BackgroundGypsy, Romany, and Traveller (GRT) populations experience poor oral health, with a recognised unmet need for care compounded by multiple barriers to accessing dental services. This scoping review aims to identify and map the available evidence, as well as gaps in the literature, relating to oral health improvement and access to care among GRT populations in Europe.MethodsStudies published in English relating to oral health improvement and/or access to care among GRT populations were included. Purpose of review: This review overviews the research base for obsessive-compulsive disorder (OCD) among three historically understudied populations; Latino/Latin Americans, Black/African Americans, and Sexual and Gender Minority individuals.
Recent findings: Findings suggest that while core OCD symptoms are broadly similar across groups, sociocultural factors such as discrimination, minority stress, stigma, family context, spirituality, and access to care significantly influence symptom presentation, severity, and treatment utilization. Recent studies have evaluated assessment tools and provided evidence supporting exposure and response prevention, while emphasizing the importance of culturally responsive assessment and treatment. This study stems from the persistent tension between legal certainty and justice and the need for an interpretative method capable of bridging the two. My research intends to bring Mark Fisher’s 2012 definition of Hauntology into conversation with the sci-fi journey in Tamsyn Muir’s Harrow the Ninth and its titular character. Using the lens of this Mythcon 54 to focus on the physical and parallel mental journey of Harrow, the physical journey traveled tracks and maps the “lost future” of the world’s inhabitants and questions the extent to which the physical road traveled can mask or hide its destructive inception. “The River” and road traveled in the novel are the result of catastrophic consumption and transformation of the Solar System by the novel's Emperor. …

Robert Heinlein, Grand Master of Speculative Fiction, was born in Butler, MO (one hour north of Route 66), raised in Kansas City, and lived much of his early adult life in Los Angeles, the westernmost terminus of Route 66. Given this proximity to the Mother Road, it’s no surprise that Heinlein spent a considerable portion of his career writing about various versions of the Road Trip, whether it was taken via foot, horse-drawn carriage, automobile or spaceship. COVID-19 and its ripple effects (e.g., social distancing, sport cancellations, quarantining) negatively affected the careers of collegiate student-athletes. The disruption to university systems and collegiate athletics and the stress and uncertainty felt globally likely increased international student-athletes' (ISAs) risk for distress. Yet, ISAs also possessed unique characteristics and were immersed within supportive sport environments, which may have shaped their psychological responses in distinct ways. The COVID-19 pandemic created an opportunity to understand how ISAs, as individuals navigating both athletic and international identities, responded and coped. Athlete migration is a visible element of the sport-globalization nexus. At the intercollegiate level, a new migration pattern has recently emerged. Numerous British universities are recruiting American postgraduate athletes for a variety of sports. International athlete migration has been examined using the cultural transition model (CTM) (Ryba et al., 2016). CTM features three phases that profile the developmental tracks migrant athletes experience in a new culture. This study used CTM to qualitatively explore the pre-transition experiences of American postgraduate athletes. As Tolkien studies has evolved, Middle-earth no longer needs to be defended as a legitimate academic subject. Today, Tolkien’s works are commonly taught as literary classics in colleges and universities alongside other mythic traditions such as The Odyssey and Beowulf. For students, however, the meaning of “classic” can be complicated. Many students encounter Tolkien long before entering college through popular media, films, games, and digital fandom, while the historical, linguistic, and cultural contexts that informed his writing (war, philology, faith, and medievalism, among others) can feel increasingly distant. In his seminal essay “On Fairy-stories” (1939), J.R.R. Tolkien introduces the concept of Faërian drama: experiences with a “realism and immediacy beyond the compass of any human mechanism,” where the viewer feels he is “bodily inside [a] Secondary World” but instead is “in a dream that some other mind is weaving.” This framing structure is used, though not called by this name, in many other texts. Kinship caregivers caring for children often lack access to services and supports they need to stabilize their families. Other kinship navigator programs have been shown to improve child permanency, child safety, child wellbeing, and adult wellbeing. This study evaluated Kinnections, a kinship navigator program provided by Wayfinder Family Services, operating since 2007 in seven counties in Northern California. Using propensity score matching, 116 kinship caregivers receiving Kinnections services were matched with 116 kinship caregivers in neighboring counties with no kinship navigator services available. The matched sample showed strong baseline equivalence on key caregiver, child, and household factors. Findings showed …

GrandFamilies: The Contemporary Journal of Research, Practice and Policy
This ethnographic study explores the caregiving experiences of grandparents in northern Vietnam who are raising grandchildren impacted by HIV/AIDS. Specifically, what is the process of skipped-generation caregiving in families affected by HIV/AIDS in Vietnam? Data were collected between 2009 and 2011 through participant observation and in-depth interviews with 26 grandparent caregivers and eight key informants. Informed by the Caregiving Career framework as a sensitizing concept and using constructivist grounded theory analytic strategies, the study identifies four interconnected caregiving phases shaped by cultural expectations, stigma, and intergenerational disruption. This improvement science dissertation seeks to address challenges in the effective implementation of a mastery-based instructional model in an open-enrollment K-12 charter school. Many students have demonstrated low motivation for completing formative learning activities, which are essential for personalized feedback and intervention. When students do not complete these activities, content mastery is hindered. This study utilizes improvement science to explore the root causes of the problem, evaluate the effects of previously used interventions, and test an intervention to improve student engagement. CONTEXT: Psychiatric comorbidity has been associated with worse perioperative outcomes and greater healthcare utilization after spine surgery.
AIMS: This study compared complications and healthcare utilization among posterior cervical decompression and fusion (PCDF) patients with depression, psychotic disorders, or no mental-health diagnosis.
SETTINGS AND DESIGN: Retrospective cohort analysis of the National Inpatient Sample from 2016 to 2022.
MATERIALS AND METHODS: The database was queried for elective adult PCDF encounters. Mental-health exposures were defined using Elixhauser indicators for depression and psychoses. Outcomes included medical complications, non-routine discharge, inpatient mortality, length of stay (LOS), and costs.

Motivation: Antibody therapeutic efficacy depends on high-affinity target engagement, yet laboratory affinity maturation is slow and costly. Most protein language models (PLMs) lack explicit training for high affinity, and current preference optimization methods introduce computational overhead without clear affinity improvements. Therefore, structure-aware and parameter-efficient approaches for antibody affinity optimization are urgently needed.
Results: We propose SimBinder-IF, a structure-aware antibody optimization model trained by freezing the Evolutionary Scale Modeling inverse folding (ESM-IF) structure encoder and fine-tuning only its decoder via Simple Preference Optimization (SimPO) to prefer stronger binders. Introduction: Dialysis patients undergoing anterior cervical discectomy and fusion (ACDF) face elevated perioperative risk due to complex comorbidities. This study aimed to (1) identify associations between dialysis status and perioperative complications after ACDF, (2) quantify differences in length of stay (LOS), hospitalization costs, and discharge disposition, and (3) provide a contemporary national benchmark for outcomes in this high-risk population. Materials and Methods: The 2016–2022 National Inpatient Sample was queried for adult elective ACDF cases. Dialysis dependence was identified through International Classification of Diseases-10 codes for end-stage renal disease with hemodialysis or peritoneal dialysis. Prolonged length of stay (LOS) in the acute care setting is a challenging issue driven by multiple factors. Increased LOS is associated with mortality, hospital-acquired infections, patient safety events, functional impairment, decreased quality of life, social isolation, and increased cost of care. The causes of increased LOS in the medical-surgical unit are unclear, necessitating an evidence-based project to address the problem. The purpose of this quality improvement initiative was to implement structured interdisciplinary barrier rounds and evaluate their effect on LOS, opportunity days, and estimated cost of care in a medical-surgical unit. What theory of constitutional interpretation has the Supreme Court followed? We search Supreme Court opinions for a wide range of sources that originalists identify as central to their methodology, over the entire universe of cases beginning in 1791. We find that for much of Supreme Court history, conventional indicators of originalist practice--citations to Founding-era sources--are largely absent, raising questions about originalism's past orthodoxy. Originalist citation practices are absent for much of the 19th century and grow sharply in the late 20th century. The Article elaborates the implications of these findings for debates in constitutional law.

Background: Allergic fungal rhinosinusitis (AFRS) is a severe subtype of chronic rhinosinusitis characterized by fungal hypersensitivity and accentuated type 2 inflammation. Dupilumab-a fully human mAb blocking IL-4/13, approved for type 2 inflammatory diseases, including chronic rhinosinusitis with nasal polyps-may benefit patients with AFRS.
Objective: We sought to understand the effect of dupilumab in AFRS.
Methods: In this phase 3 trial, patients with AFRS age ≥6 years were randomized to dupilumab or matched placebo for 52 weeks. The primary end point was Lund-Mackay computed tomography score, quantifying sinus opacification, at 52 weeks. Secondary end points were multiplicity-controlled and hierarchically tested.

Periodontitis, a chronic inflammatory disease affecting approximately 40% of U.S. adults aged 30 years and older, is characterized by dysbiosis of the dental plaque microbiome. However, although scaling and root planing (SRP) is the cornerstone of periodontal treatment, its effects on the taxonomic composition and functional potential of the dental plaque microbiome remain incompletely understood. In this study, we used whole-metagenome shotgun sequencing to characterize taxonomic composition and functional potential in dental plaque microbiomes collected from 39 patients with Stage II or III generalized periodontitis before and 3–4 months after SRP. Hospital-acquired infections (HAIs) remain a major cause of preventable morbidity, mortality, and healthcare utilization among hospitalized patients. Limited research has examined the relationship between diabetes and HAIs in Puerto Rican populations. The purpose of this quantitative cross-sectional study was to examine the prevalence of type 2 diabetes mellitus (T2DM) and its association with HAI characteristics and hospitalization outcomes among patients in southern Puerto Rico. The chronic care model provided a framework for evaluating whether diabetes status, geographic location, and substance use were associated with outcomes.
