BACKGROUND: Desirability of outcome ranking (DOOR) is an innovative approach to clinical trial design and analysis that uses an ordinal ranking system to incorporate the overall risks and benefits of a therapeutic intervention into a single measurement. Here we derived and evaluated a disease-specific DOOR endpoint for registrational trials for hospital-acquired bacterial pneumonia and ventilator-associated bacterial pneumonia (HABP/VABP).
METHODS: Through comprehensive examination of data from nearly 4000 participants enrolled in six registrational trials for HABP/VABP submitted to the Food and Drug Administration (FDA) between 2005 and 2022, we derived and applied a HABP/VABP specific endpoint. We estimated the probability that …

Stereotactic body radiation therapy (SBRT) and hypofractionation using pencil-beam scanning (PBS) proton therapy (PBSPT) is an attractive option for thoracic malignancies. Combining the advantages of target coverage conformity and critical organ sparing from both PBSPT and SBRT, this new delivery technique has great potential to improve the therapeutic ratio, particularly for tumors near critical organs. Safe and effective implementation of PBSPT SBRT/hypofractionation to treat thoracic malignancies is more challenging than the conventionally fractionated PBSPT because of concerns of amplified uncertainties at the larger dose per fraction. The NRG Oncology and Particle Therapy Cooperative Group Thoracic Subcommittee surveyed proton centers in …

This Article demonstrates how hierarchies in the international economy and in international financial institutions have facilitated the U.S.-led Global War on Terror (GWOT). Using U.S.-Pakistan relations as a case study, it shows how the United States has deployed its powerful position at the International Monetary Fund (IMF) to claim Pakistan's consent for its military activities in the region. In the GWOT's first decade, beginning in 2001, the United States openly sought forcible regime change, as in Afghanistan and Iraq; in its second and (now) third decades, the United States has waged covert counterinsurgency campaigns allegedly against militant groups in Muslim-majority …

New England Journal of Public Policy
The relationship between the United States and China has deteriorated over the past two decades and fears of escalating risks of war are regularly reported in global media. This article explores the psychological factors that contribute to the two superpowers shifting from a collaborative relationship to a competitive relationship, seeing each other as enemies, feeling increasingly threatened by each other, failing to consider the heightened sensitivities that arise from their respective traumatic pasts, triggering the collapse of thinking and unleashing of uncontainable emotionality, escalating accidents to conflict, and escalating conflict to war. It highlights the dangers of ignoring heightened trauma-related …

Profiles In Influence: Shaping American Perceptions, Attitudes, And Policy Towards China, Benjamin Merritt
Masters Theses
Pearl Buck, Henry Luce, Edgar Snow, and Richard Nixon have profoundly influenced Sino-American relations throughout the 20th century. Their impact on American perceptions, attitudes, and foreign policy has played a pivotal role in shaping the current state of relations between the United States government and the People’s Republic of China. Pivotal historical events such as WWI & WWII, the Sino-Japanese War, China’s Civil War, and the Soviet-Sino conflict—among other events—have also significantly influenced the dynamics of Sino-American relations. Despite their importance, there is a notable absence of studies that comprehensively explore the collective impact of these individuals and events. Understanding …

With climate change posing an imminent threat to our planet, it has become an ever more prominent question as to how we can curve the effects of it. One of the most popular proposed solutions to curb emissions is renewable energy. With solar, wind, and other types of renewable energy sources, we could rely on energy sources that produce zero emissions. However, it has become a hotly debated issue in the media, especially in the United States. What my research does is examine and unpack themes in the rhetoric of conservative-leaning media outlets that speak against renewable energy, examine the …

Childhood-onset essential hypertension (COEH) is an uncommon form of hypertension that manifests in childhood or adolescence and, in the United States, disproportionately affects children of African ancestry. The etiology of COEH is unknown, but its childhood onset, low prevalence, high heritability, and skewed ancestral demography suggest the potential to identify rare genetic variation segregating in a Mendelian manner among affected individuals and thereby implicate genes important to disease pathogenesis. However, no COEH genes have been reported to date. Here, we identify recessive segregation of rare and putatively damaging missense variation in the spectrin domain of spectrin repeat containing nuclear envelope …

In the modern era, the internet is becoming just as essential to the functioning of our everyday lives as food and water. Virtually everything we do relies on modern technology in some capacity, allowing our world to function efficiently. The technology that we have available has made our world smaller in a sense, because digital interconnectedness has allowed us to communicate with anyone no matter where they are on the globe in a matter of seconds, with the tap of a finger. This idea, once a phenomenon only possible in storybooks, has come to life but is not without its …
Boundaries Of Autonomy: Exploring Parallels Between Mental Hospitals And Prisons In The United States, Benjamin J. Boldt
Boundaries Of Autonomy: Exploring Parallels Between Mental Hospitals And Prisons In The United States, Benjamin J. Boldt
The Agora
Within the United States, a prevailing perception emerges that mental health facilities and prisons are unfavorably similar. This societal sentiment is quite concerning if true, as the prison system is known for its many societal failures within the U.S. Furthermore, mental hospitals aim to help individuals; thus, a comparison to a faulty system highlights a possible issue worthy of exploration. Unfortunately, little analysis exists, within academic research, comparing both institutions. The following comparative analysis looks at available research between mental hospitals and prisons to validate this issue and explore implications. Findings indicate a shared pattern between mental hospitals and prisons …

Give It A Nudge: A Comparative Analysis Of The Values And Application Of Voluntary Environmental Programs In The United States, Pianpian Wang
Give It A Nudge: A Comparative Analysis Of The Values And Application Of Voluntary Environmental Programs In The United States, Pianpian Wang
Dissertations & Theses
In recent years, companies have increased their voluntary commitments to reducing carbon emissions and implementing sustainability goals. While existing research mainly focuses on government-organized voluntary environmental programs (VEPs), exploring corporate voluntary commitments is essential. The business sector’s active role in environmental management is noteworthy. Traditionally, governments have relied on command-and-control regulations and market incentives to compel companies to protect the environment. However, companies are now demonstrating a willingness to go beyond legal requirements. Naturally, we seek answers to whether these commitments are effective, what factors can contribute to their authenticity, and how we compare these voluntary commitments to other VEPs. …

That Way: An Examination Of Male Relationships In Film During The Hays Code, Jane Knudsen
Theses/Capstones/Creative Projects
The Hays Code (1934-1968) influenced the construct of United States masculinity and the discourse surrounding masculine presentation between the 1920s to the 1960s. The Hays Code and World War II affected the culture surrounding male/male relationships in the United States. Previous research done by David Lugowski (1999) and Jeffrey Suzik (1999) shows that both World Wars led to crises of masculinity in which the hegemonic ideal of masculinity was restructured to establish men as providers and warriors, and Code-era films reflected the discourse. To understand the gender roles in the 20th century, I analyzed the Hays code, male bonds, …
