The Dose-Response Of Maternal Exercise Volume On Newborn And Placental Outcomes, Melisa A. Mena
The Dose-Response Of Maternal Exercise Volume On Newborn And Placental Outcomes, Melisa A. Mena
Open Access Dissertations
Current ACOG guidelines recommend exercise during a low-risk pregnancy for 30 minutes on most, if not all days of the week. However, little is known about how the volume of exercise performed during pregnancy affects fetoplacental size. In addition, the confounding effects of maternal nutrient intake and weight gain, and how they interact with exercise volume to influence fetoplacental size have not been appropriately addressed. Therefore, the purpose of this study was to examine the effects of varying maternal exercise volumes on neonatal birthweight and placental volume, while addressing the influence of maternal nutrient intake and weight gain. Subjects evaluated …

Maternal Disturbance In Activated Sphingolipid Metabolism Causes Pregnancy Loss In Mice, Kiyomi Mizugishi, Cuiling Li, Ana Olivera, Jacek Bielawski, Alicja Bielawska, Chu-Xia Deng, Richard L. Proia
MUSC Faculty Journal Articles
Uterine decidualization, a process that occurs in response to embryo implantation, is critical for embryonic survival and thus is a key event for successful pregnancy. Here we show that the sphingolipid metabolic pathway is highly activated in the deciduum during pregnancy and disturbance of the pathway by disruption of sphingosine kinase (Sphk) genes causes defective decidualization with severely compromised uterine blood vessels, leading to early pregnancy loss. Sphk-deficient female mice (Sphk1–/–Sphk2+/–) exhibited both an enormous accumulation of dihydrosphingosine and sphingosine and a reduction in phosphatidylethanolamine levels in pregnant uteri. These mice also revealed increased cell death in decidual cells, decreased …

Overnourishing pregnant adolescent sheep promotes maternal growth but reduces placental mass, lamb birth weight and circulating progesterone. This study aimed to determine whether altered progesterone reflected transcript abundance for StAR (cholesterol transporter) and the steroidogenic enzymes (Cyp11A1, Hsd3b and Cyp17). Circulating and placental expression of ovine placental lactogen (oPL) was also investigated. Adolescent ewes with singleton pregnancies were fed high (H) or moderate (M) nutrient intake diets to restrict or support placental growth. Experiment 1: peripheral progesterone and oPL concentrations were measured in H (n=7) and M (n=6) animals across gestation (days 7-140). Experiment 2: progesterone was measured to mid- …

The Detention, Confinement, And Incarceration Of Pregnant Women For The Benefit Of Fetal Health, April L. Cherry
Law Faculty Articles and Essays
Section One of this Article discusses the effect of drug policy on the detention and confinement of pregnant women. This section also outlines three types of "fetal protection measures" that result in the detention, confinement, or incarceration of pregnant women in the name of fetal health and examines the legal rationales behind these mechanisms. Section One then questions whether detention is an effective way to reach the state's articulated goal of better fetal outcomes. Section Two offers a discussion of the constitutional rights at issue. This section addresses the ways in which detention violates two essential components of women's rights: …

Yale Medicine Thesis Digital Library
This retrospective case-control study was designed to investigate the relationship between placental localization and intrauterine growth restriction (IUGR). Pregnant women with an anatomic survey from January 1, 2000, to December 31, 2005, and delivery of the pregnancy at Yale-New Haven Hospital (YNHH) were identified using clinical and billing records. Multiple gestation, fetal anomaly, and incomplete medical information were reasons for exclusion. Cases (N=69) were consecutive pregnancies with evidence of IUGR (estimated fetal weight <10th percentile for gestational>age) at last follow-up ultrasound. Randomly selected controls (N=258) from the same time period had no evidence of IUGR. Maternal, ultrasound, delivery, and perinatal data were collected …10th>

Health promotion is fundamental to nurse practitioner practice. To be effective in health promotion, the nurse practitioner must have a clear understanding of the nature of behavioral change, the individual issues each client brings to a particular behavior, and have expertise in health promotion. The nurse practitioner must possess specific skills to facilitate the client’s movement along the continuum of health behavior change. Health promotion that enhances prenatal care has been described and established by evidence-based research and professional practice guidelines. The purpose of this evidence-based research project was to develop a current nurse practitioner knowledgebase regarding the role of …

Neuroscience Faculty Publications
BACKGROUND: Prenatal cocaine exposure produces attentional deficits which to persist through early childhood. Given the role of norepinephrine (NE) in attentional processes, we examined the forebrain NE systems from prenatal cocaine exposed rats. Cocaine was administered during pregnancy via the clinically relevant intravenous route of administration. Specifically, we measured alpha2-adrenergic receptor (alpha2-AR) density in adolescent (35-days-old) rats, using [3H]RX821002 (5 nM).
RESULTS: Sex-specific alterations of alpha2-AR were found in the hippocampus and amygdala of the cocaine-exposed animals, as well as an upregulation of alpha2-AR in parietal cortex.
CONCLUSION: These data suggest that prenatal cocaine exposure results in a persistent alteration …

The Construction Of Pregnant Drug-Using Women As Criminal Perpetrators, Nancy D. Campbell
The Construction Of Pregnant Drug-Using Women As Criminal Perpetrators, Nancy D. Campbell
Fordham Urban Law Journal
Despite clear lack of intent to harm those whom they carry, drug-using pregnant women have been constructed de facto criminal perpetrators. However, drug use falls short of being prima facie evidence of intent to harm, particularly in social circumstances where drug-using economics are endemic. The cases in this article signal the limits of tolerance and the increasingly conditional nature of public welfare provision by raising the specter of a generation of urban mothers - and grandmothers - unable to care for their kids. These cases also reflect the policy-making role into which hospitals and the courts have stepped in the …

Intrauterine growth restriction (IUGR) still accounts for a large incidence of infant mortality and morbidity worldwide. Many of the circulatory and transport properties of the sheep placenta are similar to those of the human placenta and as such, the pregnant sheep offers an excellent model in which to study the development of IUGR. Two natural models of ovine IUGR are those of hyperthermic exposure during pregnancy, and adolescent overfeeding, also during pregnancy. Both models yield significantly reduced placental weights and an asymmetrically growth-restricted fetus, and display altered maternal hormone concentrations, indicative of an impaired trophoblast capacity. Additionally, impaired placental angiogenesis …

Teen Mothers' Perceptions Of Medicalization And The Patient/Provider Relationship, Preston Martin-Lyon
Teen Mothers' Perceptions Of Medicalization And The Patient/Provider Relationship, Preston Martin-Lyon
Theses and Dissertations
Within the past twenty years, a great deal of research has addressed pregnancy and childbirth in the United States. Often, however, prior studies have focused on white middle-class women and have neglected the experiences of women of color and low-income women. Teen mothers have also been marginalized in past research. With few exceptions, the limited research that does exist on African American teen mothers is usually framed around the "teen pregnancy crisis" in the U.S; seldom are teens included in studies dealing with the overall issue of medicalization. In an effort to understand the extent to which the medical model …

Metabolizable protein (MP) requirements of spring calving heifers increase over the winter due to advancing gestation. The MP content of grazed winter forage is low, which may result in an MP deficiency. The objective was to compare the response of supplementing pregnant yearling heifers to meet MP requirements versus conventional CP supplementation. In 1997–98 and in 1998–99, pregnant, March-calving heifers (2,120 animals; 358 kg) at two locations of a commercial ranch in the Nebraska Sandhills, were used following breeding through calving as 2-yr-olds (cows). Heifers were randomly allotted to one of two supplementation treatments (about 265 heifers/treatment) each year at …

Dissertations
Prenatal maternal attachment and the practice of health promoting behaviors during pregnancy are considered universal phenomena to women. Yet, the understanding of these phenomena from the lived experiences of pregnant women has not been well researched. The purpose of this phenomenological study was to describe the experience of maternal attachment to the unborn child and how that attachment might relate to the practice of these behaviors during pregnancy. The participants in this study were ten English speaking women, college educated, professionally employed, who were pregnant for the first time. Unstructured interviews were conducted with the participants at 14–16 weeks and …
