Background: One of the most serious and devastating effects of diabetes mellitus is diabetic nephropathy, which in many cases progresses to end-stage kidney disease (ESKD). Microalbuminuria is currently the gold standard biomarker for DN diagnosis. Microalbuminuria has a poor predictive value, though. Patients occasionally experienced progressive renal failure without microalbuminuria. A plasma protein called transferrin has a different ionic charge from albumin but is otherwise similar in weight. Aim: Our objectives were to evaluate urinary transferrin as a more accurate and earlier marker for the diagnosis of diabetic kidney disease and to investigate its relationship to the excretion of urinary …

Cities and the Environment (CATE)
In urban areas, the implementation of schoolyard facilities (especially through the greening of these spaces) aims to promote children's contact with nature to improve well-being and increase their awareness of environmental preservation. In this study, we use the prospect of schoolyard landscaping in two French schools to conduct research on pupils’ relationship with nature and well-being at school. We explore cognitive and social mechanisms that affect children's perceptions and behaviour in their relationships with nature on a daily basis and in the school context, with a view toward implementing schoolyard designs.
The research aims are (i) to gain a better …
Birth On Mother Earth: Mitigating The Maternal Health Crisis, Jill C. Engle
Birth On Mother Earth: Mitigating The Maternal Health Crisis, Jill C. Engle
Faculty Scholarship
Maternal health outcomes in the U.S. are remarkably poor, especially when compared to those in other industrialized countries. For example, our maternal death rate is more than twice that of Canada’s. Women of color in the U.S. suffer maternal morbidity and mortality at rates considerably higher than white women. Indigenous women experience the worst maternal health outcomes of all. Yet, most maternal deaths and injuries are preventable. Furthermore, rates of maternal harm, including death, are lower when midwives and similar birthing attendants are involved. This Article is the first to fully explore the unique skillset of traditional birth attendants (TBAs). …

With a median caseload of 750–1,000 clients, school nurses face setting-specific challenges involving multiple, competing demands. This necessitates effective prioritization to prevent missed opportunities to provide appropriate care. This integrative review synthesizes the current state of the literature related to prioritization among school nurses in the United States. Three themes were identified relating to prioritization among school nursing: knowledge, attitudes and values; self-efficacy and intention to act; and the nursing process. The prioritization behavior of school nurses can be facilitated by organizational and community collaboration and strong evidence-based practice recommendations. High workloads and inadequate resources among school nurses are the …

Legislators come from a range of backgrounds. Many legislators happen to be lawyers. Parliamentary rules typically allow legislators who are not members of Cabinet to practice a profession part-time. However, the part-time practice of law poses special legal ethics challenges. In this article, we consider the legal ethics issues that arise when a backbench legislator of the governing party practices criminal defence law part-time. We argue that such a dual role engages three serious, unavoidable, and perhaps even unresolvable legal ethics issues. The first issue is the time constraints imposed by outside interests. The second issue is conflicts of interest, …

Sentencing Vulnerability: Conceptualizing The Incorporation Of Personal Characteristics And Experiences At Sentencing, Adelina Iftene
Articles, Book Chapters, & Popular Press
Social inequalities that lead to criminal conduct are often not considered in sentencing; individuals subject to structural inequities receive the same formal sentence as those convicted of like crimes irrespective of the latter’s social advantage. Individual characteristics may also affect how a sentence will be experienced by an individual. While Characteristics and Experience-Sensitive Sentencing (CESS) already exists in various forms and to various degrees in sentencing theory and praxis, a more cohesive, comprehensive, and principled CESS remains wanting. Building on existing approaches, this article locates CESS as a necessary response to equality-based concerns arising from current mainstream sentencing practice. Thus, …

The ERα transcription factor that induces tumor growth is a potential target for breast cancer treatment. Each monomer of the ERα DNA-binding domain (ERαDBD) homodimer has two conserved (Cys)4-type zinc fingers, ZF1 (N-terminal) and ZF2 (C-terminal). Electrophilic agents release Zn2+ by oxidizing the coordinating Cys of the more labile ZF2 to inhibit dimerization and DNA binding. Microsecond-length molecular dynamics (MD) simulations show that greater flexibility of ZF2 in the ERαDBD monomer leaves its Cys more solvent accessible and less shielded from electrophilic attack by sulfur-centered hydrogen bonds than ZF1 which is buried in the protein. In the …

Enteroviruses initiate genomic replication via a highly conserved mechanism that is controlled by an RNA platform, also known as the 5' cloverleaf (5'CL). Here, we present a biophysical analysis of the 5'CL conformation of three enterovirus serotypes under various ionic conditions, utilizing CD spectroscopy, size-exclusion chromatography, and small-angle X-ray scattering. In general, a tendency toward a smaller monomeric hydrodynamic radius in the presence of salts was observed, but the exact structural signature of each 5'CL varied depending upon the serotype. Rhinovirus B14 (RVB14) exhibited at least two monomeric conformations and a low propensity for dimerization, while poliovirus 1 (PV1) showed …

Background and aim: Elderly individuals with diabetes mellitus (DM) are more prone to experiencing common geriatric syndromes, which include impaired cognition, dementia, frailty, falls, fractures, urinary incontinence, and disability. These can significantly impair their quality of life (QoL) and could conflict with anti-diabetic medication. Therefore, we aim to evaluate the impact of controlling diabetes on the overall burden of diabetes in the elderly. Materials and Methods: 100 elderly patients over sixty-five with type 2 diabetes mellitus (T2DM) were selected from Mansoura Specialized Medical Hospital (MSMH) between April 2022 and February 2023. The patients were divided into two groups: fifty diabetic …

Background: Community-acquired pneumonia (CAP) is a major cause of morbidity and mortality worldwide and the role of biomarkers in pneumonia has been evaluated in many clinical areas. Angiopoietin-1 and Angiopoietin -2 are glycoproteins that play role in vascular development, angiogenesis, and permeability. Aim: To conduct a trial for phenotyping and endotyping of pneumonia using Angiopoietins-1 and Angiopoietin -2. Subjects and methods: This case-control study included 30 cases and 29 controls of matched age and sex. to investigate Angiopioetin-1 & -2 serum levels in both groups and then evaluated the possible association between serum Angiopoietin -1&-2 in different pneumonia phenotypes. Results: …
