Medical exome sequencing pipelines consist of various preprocessing steps to prioritize credible causal variants before a pathologist or variant curation scientist manually interprets potential findings that are then reported to patients. The variant allele frequency (VAF), reported as the fraction of sequencing reads supporting a variant call, can be used to screen for technical artifacts, yet a specific filtering threshold has yet to be established. A total of 13,122 manually curated variants, sequenced from 289 patients using the Agilent SureSelect Focused Exome enrichment kit at the University of Kentucky Clinical Genomics laboratory from October 2019 to May 2023, were evaluated. …

Counselor Education Faculty Publications and Presentations
This interpretative phenomenological analysis study examined how adjunct instructors teaching online clinical counseling courses experienced gatekeeping in the context of the university system. The research question was: How do online, adjunct instructors teaching master’s level clinical counseling courses experience their university interactions as a part of gatekeeping? The participants described five experiential themes. These were: a) Balancing marginalization in the system with advocacy for the profession; b) Maneuvering through dependency on the system in gatekeeping; c) Finding consultation while navigating gatekeeping interactions; d) Managing the intensity of clinical courses; and e) Navigating the steep learning curve of learning how to …

Background: Within long-term care (LTC) homes, telehealth use has been found to reduce unnecessary emergency department transfers, support the care needs of rural and underserved communities, and supplement in-person physician care. Despite these benefits, it is not well understood when telehealth is not an appropriate medium for providing physician care to residents with complex health needs. This knowledge gap must be addressed given the recent rise in telehealth use in LTC homes in many health systems following the COVID-19 pandemic, when virtual care use increased in many health care sectors to limit travel and in-person exposure risks, that is expected …

Whether To Offer Interventions At The End Of Life: What Physicians Consider And How Clinical Ethicists Can Help, Joelle Robertson-Preidler, Mikaela Kim, Sophia Fantus, Janet Malek
Faculty and Staff Publications
Background: Advances in life-prolonging technologies increasingly create dilemmas for physicians who must decide whether to offer various interventions to patients nearing the end of life. Clinical ethicists are often consulted to support physicians in making these complex decisions and can do so most effectively if they understand physicians' reasons for making recommendations in this context.
Methods: Semi-structured interviews were conducted with surgeons, nephrologists, intensivists, emergency physicians, and oncologists regarding the considerations they have used to make decisions about offering interventions for patients nearing the end of life. Interview transcripts were thematically analyzed.
Results: We identified six types of considerations physicians …

Background: Inpatient medical settings lack evidence-based spiritually integrated interventions to address patient care needs within a pluralistic religious landscape. To address this gap, CCSH™ (Compassion-Centered Spiritual Health) was developed to leverage the skillsets of healthcare chaplains to improve patient outcomes through spiritual consultation. Here, we report the results of a randomized, wait-list controlled, pre-registered (NCT03529812) study that evaluated the impact of CCSH on patient-reported depression and explored putative mediators of CCSH's effects.
Method: Chaplain residents were randomized to be trained in CCSH as part of their clinical pastoral education (CPE) residency in the fall (n = 8) or …

Cisgender sexual minority women (SMW, e.g., lesbian, queer) are at greater risk for poor mental and physical health compared to heterosexual women and face challenges when accessing health care. Previous research has largely focused on general sexual and gender minority barriers to health care, but more research is needed on the experiences of specific subgroups, including cisgender SMW. The current study qualitatively explored barriers and facilitators for cisgender SMW seeking health care. Twenty cisgender SMW aged 18-40 recruited using Meta advertisements and past participant lists completed 45 min semi-structured interviews and a brief survey. Thematic analysis conducted by two coders …

Traumatic brain injury (TBI)-related growth hormone deficiency is often undertreated, despite documented physical, metabolic, and neuropsychiatric effects. Insulin-like growth factor (IGF-1), with neuroreceptors located in brain regions responsible for learning, memory, and mood, regulates cerebral blood flow, neurogenesis, and neuroplasticity. The aim of this study was to determine associations between IGF-1 levels and post-TBI symptom severity, anxiety, and depression. This retrospective observational study at an Academic Brain Injury Center included participants evaluated 3-12 months post-TBI with available IGF-1 values and complete Rivermead Post-Concussion Symptoms Questionnaire (RPQ-13), Generalized Anxiety Disorder-7 (GAD-7), and Patient Health Questionnaire-9 (PHQ-9) responses. Patients under 18 or …

Ex Vivo Single-Molecule Analysis Of Ryanodine Receptor 2 Assembly In Cardiac And Neuronal Tissue, Qianye Yin, Christopher I. Richards
Chemistry Faculty Publications
Understanding receptor assembly is critical for elucidating the mechanisms underlying their function and regulation in physiological processes. While traditional in vitro single-molecule studies rely on isolating proteins from heterologous expression systems, they often fail to capture the in vivo physiological complexity involved in the organization and assembly of cell surface proteins. This protocol employs Total Internal Reflection Fluorescence Microscopy (TIRFM) to study GFP-tagged Ryanodine Receptor 2 (RyR2) molecules encapsulated within nanoscale vesicles. These vesicles, generated from organs rapidly extracted from the animal, effectively provide a snapshot of the receptor's assembly state at the time of extraction, enabling detailed analysis of …

Positive Youth Development For Youth Living In Structurally Diverse Families, Jonathon J. Beckmeyer
Positive Youth Development For Youth Living In Structurally Diverse Families, Jonathon J. Beckmeyer
Faculty Publications - Family and Consumer Sciences
Research on family structure diversity and youth well-being has largely aligned with deficit orientations, emphasizing that living in structurally diverse families is a potential risk-factor for youth well-being and development. In this article, I advocate for increased strengths-based scholarship on youth living in structurally diverse families by centering this research with positive youth development (PYD) frameworks. Broadly, PYD frameworks orient scholars toward viewing youth holistically, attending to positive indicators of well-being as well as how youth capacities and experiences intersect with family and community resources to promote flourishing and thriving. Approaching family structure diversity and youth well-being from a PYD …

Building A Professional Learning Community To Improve Technical Services Librarians' Teaching Preparedness, Junli Diao
Publications and Research
The Applied Improvement Project (AIP) was conducted to address the inconsistency of academic librarians’ teaching engagement on the AIP site. The problem of practice was framed as the gap between having all librarians prepared to teach and technical services librarians’ unpreparedness to teach. The implemented intervention was developing a professional learning community among technical services librarians of different generations with a variety of teaching experiences to increase teaching preparedness by sharing, discussing, and reflecting. The learning topics covered library teaching basics, motivating students, self-assessment, planning to teach, using Wikipedia, embedded librarianship and credit-bearing courses, teaching and research, and building a …

When Lsat Doesn't Matter And Other Surprising Finds From A Deep Dive Into Academic Probation In Law School, Kari Milligan, Ryan Jerome Lecount
Faculty Scholarship
Numerous students nationwide are placed on academic probation and ultimately dismissed from law school each year. While students often carefully conceal their probationary status, their lack of visibility within legal communities should not be mistaken for an absence of need. These students are frequently uniquely vulnerable—facing unexpected family emergencies or entering law school unprepared. The consequences of dismissal are dire, with students experiencing adverse mental health effects and financial exigencies. During the 2023–2024 academic year alone, tuition and fees for a single year of law school ranged from $30,000 to $48,000. Yet, little research—particularly quantitative—has been conducted in service of …

Why Academic Law Librarians Quit: Results Of The Law Librarian Exit Survey, Laura J. Ax-Fultz
Faculty Scholarly Works
The pandemic exposed the struggles of workers everywhere. The complexity of handling family, work, illness, and a host of other issues led many to reconsider their employment. The phrase “The Great Resignation” summed up the zeitgeist of the early pandemic. Concurrently, open positions for academic law librarians were increasing. I sought to unravel the apparent surge in open positions by surveying academic law librarians to discover why they resigned from their positions or considered leaving them during the period of January 2020 through June 2023.
These findings were evaluated within the context of a high volume of job postings in …

This study seeks to understand the effects on researchers’ work at a large research university in the wake of the university library’s shift from a near-comprehensive journals package with a single, large publisher to a selective list of individual journal subscriptions. Analyzing historical journal usage, along with turnaway and interlibrary loan trends from the years following the changes, the authors made use of structured interviews with local researchers to bring context and meaning to the quantitative data. The interviews highlighted researchers’ strategies for gaining access to literature in their fields to which the library does not subscribe, and revealed assumptions …
