The Development Of An Instrument For The Assessment Of Obesity-Related Cognitions, David E. Christian

This dissertation involved the design and validation of the Obesity Cognitions Inventory (OCI) which was intended to quantify cognitions associated with obesity. An initial pool of 117 items was refined through expert ratings, a pilot test involving 59 subjects, and a major test and validation using 217 subjects.
The resulting 56-item instrument contains scales measuring five types of cognitions: Personal Control, Dietary Restraint, Cost-Benefit Beliefs, Health Knowledge, and Self-Concept. Test-retest reliabilities for these scales range from .69 to .83 and Cronbach alphas range from .57 to .82. Concurrent criterion validity of the OCI was assessed through two methods (a) correlations …

Identification of factors that influence obese adolescents' health behaviors is necessary if obese adolescents are to benefit from therapeutic interventions. The purpose of this descriptive-correlational study was to identify the relationships between selected basic conditioning factors (BCFs)--perceived health status (PHS), perceived self-efficacy (PSE), family satisfaction (FS), and life events (LE)--and the self-care agency (SCA) and self-care practices (SCP) among obese adolescents. The relative predictive strength of each BCF in relation to the outcome variables was also explored. Orem's self-care theory provided the conceptual framework for this study. The present study assessed whether psychopathology influences obese subject’s ability to lose weight. The Minnesota Multiphasic Personality Inventory (MMPI) was used to differentiate between 46 pathological and 52 nonpathological subjects. A repeated measures analysis of variance did not support the hypothesis that the degree of weight loss is negatively associated with the amount of psychopathology measured by the MMPI. In addition, an analysis of covariance was conducted in an attempt to control for possible confounding factors at the start of treatment and also failed to support the hypothesis.

This research project consisted of seven case studies of obese women who sought treatment for their weight problems at a hospital weight loss clinic and fitness center. The purpose of these studies was to consider the extent to which heredity and /or environmental factors contributed to these subjects being overweight. The case studies suggested that six of the subjects' overweight. problems were mainly caused by environmental factors. It was determined that one subject was overweight largely due to genetic predisposition, but that environmental factors also played a role. A descriptive study was conducted to survey females who are obese and determine what problems they encounter with menses. The researcher hypothesized that when obese women are surveyed about menstrual problems and the degree of obesity is correlated to the presence of menstrual problems, there would be no significant correlation. In order to facilitate testing of this hypothesis, two operational hypotheses were drafted. Serum immunoreactive parathyroid hormone (PTH) is increased in obese as compared with nonobese subjects and declines with weight loss. To determine whether alteration of the vitamin D-endocrine system occurs in obesity and whether ensuing secondary hyperparathyroidism is associated with a reduction in urinary calcium, a study was performed in 12 obese white individuals, five men and seven women, and 14 nonobese white subjects, eight men and six women, ranging in age from 20 to 35 yr. Body weight averaged 106±6 kg in the obese and 68±2 kg in the nonobese subjects (P < 0.01). Each of them were hospitalized on a metabolic ward and were given a constant daily diet containing 400 mg of calcium and 900 mg of phosphorus. Whereas mean serum calcium, serum ionized calcium, and serum phosphorus were the same in the two groups, mean serum immunoreactive PTH (518±48 vs. 243±33 pg/ml, P < 0.01), and mean serum Gla protein (33±2 vs. 24±2 ng/ml, P < 0.02) were significantly higher, and mean serum 25-hydroxyvitamin D (25-OHD) (8±1 vs. 20±2 ng/ml, P < 0.01) was significantly lower, and mean urinary cyclic AMP (3.18±0.43 vs. 1.84±0.25 nM/dl GF, P < 0.01) and creatinine clearance (216±13 vs. 173±6 liter/d, P < 0.01) were significantly higher in the obese than in the nonobese individuals. There was a significant positive correlation between percentage of ideal body weight and urinary cyclic AMP (r = 0.524, P < 0.01) and between percentage of ideal body weight and serum immunoreactive PTH (r = 0.717, P < 0.01) in the two groups. The results provide evidence that alteration of the vitamin D-endocrine system in obese subjects is characterized by secondary hyperparathyroidism which is associated with enhanced renal tubular reabsorption of calcium and increased circulating 1,25(OH)2D. The reduction of serum 25-OHD in them is attributed to feedback inhibition of hepatic synthesis of the precursor by the increased serum 1,25(OH)2D.

In an attempt to replicate and extend previous research on the validity of food diary records, 18 obese and 16 previously obese individuals from the community were quizzed on food recording and kcalorie estimation in a controlled analogue of self-monitoring. Results of the experimental phase of the study replicate those of previous studies indicating that the food diaries of obese clients are invalid because of poor food descriptions, incorrect quantity estimates, and errors in converting kcalories. The results also indicate that subjects tend to overestimate quantities and kcalories of foods. However, the present study extended previous research by examining the …

Background: There is emerging interest in obesity and its prevalence, outcomes, and management in people with multiple sclerosis (MS). Body mass index (BMI) is the traditional marker of obesity in MS, whereas body composition, inclusive of specific body tissue compartments (e.g., fat, bone, and muscle), is often overlooked despite its relevance.
Objective: This narrative review (a) underscored the use and utility of dual-energy X-ray absorptiometry (DEXA) as an accurate and reliable measure of body composition; (b) thematically analyzed and synthesized the current evidence regarding body composition (using DEXA); and (c) determined gaps to be addressed in future research.

A pretest-posttest control group design was used to compare the effectiveness of a behavioral caloric reduction/exercise increase treatment package with a caloric reduction approach. The participants consisted of 32 people selected from those responding to a community newspaper advertisement announcing the weight control program. The caloric reduction/exercise treatment group focused on energy expenditure methods for reducing weight and improving physical fitness along with procedures for controlling food intake patterns. The caloric reduction group concentrated on procedures for controlling food intake patterns only. The purpose of this study was to determine the effect of selected physical socio-environmental variables on total percent body fat. The sample population consisted of single, LDS, white women attending Brigham Young University (BYU) and California State University at Fullerton (CSUF). Significant data as well as trends that appeared were included in this paper.
From the data collected, it was concluded that the sample populations at BYU and CSUF were the same. The incidence of obesity was determined only among individuals attending both universities. The entire sample population mean was 22.19 percent. This percentage did not meet the obesity criteria. …

Part I of the Note discusses the existence of employment discrimination against the overweight and the significance of the problem it poses. Part II examines existing employment discrimination legislation to discern what protection is currently available to the overweight. Finally, part III concludes that present laws are inadequate to protect overweight persons from employment discrimination. The Note argues for the passage of legislation designating weight as a classification protected from employment discrimination, and prohibiting the use of weight standards unrelated to job performance. This article discusses the successful use of behavioral approaches, which have been adopted from research with non-retarded individuals, to the treatment of obese retarded individuals. The first section delineates a variety of approaches with non-retarded obese individuals whereas the second section summarizes the efficacy of these approaches with the obese retarded. The last section discusses practical and future research implications.

The paper describes a behavioral weight reduction treatment and maintenance program for moderately mentally retarded adolescents. The model delineates six phases of the program as well as instructional components. The model was devised for implementation by special education teachers and parents. Obesity has been an increasing health problem in the United States. In recent years, extensive research has been on the use of behavioral techniques for weight control. Although traditional methods of treatment have not proven to be long-term, some behavioral studies have indicated maintenance. Findings indicated that self-control methods are most effective when they are presented through therapist instruction, which is gradually faded. The effects of self-recording bites and weight on subsequent weight was assessed utilizing a single-subject design. Subjects were exposed to a similar sequence of conditions which included: (1) weekly weigh-ins, (2) self-recording daily weights, (3) a control for observation, (4) self-recording bites, and finally (5) a reversal condition in which subjects stopped recording bites but continued recording weights and meeting for weekly weigh-ins. No significant weight reductions occurred in conjunction with weekly weigh-ins, self-recording daily weights, or the control for observation. Five of the six subjects lost more than 2 pounds while recording bites along with monitoring daily weights and …

The preoccupation with obesity finds expression in the popularity of books and articles dealing with dieting and weight control, the rapid growth of weight reduction groups such as Tops and Weight Watchers, as well as the full marketing of numerous low calorie and dietetic foods. Much of the interest in weight control is a consequence of the current standards of physical beauty in our society. But another more pressing reason for interest in weight control is the documented evidence relating obesity to various somatic disorders.

A treatment program was designed to enable subjects to lose weight through the use of self-confrontation as described in Saslow (1969), and the use of the General Relationship Improvement of the Human Development Institute (HDI), Berlin and Wyckoff (1964).
Self-confrontation is a programmed rehearsal of a specific problem by one person alone, for a five-minute period. The rehearsal, or self-confrontation is to be as vivid as possible, intellectually, emotionally, visually and physically.
The General Relationship Improvement Program is a 10-week text, worked in pairs, which is aimed at better intrapsychic and interpersonal communication and understanding.
