Primary Care Experiences Of Adults Reporting Learning Disability: A Probability Sample Survey, Rohit Shankar

BACKGROUND: Adults with learning disability face multiple adversities, but evidence on their needs and primary care experiences is limited.

AIM: To compare the characteristics and primary care experiences of adults reporting learning disability with those who did not.

DESIGN AND SETTING: This was an analysis of the 2022 General Practice Patient Survey, a national probability sample survey conducted in 2022 with people registered with NHS primary care in England.

METHOD: This analysis reports descriptive profiles, weighted and with 95% confidence intervals. Logistic regression models adjusting for gender, age, ethnicity, and area-level deprivation compared experiences of adults reporting learning disability with …

A combined Single-Ended Primary-Inductor Converter (SEPIC) and Cuk DC-DC converter that provides a bipolar outputs voltage at the DC bus is widely used in applications especially solar photovoltaic (SPV) system. Maintaining constant output voltages at the DC bus under variable loads and input voltage from SPV is of great interest to many researchers. This paper presents a 400 W mathematical model of a combined SEPIC and Cuk DC-DC converter through state space averaging (SSA) in a continuous conduction mode (CCM). Urban park management generally prioritises green spaces and activities within park boundaries, rather than landscape interactions. Understanding how people experience and connect with broader landscapes is essential for improving societal environmental, economic and wellbeing outcomes. A geospatial and discourse analysis of three urban parks was conducted in Plymouth, UK. The predominant landscape gaze at each park revealed strong social ties to blue spaces–namely the coast and sea–indicating an interconnectedness between parks and coastal spaces beyond park boundaries. Textual analysis emphasised the beauty of nature, the expanse of the ocean, and the health and wellbeing benefits of blue and green spaces. …

Since bromides were first used in 1857 to treat epilepsy, numerous antiseizure medications (ASM) have been developed. Many of these are available for the treatment of epilepsy and status epilepticus today. With so many ASM available, questions arise as to whether all of these medications are needed and when should they be used. As precision medicine begins to play a larger role in determining targeted treatments for specific types of epilepsy, a complete understanding of various medications is needed. When adaptive phenotypic variation or quantitative trait loci map within an inverted segment of a chromosome, researchers often despair because the suppression of crossing over will prevent the discovery of selective target genes that established the rearrangement. If an inversion polymorphism is old enough, then the accumulation of gene conversion tracts offers the promise that quantitative trait loci or selected loci within inversions can be mapped. The inversion polymorphism of Drosophila pseudoobscura is a model system to show that gene conversion analysis is a useful tool for mapping selected loci within inversions. Aberrant repair underlies the pathogenesis of pulmonary fibrosis while effective strategies to convert fibrosis to normal regeneration are scarce. Here, we found that thyroid hormone is decreased in multiple models of lung injury but is essential for lung regeneration. Moreover, thyroid hormone receptor α (TRα) promotes cell proliferation, while TRβ fuels cell maturation in lung regeneration. Using a specific TRβ agonist, sobetirome, we demonstrate that the anti-fibrotic effects of thyroid hormone mainly rely on TRβ in mice. Cellularly, TRβ activation enhances alveolar type-2 (AT2) cell differentiation into AT1 cell and constrains AT2 cell hyperplasia. Molecularly, TRβ activation directly regulates the …

The Human Epidemiology and Response to SARS-CoV-2 (HEROS) Study is a prospective, multicity, 6-month incidence study conducted from May 2020 to February 2021. The objectives were to identify risk factors for SARS-CoV-2 infection and household transmission among children and people with asthma and allergic diseases, and to use the host nasal transcriptome sampled longitudinally to understand infection risk and sequelae at the molecular level. Xenotransplanted Human Organoids Identify Transepithelial Zinc Transport As A Key Mediator Of Intestinal Adaptation, Maame Efua S Sampah, Vered Gazit, Deborah C Rubin, Et Al.

Short bowel syndrome (SBS) leads to severe morbidity and mortality. Intestinal adaptation is crucial in improving outcomes. To understand the human gene pathways associated with adaptation, we perform single-cell transcriptomic analysis of human small intestinal organoids explanted from mice with experimental SBS. We show that transmembrane ion pathways, specifically the transepithelial zinc transport pathway genes SLC39A4 and SLC39A5, are upregulated in SBS. This discovery is corroborated by an external dataset, bulk RT-qPCR, and Western blots. Oral zinc supplementation is shown to improve survival and weight gain of SBS mice and increase the proliferation of intestinal crypt cells in vitro. Finally, …

An Iron-Rich Subset Of Macrophages Promotes Tumor Growth Through A Bach1-Ednrb Axis, Ian W Folkert, Mai T Dang, Et Al.

We define a subset of macrophages in the tumor microenvironment characterized by high intracellular iron and enrichment of heme and iron metabolism genes. These iron-rich tumor-associated macrophages (iTAMs) supported angiogenesis and immunosuppression in the tumor microenvironment and were conserved between mice and humans. iTAMs comprise two additional subsets based on gene expression profile and location-perivascular (pviTAM) and stromal (stiTAM). We identified the endothelin receptor type B (Ednrb) as a specific marker of iTAMs and found myeloid-specific deletion of Ednrb to reduce tumor growth and vascular density. Many teachers in the current public education system in the United States do not fully consider the unique societal and cultural aspects of African American and Latinx learners. As a result, many teachers could benefit from developing skills that strengthen their understanding of authentic engagement, cultural competency, and cultural congruency to aid in implementing culturally relevant and sustaining pedagogical practices that benefit the diverse learning styles of these students. The increasingly culturally diverse settings present within today’s P-12 schools necessitate improved pedagogical practices to develop more culturally inclusive and equitable learning environments. Third-space pedagogical practices demonstrate potential as an approach for increasing educational equity by promoting cultural hybridity among diverse students and stakeholders through negotiated sociocultural perspectives and interactions. This literature review summarizes recent scholarship surrounding third-space pedagogy, including discussions of how educators can foster third-space relationships and design third-space learning opportunities in diverse educational settings. In addition, lessons from COVID-19 third-space learning opportunities are discussed as a model for re-envisioning equitable instructional practices post-pandemic.

This study integrates social comparison theory and self-determination theory to examine the roles of escapism, conformity, and individual cultural values in shaping luxury brand attitudes. Concepts related to consumer upward and downward social comparison are complemented by aspects related to self-determination and autonomy. Drawing on survey data from the United States and Colombia, the research employs partial least squares structural equation modeling and fuzzy-set qualitative comparative analysis (fsQCA) to investigate the complex drivers of luxury consumption. The findings reveal that escapism and conformity, complementary motivations grounded in social comparison and self-determination theories, positively influence luxury brand attitudes. This Article challenges the common view that more stringent regulation of the digital economy inevitably compromises innovation and undermines technological progress. This view, vigorously advocated by the tech industry, has shaped the public discourse in the United States, where the country’s thriving tech economy is often associated with a staunch commitment to free markets. U.S. lawmakers have also traditionally embraced this perspective, which explains their hesitancy to regulate the tech industry to date. In the 1991 case Hernandez v. New York, the United States Supreme Court characterized bilingualism as a race-neutral trait that can be used to exclude individuals from jury service. This Note proceeds by demonstrating how the current state of the law undermines the interests of bilingual individuals and then proposes a solution. Focusing specifically on Hispanic bilingual Spanish speakers, this Note first employs Professor Jennifer Lackey’s multi-directional credibility model to show that bilingual Spanish speakers suffer injustice in the courtroom due to both credibility deficits and excesses. The World Health Organization has designated Pseudomonas aeruginosa as a critical pathogen for the development of new antimicrobials. Bacterial viruses, or bacteriophages, have been used in various clinical settings, commonly called phage therapy, to address this growing public health crisis. Here, we describe a high-resolution structural atlas of a therapeutic, contractile-tailed Pseudomonas phage, Pa193. We used bioinformatics, proteomics, and cryogenic electron microscopy single particle analysis to identify, annotate, and build atomic models for 21 distinct structural polypeptide chains forming the icosahedral capsid, neck, contractile tail, and baseplate. The COVID-19 pandemic exacerbated the already elevated stress levels among teachers, leading to contemplation of leaving the profession. As educators were compelled to transition to online teacher, they suddenly found themselves immersed in the households of their students, an entirely novel experience. Digital Twinning For Resilient Global Supply Chains: Three Case Studies, Gawon Yun, Douglas N. Hales, Leo Hong

Global supply chains have vulnerabilities, and these escalate in crisis times like pandemics or wars. Digital twinning of supply chains can provide early alerts and timely interventions. We examined three companies in different industries using Digital Twin (DT) technology to improve their supply chain performance. We found that while DT successfully enhanced performance indicators, the dollar and time costs were significant, creating barriers to wider DT adoption. We conclude by outlining the positives and negatives of implementing Digital Twins in global supply chains.
