Moderate to extreme preterm birth (< 32 weeks gestation) affects cardiopulmonary structure and function and is associated with increased risk of heart failure through adulthood. The rat hyperoxia (Hx) model (term born; postnatal Hx exposure) captures biventricular changes, including at the cell and organ scale, and pulmonary vascular remodeling seen in preterm humans. However, synthesizing these measures across scales and organ systems is challenging. We hypothesized that in silico modeling of biventricular mitochondrial, myofiber, and organ-scale function plus circulatory function could capture key features of cardiopulmonary abnormalities due to preterm birth. Therefore, we calibrated a multiscale model to subject-specific biventricular pressure–volume data previously obtained from Hx rats alongside normoxic (Nx) controls to investigate the abnormalities in cardiopulmonary function at multiple scales in this animal model of human preterm birth. The calibrated model demonstrates excellent agreement with the data and captures the expected pulmonary vascular changes and right ventricular dilation seen in preterm-born children. Our multiscale modeling approach captures cardiopulmonary abnormalities across spatial scales and provides an innovative approach to explore the consequences of preterm birth beyond preclinical experimental data alone. This is a foundational step in understanding the impact of preterm birth on cardiopulmonary disease in childhood as well as adulthood.

Autism spectrum disorder (ASD) is a multifaceted neurodevelopmental condition that affects an estimated 1 in 31 children in the United States. ASD frequently co-occurs with a wide range of developmental, emotional, sensory, and medical conditions necessitating a multifaceted and coordinated approach to care. In this paper, we describe the programmatic evaluation of a transformation of an applied behavior analysis (ABA) focused multidisciplinary therapy clinic into an interdisciplinary ASD therapy center. Using a quality improvement Plan-Do-Study-Act framework, we outline our process for building integrated teams, cultivating interdisciplinary collaboration, and enhancing respect among disciplines. High Al-content AlxGa1-xN (0.7 < x < 1) quasi-vertical Schottky barrier diodes (SBDs) with distributed polarization doping were grown on the bulk AlN substrate. They exhibit excellent rectification behavior with a large forward current density (~14 kA/cm2 ) and a high breakdown field of ~8.3 MV/cm. The SBDs also exhibited low ideality factors of (n~1.2) with a high Schottky barrier height (Φ b~ 1.7 eV). Thus, this study demonstrates the feasibility of the distributed polarization doping approach for high current–high voltage devices.

Children of mothers with childhood trauma are at an increased risk for mental health problems, yet the most influential family-related mediators of this intergenerational transmission remain unclear. This systematic review and meta-analysis synthesized effect sizes from 29 studies (n = 352,279 dyads) examining psychosocial family-level mediators between maternal childhood trauma and child mental health. We identified 380 simple and 40 serial mediation paths across four domains: child characteristics, maternal characteristics, parent–child relationship factors, and household characteristics. Maternal characteristics showed the largest pooled effects (proportion median [PM]: 31.2%; ratio median [RM]: 0.21), particularly maternal mental health (PM: 36.0%), maternal attachment style …

In 2016, Hillary Clinton was the first woman to gain the presidential nomination from a major political party in the United States, yet she was unsuccessful. The current study explores barriers to being elected as president for women generally and Hillary Clinton specifically. Using the propositions and tools of affect control theory, we demonstrate how women’s political representation shapes cultural sentiments about women and the president. In a nationwide sample of Americans surveyed shortly before the election, we find women’s representation on the state level influences voter preferences through these cultural sentiments: More women in politics makes a woman president …

The United States Department of Defense and Bureau of Labor Statistics estimated that female veterans and nurses experience work-related trauma at rates of 51% or higher. While many studies have examined their mental health outcomes, posttraumatic worsening of physical health has been less studied in this population. This generic qualitative inquiry was conducted to gain a deeper understanding of the subjective lived experiences of female nurses and veterans' work-related traumatic exacerbation of an existing respiratory disorder. The dramatic rise in ultra-processed food (UPF) consumption worldwide has sparked growing concerns about its impact on cancer development and human health. UPFs contain high levels of added sugars, unhealthy fats, chemical additives, dyes, and toxic compounds formed during cooking and processing. This study investigates the relationship between UPF consumption and cancer through a review of primary and secondary sources, alongside an observational survey conducted among 15 healthcare professionals, including physicians, nurses, and nursing educators. Survey findings indicate that 53% of participants observed higher cancer prevalence in patients consuming large amounts of UPFs, with colorectal cancer most frequently reported. The …

OBJECTIVE: To evaluate the clinical utility of combining artificial intelligence (AI) with handheld focused cardiac ultrasound (FoCUS) performed by noncardiologist physicians in clinical care settings.
PATIENTS AND METHODS: In this prospective, single-arm study conducted from July 1, 2022, through December 31, 2023 (ClinicalTrials.gov NCT05455541), 660 adult patients presenting to the emergency department or internal medicine wards were assessed with handheld ultrasound devices enhanced by AI algorithms. These algorithms provided automated analysis of ventricular function, valvular disease, pericardial effusion, and inferior vena cava size. Background: Advancements in medical technology, particularly in perfusion practices such as ECMO and heart-lung bypass, have improved patient care but also introduced novel risks. Technological errors—ranging from hardware malfunctions to software failures and AI model inaccuracies—can directly threaten patient safety.
Objective: To identify current trends and risks associated with technology in perfusion practices and synthesize evidence on the frequency, nature, and outcomes of technology-related errors.
Results: Evidence indicates substantial deficiencies in medical devices, predictive models, and health IT systems. AI-based predictive models frequently failed to detect critical health deterioration, missing 66% of injuries in some in-hospital mortality simulations. Adverse events …

BACKGROUND: Deep brain stimulation (DBS) is an evolving technique for the treatment of various psychiatric disorders refractory to conventional pharmacotherapy. T-cell acute lymphoblastic leukemia (T-ALL) is an aggressive hematologic malignancy in which activating mutations in the Notch pathway are thought to contribute to transformation, in part, through activation of c-Myc. Increased expression of c-Myc induces oncogenic stress that can lead to apoptosis through the Mdm2-p53 tumor suppressor pathway. Since the great majority of T-ALL cases maintain wildtype MDM2 and TP53, we hypothesized that T-ALL cells are selectively sensitive to MDM2 inhibition. Treatment with idasanutlin, an MDM2 inhibitor, although only inducing modest apoptosis in T-ALL cells, resulted in a significant upregulation of pro-apoptotic BH3 domain genes BAX and BBC3. The cardiovascular system is generated by mesodermal progenitors that give rise to endothelial, hematopoietic and muscle (smooth muscle and cardiomyocyte) lineages. Although vascular endothelial growth factor (VEGF), its receptor, FLK1/KDR and the transcription factor ETV2 are critically required for hemangiogenic specification, Flk1 and Etv2 expression has been observed in other mesodermal derivatives. During gastrulation, mesodermal progenitors are specified to contribute to the cardiovascular system; however, the early divergence of hemangiogenic and muscle progenitors, and their shared marker expression, complicates identification of common mesoderm progenitor populations contributing to both lineages. The rapid increase in smartphone use over the past decade led to prolonged screen time across age groups, particularly during nighttime hours. Exposure to artificial light and pre-sleep digital activities was associated with circadian rhythm disruption and alterations in individual chronotype. This systematic literature review aimed to synthesize empirical evidence on the relationship between smartphone screen time duration and individual chronotype across all age groups and to identify key moderating factors influencing this association. In this paper, we study the convergence behavior of the diffuse domain method (DDM) for solving a class of second-order parabolic partial differential equations with Neumann boundary condition posed on general irregular domains. The DDM employs a phase-field function to extend the original parabolic problem to a similar but slightly modified problem defined over a larger rectangular domain that contains the target physical domain. Background Antiplatelet therapy is widely prescribed for the prevention and management of cardiovascular disease and for mitigation of thromboembolic risk. However, its impact on post-operative periprosthetic joint infection (PJI) following total shoulder arthroplasty (TSA) remains poorly defined.MethodsA retrospective cohort study was conducted using the TriNetX US Research Network. Propensity score matching was performed to balance demographics, body mass index, and Charlson Comorbidity Index variables. The primary outcome was the incidence of PJI at 90 days, 6 months, and 1 year. Secondary outcomes included post-operative medical and surgical complications.
