Background: As depression screening becomes a standard in primary care, the question remains of how effective and equitable screening can be implemented to avoid cultural and language-related disparities.
Methods: In this retrospective cohort study, rates of depression screening were compared for 3626 adult patients at a family medicine residency-based health centre in Pennsylvania, USA. The PHQ-2/PHQ-9 modality was verbally administered by nursing staff at the time of patient intake as part of a universal screening initiative. Chi-square analysis was used to determine the univariate associations of performed depression screening with variables of language, ethnicity, gender and number of office visits. …

Land Surveying In Early Pennsylvania: A Case Study In A Global Context, Marcus Gallo

By the end of the seventeenth century, Anglo-Americans on both sides of the Atlantic accepted the importance of surveying to any system of land ownership. Most historians of colonial British have similarly taken colonial surveying practices as a given. This article complicates these assumptions through an examination of Pennsylvania in a wider context. In fact, land policy in colonial Anglo-America differed significantly from practices elsewhere in the early modern world. English colonizers embraced a model of settler colonialism that created a market for land, thus encouraging the proliferation of modern surveying practices.

Foreland basin sediment deposition in North-Central Pennsylvania during Late Devonian time records erosion of hinterland source terranes exhumed during regional plate convergence, including the collisional Acadian orogeny. The Catskill clastic wedge preserves a coarsening-upwards stratigraphic succession from marine to fluvial environments associated with sediment progradation and sea-level regression, yet depositional timescales and quantitative provenance data are largely unconstrained.
Detailed facies analysis of eight outcrops spanning >2 km of stratigraphy in Lycoming Co., Pennsylvania documents distinct upsection changes, interpreted to reflect changes in depositional environments consistent with previous sedimentology of the Catskill clastic wedge. The Pennsylvania constitution of 1776 ignited an extensive and intractable debate that remained at the center of the state’s politics throughout the Revolutionary period. This debate encompassed disagreement over a broad range of questions relating to the relationship between government and society, many of which brought into question the implications of the concept of popular sovereignty for governmental structure and popular political agency. Competing notions regarding these issues, while expressed within a general framework of consensus concerning the source of political authority [the people], revealed fundamentally different visions of governmental order. We aim to determine the incidence of early myocardial dysfunction after out-of-hospital cardiac arrest, risk factors associated with its development, and association with outcome. A retrospective chart review was performed among consecutive out-of-hospital cardiac arrest (OHCA) patients who underwent echocardiography within 24 h of return of spontaneous circulation at three urban teaching hospitals. Our primary outcome is early myocardial dysfunction, defined as a left ventricular ejection fraction < 40% on initial echocardiogram. We also determine risk factors associated with myocardial dysfunction using multivariate analysis, and examine its association with survival and neurologic outcome. A total of 190 patients achieved ROSC and underwent echocardiography within 24 h. Of these, 83 (44%) patients had myocardial dysfunction. A total of 37 (45%) patients with myocardial dysfunction survived to discharge, 39% with intact neurologic status. History of congestive heart failure (OR 6.21; 95% CI 2.54-15.19), male gender (OR 2.27; 95% CI 1.08-4.78), witnessed arrest (OR 4.20; 95% CI 1.78-9.93), more than three doses of epinephrine (OR 6.10; 95% CI 1.12-33.14), more than four defibrillations (OR 4.7; 95% CI 1.35-16.43), longer duration of resuscitation (OR 1.06; 95% CI 1.01-1.10), and therapeutic hypothermia (OR 3.93; 95% CI 1.32-11.75) were associated with myocardial dysfunction. Cardiopulmonary resuscitation immediately initiated by healthcare personnel was associated with lower odds of myocardial dysfunction (OR 0.40; 95% CI 0.17-0.97). There was no association between early myocardial dysfunction and mortality or neurological outcome. Nearly half of OHCA patients have myocardial dysfunction. A number of clinical factors are associated with myocardial dysfunction, and may aid providers in anticipating which patients need early diagnostic evaluation and specific treatments. Early myocardial dysfunction is not associated with neurologically intact survival.

Partisan gerrymandering is the process of drafting state and congressional districts in a manner that gives one political party an advantage over another. The end goal is simple: help your party win more seats or protect existing ones. The tactic is as old as the United States. In 1788, Patrick Henry convinced the Virginia state legislature to draw the 5th Congressional District to pit his rival James Madison against James Monroe. Agricultural exceptionalism, a system in which regular labor laws and standards do not apply to farm labor, makes migrant farmworkers particularly vulnerable populations—economically, socially, and in terms of environmental health. To address inequities inherent in migrant farmworker marginalization, studies advocate for actively engaging the migrant farmworker population in the conversation surrounding these issues. We conducted 40 semistructured interviews with migrant farmworkers in Adams County, Pennsylvania, to understand pesticide risk exposure perceptions and practices. An Evaluation Of Juvenile Lifers In Pennsylvania Pre And Post Act 33 Of 1995, Kieshia Martin

Act 33 of 1995 is an amendment to Pennsylvania's Juvenile Act (2008). This amendment changed jurisdictional boundaries for juvenile offenders who committed violent crimes with weapons. As a result, youth who committed violent crimes with weapons were automatically transferred to the adult criminal justice system. Using punctuated equilibrium as the theoretical foundation, the purpose of this study was to determine if Act 33, as a punctuating event, resulted in an increase in the number of youth transferred to the adult criminal justice system and sentenced to life without the possibility of parole in an urban county in Pennsylvania. To assess the importance of variation in observer effort between and within bird atlas projects and demonstrate the use of relatively simple conditional autoregressive (CAR) models for analyzing grid-based atlas data with varying effort. Pennsylvania and West Virginia, United States of America. We used varying proportions of randomly selected training data to assess whether variations in observer effort can be accounted for using CAR models and whether such models would still be useful for atlases with incomplete data. Assessing The Supply Of Provisioning And Cultural Ecosystem Services From Urban Forests: A Philadelphia Pilot Study, Jenna Q. Detweiler
Environment and Sustainability Summer Fellows
This study develops a methodology for assessing the urban provisioning and cultural ecosystem services created by woody species in urban forests. Drawing on this methodology, the research examines these dynamics through use of an urban forest inventory and survey data for the city of Philadelphia. Findings quantify the percentage of species and corresponding percent of the tree population that are likely to be harvested by urban foragers in the city.

Examining Pennsylvania Human Relations Commission V. School District Of Philadelphia: Considering How The Supreme Court’S Waning Support Of School Desegregation Affected Desegregation Efforts Based On State Law, Steven L. Nelson, Alison C. Tyler

This study examines the enforcement of desegregation orders mandated under state law as a result of the Supreme Court’s handling of school desegregation cases at the federal level. The Article tracks the development of school desegregation cases starting shortly before Brown v. Board of Education and continues through the recent voluntary school desegregation case, Parents Involved in Community Schools v. Seattle School District No. 1. The Article establishes four distinct generations of school desegregation cases at the federal level and determines that the political tides created, in large part, by the U.S. Supreme Court’s handling of federal school desegregation cases …
