Septoplasty Without Packing Or Splints: Our Experience, Jafar Fawzi Al Badran, Huda Hassan Al Abboodi
Septoplasty Without Packing Or Splints: Our Experience, Jafar Fawzi Al Badran, Huda Hassan Al Abboodi
Iraqi National Journal of Medicine
Background: Septoplasty is a common surgical procedure performed in otorhinolaryngology departments to correct nasal septum deviation. Traditionally, nasal packing or splints used to stabilize the septal mucosal flaps and promote adhesion. However, these can cause complications such as nasal obstruction until removal, dry mouth, watering of eyes, severe pain during pack removal, headache, difficulty swallowing, and hypoxia. Methods: This prospective longitudinal observational study conducted on 50 patients undergoing septoplasty for nasal obstruction without the use of nasal packing or splints. This study included 50 patients who presented to the ENT department with nasal obstruction over one year (October 2021 to …
Alternate Day Oral Iron Replacement Versus Daily Intake In Women With Iron-Deficiency Anemia In Basrah, Muntadar Jafar Mawla, Mohammed A. Al-Anssari, Qutaiba M. Dawood, Murtadha A. Jeber, Asaad A. Khalaf
Alternate Day Oral Iron Replacement Versus Daily Intake In Women With Iron-Deficiency Anemia In Basrah, Muntadar Jafar Mawla, Mohammed A. Al-Anssari, Qutaiba M. Dawood, Murtadha A. Jeber, Asaad A. Khalaf
Iraqi National Journal of Medicine
Background: Iron-deficiency anemia (IDA) remains a major global health concern, disproportionately affecting women of reproductive age. Conventional daily iron supplementation, though effective, is often associated with poor adherence due to gastrointestinal side effects. Emerging evidence suggests that alternate-day dosing may enhance iron absorption and tolerability, but comparative efficacy data in clinical settings remain limited. Aim: This study aimed to compare the hematological and iron store responses of women with IDA in Basrah, Iraq, between alternate-day and daily oral iron supplementation regimens. Methods: A total of 97 women with confirmed IDA (hemoglobin [Hb] < 12 g/dL, serum ferritin < 15 ng/mL) were enrolled in a prospective follow-up cohort study. The participants were divided into two groups: Group B (n = 48) received 100 mg of elemental iron daily, while Group A (n = 49) received the same dose every other day. Serum ferritin and hematological parameters (Hb, red blood cell [RBC] count, hematocrit) were assessed at baseline, three weeks, six weeks, and three months. ANOVA and paired t-tests were used for statistical analysis, with a significance level of P < 0.05. Results: Compared to the daily group, the alternate-day group exhibited a significantly higher mean increase in serum ferritin (28.6 ± 4.2 ng/mL vs. 22.3 ± 3.8 ng/mL, P = 0.001) and hemoglobin (2.8 ± 0.5 g/dL vs. 2.1 ± 0.4 g/dL, P = 0.001) at three months. The alternate-day group demonstrated earlier and more sustained improvements in iron reserves (P < 0.01 at all follow-ups), but both regimens increased RBC counts. Conclusion: In women with IDA, alternate-day iron supplementation is more effective in restoring hemoglobin and iron stores than daily dosing, possibly due to improved absorption and reduced hepcidin inhibition. These findings suggest potential benefits for patient adherence and clinical outcomes, supporting the use of alternate-day regimens as a superior therapeutic approach. To validate these results, further randomized controlled trials are necessary.
Central Cervical Cleft: A Very Rare Case Of Cervical Congenital Malformation, Wael J. Alshihaby
Central Cervical Cleft: A Very Rare Case Of Cervical Congenital Malformation, Wael J. Alshihaby
Iraqi National Journal of Medicine
Central cervical cleft is an rare congenital midline anomaly of the neck, typically presenting as a vertically oriented skin defect accompanied by a subcutaneous fibrous band and, in many cases, an associated skin tag. This case report presents a case involving a five-year-old female who exhibited a midline cervical lesion with intermittent clear discharge but no associated pain or functional impairment. Clinical examination identified a characteristic cutaneous defect with a prominent skin tag in the midline of the anterior neck. Radiological evaluation, including ultrasound and CT scan, confirmed the localized nature of the anomaly and excluded deeper structural involvement. After …

The integrated stress response (ISR) is an adaptive pathway hijacked by cancer cells to survive cellular stresses in the tumor microenvironment. ISR activation potently induces PD-L1, leading to suppression of antitumor immunity. In this study, we sought to uncover additional immune checkpoint proteins regulated by the ISR to elucidate mechanisms of tumor immune escape. The ISR coordinately induced cluster of differentiation 155 (CD155) and PD-L1, enhancing translation of both immune checkpoint proteins through bypass of inhibitory upstream open reading frames in their 5' untranslated regions. Analysis of primary human lung tumors identified a significant correlation between expression of PD-L1 and …

Diffuse midline glioma (DMG) is a devastating pediatric brain tumor. The oncolytic adenovirus Delta-24-RGD has shown promising efficacy and safety in DMG patients but is not yet curative. Thus, we hypothesized that activating dendritic cells (DCs) through the CD40 costimulatory receptor could increase antigen presentation and enhance the anti-tumor effect of the virus, resulting in long-term responses. This study shows that the intratumoral co-administration of Delta-24-RGD and a CD40 agonistic antibody is well tolerated and induces long-term anti-tumor immunity, including complete responses (up to 40%) in DMG preclinical models. Mechanistic studies revealed that this therapy increased tumor-proliferating T lymphocytes and …

A previously reported clinical trial in familial adenomatous polyposis (FAP) patients treated with erlotinib plus sulindac (ERL + SUL) highlighted immune response/interferon-γ signaling as a key pathway. In this study, we combine intermittent low-dose ERL ± SUL treatment in the polyposis in rat colon (Pirc) model with mechanistic studies on tumor-associated immune modulation. At clinically relevant doses, short-term (16 weeks) and long-term (46 weeks) ERL ± SUL administration results in near-complete tumor suppression in Pirc colon and duodenum (p < 0.0001). We identify a low-dose threshold for significant antitumor activity in Pirc rats given SUL at 125 ppm in the diet plus ERL at 5 mg/kg body weight via twice-weekly oral gavage (SUL125 + ERL5 × 2). Longitudinal analyses show diminished expression of MHC class I and II genes in polyps larger than Grade 5, a novel finding in the Pirc model. Treatment with ERL ± SUL upregulates the corresponding MHC and immune-associated factors in a subset of Pirc colon polyps, Pirc tumor cell lines, murine colon carcinoma cells, and FAP patient-derived organoids, with Nlrc5 playing a critical role in this effect. Imaging mass cytometry reveals that SUL125 + ERL5 × 2 increases tumor-associated Cd4+ T cells by ~2.6-fold (p < 0.05), with no apparent effect on Cd8+ T cells. The treatment also increases tumor-associated Cd68+ cells (p < 0.05) and decreases Foxp3+ (p < 0.01) and Arg1+ (p < 0.05) cells. Thus, intermittent low-dose ERL + SUL treatment enhances tumor-associated MHC expression and remodels the immune cell niche toward a more permissive "helper" immune microenvironment. We conclude that early immune-interception strategies targeting interferon-γ signaling may benefit FAP patients at drug doses below the clinical standard of care.
Can Mere Possession Of A Consumer Product Boast Your Self-Efficacy? : A Cultural Perspective, Yixuan Gao
Can Mere Possession Of A Consumer Product Boast Your Self-Efficacy? : A Cultural Perspective, Yixuan Gao
Lingnan Theses (MPhil & PhD)
Beggan (1992) introduced the mere ownership effect, suggesting that people evaluate owned objects more positively. Beggan argued that ownership fosters a psychological bond between people and their possessions, driven by self-enhancement motives (e.g., “I am good, thus my object must be good”). Yeung et al. (2017, 2020) extended the mere ownership effect, showing that ownership influences self-evaluations (e.g., I own a book so I am knowledgeable). Previous studies on the “person-based” mere ownership effect have primarily focused on East Asian participants. This thesis aims to compare cross-cultural differences, if any, in the “person-based” mere ownership effect (Study 1), and systematically …

As a key component of the traveling system of high-speed trains, the axle is crucial for its safe operation. The current research on the fretting wear and fatigue development of shafts suffers from the problems of low local simulation accuracy of the wear model and the lack of detection and validation methods for the dynamic expansion of wear and fatigue. To this end, this study firstly proposes a new node-improved form of energy dissipation wear model, which is more sensitive to the contact behavior of the asperity body in the overfilled region and the energy transfer process; it exhibits wear …

Spectroscopic observations are essential for confirming associations, measuring kinematics, and determining stellar populations in dwarf galaxies. Here, we present Keck Cosmic Web Imager spectra for 12 MATLAS survey dwarfs. For 9, we confirm recession velocities consistent with their literature-assumed host galaxies. We propose revisions of the host galaxy associations for MATLAS-631, 1494, and 1938. For MATLAS-1494, our measured redshift reclassifies it from an ultra-diffuse galaxy candidate to a dwarf galaxy that is of smaller physical size and places it in the field. It also appears old and passive, providing a challenge to models that invoke quenching by tidal effects. Additionally, …
With Vim And Courage: Confederate Soldiers And The Politics Of War, Michelle N. Norello
With Vim And Courage: Confederate Soldiers And The Politics Of War, Michelle N. Norello
LSU Master's Theses
The Confederate cause has captivated historians for many decades. At first glance, the Confederacy embodies a persistent effort to promote white supremacy, the planter class, and independence. While this assessment is accurate, individual Confederate soldiers possess diverse narratives that both challenge and support the labels assigned over the years. The political dimension of the Civil War opens new avenues for scholarly exploration concerning Confederate soldiers. But what insights might this offer regarding the nature of the Confederacy, civilian political engagement, and democracy? The answer to this question is complex and nonlinear. Previous research has acknowledged the numerous reasons why Confederate …

Objective: This study aimed to investigate serum metabolomic biomarkers associated with incident type 2 diabetes mellitus (T2DM) and evaluate their performance in improving T2DM risk prediction.
Methods: Untargeted proton nuclear magnetic resonance (1H NMR) spectroscopy-based metabolomics analyses were conducted in the Multi-Ethnic Study of Atherosclerosis (MESA; n = 3460; discovery cohort) and The Rotterdam Study (RS; n = 1556; replication cohort). Multivariable cause-specific hazards models were used to analyze the associations between 23 571 serum metabolomic spectral variables and incident T2DM. Replicated metabolites required an false discovery rate-adjusted P < .01 in MESA, P < .05 in RS, and consistent direction of association. Pathway and network analyses were conducted to elucidate biological mechanisms underlying T2DM development. The utility of the replicated metabolites in improving T2DM risk prediction was assessed based on the Framingham Diabetes Risk Score. A 2-sample Mendelian randomization was conducted to assess causal associations.
Results: Nineteen metabolites were significantly associated with incident T2DM. Pathway analyses …

South Asian individuals (SA), representing approximately one quarter of the global population, experience a disproportionately high burden of cardiovascular disease. Some of this increased susceptibility is accounted for by traditional risk factors such as diabetes, hypertension, carbohydrate-rich diets, and rising rates of obesity and metabolic syndrome. However, other previously underappreciated risk factors may also play a crucial role. These include environmental pollution, genetic factors, and Lp(a) (lipoprotein(a)). Various epidemiological and genetic studies support the role of Lp(a) as a causal and independent risk factor for atherosclerotic cardiovascular disease. SA have a higher prevalence of elevated Lp(a) levels (25% have levels …

A Systems Engineering Approach To Improve Alarm Management And Nurse Mental Workload, Benjamin D. Elmore
LSU Doctoral Dissertations
Alarm fatigue has been a consistent problem nurses have experienced for decades. The effects of excessive nuisance alarms leads to adverse effects such as desensitization, missed alarms, frustration, and increased risk to patient safety. Other research has provided explanations on alarm fatigue causes and developed solutions for reducing nuisance alarms or minimizing effects, but few have examined the problem from a sociotechnical systems perspective. This study applied the Systems Engineering Initiative for Patient Safety (SEIPS) framework to define, design, and test an alarm management intervention compatible with a Progressive Care Unit (PCU)’s sociotechnical subsystems. The SEIPS application is unique from …

LSU Doctoral Dissertations
Electrocatalytic carbon dioxide (CO2) reduction to C2+ products holds promise for low carbon intensity fuels, chemicals, and food; however, durability challenges, particularly bicarbonate (HCO3-)and carbonate (CO32-) salt precipitation, hinder their commercialization. This report considers water (H2O) and ion transport properties of conventional anion exchange membranes (AEMs) in zero-gap membrane electrode assembly (MEA) configurations using Cu electrocatalysts. Transport characterization includes H2O diffusion, permeability, potassium (K+) diffusion, and transference numbers using three commercial AEMs, including piperidinium-based (Versogen), quaternary ammonium-based (Fumasep), and imidazolium-based (Dioxide Materials) membranes. Accelerated lifetime …

The Philosophical Foundations Of The Civil-Military Relationship, John Mark Mattox
The Philosophical Foundations Of The Civil-Military Relationship, John Mark Mattox
Joint Force Quarterly
This article reexamines civil-military relations through a philosophical lens, arguing that they are far more complex than simple military subordination to civilian authority. Drawing on foundational models from Hobbes, Plato, medieval Christendom, and Clausewitz, it reveals a recurring tripartite structure of society: rulers, warriors, and citizens bound together by mutual responsibility and moral obligation. From Plato’s emphasis on virtue and justice to Clausewitz’s "paradoxical trinity" of people, military, and government, the article shows that the strength of any state depends on trust and collaboration among all three. True civil-military relations, it concludes, must be understood as a dynamic moral partnership …

This article evaluates the modernization of the People’s Liberation Army Air Force (PLAAF) from 2007 to 2025 using the Saunders-Quam framework, which identifies four core trade-offs: changes in roles and missions, domestic versus foreign procurement, high-tech versus low-tech systems, and combat versus support aircraft. While the framework remains a useful analytical tool, it underestimated the Chinese Communist Party’s willingness to fund expensive modernization efforts and the rapid advancements in China’s defense industrial base. The article identifies key factors missing from the original framework such as doctrinal shifts, increased emphasis on counterintervention, maritime expansion, and the rise of unmanned systems. It …
Is Mobilization A Major Question?, Evan J. Ward
Is Mobilization A Major Question?, Evan J. Ward
Joint Force Quarterly
The combination of the Supreme Court’s new Major Questions doctrine and its decision in Loper Bright v. Raimondo threatens to hobble the Executive Branch’s ability to effect a major mobilization under the Defense Production Act. The Defense Production Act followed the War Powers Acts of WWII to consolidate and update the presidential powers that were so famously marshaled to mobilize the arsenal of democracy. The Department of Defense has long planned to rely on Defense Production Act authority to mobilize should the need arise, but the risk of litigation brought about by new legal developments means that the authorities the …

Joint Force Quarterly, Issue 118, 3rd Quarter 2025, National Defense University Press
Joint Force Quarterly
This issue of Joint Force Quarterly addresses key strategic, operational, and doctrinal questions shaping the future of the joint force. Articles examine mobilization as a strategic imperative, PLA Air Force modernization, Southeast Asia’s geographic role in U.S. defense strategy, and leader development for strategic lethality. Contributions also explore AI’s role in detecting deepfakes, the civil-military relationship, and innovations in military logistics and cyber force design. A retrospective on the Joint Warfighting Concept and critical reviews of recent defense literature provide context and continuity. Collectively, these pieces offer insight into how the joint force must evolve to meet emerging challenges.

Detecting geometric road anomalies in digital maps at a large scale can present a challenge, as some conventional methods may exhibit limitations in precision and result in a higher rate of false positives. A method and system are described that can identify these anomalies by simulating a vehicle's traversal of a given road segment to assess its kinematic feasibility. The model can analyze the change in the road centerline's bending energy before and after lane rendering and can predict the probability of a vehicle spin-out based on lateral acceleration. This approach may allow for accurate, scalable detection and quantification of …

On-Product And On-Advertisement Encoding For Visual Product Identification, Tal Cohen, Praveen Krishnakumar
Defensive Publications Series
This disclosure describes a method for on-product and on-advertisement encoding for visual product identification. Visual search technologies can have difficulty distinguishing between product models that are highly similar or visually identical, which may lead to incorrect identifications and a suboptimal user experience. The described method involves embedding a unique product identifier, like a Global Trade Item Number (GTIN), onto a physical product or into its advertisements through an invisible watermark. This watermark is composed of subtle, algorithmically detectable variations in color or pattern that are imperceptible to the human eye, such as stripes of nearly identical shades on a fabric. …
Verifying Ownership Of Online Profiles And Web Sites For Creators And Publishers, Nick Krasney, Yi Li, Govind Kaushal
Verifying Ownership Of Online Profiles And Web Sites For Creators And Publishers, Nick Krasney, Yi Li, Govind Kaushal
Defensive Publications Series
Verifying that a single creator or publisher controls multiple disparate online properties, such as websites and social media profiles, can be a challenge. Existing methods may rely on bilateral agreements between platforms, which may not be scalable and can create friction. This disclosure describes a decentralized cryptographic method for verifying ownership. A creator may generate a unique public-private key pair and can use the private key to sign the canonical URL of each online property, creating a digital signature. The creator can then provide the public key and the corresponding digital signature to each platform, which may embed this information …
Techniques For Detecting Contraflow On Divided Roadways And Adjusting Vehicle Routes, Daniel Klein
Techniques For Detecting Contraflow On Divided Roadways And Adjusting Vehicle Routes, Daniel Klein
Defensive Publications Series
Navigation systems may encounter routing anomalies when one side of a divided highway is closed, for example for construction, and traffic is temporarily rerouted into a two-way flow on the remaining side. In such situations, detecting the temporary traffic pattern can allow a navigation system to avoid providing erroneous instructions, such as "make a U-turn" or undesirable detours. A disclosed method relates to using aggregated geospatial data, such as Global Positioning System (GPS) data, and traffic flow data to detect these temporary traffic patterns. A system can identify anomalies such as a threshold number of vehicles traveling on the "wrong" …

Indigenous communities have endured the long-standing impacts of U.S. Federal Indian boarding school policy. The history of Indigenous boarding school policies and the underlying intent to enact cultural genocide is unknown and unacknowledged. The perspectives of Indigenous Peoples have been excluded from dominant narratives, and until recently, from heritage site programming, leading to intergenerational trauma, fragmented identities, and the misrepresentation of Indigenous cultures and experiences. This basic qualitative study asked the question: When the four-truth framework, developed by the South African Truth and Reconciliation Commission, is applied to U.S. Federal Indian boarding school policy, Native American boarding school history, and …
