Background: Optimized vancomycin dosing is associated with improved clinical outcomes, especially decreased acute kidney injury. Free, online, grey literature training materials are available for vancomycin dosing; however, their utility is unclear. Objectives: This study evaluates the utility of these tools for just-in-time (e.g. refresher courses) training for general clinicians dosing vancomycin. Methods: A structured grey literature review of websites was performed, and the Online Course Quality Review (OSCQR) rubric was modified for this study. Eligible resources included free, web-based tutorials aimed at general clinicians. The materials were evaluated using the rubric. Data were summarized as counts and proportions. Results: A …

Eccentricity faults in electric machines remain a critical concern, as they generate uneven magnetic forces that increase vibration and noise, ultimately raising the risk of premature motor failure. This study proposes a method for the early detection of dynamic eccentricity (DE) faults in hydropower plants through an advanced optimization-based parameter identification technique integrated with finite element analysis (FEA). Finite element modeling (FEM) is first used to analyze an existing salient-pole synchronous generator (SPSG) from a hydroelectric power plant in Türkiye. Recent advances in machine learning and deep learning have greatly improved how we detect plant diseases, making diagnoses more accurate, faster, and easier to scale. However, many existing solutions depend on large, pretrained models that need powerful hardware, which limits their use in the field, especially in areas with limited resources. To tackle this, we designed a custom lightweight convolutional neural network (CNN) built from scratch using 20,000 carefully selected images from the PlantVillage tomato dataset. Our model uses Squeeze-and-Excitation (SE) blocks and Swish activation functions to boost performance, reaching an accuracy of 97.7% while using far fewer computing resources …

Nonorthogonal multiple access (NOMA) communication presents a promising solution to the limitations of traditional orthogonal multiple access techniques, offering potential improvements in achievable rates. Multiple-input multiple-output (MIMO), when combined with NOMA (MIMO-NOMA), further enhances these benefits by leveraging the diversity advantages of multiple antennas. Looking ahead, the future of wireless communication hinges on deploying heterogeneous networks (HetNets), facilitating the coexistence of various wireless access networks in a hierarchical fashion. Traffic signal management is a critical challenge due to its environmental, economic, and public health impacts. The maximum weighted flow method (MaxWeightedFlow) was developed to optimize traffic flow at isolated and coordinated urban intersections. This study proposes a new method, the novel MaxWeightedFlow, which includes two key strategies to enhance the classical approach. The first strategy reduces computational burden by estimating vehicle approach times based on instantaneous speeds, improving real-time performance. The second employs regression analysis to optimize the alpha parameter, representing the vehicle waiting coefficient. In this study, a dataset comprising 3600 discrete operational snapshots (rather than continuous time-series data) derived from real-field operations is used to obtain a high-accuracy driving profile equation using a second-degree Polynomial Regression method. This equation demonstrates the model’s interpretability. The performance metrics obtained with the second-degree polynomial regression model’s equation are as follows: a coefficient of determination (R2) of 0.84, a Pearson Correlation Coefficient of 0.91, and an RMSE of 11.13. These results indicate the effectiveness of artificial intelligence-based approaches in improving the efficiency of the railway signaling system. Now that artificial intelligence tools for lawyers are widely available, we decided to integrate them for a semester in our Entrepreneurship Clinic. We have some important takeaways for legal education in general and the transactional practice of law in particular.
First, employers and educators need to account for law students who already are using AI tools in their legal work and guide new lawyers about how to use such tools appropriately.

Student Disruptions And Teacher Turnover, Kyuhan Choi, Ying Shi, Maria Zhu

This paper examines how exposure to disruptive students affects teacher retention using linked teacher–student administrative records from North Carolina. To address non-random classroom assignment, we instrument for classroom exposure using the school-by-grade share of disruptive students based on prior-year disciplinary infractions. A 10 percentage point increase in the share of disruptive students raises the probability of an early-career teacher leaving the school in the following year by 1.0 percentage points. Notably, working in a school environment with supportive leadership and greater teacher autonomy, particularly around student conduct policies, mitigates the impact of student disruptions.

Lipid nanoparticle (LNP)-encapsulated nucleoside-modified mRNA vaccines elicit robust CD4+ T cell responses, yet the mechanisms underlying this T cell priming remain unknown. Antigens presented to CD4+ T cells on major histocompatibility complex class II (MHC II) are traditionally acquired by antigen presenting cells (APCs) from extracellular sources. Here we show that vaccine specific CD4+ T cell responses instead rely on antigen directly expressed within APCs, without extracellular transit. Murine APCs treated with mRNA-LNP vaccines activate T cells more efficiently when presenting antigen produced internally, rather than acquired externally. Immunization with mRNA-LNP vaccines engineered to inhibit antigen expression in APCs results …

Maintaining reliable and high-quality power delivery becomes increasingly complex with expanding power grids. The lack of protection coordination poses a significant threat, compromising overall system reliability. This research addresses this challenge by proposing a method for coordinating protective devices within the distribution system, specifically during network faults. The proposed approach utilizes a stochastic timed Petri net (STPN) based methodology to model protective device coordination across various fault scenarios. PurposeThe number of Experts by Experience (EbE) working in mental health services has grown in recent years. The National Health Service (NHS) five-year plan states EbE should be used across different levels, including service delivery, design and commissioning. While current research explores the role of EbE in service delivery, little is known about their experiences of co-designing services. BACKGROUND: AVEIR DR, an industry-first dual-chamber leadless pacemaker (LP), provides continuous atrioventricular synchrony through implant-to-implant (i2i) communication between atrial and ventricular LP devices. It is important to evaluate the early real-world comparative safety of AVEIR DR LP.
OBJECTIVE: To compare complications and mortality between AVEIR DR LP and dual-chamber transvenous pacemakers (TP).
METHODS: De novo LP and TP patients were identified in Medicare Fee-for-Service claims (October 2023 and December 2024). Outcomes were 30-day and 6-month complications, reinterventions, heart failure hospitalizations, and all-cause mortality. Comparisons were adjusted for demographics, comorbidities, and hospital encounter characteristics.

Sensory perception is shaped by experience, giving stimuli behavioral significance. Basal forebrain (BF) cholinergic neurons in mice, which are crucial for arousal and motivation, also regulate sensory processing. Within BF nuclei, glutamatergic (vGlut2BF) neurons receive cholinergic input and modulate behaviors, but their roles in encoding sensory significance remain unclear. Using in vivo calcium imaging, we found that vGlut2BF neurons initially poorly encoded odor identity. However, their response to conditioned odors increased following associative learning, and their population activity more distinctly encoded paired stimuli, reflecting emergent value representation. Acute myeloid leukemia (AML) is a clonal disorder characterized by immature blasts and arrested differentiation that primarily affects the bone marrow (BM) and occasionally presents as extramedullary (EM) disease. EM manifestations highlight AML's adaptability to distinct microenvironments, which we examined using spatial analyses of medullary and EM tissues. We describe a workflow for Visium-based spatial transcriptomics in medullary and EM AML, revealing insights into cell-cell communication and the spatial organization of AML hierarchies. In BM, monocytes and granulocyte-monocyte progenitors colocalized with leukemic populations, sharing molecular signatures with those in EM sample. CXCL12-CXCR4-mediated communication correlated with PI3K/AKT/mTOR signaling in inflammatory niches.

This research aims to investigate healthcare professionals’ (HCPs) attitudes towards patient safety and to explore the role ofvarious work-related factors that could be the barriers to safety-events reporting to modify these attitudes. A cross-sectionalsurvey was conducted, involving 629 HCPs who are working across health care sectors in the United Arab Emirates. HCPs in theUnited Arab Emirates, comprising 71.5% women and 27.8% men, displayed a generally positive attitude towards patient safety(mean score = 3.91). Sub-comparisons indicated high scores for team functioning (4.58 ± 0.62) and low scores for professionalincompetence as an error cause (2.86 ± 0.50). Common barriers to reporting safety …

Purpose: RAPGEF2 encodes a guanine nucleotide exchange factor (GEF) that activates small GTPases and has not been linked to a Mendelian disorder. RAPGEF2 is highly intolerant to loss-of-function variants. We report 5 de novo heterozygous variants in RAPGEF2 in unrelated individuals with developmental delay, attention deficit hyperactivity disorder, epilepsy, dysmorphic features, or other manifestations. We used a Drosophila model to assess the functional impact of the identified human variants.
Methods: We generated a Kozak-GAL4 null allele of the Drosophila ortholog of RAPGEF2, PDZ-GEF, and used the allele to determine the gene expression pattern as well as the loss-of-function phenotypes. We …

This study evaluates the fate and variability of soil organic carbon (SOC) stocks and nitrogen species using the latest version of the Soil and Water Assessment Tool– Carbon (SWAT-C) and assesses how conservation practices influence their dynamics in the Raccoon River Watershed (RRW). Dominated by intensive agricultural pro- duction, the RRW is a significant contributor of sediment and nutrient loads to local rivers and the Mississippi River. This SWAT-C model simulates the export of SOC and nitrogen species and evaluates their responses under varying management scenarios. Aims: Among statin-treated participants with elevated triglycerides and known cardiovascular disease or with diabetes and other risk factors, icosapent ethyl reduced the risk of cardiovascular events in the REDUCE-IT study. In this post hoc analysis of REDUCE-IT, we quantified the effects of icosapent ethyl on total hospitalizations and days lost to hospitalization and death.
Methods: Randomization to treatment with 2 g twice daily of icosapent ethyl or matching placebo was performed among 8179 participants receiving statin therapy with established cardiovascular disease or age ≥50 years with diabetes and ≥1 additional risk factor, fasting triglyceride 1.69-5.63 mmol/L, and low-density lipoprotein cholesterol …
