Importance
Fibromyalgia (FM) is a chronic pain condition associated with substantial quality-of-life impairments and economic burden. Although multiple pharmacologic options are recommended in clinical guidelines, the relative cost-effectiveness of off-label amitriptyline compared with FDA-approved treatments remains poorly defined due to a scarcity of direct economic comparisons and the use of heterogenous outcome measures in the extant literature.
Objective
To evaluate the cost-effectiveness of pregabalin, duloxetine, and milnacipran compared with amitriptyline in adults with moderate to severe FM.

Ai-Guided Hemodynamic Management In Non-Emergency Cardiac Surgery, James P. Warren, Maria Ledbetter

This scholarly project examines the role of artificial intelligence (AI)–guided hemodynamic management in reducing intraoperative hypotension during non-emergent cardiac surgery. Intraoperative hypotension, defined as mean arterial pressure (MAP) below 65 mmHg, is a well-established risk factor for myocardial injury, acute kidney injury, and increased postoperative morbidity and mortality. Despite vigilant monitoring, traditional clinician-directed management remains largely reactive, intervening only after hypotension has occurred.
The project centers on a clinical case involving a 67-year-old male undergoing elective on-pump coronary artery bypass grafting, during which several transient hypotensive episodes occurred despite standard anesthetic care. These events served as a catalyst for an …

Sugammadex is commonly used to antagonize neuromuscular blockade and has a clinically significant interaction with hormonal contraceptives by binding to plasma progesterone, reducing contraceptive efficacy and mimicking a missed dose. This case involved a healthy 21-year-old woman undergoing elective anterior cruciate ligament and medial collateral ligament repair whose only home medication was an oral contraceptive. She received midazolam prior to anesthesia assessment, limiting her ability to retain education, and counseling occurred at a suboptimal time without a support person present. Postoperative education was verbally relayed without standardized documentation or accountability. This project examined whether implementation of a standardized protocol regarding …

The Business Bankruptcy “Big 3” And The Unanticipated Benefits Of Subchapter V, Marshall V. Ringwood

The liberal bankruptcy venue rules in the United States have their defenders and advocates. Subchapter V of the Bankruptcy Code came into effect in 2020, justified as a bipartisan solution to a longstanding problem in corporate bankruptcy where restructuring under Chapter 11 was prohibitively expensive for small-business debtors. On June 21, 2024, Subchapter V’s extended debt limit of $7,500,000 in liabilities reverted back to a statutorily defined $3,024,725. In addition to the justifications offered by organizations such as the American Bankruptcy Institute (ABI) for both Subchapter V, generally, and a permanent increase to its debt limit, I argue that Subchapter …

Background: Fenestrated and branched endovascular aneurysm repair can be complicated by branch vessel occlusion in the absence of structural stenosis. We hypothesized that computational flow simulation could identify adverse hemodynamic features associated with postfenestrated and branched endovascular aneurysm repair branch occlusion.
Methods: Patients undergoing 4-vessel fenestrated and branched endovascular aneurysm repair for Extent II to IV thoracoabdominal aortic aneurysms were retrospectively reviewed. Branches that occluded without identifiable kinking or stenosis on computed tomography were included, along with an equal cohort of anatomy-matched patent controls. Patient-specific pulsatile rigid-wall simulations were performed using SimVascular with individualized geometries and boundary conditions. Abnormal time-averaged …

KBG syndrome is a rare autosomal dominant disorder characterised by developmental delay, characteristic facial features, macrodontia and skeletal anomalies, caused by mutations in the ANKRD11 gene. We report a 5.5-year-old Moroccan boy who presented in 2022 to a tertiary military teaching hospital in Rabat, Morocco, with psychomotor delay, autistic traits, epilepsy, bilateral hearing loss with chronic otomastoiditis and radiologically-detected macrodontia before clinical eruption, in whom artificial intelligence-assisted facial phenotyping suggested the diagnosis, subsequently confirmed by identification of a novel nonsense mutation (c.1977C>G; p.Tyr659Ter). Objectives: This study investigated Vietnamese English as a foreign language (EFL) teachers’ perspectives on the influence of artificial intelligence (AI) writing tools on students’ writing content and organization, examining tool selection patterns and effectiveness relationships through a mixed-methods approach.
Methods: An exploratory sequential mixed-methods design was employed, combining semi-structured interviews and classroom observations with six EFL teachers, followed by a survey of 52 teachers from various Vietnamese universities. Data analysis integrated thematic analysis of qualitative data with correlation and regression analyses of survey responses to examine relationships between usage patterns, institutional factors, and effectiveness ratings.

This study investigates the comparative usability of two common bare-hand input techniques, touch and raycast, in the context of an educational extended reality (XR) application. An XR educational solar system application developed by the research team, Solar-XR, was used in the study. Thirty-four participants completed a series of tasks using both input methods in a within-subjects design. Usability was assessed using the validated VRSUQ questionnaire, performance was measured through task completion times, and user preferences were collected alongside qualitative feedback. The thesis, consisting of nine chapters, explores the methodology of building testable brain computational models using Deep Neural Networks (DNN), which are trained by psychophysics data. Psychophysics is the quantitative study of perception of physical stimuli. In psychophysics experiments, one or more parameters associated with the stimuli are changed, and the human subject’s responses to the stimuli are recorded. This thesis encompasses both experimental psychophysics works, as well as computational models of the phenomena involved. Calcium oxalate stones comprise greater than 70% of all kidney stones. In the current conceptual framework, the initial stone nidus is thought to include the aggregation of inorganic crystallites, the formation of which is favored by elevated concentrations of dissolved constituents. Here, we show that this highly prevalent stone type comprises a form of organic-inorganic polycrystalline biocomposite with integrated bacterial biofilms. Evidence from electron microscopy and fluorescence microscopy reveal the unanticipated internal structure of kidney stones from human patients, where bacterial biofilms are intercalated between polycrystalline mineral layers, even in stones identified as "noninfectious" clinically, including those in patients without …

Methods:The methodology for this study involved analyzing survey data using statistical methods, such as descriptive analysis, correlation analysis, analysis of variance (ANOVA), regression, and factor analysis to evaluate the effectiveness of innovative methods in computer science education. This note examines the intersection between shareholders agreements and intellectual property ownership in Delaware corporations after Delaware Senate Bill No. 313. Delaware corporate governance law has created extensive statutory and case law balancing the competing interests of corporate actors. The board of directors maintains an exclusive right to manage the affairs of a corporation, unless otherwise provided in the certificate of incorporation. Delaware law defers to director’s management authority through the business judgement rule, but fiduciary duties govern a director’s responsibly to shareholders. Clinical trial data occupy a contested space between private commercial assets and public goods. While the United States Food and Drug Administration (FDA) has historically shielded these data through expansive trade-secret interpretations, the European Union (EU) has adopted a more open disclosure regime. This Article compares the two regimes and traces how the EU’s two-decade journey toward transparency has become a key reference for researchers and advocates of open data-sharing.

Conventional scholarly wisdom tends to discount the importance of copyright as a tool for delivering remuneration to the artists that sustain the digital creative ecosystem or as an enabling mechanism that facilitates the licensing, distribution, and financing transactions that constitute the economic infrastructure of the media and entertainment industries. This position runs counter to the evolution of digital content markets, which have experienced renewed investment and growth concurrently with the development of technological tools that have enabled content owners to correct for the “copyright deficit,” regulate access, and extract payment from platforms and users.
