Purpose: The PI3K pathway is frequently altered in triple-negative breast cancer (TNBC). Limited cell line and human data suggest that TNBC tumors characterized as mesenchymal (M) and luminal androgen receptor (LAR) subtypes have increased incidence of alterations in the PI3K pathway. The impact of PI3K pathway alterations across TNBC subtypes is poorly understood.
Methods: Pretreatment tumor was evaluated from operable TNBC patients enrolled on a clinical trial of neoadjuvant therapy (NAT; A Robust TNBC Evaluation fraMework to Improve Survival [ClinicalTrials.gov identifier: NCT02276443]). OBJECTIVE: The Supreme Court of the United States’ decision in Dobbs v. Jackson Women’s Health Organization in June 2022 overturned Roe v. Wade and ended federal protection of abortion rights. Given the drastic policy changes as a result of the ruling and high exposure to media related to abortion, women opposed to the decision may have experienced distress, which could trigger maladaptive coping strategies, such as alcohol use. Endovascular thrombectomy (EVT) is one of the most effective therapies for acute ischemic stroke attributable to large-vessel occlusion but, despite successful treatment, there remains a significant number of patients with disability. The phenomenon of incomplete microcirculatory reperfusion or no reflow is thought to underlie a substantial proportion of cases with unfavorable outcome. This phenomenon likely arises from platelet aggregation and endothelial edema impacting the cerebral microvasculature, vessels that are below the resolution of digital subtraction angiography. Although incomplete microcirculatory reperfusion prevents tissue recovery and poses a significant clinical challenge, there are multiple therapeutic options administered early after recanalization that have …

Background: Adults with PKU have difficulty maintaining plasma phenylalanine (Phe) in the range that is safe for neurologic function. Elevated plasma Phe is a risk factor for congenital anomalies and developmental delay in offspring resulting from pregnancies with poor Phe control in women with PKU. Enzyme supplementation with pegvaliase allows adults with PKU to eat an unrestricted diet and have plasma Phe levels in a safe range for pregnancy but pegvaliase has not been approved for use in pregnant females with PKU. In contrast to the canonical view that genomes cycle only between haploid and diploid states, many eukaryotes have dynamic genomes that change content throughout an individual’s life cycle. However, the few detailed studies of microeukaryotic life cycles render our understanding of eukaryotic genome dynamism incomplete. Foraminifera (Rhizaria) are an ecologically important, yet understudied, clade of microbial eukaryotes with complex life cycles that include changes in ploidy and genome organization. Prehospital care currently finds itself in a critical crisis as the number of emergency medical technicians continues to decline nationwide. This is especially alarming as such shortages have been exacerbated in the wake of the COVID-19 pandemic. Emergency medical service agencies are looking to educational institutions to ensure that an adequate number of providers are entering the field. Objective: Acute limb ischemia (ALI) is associated with high rates of amputation and consequent morbidity and mortality. The objective of this study is to report on the safety and efficacy of aspiration thrombectomy using the Indigo Aspiration System in patients with lower extremity (LE) ALI.
Methods: The STRIDE study was an international, multicenter, prospective, study that enrolled 119 participants presenting with LE-ALI. Patients were treated firstline with mechanical thrombectomy using the Indigo Aspiration System, before stenting or angioplasty, or other therapies as determined by treating physician. The primary end point was target limb salvage at 30 days after the procedure. …

Definitely, image processing operations without advanced and expensive microprocessors consume more time, power, and larger programs. So, improving the reasonable cost of microprocessors is crucial in this situation. This paper proposes an improvement for the MIPS_32 architecture that is called a Customized MIPS_32 (CMIPS_32) to enhance the capabilities of image processing (IP) operations. Jellyfish are a ubiquitous occurrence in the world’s oceans. During the past decades, dramatic spatial increases and temporal shifts in jellyfish distributions have been reported around the world. iPydisp is an open-source, Python-based tool designed to model the spatio-temporal dispersal of insect species such as parasitoids. It provides an accessible interface for interactive spatio-temporal analysis, using cellular automata algorithms for easy data processing and updates. The tool's main features include intuitive data loading, easy setting of dispersal constraints, and dynamic visualization of dispersal patterns. iPydisp has the potential to improve our understanding of parasitoid dispersal and enhance pest management strategies.

Engineers create technologies but do not sufficiently care for their biased and inequitable outcomes despite the engineering profession’s statutory mandate to protect the public interest. The engineering and legal professions’ similar admission processes and statutory provisions mandating the protection of public interest may justify applying the Supreme Court of Canada’s interpretation of the legal profession’s public interest mandate in the Trinity Western University decisions to the engineering profession. The Supreme Court’s interpretation centres on equal access to the profession, diversity within its membership, and protection of 2SLGBTQ+ persons. Reducing micronutrient malnutrition (“hidden hunger”) in low-income countries is a global challenge, particularly among women, children, and high-poverty households. Countries like Tanzania have developed diverse strategies to combat malnutrition, including the biofortification of staple foods. However, broad awareness and knowledge of micronutrient deficiencies and beneficial foods are needed for these strategies to be effective. The objectives of this study were to (i) examine Tanzanian consumers' initial awareness and knowledge of deficiencies for four micronutrients and associated biofortified foods, and (ii) to examine the effectiveness of targeted communication approaches (i.e., information and branding) to improve knowledge. This study investigates purity expectations directed towards girls and women in the Church of Jesus Christ of Latter-day Saints (LDS), approaching a largely underresearched topic and establishing findings such as the mutual embeddedness of purity and gender-based violence for girls and women in the LDS Church. Through a discourse analysis, this study creates a historical timeline of over 100 years of documents, including church-produced media as well as separate writings by LDS women, focusing on how purity has changed and has been politicized over time within various historical moments. Heart failure (HF) is a complex syndrome traditionally classified by left ventricular ejection fraction (LVEF) cutpoints. Although LVEF is prognostic for risk of events and predictive of response to some HF therapies, LVEF is a continuous variable and cutpoints are arbitrary, often based on historical clinical trial enrichment decisions rather than physiology. Holistic evaluation of the treatment effects for therapies throughout the LVEF range suggests the standard categorization paradigm for HF merits modification. The multidisciplinary Heart Failure Collaboratory reviewed data from large-scale HF clinical trials and found that many HF therapies have demonstrated therapeutic benefit across a large range of …

Objective: To evaluate the screening performance characteristics of existing tools for the diagnosis of sepsis during delivery admissions.
Methods: This was a case-control study using electronic health record data, including vital signs and laboratory results, for all delivery admissions of patients with sepsis from 59 nationally distributed hospitals. Patients with sepsis were matched by gestational age at delivery in a 1:4 ratio with patients without sepsis to create a comparison group. Patients with chorioamnionitis and sepsis were compared with a complete cohort of patients with chorioamnionitis without sepsis.
