Background: This secondary analysis of the CONFIDENCE (combination effect of fInerenone and empagliflozin in participants with chronic kidney disease and type 2 diabetes using a UACR endpoint) trial (REGISTRATION: URL: https://www.clinicaltrials.gov; Unique identifier: NCT05254002) assessed systolic blood pressure (SBP) changes with empagliflozin/finerenone combination therapy versus monotherapy in people with chronic kidney disease and type 2 diabetes across baseline SBP categories. We assessed SBP response predictors and whether early (day 30) SBP changes mediate urinary albumin-to-creatinine ratio reductions.
Methods: Participants (n=800) were stratified by baseline SBP (< 130 mm Hg; ≥130 mm Hg). Outcomes included SBP response (≥10 mm Hg reduction from baseline at day 180) and longitudinal SBP changes, analyzed by linear mixed-effects models and logistic regression. Causal mediation analysis, a secondary exploratory analysis, evaluated the effect of day 30 SBP change on day 180 urinary albumin-to-creatinine ratio reduction.
Results: At baseline, 532 (66%) participants had SBP ≥130 mm Hg; 268 (34%) …

Histone deacetylase inhibitors (HDIs) are approved for treating hematologic cancers and are currently being evaluated in hundreds of clinical trials for various cancers and other diseases, although their mechanisms of action remain poorly understood. Here, our unbiased bioinformatics analyses found that, for most cancer types, expression levels or genetic variants of histone deacetylase (HDACs) do not consistently correlate with carcinogenesis, do not predict cancer patient survival, and do not associate with cellular responses to HDIs. Whole-genome CRISPR library screens did not identify HDACs as genes affecting cellular responses to HDIs. Overexpression of dominant-negative Class I HDACs causes similar protein hyperacetylation …

Functional brain network organisation, measured by functional connectivity (FC), reflects key neurodevelopmental processes for healthy development. Early exposure to adversity, for example undernutrition, affects neurodevelopment, observable via disrupted FC, and leads to poorer outcomes from preschool age onwards. We assessed longitudinally the impact of early growth trajectories on developmental FC in a rural Gambian population from age 5-24 months. To investigate how these early trajectories relate to later childhood outcomes, we assessed cognitive flexibility at 3-5 years. We observed that early physical growth before the fifth month of life drove optimal developmental trajectories of FC that in turn predicted cognitive …

Entre La Lucha: Familismo And Well-Being Of Latino Immigrant Families Navigating Manual Labor And Anti-Immigrant Sentiment, Faviola Robles Saenz
Dissertations and Theses
The United States' history of treatment toward Latino immigrants has been shaped by anti-immigrant sentiment and exploitative labor practices, creating psychological violence that harms their families' well-being. Although previous research has examined anti-immigrant sentiment, labor conditions, and familismo among Latinos, these areas are often studied in isolation, with limited attention to their intersections and how they are experienced across generations. Informed by history, prior research, and my lived experiences, this study qualitatively explored the intersections of anti-immigrant sentiment and parents' manual labor to understand how these contexts shape familismo and well-being among Latino immigrant families. This work was grounded in …

Background/Objectives: Circadian misalignment, including mistimed sleep or eating, is associated with altered glucose metabolism. The importance of eating window timing for time-restricted eating (TRE) is increasingly recognized. This secondary analysis examined associations between meal to sleep timing intervals and glycemic parameters in individuals with obesity across three dietary interventions [TRE, CR: caloric restriction, and UE: unrestricted eating]. Methods: Participants aged 18-65 years with obesity were randomized to a 12-week intervention: TRE (8 h eating window), CR (15% reduction in daily caloric intake), or UE (usual eating habits). CGM and actigraphy were assessed over two weeks at baseline and end-intervention. Mixed …

Necrotizing enterocolitis (NEC) remains one of the most serious gastrointestinal emergencies in preterm infants, and imaging plays a central role in diagnosis and clinical management. Historically, evaluation has relied primarily on abdominal radiography, which remains widely available and embedded in established diagnostic frameworks. However, the hallmark radiographic signs of NEC (i.e., pneumatosis intestinalis, portal venous gas, and free air) reflect relatively advanced manifestations of intestinal injury that indicate established mucosal disruption or transmural necrosis. Bowel ultrasound has increasingly complemented radiography by enabling real-time assessment of bowel wall integrity, perfusion, motility, and intra-abdominal fluid, providing physiologic information that may refine clinical …

Stress-Activated Pathways Mediate Pfas Effects On Human Placental Syncytiotrophoblast Cells., Keisuke Kozai, Kaela Varberg
Manuscripts, Articles, Book Chapters and Other Papers
Per- and polyfluoroalkyl substances (PFAS) are persistent environmental pollutants associated with placenta-mediated pregnancy complications, including preeclampsia, fetal growth restriction, and preterm birth. The syncytiotrophoblast (STB), which forms the placental barrier at the maternal-fetal interface and is directly exposed to maternal blood, is a primary site of PFAS exposure. Although PFAS induce STB apoptosis, the upstream stress-signaling pathways involved remain poorly defined. Here, we investigated stress-responsive signaling mechanisms mediating PFAS-induced STB cell death. STB differentiated from human trophoblast stem cells were exposed to vehicle or an environmentally relevant mixture of 5 PFAS (perfluorooctanoic acid, perfluorooctanesulfonic acid, perfluorohexane sulfonate, perfluorononanoic acid, and …

Light exposure plays an important role in overall health because it entrains circadian rhythms. Recent technological advances in wearable light loggers allow measurement daily light exposure habits. Using a chest-worn light logger, our goal was to (1) develop methodology for differentiating adherent versus non-adherent use, and (2) define differences in lighting intensity in indoor and outdoor environments, to improve data reliability in future clinical studies using this technology. Four testers used a 10-channel chest worn light logging device under different conditions of wear and non-wear (experiment 1), and another tester made measurements with the light logger across a variety of …

Diamond Open Access Journals In India: Status, Sustainability And Challenges, Mallikarjun Dora, Kanagasabai K, Raj Kishor Kampa
FORCE 2026
Open-access publishing has transformed scholarly communication in recent decades. The share of OA articles between 2014 and 2024 increased by 26%, from a modest 14% in 2014 to 40% in 2024. OA journals have evolved over time and exist in various models based on funding mechanisms, including the Gold, Green, Hybrid, and Diamond models. While OA publishing emerged in response to scientific inequality, middle- and low-income countries face the barrier of high Article Processing Charges (APC), which hinder them from publishing in those journals (Alperin, 2022). The Article Processing Charges are part of the Gold and Hybrid OA models, where …

Introduction: Accurate interpretation of serum marker measurements is essential in clinical laboratory practice. Carbohydrate antigen 19-9 (CA 19-9) and carcinoembryonic antigen (CEA) are widely used biomarkers, yet their serum concentrations may be influenced by demographic and non-malignant factors. This study aimed to establish age- and gender-stratified reference intervals (RIs) for a Saudi population and quantify the impact of these local limits on diagnostic specificity and stewardship.
Methods: We retrospectively analysed 1,567 adult laboratory results (aged 15–100) from nationwide diagnostic centres. This study followed the CLSI EP28-A3c guidelines for the indirect estimation of reference intervals. Statistical analysis included Kruskal–Wallis and Mann–Whitney …
