Assisted living facilities are expanding in response to the growing population of older adults in the United States. Assisted living facilities offer a wide array of services including memory care. Research has shown that a majority of assisted living facilities have residents with cognitive impairments living along with non-cognitively impaired individuals (i.e., mixed populations). Little is known about the conflicts that may arise between cognitively and non-cognitively impaired residents in a mixed population. Issues Of Elder Care Among Migrant Workers In Contemporary Rural China: Filial Piety Redefined From A Foucauldian Perspective, Longtao He, Kate Van Heugten, Maria Perez-Y-Perez, Yongqiang Zheng

This article reports on research that explored rural migrant workers’ experiences of returning to their hometown to provide care for elderly parents diagnosed with cancer. The authors used a culturally integrated approach to Foucauldian discourse analysis to consider how 24 participants narrated their experiences of care in China. The discourse of care demonstrated a strong commitment to filial piety despite their unique care challenges, and this commitment was bolstered by discourses that emphasized how much parents had sacrificed, as well as by a persistent forgetting of experiences or background details that suggested any lack of parental sacrifice in recent Chinese …

The exploitation of unpaid healthcare staff led to a volunteer flight from long-term residential facilities during the COVID-19 pandemic. A literature gap existed on human resource policies to support volunteer eldercare workers who had remained at their posts and taken charge of work in long-term care facilities during the public health crisis. The purpose of this qualitative narrative inquiry study was to explore how volunteers in eldercare facilities viewed their daily experiences and interpreted their role in eldercare facilities during the COVID-19 pandemic. Falls among hospitalized, geriatric patients are a leading cause of injury. Geriatric patients with psychiatric illnesses are at an even greater risk of falls. The project site was experiencing an increase in the number of patient falls and identified a need for staff education on fall risk assessment and implementation of interventions specific to the hospitalized, geriatric, psychiatric patient. Hospice patients use the emergency department (ED) for the management of symptoms, making it necessary for ED nurses to be equipped with the knowledge and skills to care for hospice patients. EDs should have staff specially trained to address end-of-life needs due to ED staff’s minimal knowledge of treating and managing hospice patients. When healthcare organizations fail to provide clinical practice guidelines (CPG) and collaborate with significant stakeholders adequately, gaps in care delivery can occur, and hospice care is often not delivered safely. Interpersonal violence (IPV) can result in the traumatization of primary survivors, as well as the mental health professionals who support them, which is referred to as vicarious trauma (VT). VT is an occupational hazard that if unaddressed can lead to significant personal and professional disruption. Clinical mental health counselors (CMHCs) who provide counseling services to IPV survivors are at increased risk of integrating the traumatic session content into their worldview. VT can lead to psychological distress including symptoms of post-traumatic stress disorder and can threaten the quality and ethical nature of the counseling relationship. Elderly nursing home patients’ high rates of hospitalization due to sepsis impact their morbidity and mortality and significantly cost the U.S. healthcare system. Skilled nursing facility (SNF) nurses who have the knowledge to assess older adults for signs and symptoms of sepsis and communicate findings to providers are instrumental in providing improved care for patients. Before biomarkers can be used in clinical trials or patients' management, the laboratory assays that measure their levels have to go through development and analytical validation. One of the most critical performance metrics for validation of any assay is related to the minimum amount of values that can be detected and any value below this limit is referred to as below the limit of detection (LOD). Most of the existing approaches that model such biomarkers, restricted by LOD, are parametric in nature. This dissertation presents the results of a grounded theory study of the key factors of sibling conflict when caregiving for an aging parent. Following interviews with 30 participants, I conducted initial coding, focused coding and theoretical coding, which led to the development of a theory that explains the relationship of conflict factors such as: beliefs & values, feelings & emotions, roles & expectations, and behaviors & responses. The dissertation includes a discussion of how birth order theories, power theories and psychodynamic theories apply to sibling conflict. Much research has documented the mental health consequences of the terrorist attacks of September 11, 2001; however, little is known about how the 9/11 attacks affect the mental health of Latino Americans. This study uses a nationally representative sample of Latino Americans (N = 2,346) from the National Latino and Asian American Study (NLAAS) to examine the relationships between 9/11-induced negative life experiences and mental disorders. The former includes losing a job, reducing family income, feeling less safe and secure, discrimination, loss of optimism, and inability to cope with things. Hearing loss is the most common sensory deficit noted in aging adults. It is commonly known to reduce an individual’s ability to detect, identify, and localize sounds and speech and to cause issues in communication. However, there are other less commonly discussed impacts that hearing loss has beyond the auditory system. Literature suggests a correlation between hearing loss and cognition in aging adults. Similar to hearing loss, the domains of cognition experience performance and functional changes across the life span. This article provides an overview of medical music therapy theory, practice, and research in the United States. The breadth and depth of treatment as well as clinical goals for those with a wide range of medical problems is described. Medical music therapy uses a range of music interventions. Differences between medical music therapy and other music practices in hospitals, e.g., music medicine are detailed. Results of systematic reviews published in the distinguished Cochrane Library are summarized. Recommendations for theory, practice, training, and research are provided as a means to advance medical music therapy in the future.

This quality improvement project was implemented to improve self-care in home health patients using video education provided on the tablet-based monitoring system. Heart failure is a chronic, progressive disease with high hospital readmission rates. However, consistent use of self-care behaviors improves patient quality of life and reduces hospital readmission rates in heart failure patients. Literature has shown that continued self-care education is essential for the success of the heart failure patient. The purpose of this study was to summarize the effects of physical activity (PA) as a nonpharmacological preventative measure in people at risk of Alzheimer's disease (AD) or all-cause dementia. It was hypothesized that persons at risk of AD or all-cause dementia who engage in sustained and continuous PA would fare better in terms of physical health, cognition, and activities of daily living (ADLs). Challenges are faced, however, when developing and implementing stimulating and meaningful activities for this population. Objective: MicroRNAs were identified as master-switch molecules limiting acute inflammatory response. This study investigated the potential role of microRNA (miR)-223 in the mechanism of gout.
Methods: Wild-type (WT) and miR-223 knock-out (KO) mice were used to evaluate the phenotypes of gout models. Inflammatory cytokines were measured in air pouch and peritoneal cavity lavage fluid. In addition to miR-223 level in gout patients, miR-223 and pro-inflammatory genes were examined in bone marrow-derived macrophages (BMDMs) from mice as well as peripheral blood mononuclear cells from healthy controls (HC) treated with monosodium urate (MSU) crystals in vitro.

Background and Objectives License inspection data have commonly been used as a quality measure for nursing homes but has not yet been used to assess the quality of assisted living/residential care (AL/RC) communities. Drawing on resource dependency theory, we test the hypothesis that structural and environmental characteristics influence AL/RC quality as measured by deficiency citations (“deficiencies”) issued during license inspections.
Research Design and Methods Using data from 526 licensed AL/RC communities in Oregon that received a license inspection visit between 2008 and 2016, we examined the prevalence of deficiencies by type and year. We estimated regression models to identify structural …

Most safety performance analysis employs cross-sectional and time-series datasets, posing an important challenge to safety performance and crash modification analysis. The traditional safety model analysis paradigm relying on observed data only allows relative comparisons between analysis methods and is unable to establish how well the methods mimic the true underlying crash generation process. Assumptions are made about the data, but whether the assumptions truly characterize the safety data generation in the real world remains unknown. The Health and Retirement Study is a longitudinal study that is conducted every other year in the United States and has served as a precursor to similar studies across the world, such as the Mexican Health and Aging Study. The purpose of the current project was evaluating the relationship between depressive symptoms, inflammation and physical activity levels using data from the HRS and MHAS studies.
