The older adult population is rapidly increasing, with projections indicating that 21% of the general population will be 65 years or older by 2030. This demographic shift is accompanied by a rise in health co-morbidities such as obesity, heart disease, certain types of cancers, and diabetes, which are exacerbated by inadequate nutrition and decreased physical function. งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับการเข้ารับบริการรักษาพยาบาลเมื่อเจ็บป่วยในสถานพยาบาลและค่าใช้จ่ายที่ผู้ป่วยต้องจ่ายเอง ในช่วงเวลาก่อนและระหว่างที่มีการระบาดของโควิด-19 โดยใช้ข้อมูลจากโครงการสำรวจอนามัยและสวัสดิการ พ.ศ. 2562 (n= 9,634) และ 2564 (n= 7,247) สำหรับการวิเคราะห์ในส่วนที่หนึ่งเป็นการศึกษาปัจจัยที่มีความสัมพันธ์กับการตัดสินใจเลือกของวัยแรงงานเมื่อเจ็บป่วยซึ่งมีสามทางเลือกคือ ไม่ได้รักษา ซื้อยามากินเอง และการไปรักษาที่สถานพยาบาล โดยใช้วิธีการถดถอยโลจิสติก และในส่วนที่สองเป็นการศึกษาปัจจัยที่มีความสัมพันธ์กับค่าใช้จ่ายที่ผู้ป่วยวัยแรงงานต้องจ่ายเอง โดยใช้แบบจำลองโทบิต ผลการศึกษาในส่วนที่หนึ่งด้วยวิธีการถดถอยโลจิสติก กำหนดให้กลุ่มของประชากรที่ป่วยและไม่รักษาเป็นกลุ่มอ้างอิง ผลการศึกษาพบว่า ในช่วงเวลาก่อนและระหว่างที่มีการระบาดของโควิด-19 ประชากรที่มียานพาหนะในครัวเรือน ประชากรกลุ่มผู้สูงอายุตอนปลาย ประชากรที่มีสิทธิ์ข้าราชการที่ใช้ในการรักษา ประชากรที่มีโรคประจำตัว มีความสัมพันธ์กับการซื้อยามากินเองเมื่อเจ็บป่วยเมื่อเทียบกับการไม่รักษา ประชากรที่มียานพาหนะในครัวเรือน การมีโรคประจำตัว ประชากรผู้สูงอายุตอนปลาย มีความสัมพันธ์กับการเข้ารับบริการรักษาพยาบาลเมื่อเจ็บป่วยในสถานพยาบาลเมื่อเทียบกับการไม่รักษา ผลการศึกษาในส่วนที่สองด้วยแบบจำลองโทบิต พบว่า ในช่วงเวลาก่อนและระหว่างที่มีการระบาดของโควิด-19 ประชากรที่อาศัยในภาคเหนือ ผู้มีสิทธิ์รับการรักษาในประกันสังคม สิทธิ์ข้าราชการ รวมถึงผู้ป่วยที่เข้ารับบริการในสถานพยาบาล จะมีค่าใช้จ่ายที่ผู้ป่วยต้องจ่ายเองต่ำกว่า ในขณะที่ปัจจัยพื้นฐานทางเศรษฐกิจของครัวเรือน แสดงให้เห็นว่าผู้ป่วยจากครัวเรือนที่มีรายได้สูง จะมีค่าใช้จ่ายที่ผู้ป่วยต้องจ่ายเองสูงตามด้วย

This dissertation aims to examine the limitations of secular values within the context of all twenty constitutions of the Kingdom of Thailand. The analysis focuses in particular on rights and liberties as core secular values. The study considers their origins, development, and the obstacles that have hindered the realization of rights and liberties in Thai society. The findings suggest that secular values—especially modern concepts of rights and liberties—emerged during the Reformation period under King Rama V, when Siam sought to adjust itself to the conditions of modernity. Biofouling remains a significant challenge in marine industries, as conventional coatings often lose their antifouling efficacy once they become damaged. Incorporating self-healing capabilities into antifouling coatings has therefore emerged as a promising strategy to extend service life and maintain long-term performance. In this study, self-healing polymers were developed based on a bio-derived benzoxazine resin (E-fa), synthesized from renewable precursors including eugenol (a phenol source), furfurylamine (an amine source), and paraformaldehyde, and subsequently blended with poly(ethylene glycol) (PEG). This research explores the Thai television series The Gifted (GMMTV, 2018) as a cultural text that reflects the power dynamics and social inequalities embedded in Thailand’s educational system. Drawing on Michel Foucault’s concept of disciplinary power, the research examines how the series portrays mechanisms of control, compliance, and resistance within a hierarchical school environment. Textual analysis is complemented by a reception study of audience responses collected from YouTube, IMDb, and MyDramaList, revealing how viewers interpret and emotionally engage with the drama’s social implications. Comparative Analysis Of Gut Microbiota In Post-Mortem Chronic Alcoholics Versus Minimal To No Alcohol Users In The Thai Population, Laiba Pervez

Chronic alcohol consumption induces long-term disruption of the gut microbiota, contributing to inflammation, metabolic imbalance, and impaired intestinal integrity. This study investigated 24 postmortem gut microbiota samples from the Thai population, including 12 chronic alcoholic and 12 non-alcoholic cadavers, focusing on cecal and fecal samples to identify site-specific microbial signatures of alcohol-associated dysbiosis. Following DNA extraction, the V3–V4 regions of the 16S rRNA gene were sequenced on the Illumina MiSeq platform. Data were processed in QIIME for OTU clustering and taxonomic profiling. Alpha (Shannon, Chao1, ACE) and beta (Bray–Curtis) diversity were analyzed with PERMANOVA (p< 0.05) while LEfSe, cladogram, and STAMP analyses identified differentially abundant taxa.. At the phylum level, alcoholic cecal samples (AC) showed a decrease in Firmicutes (59.07%) and increases in Proteobacteria (5.54%), Fusobacteria (8.48%), and Actinobacteria (2.78%) compared with controls (NAC). The rise in Actinobacteria was statistically significant. The class Bacilli was also elevated, reflecting a more oxygenated and inflammatory intestinal environment. At the genus level, Paeniclostridium was enriched in alcoholics, while the butyrate-producing Coprococcus was reduced, suggesting impaired SCFA metabolism and gut barrier function. In fecal samples (AF vs NAF), phylum-level differences were minimal, although Clostridium sensu stricto 1 was notably higher in alcoholics, consistent with inflammation-associated dysbiosis. When comparing gut regions, distinct spatial heterogeneity was observed. Within the alcoholic group, Firmicutes were more abundant in cecum, while Actinobacteria were lower compared to feces. Among controls, Actinobacteria and Bacteroidetes were enriched in feces, whereas Firmicutes predominated in the cecum. Alpha diversity analysis using ACE and Chao1 indices showed significant differences between cecal and fecal samples of the control group. Overall, postmortem gut microbiota retained distinct alcoholassociated signatures, with the cecum preserving clearer microbial patterns than feces. These findings highlight the cecum’s potential as a reliable site for detecting alcohol-related dysbiosis in forensics.

Smartphones are gateways to financial, health, and personal data; consequently, mobile authentication must be accurate, fast, privacy-preserving, and scalable. This thesis presents an end-to-end palmprint authentication framework that addresses a practical trilemma: label-efficient learning, on-device efficiency, and cloud-scale identification. We pretrain a ResNet-18 encoder with self-supervised contrastive learning on unlabeled palm images, distill its representation to a lightweight MobileNetV3 student for real-time inference on phones, and support both 1:1 on-device verification and 1:N cloud identification using FAISS/HNSW. On public datasets, the system attains 99.2% accuracy, a 0.15% equal-error rate (EER), and ~87 ms end-to-end latency on iPhone-class hardware. FAISS scales …

Pulmonary fibrosis is a suffocating disease. การศึกษานี้เป็นการวิจัยกึ่งทดลองแบบวัดเฉพาะหลังการทดลองมีกลุ่มเปรียบเทียบ (Posttest-only Control Group Design) โดยมีวัตถุประสงค์เพื่อประเมินผลของโปรแกรมการส่งเสริมความสุขสบายต่อคุณภาพการฟื้นตัวหลังการผ่าตัดศัลยกรรมกระดูกในเด็กวัยเรียน กลุ่มตัวอย่างเป็นเด็กอายุ 7–12 ปี จำนวน 40 ราย ที่เข้ารับการผ่าตัดใหญ่ศัลยกรรมกระดูกส่วนรยางค์และพักรักษาในหอผู้ป่วยกุมารศัลยกรรมกระดูก สถาบันสุขภาพเด็กแห่งชาติมหาราชินี โดยแบ่งเป็นกลุ่มทดลอง 20 ราย ซึ่งได้รับโปรแกรมการส่งเสริมความสุขสบาย และกลุ่มควบคุม 20 ราย ซึ่งได้รับการพยาบาลตามปกติ เครื่องมือวิจัยประกอบด้วย (1) โปรแกรมการส่งเสริมความสุขสบายแบบองค์รวมที่พัฒนาตามแนวคิดความสุขสบายของ Kolcaba (1992) (2) แบบประเมิน Children’s Comfort Daisies และ (3) แบบประเมินคุณภาพการฟื้นตัวหลังผ่าตัด 15 (Quality of Recovery–15: QoR–15) เครื่องมือทั้งหมดผ่านการตรวจสอบคุณภาพ โดยมีค่าดัชนีความตรงตามเนื้อหา (CVI) เท่ากับ 1.00, 1.00 และ 0.96 ตามลำดับ และแบบประเมิน QoR–15 มีค่าความเที่ยง Cronbach’s alpha เท่ากับ 0.86 การวิเคราะห์ข้อมูลใช้สถิติ Independent t-test ที่ระดับนัยสำคัญทางสถิติ .05 ผลการวิจัยพบว่า เด็กในกลุ่มทดลองมีคะแนนคุณภาพการฟื้นตัวหลังผ่าตัดเฉลี่ยในช่วง 48 และ 72 ชั่วโมง สูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < .001) โดยที่ 48 ชั่วโมงหลังผ่าตัด กลุ่มทดลองมีคะแนนเฉลี่ย 130.80 (SD = 16.43) ขณะที่กลุ่มควบคุมมีคะแนนเฉลี่ย 101.45 (SD = 21.13) และที่ 72 ชั่วโมงหลังผ่าตัด กลุ่มทดลองมีคะแนนเฉลี่ย 140.90 (SD = 10.76) ขณะที่กลุ่มควบคุมมีคะแนนเฉลี่ย 118.20 (SD = 17.19) ผลลัพธ์ดังกล่าวแสดงให้เห็นว่าโปรแกรมการส่งเสริมความสุขสบายแบบองค์รวมมีประสิทธิผลในการส่งเสริมคุณภาพการฟื้นตัวหลังการผ่าตัดศัลยกรรมกระดูกในเด็กวัยเรียนอย่างมีนัยสำคัญทางสถิติ

การศึกษาครั้งนี้เป็นการวิจัยแบบบรรยายเชิงทำนาย เพื่อศึกษาความสามารถของปัจจัยคัดสรรในการทำนายพฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้าน โดยปัจจัย ประกอบด้วย 1) ความรู้เกี่ยวกับวัณโรค 2) ทัศนคติต่อวัณโรค 3) แรงจูงใจในการป้องกันวัณโรค 4) การรับรู้ความสามารถของตนเองในการป้องกันวัณโรค และ 5) การรับรู้อุปสรรคเกี่ยวกับการป้องกันวัณโรค กลุ่มตัวอย่างเป็นผู้สัมผัสวัณโรคร่วมบ้าน จำนวน 111 คน ซึ่งเลือกอย่างเจาะจงจากโรงพยาบาล 3 แห่งในเขตกรุงเทพมหานคร เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเอง วิเคราะห์ด้วยสถิติเชิงพรรณนาและการถดถอยพหุคูณ ผลการวิจัย พบว่า พฤติกรรมการป้องกันวัณโรคในผู้สัมผัสวัณโรคร่วมบ้านโดยรวม อยู่ในระดับดี (Mean = 44.2, SD = 6.5) ปัจจัยที่สามารถทำนายพฤติกรรมการป้องกันวัณโรคอย่างมีนัยสำคัญทางสถิติ ได้แก่ ทัศนคติต่อวัณโรค (b = .374, p<.05) การรับรู ้ความสามารถของตนเอง ในการป้องกันวัณโรค (b = .244, p <.05) และ การรับรู ้อุปสรรคเกี ่ยวกับการป้องกันวัณโรค (b = .166, p <.05) ปัจจัยดังกล่าวสามารถร่วมทำนายพฤติกรรมการป้องกันวัณโรคได้ร้อยละ 32.8 อย่างมีนัยสำคัญทางสถิติ (R2 = .328, p <.05) อย่างไรก็ตามในการศึกษานี้ ความรู ้เกี ่ยวกับวัณโรค และแรงจูงใจในการป้องกันวัณโรค ไม่สามารถทำนายพฤติกรรมการป้องกันวัณโรคในผู ้สัมผัสวัณ โรคร่วมบ้าน

การวิจัยแบบกึ่งทดลองนี้มีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันต่อพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม เบาหวาน ความดันโลหิตสูง ไขมันในเลือด 1 โรคขึ้นไป ที่มีอายุตั้งแต่ 35 – 80 ปี คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 23 คน จับคู่ทั้งสองกลุ่มให้มีลักษณะคล้ายคลึงกันในเรื่องอายุ จำนวนโรคร่วม จำนวนกลุ่มยาที่ใช้รักษาโรค เปรียบเทียบข้อมูลพื้นฐานด้วยสถิติ Chi-square test และ Fisher’s Exact Test กลุ่มตัวอย่างทั้งสองกลุ่มมีคุณลักษณะพื้นฐานที่ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติในทุกตัวแปร (p > .05) กลุ่มควบคุมจะได้รับการพยาบาลปกติ ในขณะที่กลุ่มทดลองได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชัน เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารของผู้ป่วยที่มีภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม และแบบสอบถามประเมินความรอบรู้ด้านสุขภาพเกี่ยวกับพฤติกรรมการจัดการตนเองด้านการบริโภคอาหาร ทดสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงตามเนื้อหาเท่ากับ 0.92 และ 1.00 และค่าความเที่ยงเท่ากับ 0.91 และ 0.73 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิตทดสอบค่าที ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม กลุ่มทดลองหลังได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่าก่อนได้รับโปรแกรม ฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการจัดการตนเองด้านการบริโภคอาหารในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีโรคร่วม ที่ได้รับโปรแกรมเสริมสร้างความรอบรู้ด้านสุขภาพผ่านไลน์แอปพลิเคชันสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อศึกษาผลของโปรแกรมสนับสนุนการให้ข้อมูลแบบรูปธรรม-ปรนัยต่อความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกในห้องฉุกเฉิน โดยกลุ่มตัวอย่างในการศึกษาครั้งนี้คือ สมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองครั้งแรกที่นำส่งผู้ป่วยมารับบริการที่ห้องฉุกเฉินของโรงพยาบาลสรรพสิทธิประสงค์ แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 30 คน โดยคัดเลือกตามแบบเฉพาะเจาะจงตามเกณฑ์ที่กำหนด โปรแกรมรูปธรรม-ปรนัย และแบบสอบถาม ผ่านผู้ทรงคุณวุฒิ 5 ท่าน ได้ค่าความเที่ยง 0.84 โปรแกรมการสนับสนุนการให้ข้อมูลแบบรูปธรรม–ปรนัยต่อความวิตกกังวล มีเครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ประกอบด้วย ข้อมูลทั่วไปของผู้ป่วยโรคหลอดเลือดสมอง และแบบประเมินความวิตกกังวลขณะเผชิญ (The State Anxiety Inventory (STAI) Form Y) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติทดสอบ Independent t-test และสถิติ paired t-test ผลการวิจัย: ผลการวิจัยพบว่าสมาชิกครอบครัวของผู้ป่วยโรคหลอดเลือดสมองที่ห้องฉุกเฉิน ในกลุ่มทดลอง ภายหลังได้รับโปรแกรมคะแนนความวิตกกังวลตํ่ากว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (p < .05) และคะแนนความวิตกกังวลของสมาชิกครอบครัวของผู้ป่วยในกลุ่มทดลองภายหลังได้รับโปรแกรมตํ่ากว่าในสมาชิกครอบครัวของผู้ป่วยได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติ (p < .05)

The present study examined knowledge updating abilities in individuals with and without ADHD when forced to metacognitively evaluate the effectiveness of different study strategies. Participants with and without ADHD were randomly assigned to either a spontaneous or a forced knowledge updating condition, with those in the latter condition being asked to keep track of encoding strategy-recall outcome results, but no additional instructions for those in the spontaneous condition. This work presents a proof-of-concept implementation of an automation system to address these challenges for a large scale fragmented software systems. Succinctly we address these challenges in the context of Satellite Needs Working Group (SNWG), which is responsible for collecting and communicating federal agencies' Earth observation needs to National Aeronautics and Space Administration (NASA). This thesis explores the performance of higher-order sliding mode differentiators applied to sensor signals under varying sampling rates and switching functions. Two differentiator designs are evaluated: a hybrid algorithm that combines a super-twisting algorithm with a linear observer, and a fixed-time higher-order sliding mode differentiator. Each was tested on a physical sensor testbed using position data from a servomotor and electronic compass. The differentiators were implemented in a ROS2 environment, and their estimates were evaluated by obtaining the minimum and maximum position and velocity values, as well as analyzing graphs to capture the nature of the algorithms' noise reduction. Federated Learning (FL) enables collaborative model training across distributed clients without sharing raw data, thereby preserving privacy. However, conventional FL depends on a centralized aggregator, exposing it to poisoning and backdoor attacks that threaten model integrity. This thesis introduces B-ZkFed, a blockchain-based and zero-knowledge proof (ZKP)-enhanced FL framework designed to ensure verifiable trust, transparency, and robustness. The blockchain layer decentralizes aggregation through smart contracts, while the ZKP module allows clients to cryptographically prove the correctness of updates without revealing private data. While reward-based crowdfunding has widespread popularity, the motivations driving funders, balancing self-interest and altruism, have remained puzzling. Prior research has been constrained by examination methods and produced mixed findings regarding the weight of altruism versus self-interest among funders. Our study takes a fresh perspective, delving into funder behavior amid a major crisis—the tumultuous backdrop of the COVID-19 pandemic. Our findings reveal that funders not only display an increased willingness to contribute but also significantly amplify their contributions, particularly to projects in crisis-affected regions, irrespective of external incentives like rewards. Blockchain technology (BT) has the potential to enhance security and robustness of transactions through a distributed ledger bookkeeping process. This study employs technology-organization-environment (TOE) framework and threat-rigidity theory (TRT) to examine whether perceived disruption caused by COVID-19 pandemic significantly impacted the adoption of BT, and inclination to adopt BT in the US. The COVID-19 pandemic provided a unique backdrop, as it affected businesses across all industries, sizes, and geographies. Results show a non-significant effect of perceived pandemic disruption on the current stage of BT adoption and intention to adopt BT. Nikethamide, N,N-diethyl-3-pyridinecarboxamide (Coramine®) is a central nervous system stimulant at one time used therapeutically in human medicine but no longer marketed in the US. Nikethamide is classified a Drug Class 1, Penalty Class A substance by the Association of Racing Commissioners International (ARCI) and as a Horseracing Integrity and Safety Authority (HISA) SO or banned Stimulant that lacks FDA approval. Identifications of Nikethamide or its metabolite N-Ethylnicotinamide in racing samples can result in significant penalties in horse racing. Purpose
The global Assessing long-teRm Outcomes in dupiluMAb (AROMA) registry study aims to characterize the long-term, real-world use of dupilumab in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). This paper reports interim analysis of the baseline characteristics for the first 303 patients enrolled in AROMA.
Methods and materials
AROMA is currently ongoing in adult patients with CRSwNP who initiated dupilumab for up to 36 months. This interim analysis describes baseline demographics; systemic/oral corticosteroid (SCS/OCS) and antibiotic use; disease burden; healthcare resource utilization; and medical history. All outcomes are summarized descriptively.

Objective: To analyze a first-century Roman account of surgery for ozena-a tetrad of nasal crusting, ulceration, bony involvement, and foul odor-with a focus on historical context, etymology, and surgical management.
Data Sources: This historical review draws from the authors' translation of De Medicina. Primary sources detailing Roman surgical instruments from the House of the Surgeon in Pompeii, as well as secondary sources of surgical techniques, were utilized.

Tympanoplasty and mastoidectomy are common procedures performed by otolaryngologists that can result in complications for which patients may seek compensation. Medical malpractice case analyses may offer insight into how clinicians can avoid risk and improve patient satisfaction. We aimed to comprehensively characterize litigation after mastoidectomies and tympanoplasties in the United States.
Methods
The Westlaw Campus Research legal database was searched for all available court decisions associated with claims of medical malpractice after tympanoplasty and/or mastoidectomy in the United States between 1975 and 2022.
