Background Stroke remains one of the leading causes of disability and death in the United States. We characterized 10-year nationwide trends in use of comfort care interventions (CCIs) among patients with ischemic stroke, particularly pertaining to acute thrombolytic therapy with intravenous tissue-type plasminogen activator and endovascular thrombectomy, and describe in-hospital outcomes and costs. Methods and Results We analyzed the National Inpatient Sample from 2006 to 2015 and identified adult patients with ischemic stroke with or without thrombolytic therapy and CCIs using validated

Usability Of Electronic Health Record-Generated Discharge Summaries: Heuristic Evaluation., Patrice Dolhonde Tremoulet, Priyanka D Shah, Alisha A Acosta, Christian W Grant, Jon T Kurtz, Peter Mounas, Michael Kirchhoff, Elizabeth Wade

College of Science & Mathematics Departmental Research

BACKGROUND: Obtaining accurate clinical information about recent acute care visits is extremely important for outpatient providers. However, documents used to communicate this information are often difficult to use. This puts patients at risk of adverse events. Elderly patients who are seen by more providers and have more care transitions are especially vulnerable.

OBJECTIVE: This study aimed to (1) identify the information about elderly patients' recent acute care visits needed to coordinate their care, (2) use this information to assess discharge summaries, and (3) provide recommendations to help improve the quality of electronic health record (EHR)-generated discharge summaries, thereby increasing patient …

Real-World Outcomes Of Patients With Locally Advanced Or Metastatic Epithelioid Sarcoma, Mrinal M Gounder, Brian A Van Tine, Et Al.

2020-Current year OA Pubs

BACKGROUND: Limited data are available on the real-world effectiveness and safety of systemic therapies for advanced (surgically unresectable and/or metastatic) epithelioid sarcoma (ES).

METHODS: A retrospective medical records review was conducted in patients with advanced ES who were initiating first-line or ≥2 lines of systemic therapy (2000-2017) at 5 US cancer centers. The real-world overall response rate (rwORR), the duration of response (rwDOR), the disease control rate (rwDCR) (defined as stable disease for ≥32 weeks or any duration of response), and progression-free survival (rwPFS) were assessed by radiology reports. Overall survival (OS), rwDOR, and rwPFS were estimated from the time …

A Comparative Analysis Of Distribution And Allocation Of Covid-19 Vaccines Between Two Administrations, Leila Halawi, J. Mayanja, J. Chilipamushi, Y. Zhang, B Albury, Jude Edwards

Publications

The pandemic of COVID-19 is a life-threatening disease that entailed all governments but especially the U.S government, to quickly prevent methods of spreading the virus while strategically putting in place a plan for biological testing for a vaccine for immunity. The purpose was to determine how effective the formulation of a COVID-19 vaccine and the distribution would impact the mortality with efficacy rates. Besides, this research paper underlines the principles for ethical and equitable distribution for mass immunization. A comparative analysis between the Trump Administration and the Biden Administration will be presented regarding the funding stream, distribution of vaccinations, and …

Redefining Affordable Housing In Ma, Maria Fernanda De La Fuente Martinez Apr 2021

Redefining Affordable Housing In Ma, Maria Fernanda De La Fuente Martinez

Honors Program Theses and Projects

Housing is considered affordable if it costs 30% or less of a household’s income, and “is deed-restricted to income-eligible low- or moderate-income residents” (MAPC, n.d.). According to the Joint Center of Housing Studies (JCHS), as of 2017, 31.5% of all American households were considered cost-burdened by rent, meaning they spent more than 30% of their income on rent. That same year, the percentage of severely cost-burdened households (those who spent over 50% of their income in rent) was 15.2%. This means that almost half of all Americans struggled to pay rent in 2017 (Veal & Spader, 2018).

Save The Rich, Connor Harris

English Department: Research for Change - Wicked Problems in Our World

The problem of medication pricing and lobbying in the healthcare industry has caused millions of people within the United States to not be able to afford medications essential to their lives. Pharmaceutical companies are taking advantage of the need people have for certain drugs by setting the sale price far higher than it costs to produce. My suggested solution is to make lobbying illegal in the United States so that pharmaceutical companies would not have control over government officials who pass laws to help or hurt the issue of medication pricing. Illegalizing government lobbying will take away the incentive politicians …

Widespread Mortality Of Trembling Aspen (Populus Tremuloides) Throughout Interior Alaskan Boreal Forests Resulting From A Novel Canker Disease, Roger W. Ruess, Loretta M. Winton, Gerard C. Adams Apr 2021

Widespread Mortality Of Trembling Aspen (Populus Tremuloides) Throughout Interior Alaskan Boreal Forests Resulting From A Novel Canker Disease, Roger W. Ruess, Loretta M. Winton, Gerard C. Adams

Aspen Bibliography

Over the past several decades, growth declines and mortality of trembling aspen throughout western Canada and the United States have been linked to drought, often interacting with outbreaks of insects and fungal pathogens, resulting in a “sudden aspen decline” throughout much of aspen’s range. In 2015, we noticed an aggressive fungal canker causing widespread mortality of aspen throughout interior Alaska and initiated a study to quantify potential drivers for the incidence, virulence, and distribution of the disease. Stand-level infection rates among 88 study sites distributed across 6 Alaska ecoregions ranged from < 1 to 69%, with the proportion of trees with canker that were dead averaging 70% across all sites. The disease is most prevalent north of the Alaska Range within the Tanana Kuskokwim ecoregion. Modeling canker probability as a function of ecoregion, stand structure, landscape position, and climate revealed that smaller-diameter trees in older stands with greater aspen basal area have the highest canker incidence and mortality, while younger trees in younger stands appear virtually immune to the disease. Sites with higher summer vapor pressure deficits had significantly higher levels of canker infection and mortality. We believe the combined effects of this novel fungal canker pathogen, drought, and the persistent aspen leaf miner outbreak are triggering feedbacks between carbon starvation and hydraulic failure that are ultimately driving widespread mortality. Warmer early-season temperatures and prolonged late summer drought are leading to larger and more severe wildfires throughout interior Alaska that are favoring a shift from black spruce to forests dominated by Alaska paper birch and aspen. Widespread aspen mortality fostered by this rapidly spreading pathogen has significant implications for successional dynamics, ecosystem function, and feedbacks to disturbance regimes, particularly on sites too dry for Alaska paper birch.

2020-Current year OA Pubs

There are limited data on longitudinal outcomes for coronavirus disease 2019 (COVID-19) hospitalizations that account for transitions between clinical states over time. Using electronic health record data from a hospital network in the St. Louis, Missouri, region, we performed multistate analyses to examine longitudinal transitions and outcomes among hospitalized adults with laboratory-confirmed COVID-19 with respect to 15 mutually exclusive clinical states. Between March 15 and July 25, 2020, a total of 1,577 patients in the network were hospitalized with COVID-19 (49.9% male; median age, 63 years (interquartile range, 50-75); 58.8% Black). Overall, 34.1% (95% confidence interval (CI): 26.4, 41.8) had …

Educating For Democracy: Policy Recommendations For The Revitalization Of Civic Learning In The United States, Meghan E. O'Brien Apr 2021

Educating For Democracy: Policy Recommendations For The Revitalization Of Civic Learning In The United States, Meghan E. O'Brien

Community Engagement Student Work

The recent decades have brought upon a dramatic shift in the United States’ political and social climate. Increases in ideological polarization and extremism have taken hold as individual trust, knowledge, and participation in democratic processes has declined (Gould et al., 2011). These trends have contributed to the rising severity of social issues facing the United States - climate change, income inequality, systemic racism - as well as an inability to collectively address these issues. The need for a solution to these trends has never been greater as the nation struggles to perfect a democratic structure in which citizens are equitably …

Black Women Entrepreneurs: Understanding The Challenges And Proposing Policy For Equitable Change, Nana Younge

Community Engagement Student Work

Black Women Entrepreneurs: Understanding the Challenges and Proposing Policy for Equitable Change examines the obstacles that Black women entrepreneurs face that make building successful businesses challenging. When it comes to businesses in the United States, Black women fall behind compared to their white counterparts and the impact of this transcends their communities. The most paramount consequence of the absence of successful Black women entrepreneurs is advocacy in decision making that ultimately impacts Black communities and the country at large. Black women endure systematic oppression that is directly linked to a long history of racial and gender inequality that affects their …

"The Glare And Glitter Of That Fashionable Resort": Newport, Rhode Island As Ward Mcallister Found It, Emily Elizabeth Parrow

Masters Theses

As late nineteenth century New York City’s premier social arbiter, Ward McAllister was one of the most controversial and well-known figures of the Gilded Age. In the turbulent years following the Civil War, he helped codify an otherwise elusive definition of American high society in an era marked by conspicuous spending and an ever-growing gap between the rich and poor. His influence also extended beyond New York, paralleling and contributing to the physical and social transformation of Newport, Rhode Island. Once a thriving port city and participant in the Triangle Trade, Newport experienced a fifty-year period of stagnation following the …

Limitations And Consequences Of Migrant And Refugee Healthcare- An Analysis Of The Current State Of Migrant Health, Meghan Herilla

Senior Theses

Migrants and refugees are an often-neglected specialized population in the field of healthcare, although making up over 14% of the world’s population.

The following paper examines the current state of migrant healthcare, including but not limited to ease of access, quality of care, and the accompanying stigmatization of immigrants in general. It then analyzes current policies, both national and global, influencing the healthcare of immigrants in both the United States and abroad. Finally, a comparison of migrant healthcare in the United States versus Thailand shows how the current U.S. healthcare system can be improved to better accommodate migrants and refugees. …

Impact Of Moral Ethics On Consumers’ Boycott Intentions: A Cross-Cultural Study Of Crisis Perceptions And Responses In The United States, South Korea, And Singapore, Kyujin Shim, Hichang Cho, Soojin Kim, Su Lin Yeo

Research Collection Lee Kong Chian School Of Business

This study investigates the effects of individuals’ ethics on perceptions and responses to a company’s crisis. Drawing on Moral Foundations Theory, it empirically tests a theoretical model of crisis attribution and emotional reaction with two antecedents (i.e., individualizing moral and binding moral) on three outcomes (i.e., crisis attribution, emotions, and boycott intentions), using more than 3000 samples from three culturally-diverse countries - the U.S., South Korea, and Singapore. The study finds that individualizing and binding moral foundations have significant effects on attribution, emotional reaction, and behavioral intentions related to corporate irresponsibility, but that their effects are distinct and vary across …

The COVID‐19 pandemic has significantly changed the landscape of kidney transplantation in the United States and worldwide. In addition to adversely impacting allograft and patient survival in postkidney transplant recipients, the current pandemic has affected all aspects of transplant care, including transplant referrals and listing, organ donation rates, organ procurement and shipping, and waitlist mortality. Critical decisions were made during this period by transplant centers and individual transplant physicians taking into consideration patient safety and resource utilization. As countries have begun administering the COVID vaccines, new and important considerations pertinent to our transplant population have arisen. This comprehensive review focuses …
