Background. Little is known about the effect of provider type and experience on outcomes, resource use, and processes of care of hospitalized patients with human immunodeficiency virus (HIV) infection. Hospitalists are caring for this population with increasing frequency.

Methods. Data from a natural experiment in which patients were assigned to physicians on the basis of call cycle was used to study the effects of provider type—that is, hospitalist versus non hospitalist—and HIV-specific inpatient experience on resource use, outcomes, and selected measures of processes of care at 6 academic institutions. Antitrust Liability For Refusal To License Intellectual Property: A Comparative Analysis And The International Setting, Rita Coco

Antitrust and IP law both share the goals of promoting innovation and benefiting consumers. A potential for conflict exists, however, when a dominant firm's refusal to license IP rights affects the dynamics of competition. Antitrust intervention in IP rights can reduce incentives to invest, whereas a failure to allow anticompetitive behavior can harm consumers and competitors while reducing the efficiency of the economic system. The author reviews the European and United States approaches to monopolization claims involving IP rights. The European approach is limited by the mismatch between national enforcement of IP rights and community enforcement of antitrust law. The …

Middle-aged and older adults remain sexually active, but the combination of sexual longevity, inconsistent safe sex practices, and poor condom knowledge, increases their likelihood to risk their sexual health. Menopause may influence sexual risk, as postmenopausal women may engage in risky sexual behaviours, while hormonal changes increase physiological vulnerability to sexually transmitted infections. This project explored whether menopause and type of post-menopause (natural versus surgically- induced) influenced sexual behaviour and herpes simplex virus type 2 (HSV-2) seroprevalence, using logistic regression modeling of the U.S. National Health and Nutrition Examination Survey data. BACKGROUND. Lung cancer incidence rates among American Indians and Alaska Natives (AI/ANs) in the United States have not been described well, primarily because of race misclassification and, until the 1990s, incomplete coverage of their population by cancer registries. Smoking, the predominant cause of lung cancer, is particularly prevalent among this population.

METHODS. Data from the National Program of Cancer Registries and the Surveillance, Epidemiology, and End Results Program were combined to estimate age adjusted incidence rates of lung cancer during 1999 through 2004. BACKGROUND. The misclassification of race decreases the accuracy of cancer incidence data for American Indians and Alaska Natives (AI/ANs) in some central cancer registries. This article describes the data sources and methods that were used to address this misclassification and to produce the cancer statistics used by most of the articles in this supplement.

METHODS. Records from United States cancer registries were linked with Indian Health Service (IHS) records to identify AI/AN cases that were misclassified as non-AI/AN. BACKGROUND. American Indians and Alaska Natives (AI/AN) experience higher morbidity and mortality from primary liver cancer than other United States (US) populations, but racial misclassification in medical records results in underestimates of disease burden.

METHODS. To reduce misclassification, National Program of Cancer Registries and Surveillance, Epidemiology, and End Results data were linked with Indian Health Service (IHS) enrollment records to compare primary liver cancer incidence and stage at diagnosis between AI/AN and non-Hispanic whites (NHW) living within the regionalized IHS Contract Health Service Delivery Area counties. BACKGROUND. Cancer incidence rates vary among American Indian and Alaska Native (AI/AN) populations and often differ from rates among non-Hispanic whites (NHWs). However, the misclassification of race for AI/AN cancer cases in central cancer registries may have led to underestimates of the AI/AN cancer burden in previous reports.

Rickets, the bone disease classically caused by Vitamin D deficiency, was one of the most common diseases of children 100 years ago. It has been recognized as a disease of urban living and linked to issues of race and culture for generations. This paper uses unpublished patient records from 1904 to 1909 and archival and published materials from multiple community-based trials, including the New Haven Rickets Study (1923-1926), to explore how the definition, diagnosis, and treatment of rickets shifted in the first decades of the twentieth century in the United States. The northern extension of the Florida reef tract and a complex of limestone ridges run parallel to the subtropical Atlantic coastline of southeast Florida. Spanning 170 km from the northern border of Biscayne National Park (BNP) in Miami-Dade County to the St. Lucie Inlet in Martin County, the reefs and hardbottom areas in this region support a rich and diverse biological community (Figure 5.1). Nearshore reef habitats in southeast Florida include hardbottom areas, patch reefs and worm reefs (Phragmatopoma spp.) exhibiting abundant octocoral, macroalgae, stony coral and sponge assemblages. Recently, countries from China and Brazil to Malaysia and South Africa have passed laws promoting the patenting of publicly funded research, and a similar proposal is under legislative consideration in India. These initiatives are modeled in part on the United States Bayh-Dole Act of 1980. Bayh-Dole (BD) encouraged American universities to acquire patents on inventions resulting from government-funded research and to issue exclusive licenses to private firms, on the assumption that exclusive licensing creates incentives to commercialize these inventions. During a disaster emergency medical services (EMS) plays a critical role in supporting mass casualty response. However, the processes and procedures used in a disaster are different than those which the prehospital providers encounter during routine emergency response. With limited time and resources, new approaches to training should be considered. This thesis presents research on EMS training, disaster response, distance learning, and instructional technology. Survey and interview results are analyzed providing a foundation for the development of a proposed software model using Internet simulation games to train prehospital providers for mass casualty response.

Women are significantly underrepresented in the hard sciences and engineering. While the number of women seeking degrees in these fields has increased in the last forty years, a substantial gap still exists between the sexes. The purpose of this dissertation is to examine one area of influence on career choice -- the curriculum. Women scientists are underrepresented in the school curriculum. This dissertation examines the discourse of curriculum and the role it has in the gendering of the field of science. Prostate cancer is the second leading cause of cancer-related deaths in men in the United States. The molecular pathogenesis of this disease includes genetic alterations, infection, and exposure to inflammatory or dietary oxidants. Recently, research has focused on the role of host defense peptides in tumor immunity. Defensins, a highly conserved multigene family of proteins that possess antimicrobial and antiviral properties, play an essential role in innate and adaptive immunity. These peptides are categorized into alpha- and beta-defensins based upon the spacing and connection between the six conserved cysteine residues of the mature peptide. Chemical characteristics of permanent stratification in cave pools (meromixis) may provide insight into the geochemical origin and evolution of cave pool waters. The objective of this study was to test the hypothesis that some pools in Lechuguilla Cave may be subject to ectogenic meromixis, where permanent chemical stratification is induced by input of relatively saline or fresh water from an external source. However, because organic C concentrations in Lechuguilla waters are low (typically < 1 mg L-1), biogenic meromixis resulting in O2(g)-depleted subsurface waters is not expected. Despite the acknowledged relevance of aerosol-derived water-soluble organic carbon (WSOC) to climate and biogeochemical cycling, characterization of aerosol WSOC has been limited. Electrospray ionization Fourier transform ion cyclotron resonance mass spectrometry (ESI FT-ICR MS) was utilized in this study to provide detailed molecular level characterization of the hi(,h molecular weight (HMW; m/z>223) component of aerosol-derived WSOC collected from rural sites in Virginia and New York, USA. More than 3000 peaks were detected by ESI FT-ICR MS within a m/z range of 223-600 for each sample. Approximately 86% (Virginia) and 78% (New York) of these peaks were assigned molecular formulas …

This Article deals with Cox v. City of Dallas, Halprin v. Prairie Single Family Homes of Dearborn Park Ass’n, and the issue of whether the Federal Fair Housing Act (FHA) should be interpreted to outlaw discrimination in the provision of services by local governments. Part I describes the Cox litigation and its connection with Halprin. Part II surveys the pre-Cox cases that have dealt with discriminatory municipal services. Traditionally, studies of the Loyalists from the American Revolution highlight their wartime experience or explore their post-war experience as exiles in other areas of the British Empire. Instead, this study begins in 1783 and focuses on the majority of Loyalists who stayed in the United States after Independence. Using legal documents, personal correspondence, and popular newspapers the "Loyalist Problem in the Early Republic," analyzes the legal and cultural dimensions of citizenship from the Loyalist's perspective.
