BACKGROUND: Proximal junctional kyphosis (PJK) and failure (PJF) remain challenging and incompletely predictable adverse event following adult spinal deformity (ASD) surgery. Machine learning (ML) models have been proposed to improve risk stratification, but performance varies and clinical applicability remains uncertain. A systematic review and diagnostic test accuracy meta-analysis was performed to synthesize the predictive performance of ML-based models for proximal junctional pathology after ASD surgery.

Background: This study evaluated medication utilization in children, adolescents, and adults with obsessive-compulsive disorder (OCD), a chronic psychiatric condition characterized by intrusive thoughts and repetitive behaviors. Although first-line treatments include selective serotonin reuptake inhibitors (SSRIs) and cognitive behavioral therapy (CBT), the heterogeneous biological underpinnings contribute to suboptimal outcomes, with 40-60% of individuals not responding to SSRIs. This complex phenotype often leads to psychotropic polypharmacy, which may be mitigated by incorporating combinatorial pharmacogenomic (PGx) testing into protocol-based care to identify potential gene-drug interactions.

X-linked adrenoleukodystrophy (X-ALD) is a progressive neurodegenerative disorder caused by a loss-of-function (LOF) mutation in the ATP-binding cassette subfamily D member 1 (ABCD1) gene, leading to the accumulation of very long-chain fatty acids (VLCFAs). This disorder exhibits striking heterogeneity; some male patients develop an early childhood neuroinflammatory demyelination disorder, while other patients, including adult males and most affected female carriers, experience a chronic progressive myelopathy. Adrenocortical failure is observed in almost all male patients, with the age of onset varying, sometimes being the first diagnostic finding. This study compares accessibility on the Izu Islands, an inhabited archipelago. part of Tokyo Prefecture, from around the 1910s to the 1930s, during the early years of tourism development. Coming from over 10 years of experience in cave monitoring in northeast Italy, Gruppo Speleologico Talpe del Carso, has designed Riki and Dolphin: customizable, low cost, and easy to assemble tools for getting real time data transmission from the bottom of a cave, even underwater, to a webserver. Their use has been tested to monitor air temperature inside the Abisso Bonetti Cave (Classical Karst, Italy), proving for the first time that a cave in Gorizian Karst can systematically be colder than the outdoor temperature even in winter, recording an internal temperature even lower than 0°C. Heart failure is one of the leading causes of hospitalization in the United States, with nearly 1 in 4 patients readmitted within 30 days of discharge. Clinical severity largely accounts for this risk, but social determinants of health (SDOH), including income, housing stability, transportation access, and health literacy, also play a role in post-discharge outcomes. This review synthesized seven cohort, observational, and qualitative studies from PubMed (2018–2025) examining the relationship between SDOH and 30-day readmission in heart failure patients.

Background: As ambient temperature rises, regions experiencing extreme heat face serious health concerns. This disproportionately affects lower socioeconomic status (SES) individuals in urban areas. This review examines how these “heat islands” affect the health outcomes for those in different socioeconomic statuses.

Background: Digital screens, such as smartphones, computers, and tablets, emit blue light in the short wavelength range of ~400–500 nm. This exposure impacts the suprachiasmatic nucleus, the brain's biological clock, which regulates the sleep-wake cycle via melatonin secretion from the pineal gland. This thesis examines how separation from—and later discovery of—an Indigenous family and culture shapes ethnic identity formation. Grounded in Charles Horton Cooley’s theory of the looking-glass self, it uses collaborative life history methodology to analyze the process of Indigenous identity formation experienced by Robert O’Brien, a Native American orphan who discovered his Mandan, Hidatsa, and Arikara (MHA) heritage as an adult. Drawing on two decades of reflexive research, unstructured interviews, participant observation, and archival documents, the case study shows how O’Brien’s sense of identity developed in response to shifting and often contradictory social mirrors.

Sickle cell disease (SCD) is a hereditary hemoglobin disorder characterized by vaso-occlusion and recurrent acute painful crises, which are the leading cause of hospitalization in affected individuals (1,2). Opioids remain the primary treatment for vaso-occlusive crisis (VOC) pain but are associated with adverse effects, tolerance, and risk of dependence, prompting investigation into opioid-sparing adjuncts (1,2). Intravenous ketamine, an NMDA receptor antagonist, has shown modest opioid reduction in randomized and observational studies, though pain score improvements are inconsistent (3–6). This project developed a new universal caregiver resource guide for caregivers within the Area Agency on Aging, in order to improve resource navigation and workflow efficiency. Resources were collected, verified, and organized into a new, streamlined guide via the ARIA chatbot, covering multiple needs. This study examines the correlation between taking a financial literacy class in high school and an individual’s actual financial literacy. It is inspired by the recent Kentucky House Bill 342, which mandates that a financial literacy class be taken during high school. According to recent studies, Generation Z scores the lowest on a financial literacy index among the past three generations. To address this, many U.S. states have introduced mandatory financial literacy classes in K-12 education. Multilingual learners in the United States consistently underperform in reading comprehension compared to monolingual peers, which is due to limited oral language proficiency and academic vocabulary. This academic achievement widens across grade levels, which emphasizes the need for instruction that integrates language development with literacy. This project designs an instructional model to support reading comprehension in upper elementary classrooms through explicit instruction, academic vocabulary, and structured oral opportunities. Substance use disorder (SUD) frequently co-occurs with depression and increases relapse risk and healthcare utilization. Although medication-assisted treatment (MAT) is an evidence-based intervention for SUD, outcomes for adults with co-occurring depression remain inconsistently reported due to variation in study designs, treatment settings, and measures. This scoping review examines evidence on MAT use among adults with co-occurring depression and SUD and summarized reported clinical and healthcare utilization outcomes.

Theory generation has long been subsumed under categories such as creativity, genius, and innovation. These categories do not distinguish assembly-based conceptual synthesis from fixed-point theory generation.

This paper makes that distinction explicit. The termination condition of assembly is external: data, citation, endorsement, usability. The termination condition of fixed-point theory generation is internal: consistency with internally held constraints, resolution of structural contradiction. The two operate under different processing conditions.

The historical record confirms this distinction. What Darwin, Einstein, Spinoza, and Kant produced was not assembly. Their processes involved unresolved branch retention and decomposition necessity, arriving at internally constrained fixed-point termination. Freud …

Chronic neuropathic pain (CNP) can be refractory to traditional pharmacologic therapies, giving rise to the investigation of non-invasive alternatives such as repetitive transcranial magnetic stimulation (rTMS). rTMS utilizes magnetic fields to modulate neuronal activity within specific brain regions, offering a targeted approach to analgesia without the side effects of systemic medication1,2,3,4.

Epilepsy affects approximately 1% of children in the United States, representing over 456,000 pediatric patients with active seizure disorders. Despite advances in antiseizure medications and surgical interventions, the benefits of these developments are not distributed equally across racial, socioeconomic, and geographic groups.
