Clinical Research Faculty Publications

In the Look AHEAD trial, randomization to Intensive Lifestyle Intervention (ILI) or Diabetes Support and Education (DSE) did not result in differences in cognitive outcomes. However, menopause and APOE genotype are factors that affect the response to this intervention. The effect of this intervention on a single cognitive assessment was examined in 3 groups of women: premenopausal or < 5 years postmenopausal (N = 594), within 5–10 years (n = 388), and ≥10 years postmenopausal (n = 963), and as a function of continuous years since menopause. The late postmenopausal group in the ILI had worse composite z-scores compared to those in the DSE, whereas the younger premenopausal or early postmenopausal women in the ILI had better composite z-scores than the DSE. A significant interaction between years since menopause and intervention arm, but not baseline age, was observed on executive function domains. ILI appeared only to benefit cognitive function among non-APOE4 carriers during premenopause or early postmenopause. These findings emphasize the importance of assessing menopause and APOE status to understand how weight loss impacts cognition.

Preventing Polypharmacy Amongst The Elderly In An Acute Care Setting Through The Integration Of The Deprescribing Tools Start, Stopp, And Beers Criteria, Samantha Bonita Wilkins-Copeland

Doctoral Dissertations and Projects

A significant percentage of medications prescribed in the United States are prescribed to elderly adults, which is due to the increase in chronic illness as Americans age. This increase in the use of medications amongst the elderly population has led to the issue of polypharmacy, which can lead to negative patient outcomes. An evidence-based practice pilot project was performed at a large tertiary care facility in Virginia. The aim of the scholarly project was to integrate three deprescribing tools into the everyday practice of pharmacists at the facility. Three outcomes were achieved: (1) Pharmacists understood how to accurately utilize the …

Is Exposure To Epidemic Associated With Older Adults’ Health Behavior? Evidence From China’S 2002-2004 Sars Outbreak, Hong Zou, Sha Wen, Hongwei Xu

Publications and Research

Objectives: To determine whether exposure to an epidemic is associated with better health behaviors.

Methods: Using nationally representative survey data collected in 2011 and 2014, we identified middle-aged and older Chinese adults whose communities experienced an outbreak of the 2002–2004 severe acute respiratory syndrome (SARS). We estimated logistic models of health behaviors in the years after the SARS epidemic.

Results: Compared to those who lived in communities not hit by the epidemic, respondents who lived in communities with a SARS outbreak in 2002–2004 were more likely to get a physical examination in 2010–2011 and have their blood pressure checked and …

Creating A Quality Improvement Toolkit To Identify & Address Gaps In The Care Of Older Adults With Hip Fracture, Amanda J. Covell Aug 2020

Creating A Quality Improvement Toolkit To Identify & Address Gaps In The Care Of Older Adults With Hip Fracture, Amanda J. Covell

Doctoral Projects

Introduction: Traumatic hip fracture remains a major cause of disability, decline, and mortality for adults over 65. Gaps remain in the consistency of care and services hip fracture patients receive during hospitalization and post-discharge, leading to adverse outcomes, including MI, surgical site infection, poorly managed pain, and increased mortality. To help address these inconsistencies, the AAOS released updated clinical practice guidelines for management of hip fracture care in the elderly in 2014. These guidelines examine the entire hospitalization process for this population and set forth evidence-based standards to assist with improving outcomes for hip fracture patients.

Objectives: Five of the …

Investigating A Multimodal Approach To Clinical Diagnosis Of Mild Cognitive Impairment And Alzheimer’S Disease, Sean M. Flannery Aug 2020

Investigating A Multimodal Approach To Clinical Diagnosis Of Mild Cognitive Impairment And Alzheimer’S Disease, Sean M. Flannery

Psychology Theses & Dissertations

An estimated 5.8 million Americans suffer from dementia due to Alzheimer’s disease (AD), with that number projected to grow to 13.8 million by mid-century (Alzheimer’s Association, 2019). Mild cognitive impairment (MCI) describes the stage between normal cognitive decline that comes with aging and a dementia diagnosis (Peterson, 1999). Due to a lack of a cure or particularly effective treatment, a major goal of treatment is to focus on improving quality of life (Budson & Solomon, 2016). An early and accurate diagnosis can address this goal in a variety of ways. Despite the high prevalence and immense amount of research in …

Dementia Prevention, Intervention, And Care: 2020 Report Of The Lancet Commission, G Livingston, J Huntley, A Sommerlad, Et Al

Peninsula Medical School

The number of older people, including those living with dementia, is rising, as younger age mortality declines. However, the age-specific incidence of dementia has fallen in many countries, probably because of improvements in education, nutrition, health care, and lifestyle changes. Overall, a growing body of evidence supports the nine potentially modifiable risk factors for dementia modelled by the 2017 Lancet Commission on dementia prevention, intervention, and care: less education, hypertension, hearing impairment, smoking, obesity, depression, physical inactivity, diabetes, and low social contact. We now add three more risk factors for dementia with newer, convincing evidence. These factors are excessive alcohol …

We discuss how our findings will be used for future efforts addressing the topic for older adults and the implications of our findings and method for others in Extension.

From Territorial To Transformational: A New Framework For Master Volunteer Engagement, Lisa T. Washburn, Andrew B. Crocker, Joanne Leatherman Aug 2020

From Territorial To Transformational: A New Framework For Master Volunteer Engagement, Lisa T. Washburn, Andrew B. Crocker, Joanne Leatherman

Journal of Extension

Meaningful volunteer engagement depends on the Extension professional's volunteer management philosophy, training, and organizational support for using volunteers. Volunteer development and leadership development are typically absent from management-focused volunteer models used in Extension. Professional development of the Extension professional, beyond discrete management tasks, is lacking but is needed for authentic volunteer engagement through master volunteer programs. A volunteer engagement framework is described to guide a shift from volunteer management to engagement, including use of principles of the community-based participatory approach. The volunteer engagement framework can help professionals identify and self-assess the skill set needed for authentic and sustained volunteer involvement …

Intussusception In An Infant As A Manifestation Of Covid-19, Zorays Moazzam, Areej Saleem, Alina Ashraf, Fyezah Jehan, Muhammad Arshad

Medical College Documents

Gastrointestinal manifestations of COVID-19 are rare and have primarily been limited to diarrhea or vomiting. Intussusception is the most common cause of bowel obstruction in infants, with up to 30% of pediatric intussusception cases having a preceding viral illness. We present the rare case of intussusception in a SARS-CoV-2 positive infant. This is the first documented case of survival in a SARS-CoV-2 positive patient presenting with intussusception as the primary manifestation. As our knowledge of this disease evolves, surgeons need to remain suspicious for possible gastrointestinal manifestations of COVID-19.

Rights And Representation: Media Narratives About Disabled People And Their Service Animals In Canadian Print News, Lana Kerzner, Chelsea Temple Jones, Beth Haller, Arthur Blaser

Political Science Faculty Articles and Research

Canadian news coverage is reflecting and shaping an evolution of thought about how we must publicly account for animals’ roles in the disability rights movement. Through a textual analysis of 26 news media articles published between 2012 and 2017, this research demonstrates that the media play a key role in reporting on discrimination, yet media narratives about service animals and their owners too often fail to capture the complexity of policies and laws that govern their lives. In Canada, there is widespread public confusion about the rights of disabled people and their service animals. This incertitude is relevant to both …

Research has shown that social networking sites (SNS), such as Facebook and WhatsApp, make it more convenient for older adults to bond with friends and family. However, despite such benefits, studies also found few older adults use SNS, and even fewer older adults use SNS in less developed nations. Therefore, it is important to identify factors that can influence intention to use SNS in older adults, especially among the Malaysian Chinese population, as they will face an aging society earlier than other ethnic groups in Malaysia. This study used the Technology Acceptance Model and Reasoned Action Approach to examine whether …
