhancing the Knowledge Management (KM) program at USMEPCOM, aiming to align decision-making with the organization’s values and goals. Through evaluating the current knowledge flow and policy drafts, it proposes categorizing command messages, establishing a centralized information repository, and scheduling a daily order release to improve information accessibility and operational readiness. Although no alternative offers a perfect solution, implementing Command Message Categorization is expected to significantly enhance operational efficiency and prepare USMEPCOM for future challenges.

The United States Air Force (USAF) and municipal utilities rely on asset management processes to maximize utility of infrastructure assets while decreasing maintenance and repair costs. One of the core tenants of asset management is assessing the condition of the asset, which for water distribution pipes is challenging and costly to obtain. Tert-butanol (TBA), a versatile chemical widely used in industrial processes, poses exposure risks through inhalation, ingestion, and skin contact. Environmental contamination, often from industrial activities, emphasizes the importance of robust waste management and monitoring to protect water supplies from potential TBA migration and ensure drinking water safety. This study employed hydrogen peroxide (H2O2): TBA molar ratios of 100, 200, 400, and 500:1 in a Continuous Flow Stirred-Tank Reactor (CSTR) with UV-LED as the TBA degradation mechanism in an Advanced Oxidation Process (AOP). This retrospective cohort epidemiological study investigated the relative risks of hearing loss associated with ototoxicants in combination with noise exposure. Utilizing Department of Defense (DoD) industrial hygiene and hearing conservation data, this research expanded on a 2020 thesis conducted on Tinker Air Force Base (AFB), applying a similar methodology to Hill AFB, adding 893 evaluated individuals. Grouped into twelve exposure combinations with a minimum of three years of exposure duration, the study assessed various hearing loss indicators, including DoD Standard Threshold Shift (STS) and NIOSH STS. Ototoxic substances consistently elevated relative risk (RR) compared to noise-only exposure groups, but none …
