As the numbers of primary care physicians decline, nurse practitioners (NPs) are filling this gap as primary care providers in many settings. The number of adults presenting to their primary care provider with signs and symptoms of posttraumatic stress disorder (PTSD) each year is unknown. The devastating effects of untreated PTSD include high rates of homelessness, violence, drug abuse, suicidality and a decreased quality of life. Little is known as to the breadth of NPs knowledge of PTSD sequelae, epidemiology and screening protocols. Increased perceived self-efficacy, tied to positive attitudes about PTSD knowledge and screening, result in overall higher rates …

Background: Recent studies cast doubt on the net effect of antipsychotics for delirium.
Aim: To investigate the influence of these studies and other factors on clinicians’ delirium treatment practice and practice change in palliative care and other specialties using the Theoretical Domains Framework.
Design: Australia-wide online survey of relevant clinicians.
Setting/participants: Registered nurses (72%), doctors (16%), nurse practitioners (6%) and pharmacists (5%) who cared for patients with delirium in diverse settings, recruited through health professionals’ organisations.
Results: Most of the sample (n=475): worked in geriatrics/aged (31%) or palliative care (30%); in hospitals (64%); and saw a new patient with delirium …

โปรแกรมการเรียนรู้ระหว่างวัยถือเป็นเครื่องมือในการแก้ปัญหาความแปลกแยกและลดการพึ่งพาทางสังคมของผู้สูงอายุ และยังสร้างให้เกิดความสัมพันธ์เชิงบวกระหว่างวัยจากการการแลกเปลี่ยนเรียนรู้ในลักษณะต่างตอบแทนของผู้สูงอายุและเด็ก งานวิจัยนี้มีวัตถุประสงค์เพื่อ 1) วิเคราะห์ความต้องการจำเป็นในการส่งเสริมความสัมพันธ์ระหว่างผู้สูงอายุและเด็กผ่านโปรแกรมการเรียนรู้ระหว่างวัยที่เหมาะสมกับบริบทไทย 2) พัฒนาหลักการออกแบบและต้นแบบโปรแกรมการเรียนรู้ระหว่างวัยเพื่อส่งเสริมความสัมพันธ์ระหว่างผู้สูงอายุและเด็ก และ 3) ทดลองและประเมินผลการใช้ต้นแบบโปรแกรมการเรียนรู้ระหว่างวัยเพื่อส่งเสริมความสัมพันธ์ระหว่างผู้สูงอายุและเด็ก และนำเสนอหลักการออกใหม่ที่ได้จากบทเรียนจากการวิจัย แบ่งการดำเนินวิจัยเป็น 3 ระยะตามวัตถุประสงค์การวิจัย โดยตัวอย่างในการวิจัยประกอบด้วยนักปฏิบัติจากในเมืองและนอกเมือง จำนวน 24 คน ตัวอย่างวิจัยระยะที่ 2 ประกอบด้วย นักวิชาการที่มีประสบการณ์ จำนวน 9 คน และ ตัวอย่างวิจัยในระยะที่ 3 ของการดำเนินการวิจัย คือ ผู้สูงอายุที่มีอายุ 60-69 ปี และเด็กที่มีอายุระหว่าง 10-12 ปี อย่างละ 17 คน เครื่องมือที่ใช้ในการวิจัยคือ โปรแกรมการเรียนรู้ระหว่างวัยเพื่อส่งเสริมความสัมพันธ์ระหว่างผู้สูงอายุและเด็ก วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์เนื้อหา การวิเคราะห์โดยใช้สถิติเชิงบรรยาย ผลการวิจัยสรุปได้ดังนี้ ผลการวิเคราะห์ความต้องการจำเป็นในการส่งเสริมความสัมพันธ์ระหว่างผู้สูงอายุและเด็กผ่านโปรแกรมการเรียนรู้ระหว่างวัยที่เหมาะสมกับบริบทไทย พบว่า นักปฏิบัติควรได้รับการส่งเสริมทั้งสิ้น 4 ประการ คือที่ 1) ความตระหนักรู้ในความสำคัญของการส่งเสริมความสัมพันธ์ระหว่างวัย 2) ความต้องการจำเป็นด้านความรู้ความเข้าใจเกี่ยวกับแนวคิดในการจัดกิจกรรมเพื่อส่งเสริมความสัมพันธ์ระหว่างวัย 3) ความต้องการจำเป็นด้านทักษะการจัดกิจกรรมเพื่อส่งเสริมความสัมพันธ์ระหว่างวัย 4) ความต้องการจำเป็นด้านความพร้อมในการจัดกิจกรรมส่งเสริมความสัมพันธ์เรียนรู้ระหว่างวัย หลักการออกแบบและต้นแบบโปรแกรมการเรียนรู้ระหว่างวัยพัฒนาจากแนวคิดทฤษฎีการเรียนรู้ระหว่างวัยและแนวคิดการศึกษานอกระบบ ร่วมกับผลจากการระดมความคิดของนักปฏิบัติ ได้มาซึ่งร่างหลักการออกแบบตามแนวคิดของ Sandoval (2014) ซึ่งมีหลักการออกแบบทั่วไป 10 ประการ คือ 1) การสร้างความรู้สึกปลอดภัยในการเข้าร่วมกิจกรรมโดยให้สมาชิกที่การตัดสินด้วยตนเอง 2) การจัดกิจกรรมที่ทำให้ผู้เข้าร่วมกิจกรรมมีความรู้สึกว่าสามารถทำกิจกรรมได้ง่ายและสนุก 3) การส่งเสริมให้เกิดความเข้าใจในความแตกต่างระหว่างวัย 4) การกำหนดบทบาทของการมีส่วนร่วมของพ่อแม่ในการสร้างการเรียนรู้ระหว่างวัย 5) การจัดกิจกรรมที่ตอบสนองความต้องการของชุมชน 6) การจัดกิจกรรมที่ตอบสนองตามความต้องการของข้อเสนอของผู้สูงอายุและเด็ก 7) การใช้การสื่อสารเชิงบวกในการจัดกิจกรรม 8) การส่งเสริมให้เกิดความเชื่อถือไว้วางใจกันของสมาชิกที่เข้าร่วมกิจกรรม 9) การสร้างเงื่อนไขและสถานการณ์ที่ทำให้ผู้สูงอายุและเด็กมาพบและทำกิจกรรมร่วมกัน 10) การส่งเสริมศักยภาพในการจัดกิจกรรมของนักปฏิบัติในพื้นที่เพื่อให้สามารถพึ่งพาตนเองในการจัดโปรแกรมการเรียนรู้ระหว่างวัยได้ด้วยตนเองและยั่งยืน ผลการทดลองและประเมินผลการใช้ต้นแบบโปรแกรมการเรียนรู้ระหว่างวัย พบว่า ช่วยส่งเสริมความสัมพันธ์ระหว่างผู้สูงอายุและเด็ก โดยส่งผลให้เกิดตัวแปรผลลัพธ์ 7 ตัวแปรย่อย ได้แก่ ความเข้าใจและยอมรับในความแตกต่างระหว่างวัย อันนำมาซึ่งทัศนคติที่ดีต่อคนต่างวัย 2) มุมมองเชิงคุณค่าต่อคนต่างวัย 3) ความผูกพันธ์ระหว่างวัย …

Frailty is roughly defined as an accumulation of physiological, emotional, cognitive, and social deficits that impair a person’s response to stressful events. A frailty diagnosis has been associated with poor outcomes following surgical procedures. Cancer surgical patients aged 65 or older represent a vulnerable population susceptible to being frail and the potential associated complications that can accompany frailty. Measuring frailty is an objective risk assessment that identifies increased risk better than age or American Society of Anesthesiologists Physical Status (ASA-PS) score such that frailty can independently predict poor surgical outcomes.

Hurricane María devastated Puerto Rico in 2017 and resulted in adverse long-term outcomes. Psychological sense of community (PSOC) may serve as a protective factor against the effects of Hurricane María for older adults in Puerto Rico. Using a three-paper format, this dissertation draws on a resilience framework and theories of PSOC and the Ecological Theory of Aging to examine the role of PSOC among older adults in Puerto Rico two years after Hurricane María.

Objectives: To describe and identify the socio-behavioral factors predicting willingness towards deprescribing among older adults with osteoporosis.Design, Setting, and Participants: For this population-based survey study of US Medicare beneficiaries, data was obtained from the Medication Attitudes Module distributed to the participants in Round 6 (2015) of the National Health and Aging Trends Study. The 2019-2020 SARS-related coronavirus-2 (SARS-CoV-2) pandemic has brought unprecedented challenges to healthcare sectors around the world. As of November 2020, there have been over 64 million confirmed cases and approaching 2 million deaths globally. Declining estrogen levels before, during, and after menopause can affect memory and risk for Alzheimer's disease. Undesirable side effects of hormone variations emphasize a role for hormone therapy (HT) where possible benefits include a delay in the onset of dementia—yet findings are inconsistent. Effects of HT may be mediated by estrogen receptors found throughout the brain. Effects may also depend on lifestyle factors, timing of use, and genetic risk. We studied the impact of self-reported HT use on brain volume in 562 elderly women (71–94 years) with mixed cognitive status while adjusting for aforementioned factors. Evidence-based practice integrates and supports the best interventions in clinical practice. However, a gap of about ten years may elapse between the production of evidence and integration of best practices in clinical settings. Some technologies, such as podcasting, have become increasingly popular and are emerging as an innovative teaching modality that can support knowledge acquisition and integration. This scoping study gathered and synthesized the information in the literature regarding the use of podcasts by occupational therapy clinicians and students as a means for teaching and learning evidence-based knowledge. Introduction: Persons with preclinical Alzheimer’s disease (AD) show impairments in semantic and associative memory years before the onset of clinical symptoms. However, many tasks used to assess these deficits make use of previously learned stimuli, often within well-established semantic networks that may be more resistant to disease-related changes. No research has been conducted examining trained and derived relational responding (TDRR) in older adults or in the presence of AD risk factors. บทความวิจัยเรื่อง “ความรุนแรงต่อผู้สูงอายุ: สถานการณ์ สาเหตุ และการรับมือ” เป็นการวิจัยเชิงปริมาณ มีวัตถุประสงค์เพื่อศึกษาถึงสถานการณ์ปัญหาความรุนแรงต่อผู้สูงอายุของสังคมไทยในปัจจุบันและสาเหตุของความรุนแรงต่อผู้สูงอายุ รวมถึงแนวทางการสนับสนุนจากเครือข่ายในการป้องกันและแก้ไขความรุนแรงต่อผู้สูงอายุ ดำเนินการใน 5 พื้นที่ ได้แก่ กรุงเทพมหานคร ปทุมธานี เชียงใหม่ หนองบัวลำภู และสงขลา จากกลุ่มตัวอย่างทั้งหมดจำนวน 1,215 ราย ผลการศึกษาพบว่าผู้สูงอายุส่วนใหญ่ถูกกระทำความรุนแรงทางด้านอารมณ์และจิตใจมากที่สุด ซึ่งสาเหตุความรุนแรงส่วนใหญ่มาจากภาวะความเครียดจากบุคคลที่เกี่ยวข้องอันสืบเนื่องมาจากปัญหาทางด้านความสัมพันธ์ระหว่างบุคคลภายในครอบครัวหรือคนใกล้ชิด ซึ่งผู้กระทำความรุนแรงส่วนใหญ่มักเป็นคู่สมรส ลูกหลาน ญาติพี่น้อง คนใกล้ชิดเพื่อน และเพื่อนบ้าน ที่ต้องพึ่งพาอาศัยซึ่งกันและกัน และแนวทางการสนับสนุนจากเครือข่ายในการป้องกันและแก้ไขความรุนแรงต่อผู้สูงอายุ พบว่าผู้สูงอายุส่วนใหญ่จะได้รับการสนับสนุนในด้านของการให้คำปรึกษา แนะนำ ให้กำลังใจจากสมาชิกในครอบครัว เครือญาติ เพื่อนบ้านและทีมสหวิชาชีพ

Cerebrovascular pathologies are commonly associated with dementia. Because air pollution increases arterial disease in humans and rodent models, we hypothesized that air pollution would also contribute to brain vascular dysfunction. We examined the effects of exposing mice to nanoparticulate matter (nPM; aerodynamic diameter ≤200 nm) from urban traffic and interactions with cerebral hypoperfusion. C57BL/6 mice were exposed to filtered air or nPM with and without bilateral carotid artery stenosis (BCAS) and analyzed by multiparametric MRI and histochemistry. Researchers are actively exploring how historical institutionalized silences contribute to health disparities for lesbian, gay, bisexual, and transgender (LGBT) older adults. Yet, none have considered LGBT elders within the specific context of community-based senior centers. My study addressed the research question: how does the discourse of senior centers create welcoming environments, or alternately construct silences, for LGBT older adults? Myofascial pain syndrome (MPS) is one of the most common musculoskeletal conditions characterized by tight bands of fascia and muscle fibers known as myofascial trigger points (MTrP’s). Empirical evidence suggests that deactivation of MTrP’s is best accomplished with invasive needling therapy such as dry needling, trigger point injections (TPI’s), and botulinum toxin (BTX-A) injections. Anticholinergic medications cause a therapeutic or negative effect in the human body by blocking neuronal cholinergic receptors, thereby inhibiting the binding of acetylcholine in the central nervous system and the peripheral nervous system. These medications have short-term side effects including dry mouth, constipation, visual impairments, or delirium; however, research regarding the long-term side effects is limited. This research aims to investigate the scope of fisetin (FIT)/β-cyclodextrin (β-CD) inclusion complexes with the ratios of 1:2 and 2:1 mole by means of freeze-drying, kneading and physical mixing methods (controlled process). Moreover, the complex compounds are analyzed by Fourier-Transform Infrared Spectroscopy (FTIR) and Differential Scanning Calorimetry (DSC). The results show that the most suitable preparation method of fisetin (FIT)/β-CD complex is the freeze-drying method with the fisetin (FIT) to β-CD ratio at 1:2. The special circumstances of living plus innovation and promotion of mobile applications (apps) during the pandemic influenced mobile technology use behavior. The life sequence and human capital accumulation of women have changed in the US. This dissertation intends to propose three interrelated studies that investigate the possible trajectories of human capital accumulation after women’s transition to motherhood, the determinants of these trajectories, and the implications for women and their children’s wellbeing. Driven by the life course perspective, the cumulative advantage and disadvantage theory, and Bourdieu’s cultural framework, this dissertation uses mothers’ vocational training experiences to identify distinct trajectories over time in the first study. Background: Abuse of elderly women is of great concern and yet relatively little is known about interventions.
Objectives: The aim of this study was to develop and test a culturally informed treatment, based on Intervention Mapping (IM), for primary healthcare settings. The intervention targets family members of elderly women and seeks to reduce elder abuse.
Methods: N = 80 family members of elderly women were randomized to intervention or control. Elderly women completed assessment prior to randomization. Elder abuse was measured by self-reported frequency of neglect, physical, psychological, and financial abuse in the last 2 months across 16 items. Intervention …

In this study, formation and quantification of mechanochemically generated free radicals of lignin were evaluated after the extraction of lignin from olive seeds and detailed lignin characterization was performed. Lignin was extracted from crushed olive seeds as an insoluble solid using Klason method. Isolated lignin was mechanochemically grinded under cryo conditions using Cryomill and particlesizes were determined by using Zeta Sizer, structural changes were followed by XRD and FTIR-ATR; thermal stabilities were tracked by TGA and DSC. Young adults (aged 18-25) were identified as the most stressed age group due to internal and external factors that are relatively unique to their group. High stress levels are alarming as they coincide with a multitude of adverse effects on psychological health, including difficulty falling and staying asleep, reduction in overall sleep quality, and exacerbated depression and anxiety symptomatology. Young adults attending post-secondary education institutions are at particular risk for developing and maintaining these adverse psychological effects of stress, especially considering the low rate in which they seek treatment for psychiatric difficulties.
