Background

Breast cancer patients who undergo regional nodal irradiation therapy (RNI) following neoadjuvant chemotherapy and mastectomy or lumpectomy with axillary lymph node dissection (ALND) frequently encounter breast cancer-related lymphedema (BCRL). Early detection and treatment can significantly improve outcomes. Previously, near-infrared fluorescence lymphatic imaging (NIRF-LI) detection of dermal backflow, or retrograde lymphatic flow into initial lymphatics, was shown to precede clinical BCRL diagnosis by 8–23 months. Herein, we hypothesize that lymphatic pump failure is responsible for dermal backflow and may be a target to prevent onset of BCRL symptoms.

This article analyses the legal situation in Japan regarding the relaxation of or deviation from mandatory regulations in labour law through collective and individual agreements. Although many statutory labour laws in Japan have a mandatory effect over individual employment contracts, several statutory provisions provide that such mandatory regulations can be relaxed through collective agreements under certain circumstances, as is the case in other industrialised countries. The promotion of derogability clauses, which allow collective agreements to deviate from statutory protection to the detriment of employees, has been an issue in the political debate on German labour law for some 20 years. While such derogability clauses can effectively allow for flexibility at the company or sectoral level, there are differing views on their suitability for promoting collective bargaining. This article highlights the paradox of such derogability clauses: They are legitimised by the presumption of fairness that underpins collective autonomy; at the same time, they create incentives for employers that challenge the very presumption of fairness. Radiotherapy is a mainstay in the management of locally advanced lung cancer; however, intrinsic and acquired radioresistance contribute to poor prognosis. S6K1, a serine/threonine kinase, regulates cell growth, protein synthesis, and survival, and is increased in tumors, which is linked to enhanced survival under therapeutic stress, including radiation. The mechanisms, however, are not fully understood. This study investigates the role of S6K1 in lung cancer radioresistance and the mechanisms involved. Intrinsic radioresistance in lung cancer cells was associated with increased S6K1 activation. Importance: The management of patent ductus arteriosus (PDA) in preterm infants is controversial.

Objective: To determine whether expectant management compared with active treatment of a protocol-defined PDA in preterm infants decreases the incidence of death or bronchopulmonary dysplasia (BPD).

Design, setting, and participants: A randomized clinical trial including infants born at 22 to 28 weeks' gestation and diagnosed with a protocol-defined PDA between the age of 48 hours and 21 days at screening. Although higher education increasingly focuses on both community engagement and adult learners, community-engaged research rarely considers them together. This article bridges the gap in the literature between undergraduate adult learners and community engagement. Providing one example of a partnership that considers adult learners as necessary partners for community engagement, Calvin University’s 49507 Initiative is a place-based community educational project in southeast Grand Rapids, Michigan, focusing on the goals and needs of adult learners in the 49507 zip code and adjacent neighborhoods. Pancreatic ductal adenocarcinoma (PDAC) remains one of the deadliest malignancies, with a 5-year survival rate of just 13%. While the development and early clinical use of small molecules targeting oncogenic KRAS mutations, key drivers of PDAC, have shown promise, resistance to these targeted therapies remains a significant challenge. We recently identified Syndecan-1 (SDC1), a highly expressed heparan sulfate proteoglycan, as a critical KRAS effector protein that promotes nutrient salvage and tumor growth. Te self-assembly of enzyme proteins on 2D nanomaterials has enabled the construction and functional control of viable biochemical pathways. However, enzymatic cascades, which combine essential components of the photosynthetic and respiratory electron transport chains in tandem, have thus-far remained elusive. Herein, we have investigated a galvanic biohybrid nano system coupling photosystem I and cytochrome c oxidase on the surface of graphene oxide nanosheets in colloidal suspension. Te oriented immobilization of the enzymes was facilitated by Ni-coordination sites tethered to the carbon basal plane, with negligible parasitic O2 consumption. The American Society of Hematology (ASH) proposed the median time to first emergency department (ED) administration of pain medication for patients with sickle cell disease (SCD) vaso-occlusive pain episodes (VOE) as a site-level quality measure. Generalizable studies assessing current guideline adherence recommending pain medications within 60 minutes are lacking. We leveraged multisite electronic health record data from Epic's Cosmos research platform to analyze ED encounters for SCD VOE from 1 January 2019 to 31 December 2024, with administration of at least 1 pain medication. Triple-negative breast cancer (TNBC) exhibits heterogeneous treatment responses, yet molecular subtypes based on predefined biological pathways show limited prognostic value. We introduce tumor-specific total mRNA expression (TmS), a pathway-agnostic deconvolution metric derived from matched RNA/DNA sequencing, as a robust stratification tool. Analyzing 575 TNBC patients across Western and East Asian populations, TmS outperforms established subtypes in predicting chemotherapy outcomes, stratifying patients into high TmS with favorable prognosis and low TmS with poor prognosis. Stromal enrichment with immune exclusion emerges as a universal feature of chemotherapy-resistant low-TmS tumors across all cohorts. Liquefaction and cyclic softening of fine-grained soils pose a significant hazard during earthquakes, yet the cyclic behavior of transitional soils such as silts remains poorly understood. Existing empirical liquefaction evaluation frameworks and ground improvement techniques are largely developed for sands and are not directly applicable to silts, particularly beneath existing infrastructure where conventional mitigation methods are often impractical or cost prohibitive. This knowledge gap is critical in regions such as the Pacific Northwest, where young, loosely deposited silts are prevalent and susceptible to seismic loading.

Under the Individuals with Disabilities Education Act (IDEA), states establish their own autism spectrum disorder (ASD) eligibility criteria, resulting in substantial interstate variation and frequent misalignment with medical diagnostic standards. In 2019, Oregon revised its ASD eligibility criteria by directly incorporating DSM-5 diagnostic criteria, reflecting proponents' theory of action that this alignment would improve conceptual clarity, identification accuracy, and coherence across educational and medical systems. Bergmann glia (BG) are a specialized glial population essential for cerebellar development, yet their developmental timeline and molecular identity in the human cerebellum remain poorly understood. Here, we combined detailed histopathological analysis with spatial transcriptomics and single-nucleus RNA sequencing to generate a developmental atlas of human cerebellar BG. Histology revealed that BG emerge around 11 post-conception weeks (PCW), initially serving as a scaffold for Purkinje cells (PCs) migrating into the PC layer of the cerebellar cortex. As computing education expands globally, the dominance of English in programming languages can present barriers for students who are not native English speakers. This research work investigates whether language matters in programming by examining how a bilingual programming workshop influences student attitudes, perceived understanding and comfort. Sixty bilingual participants from San Jose State University, 40 with little to no prior programming experience and 20 experienced programmers completed a two-hour workshop using the multilingual Hedy platform. Pre- and post-surveys measured changes in confidence, enjoyment, motivation, identity, and usefulness, complemented by open-ended questions about participants’ experiences. Introduction: Treatment for childhood acute lymphoblastic leukemia (ALL) can result in hepatotoxicity. Despite being a common complication of ALL therapy, mechanisms and biomarkers of treatment-associated hepatotoxicity (TAH) are not well described.

Methods: We conducted lipidomic profiling to identify plasma lipids associated with TAH in children receiving ALL therapy utilizing a nested case-control framework. TAH was defined as (1) transaminitis: ALT/AST ≥ CTCAE grade 3, and/or (2) conjugated hyperbilirubinemia: > 3.0 mg/dL during induction therapy or > 2.0 mg/dL post induction. BACKGROUND: Community engagement is increasingly part of implementation research. However, some implementation scientists are less familiar with how to use community engagement approaches in implementation research. This hermeneutic review explores the gaps in community engagement within implementation research and examines community engagement approaches for implementation researchers.

METHODS: We conducted a hermeneutic review to synthesize information in the literature on community engagement in implementation research. The Relationship Between Mindful Awareness And Acculturative Stress For International Undergraduate Students In America, Nandini Narula Bajpai, Mary Stone

International Journal of Undergraduate Research and Creative Activities

This study examines mindful awareness and its role in acculturative stress experienced by international undergraduate students in American colleges. Data from 101 students from 37 countries attending 35 colleges revealed a significant negative correlation between mindful awareness and acculturative stress (r[99] = -0.51, p < 0.001). Higher mindful awareness predicted lower general acculturative stress and specific subdomains. These findings aim to refine cultural adaptation frameworks and inform evidence-based support systems for international students.
