“Dead Or Alive?” Regression Imputation Of The Binary End-Of-Event Outcome Indicator For The Nemsis Public Research Dataset, Mary E. Helander

Social Science - All Scholarship

Objectives: Recent studies address missing survival outcomes in the National Emergency Medical Services Information System (NEMSIS), a system that stores and shares standardized EMS data from U.S. States and Territories. This study aspired to improve imputed EMS outcome accuracy and establish benchmarks for common medical emergencies treated in prehospital care. Methods: This retrospective cohort study of ground-transported EMS patients with definitive end-of-EMS-event outcomes, based on dataset years 2017 to 2023, predicted the probability of “dead” or “alive” status using logistic regression. K-fold cross-validation assessed over-fitting and internal generalization. Threshold optimization and machine learning classification metrics enabled comparison with a …

Why Is The Mycobacterium Tuberculosis Beijing Genotype So Prevalent In Colón, Panama? Uncovering The Risk Factors Of A Dangerous Strain, Daniela Candanedo Aug 2024

Why Is The Mycobacterium Tuberculosis Beijing Genotype So Prevalent In Colón, Panama? Uncovering The Risk Factors Of A Dangerous Strain, Daniela Candanedo

International Programs

Tuberculosis (TB) remains one of the leading causes of morbidity and mortality worldwide (WHO, 2021). In Panama, TB incidence is 45.3 per 100,000 population, affecting underdeveloped areas like the Colón province (MINSA, 2023). The Beijing genotype (Lineage 2), known for its high transmissibility and multidrug resistance, poses a significant challenge (Pérez-Lago et al., 2019). A novel molecular surveillance method using allele-specific oligonucleotide PCR (ASO-PCR) was developed to identify Beijing, Haarlem and LAM genotypes in Colón, Panama (Domínguez et al., 2019). This technique detected 44.1% of Beijing strain and <5% of Haarlem and LAM strains between 2018-2020 (Acosta et al., 2020). This method was also applied between 2021-2022, identifying 29.1% of Beijing strain (Candanedo et al., 2023). This study aims to identify and analyze the epidemiological risk factors associated with the Beijing strain infection in Colón, Panama between 2025-2026. Demographic, clinical, and geographic variables will be assessed to better understand the infection and transmission dynamics. The findings will provide critical information to enhance TB control and prevention strategies in Colón, influence health policy formulation, and optimize resource allocation.

Breaking The Silence: Addressing Stigma On Seeking Professional Mental Health Care In Rwanda Through Effective Communication, Anitha Dushimirimana Aug 2024

Breaking The Silence: Addressing Stigma On Seeking Professional Mental Health Care In Rwanda Through Effective Communication, Anitha Dushimirimana

International Programs

Mental health stigma remains a significant barrier to accessing and usage of mental health services globally, and Rwanda is no exception. According to the Rwanda Ministry of Health, one in five people between the ages of 14-65 in Rwanda has at least one mental health issue, with depression being the most common (Rwanda Ministry of Health, 2018). Despite the Ministry of Health's efforts to decentralize mental health services at community and health center levels, stigma and prejudices hinder their utilization (Mukamana et al., 2019). “Overall, 61.7% had awareness of mental health services while only 5.3% reported to have used existing …

Impact Of A Public-Private Interface Agency For Tuberculosis Reporting During Covid-19 Pandemic In Vietnam, Lan Anh Hoang Aug 2024

Impact Of A Public-Private Interface Agency For Tuberculosis Reporting During Covid-19 Pandemic In Vietnam, Lan Anh Hoang

International Programs

Background: Viet Nam’s second prevalence survey uncovered a higher than expected burden of TB. Studies have indicated that people with TB who are ‘missed’ by existing services are equally split between those seeking care in the private sector and those not accessing healthcare. Intervention: We established a public-private interface agency (PPIA) to engage private doctors for TB treatment reporting in 14 districts across Ho Chi Minh City (HCMC) and Hai Phong, Viet Nam. Treatment data were collected and patients missing sputum results were offered an Xpert MTB/RIF test to increase the rate of bacteriologically-confirmed TB and to identify drug resistance. …

Trends In Clinical Deterioration Events Mortality: A Call For Action, Eliana Lopez-Baron Aug 2024

Trends In Clinical Deterioration Events Mortality: A Call For Action, Eliana Lopez-Baron

International Programs

Children's mortality is still a public health concern. The World Health Organization (WHO) has been attempting to lower the death rate among children under five years old which represented the most important burden of mortality in children. In certain countries these diseases have moved from high mortality rates secondary to under-5 mortality such as infections in neonates, respiratory, and gastrointestinal disorders, to other diseases that are more commonly seen in older children and for which there is an urgent need to identify in order to improve these outcomes. An assessment of clinical deterioration events, which, if left undetected, may result …

Antimicrobial Studies Of Novel Pyrazole And Thiazole-Based Antibacterial Agents, Hansa Raj Kc Aug 2024

Antimicrobial Studies Of Novel Pyrazole And Thiazole-Based Antibacterial Agents, Hansa Raj Kc

Student Theses and Dissertations

In recent years, the rise of antibiotic-resistant bacteria has become a global concern, resulting in fewer effective antibiotics. Several conventional antibacterial agents are ineffective against these, necessitating novel molecules with enhanced efficacy and diverse actions. We developed pyrazole and thiazole-based antibacterial agents and demonstrated that they have significant potency (minimum inhibitory concentration as low as 0.39 µg/mL) against antibiotic-resistant and susceptible bacteria, as well as promising anti-biofilm and anti-persister activity. In vivo toxicity tests on Caenorhabditis elegans showed no significant harm to this model host organism at low compound concentrations. These compounds rescued C. elegans from methicillin-resistant Staphylococcus aureus (MRSA) …

Publications and Research

Osteoclasts are multinucleated bone-resorbing cells, and their formation is tightly regulated to prevent excessive bone loss. However, the mechanisms by which osteoclast formation is restricted remain incompletely determined. Here, we found that sterol regulatory element binding protein 2 (SREBP2) functions as a negative regulator of osteoclast formation and inflammatory bone loss. Cholesterols and SREBP2, a key transcription factor for cholesterol biosynthesis, increased in the late phase of osteoclastogenesis. The ablation of SREBP2 in myeloid cells resulted in increased in vivo and in vitro osteoclastogenesis, leading to low bone mass. Moreover, deletion of SREBP2 accelerated inflammatory bone destruction in murine inflammatory …

Einstein Health Papers

Anti-melanoma differentiation-associated gene 5 antibody (anti-MDA-5 Ab) is associated with amyopathic dermatomyositis (DM). These patients are particularly at high-risk for developing acute and rapidly progressive interstitial lung disease (ILD). Given the lack of muscle-related symptoms, along with its sudden onset and rapid clinical progression, the diagnosis of anti-MDA-5 Ab + ILD represents a challenge for clinicians. Even after the diagnosis is established, prognosis remains dismal owing to a hyperinflammatory state, mimicking cytokine storm, commonly refractory to potent immunosuppressive therapy. Hence, we present an elderly African American man who developed acute and rapidly progressive ILD in the setting of positive anti-MDA5 …

Department of Pharmacology, Physiology, and Cancer Biology Faculty Papers

Kindlin-2 is a cytoskeletal adapter protein that is present in many different cell types. By virtue of its interaction with multiple binding partners, Kindlin-2 intercalates into numerous signaling pathways and cytoskeletal nodes. A specific interaction of Kindlin-2 that is of paramount importance in many cellular responses is its direct binding to the cytoplasmic tails of integrins, an interaction that controls many of the adhesive, migratory and signaling responses mediated by members of the integrin family of cell-surface heterodimers. Kindlin-2 is highly expressed in many cancers and is particularly prominent in prostate cancer cells. CRISPR/cas9 was used as a primary approach …

Osteosarcoma is the most common primary bone malignancy in children and young adults, and it has few treatment options. As a result, there has been little improvement in survival outcomes in the past few decades. The need for models to test novel therapies is especially great in this disease since it is both rare and does not respond to most therapies. To address this, an NCI-funded consortium has characterized and utilized a panel of patient-derived xenograft models of osteosarcoma for drug testing. The exomes, transcriptomes, and copy number landscapes of these models have been presented previously. This study now adds …

Underrepresentation of racial and ethnic subgroups in cancer clinical trials remains a persistent challenge. Restrictive clinical trial eligibility criteria have been shown to exacerbate this problem. We previously identified that up to 24% of patients treated with standard immunochemotherapy would have been excluded from recent first-line trials in diffuse large B-cell lymphoma (DLBCL) based on 5 laboratory-based criteria. These ineligible patients had worse clinical outcomes and increased deaths related to lymphoma progression, suggesting the potential exclusion of patients who could have benefited most from the novel therapies being evaluated. Using data from the prospectively enrolled Lymphoma Epidemiology Outcomes cohort study, …

We investigate JN.1-derived subvariants SLip, FLiRT, and KP.2 for neutralization by antibodies in vaccinated individuals, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-infected patients, or class III monoclonal antibody S309. Compared to JN.1, SLip, KP.2, and especially FLiRT exhibit increased resistance to bivalent-vaccinated and BA.2.86/JN.1-wave convalescent human sera. XBB.1.5 monovalent-vaccinated hamster sera robustly neutralize FLiRT and KP.2 but have reduced efficiency for SLip. All subvariants are resistant to S309 and show decreased infectivity, cell-cell fusion, and spike processing relative to JN.1. Modeling reveals that L455S and F456L in SLip reduce spike binding for ACE2, while R346T in FLiRT and KP.2 strengthens …

BRCA1/2 proteins function in genome stability by promoting repair of double-stranded DNA breaks through homologous recombination and by protecting stalled replication forks from nucleolytic degradation. In BRCA1/2-deficient cancer cells, extensively degraded replication forks can be rescued through distinct fork recovery mechanisms that also promote cell survival. Here, we identified a novel pathway mediated by the E3 ubiquitin ligase RAD18, the E2-conjugating enzyme UBC13, the recombination factor PALB2, the E3 ubiquitin ligase RNF168 and PCNA ubiquitination that promotes fork recovery in BRCA1- but not BRCA2-deficient cells. We show that this pathway does not promote fork recovery by preventing replication fork reversal …

Selective androgen receptor modulators (SARMs) have gained popularity for their alleged ability to selectively target androgen receptors, potentially offering muscle-building benefits with fewer side effects than traditional steroids. However, the safety profile of SARMs, including RAD-140, is not fully understood. This case report presents a 29-year-old male who developed liver injury after taking RAD-140. The patient experienced jaundice and elevated liver enzymes after three months of RAD-140 use. A liver ultrasound revealed hepatic steatosis and a hyperechoic lesion. Symptoms resolved after discontinuing RAD-140. Similar cases of liver injury associated with RAD-140 have been reported, highlighting the potential hepatotoxicity of this …
