Proper hand hygiene is crucial for preventing,, infections in intensive care units (ICUs) and neonatal intensive care units (NICUs). Despite the awareness among healthcare personnel and visitors of its importance, compliance with regulations is often inadequate. Advanced nurse educators and infection control professionals can significantly improve these habits. This review investigates whether advanced nursing positions improve hand hygiene among ICU and NICU families, as well as visitors, focusing on enhancing hand hygiene programs, identifying effective solutions, recognising practice shortcomings, and providing recommendations. Background/Objectives: Gene rearrangements involving oncogenes are major drivers in acute leukemia, influencing disease classification, prognosis, and therapeutic decision-making. Targeted RNA sequencing (RNA-Seq) panels capable of detecting intergenic and intragenic fusions across multiple genes are increasingly used in diagnostic settings. However, comparative evaluation with orthogonal technologies remains limited.
Material and Methods: We compared the performance of a 108-gene anchored multiplex PCR (AMP)-based RNA-Seq panel with that of Optical Genome Mapping (OGM) in 467 acute leukemia cases. Metastatic disease is the primary driver of cancer-related mortality. In prostate cancer (PCa), patient survival decreases dramatically once the disease spreads to distant organs, with over 80% of metastases occurring in the bone. This dissemination is often followed by a clinically silent period of tumor dormancy that can last for years, creating a critical reservoir of cells poised for lethal relapse. A significant barrier to understanding and targeting this dormant phase has been the lack of robust experimental models. Abstract This qualitative study examines the Family Language Policy (FLP) of second-generation Iranian American (SGIA) parents regarding the maintenance of their heritage language (HL), specifically Persian, within two U.S. contexts: Orange County, California, and Memphis, Tennessee. Based on Curdt-Christiansen and Huang’s (2020) dynamic FLP model and Bronfenbrenner’s (1979) ecological systems theory, the research examines parental language ideologies and the impact of perceived ethnolinguistic vitality of their communities on their language ideologies. Semi-structured interviews and background questionnaires with ten families (primarily mothers, with some fathers) revealed complex and hybrid language ideologies. MXenes have attracted considerable attention due to their tunable surface chemistry, high electrical conductivity, and ease of solution processing, making them promising candidates for a wide array of applications. The inherent tendency of MXenes to degrade under environmental conditions constrains their compositional diversity and limits certain practical applications. Our computational study shows that degradation of defect-free Ti3C2Txis kinetically limited, whereas common defects markedly lower the activation barriers for water attack. Valemetostat tosylate (valemetostat) is an oral, potent, dual inhibitor of enhancer of zeste homolog (EZH)2/1, under investigation in non-Hodgkin lymphomas (NHLs) and solid tumors. In vitro, it inhibits cytochrome P450 3A (CYP3A) and P-glycoprotein (P-gp) when combined with sensitive CYP3A or P-gp substrates. This drug–drug interaction (DDI) sub-study is part of the phase 1 trial of valemetostat monotherapy (DS3201-A-J101; NCT02732275), assessing the effect of valemetostat on the pharmacokinetics (PK) of sensitive CYP3A and P-gp substrates midazolam and digoxin, respectively, in patients with relapsed or refractory NHL, and its safety and efficacy. Introduction: Chagas disease (CD) - Trypanosoma cruzi infection - in people living with HIV (PWH), particularly those with advanced CD4 T cell depletion, can lead to severe syndromes affecting the central nervous system and the heart.
Methods: We performed a cross-sectional study to screen for CD among PWH in Iquitos, Peru, between October 2023 and May 2024, with the objective of understanding the frequency of infection in this population. Adults with confirmed HIV attending outpatient services at two regional hospitals were enrolled. Stem cell niches are essential for regulating stem cell self-renewal and differentiation during tissue repair and regeneration. However, the mechanisms supporting stem cell function in highly regenerative organisms, such as the freshwater planarian Schmidtea mediterranea, remain unclear. Using spatial transcriptomics, we identified two cell types associated with planarian stem cells: secretory cells we term "hecatonoblasts" and intestinal cells. Surprisingly, while hecatonoblasts were in close physical proximity to stem cells in the mesenchyme, they were dispensable for regeneration. In contrast, intestinal cells, despite lacking direct contact with stem cells, regulated both their position and function during regeneration. Electron microscopy revealed diverse …

Controlling stem cell differentiation is a longstanding goal in biomedical research. Here we explore how cell fate is influenced by RNA condensates, specifically P-bodies, which modulate gene expression posttranscriptionally. We profiled the transcriptomes of biomolecular condensates in diverse developmental contexts spanning multiple vertebrate species. Our analyses revealed conserved, cell type-specific sequestration of untranslated RNAs encoding cell fate regulators. P-body RNA contents do not reflect active gene expression in each cell type but are enriched for translationally repressed transcripts characteristic of the preceding developmental stage. Sirtuin 1 (SIRT1), an NAD+-dependent histone deacetylase, exerts complex and context-dependent effects in breast and gynecological cancers. By deacetylating histone and non-histone proteins such as p53, FOXO, and NF-κB, SIRT1 regulates essential processes including DNA repair, apoptosis, metabolism, and stress response. In breast cancer, SIRT1 may act as a tumor suppressor in early stages by maintaining genomic stability but promotes epithelial–mesenchymal transition, metastasis, and chemoresistance in aggressive subtypes such as triple-negative breast cancer. Similarly, in gynecological cancers, SIRT1 displays dual roles: promoting proliferation via estrogen signaling and p53/FOXO1 inhibition in Type I endometrial cancer yet potentially supporting DNA repair in …

Background: Parenting practices contribute to children’s eating and physical activity behaviors, and thereby their risk of obesity and associated medical conditions. Fathers may engage in different parenting practices than mothers. The objective of this study was therefore to establish psychometrically sound tools in English and Spanish for measuring Physical Activity Parenting Practices (PAPP) and Food Parenting Practices (FPP) for Hispanic and non-Hispanic fathers of diverse races for use in future studies, such as evaluations of family-based obesity interventions targeted at fathers.

CD8+ T cell exhaustion limits immune responses during cancer and chronic infection. We identify CD7 as a tissue-specific regulator of terminally exhausted CD8+ T cells during chronic infection. CD7 expression progressively increases during exhaustion, reaching its highest levels on a subset of CD101+Tim3low terminally exhausted cells that arise in the liver. Transcriptomic analysis revealed that CD7-deficient terminally exhausted cells display altered expression of co-stimulatory, translational, and effector genes, correlating with markedly reduced persistence and increased apoptotic susceptibility. Importantly, CD7 is preferentially upregulated on PD-1+CD39+ tumor-infiltrating lymphocytes (TILs) in human head and neck squamous cell carcinoma (HNSCC), suggesting CD7 may play …

Background: Real-world data on the benefit of high-dose inhaled corticosteroid (ICS) post-exacerbation are limited.
Objective: To investigate whether stepping-up to high-dose inhaled corticosteroid (ICS) post-severe exacerbation reduces future asthma exacerbation risk in a real-world setting.
Methods: This was a retrospective cohort study using the Optum® Clinformatics® database (October 2015-December 2023). Patients with asthma, aged ≥12 years, with ≥1 severe exacerbation and ≥2 ICS maintenance prescriptions in the 12-months pre-, and ≥1 ICS maintenance prescription in the 3-months post-exacerbation were included. Objective: Patient-reported outcomes (PROs) contain valuable information that can be leveraged by providers to perform timely interventions and improve quality of life and survival. However, the implementation of electronic PROs (ePROs) remains a challenge from technical and evaluation perspectives. Our objective was to construct a robust electronic health record (EHR)-integrated ePRO information infrastructure founded on RE-AIM (reach-effectiveness-adoption-implementation-maintenance) principles.

Introduction: The growing emphasis on patient-centered care, along with the rise of open-access publications and online platforms, has increased patient demand for education about their medical conditions. In this context, patient-friendly language summaries of fundamental cancer clinical trials may be valuable resources for patients to learn more about their disease and treatment options.
Methods: We surveyed patients diagnosed with non-small cell lung cancer (NSCLC) to assess the availability and need for easy-to-understand resources related to treatment decision-making and side effect profiles. Patients were also provided an example patient-friendly language summary and asked for feedback.

Generating Context-Aware Code-Switched Text Using Probabilistic Modeling And Generative Refinement, Mithun Kumar S R

Artificial intelligence models can experience difficulty understanding and generating code-switched text, and some existing synthetic data generation methods can produce grammatically incorrect or unnatural-sounding results. This disclosure describes a multi-stage system for generating code-switched text from monolingual corpora. The method can employ a probabilistic constraint model to analyze a source sentence and identify syntactically and semantically appropriate points for a language switch. Based on these identified points, a candidate generation engine can create one or more raw code-switched sentences. A large generative model may then refine these raw candidates to address potential grammatical errors at the language junction and improve …

P–12 District Equity “Shutdown”: Equity Directors’ Experiences Amid Anti-Equity Organizing, 2020–2022, Andrew Matschiner

This article examines local experiences of the national attack on district equity work intended to better support Students of Color and LGBTQ+ students over the 2020–2022 school years. Drawing on theory on zones of mediation and interviews with over 70 district “equity directors” (EDs) nationally, I analyze how together national, state, and local pressure led to district equity “shutdown.” Forms of district equity shutdown included censoring equity-focused communication and language, eliminating district equity-focused programming or personnel, and restricting books and learning resources, with 40% of EDs reporting at least one form of district equity shutdown.

Since 2022, The TMC Library started using Digital Commons Exhibits, adding 15k items to its 18yr institutional repository to include more library archival materials. Traffic of archival collections jumped from 4k, 36k to 107k downloads in 2024, bringing greater impact to not only archives, but the entire library community.
Other beneficial impacts have been identified since additional archival materials have been integrated within our institutional repository. Other than more people who have been accessing McGovern Historical Center using their services and materials, The TMC Library has noticed changes since more download traffic has been noticed on our institutional repository. To address the challenge of pile foundation waste slurry disposal, this study proposes a resource utilization approach based on alkali activation technology, enabling rapid solidification of the waste slurry into a flowable solidified soil suitable for engineering backfill. An orthogonal experimental design was employed to determine the optimal mix ratio of the solidifying agent, identified as cement:slag:fly ash = 0.5:0.35:0.15, with NaOH accounting for 6% of the total mass. Based on this, the mechanical and durability properties of the solidified soil were systematically investigated. This study was carried out to investigate the effects of three different crop rotation practices and three cycle periods on tomato seed yield and quality and some soil characteristics in organic farming conditions. The crop rotations included tomato as the primary crop along two intercrops (CR-1: tomato-leek-okra crop rotation, CR-2: tomato-head cabbage-lettuce crop rotation, CR-3: tomato-watermelon-onion crop rotation). This study aimed to explore the individual, institutional, and job-related factors influencing the job performance of technical staff in the Regional Forestry Directorates located in the Mediterranean Region of Türkiye. Data were collected through a mixed-method approach, including mostly face-to-face interviews, complemented by partially online questionnaires. Data were obtained from 650 technical staff, including regional directors, enterprise managers, section chiefs, and forest engineers. Structural equation modeling was applied, and variables were validated using confirmatory factor analysis. Model fit indices were within acceptable limits (e.g., RMSEA = 0.069, CFI = 0.838, GFI = 0.809). The results indicate that institutional factors had …

This study investigated the effect of reduced nitrogen (N) fertilizer dosage on the vegetative and generative potential, physical and chemical characteristics, and fruit yield of raspberry (Rubus idaeus L.). Two raspberry cultivars (Willamette and Meeker) were treated with low and high doses of calcium ammonium nitrate and urea over 3 consecutive years (2021–2023) in Western Serbia. Basic floricane traits and fruit quality parameters were analyzed using widely accepted methodologies. The results showed significant variations in primocane growth, floricane traits, and berry quality in response to different N fertilizer doses, whereas soluble solid content (SSC) was more stable. To accurately assess the impact of structural cracks on the strength of pavement structures and establish corresponding evaluation indicators based on deflection response, field tests were conducted on multiple sections of expressways in Jiangsu Province. A falling weight deflectometer was used to test a total of 15 measurement points within a 3-meter range on both sides of each crack. The center deflection values at each measurement point were plotted into curves, and three evaluation indicators for cracks, namely deflection range, maximum influence distance on one side of the crack, and influence area, were proposed based on the characteristics of the …
