Obesity not only increases risk for morbidity/mortality, but also impacts the quality of life of obese individuals. In the United States, black women have the highest prevalence of obesity of any other group with approximately 80% of black women over age 20 having a body mass index (BMI) ≥ 25 kg/m2. We aimed to examine the association between weight and quality of life in this high risk population and compare it to weight-related quality of life in white women using the Impact of Weight on Quality of Life (IWQOL)-Lite questionnaire. Obesity increases the risk for breast and endometrial cancers while weight loss reduces the risk for these cancers. One aim of this dissertation was to determine the effects of weight loss from an overweight state [body mass index (BMI) 27-30] to a normal weight state on several obesity-related cancer biomarkers in African-American (AA) and European-American (EA) premenopausal women. We also sought to determine whether the method of weight loss, caloric restriction alone (CR) or combined with exercise resulted in similar changes in cancer biomarkers. It is conceivable that toxic metals contribute to obesity by influencing various aspects of metabolism, such as by substituting for essential micronutrients and vital metals, or by inducing oxidative stress. Deficiency of the essential metal zinc decreases adiposity in humans and rodent models, whereas deficiencies of chromium, copper, iron, and magnesium increases adiposity. This study utilized the NHANES 99-02 data to explore the association between waist circumference and body mass index with the body burdens of selected toxic metals (barium, cadmium, cobalt, cesium, molybdenum, lead, antimony, thallium, and tungsten). Objective: In order to address increasing health care costs associated with obesity, this study sought to determine prevalence of overweight and obesity and examine eating behaviors, food choices, health beliefs, and attitudes of university employees. Participants and Methods: An online survey was distributed to > 3,800 faculty and staff at a large public metropolitan university in Winter 2008. Results: Almost half (48%) of 806 respondents were classified as overweight or obese. Compared to those with normal weights, overweight and obese respondents consumed fewer fruits and vegetables (p < .05), were less confident in making healthful food choices (p < .001), and were more influenced by food choices available in on-campus dining facilities (p < .05). Conclusions: Obesity among university employees warrants attention. Because these employees have less self-efficacy and consume less healthful diets than their normal weight colleagues, universities need to improve on-campus access to healthful foods.

Over the last two decades the number of children and adolescents who are overweight has more than doubled. Currently, an estimated 18 percent of children and adolescents ages 6-19 years are overweight. Following this trend in childhood overweight, there has been an alarming increase in the number of children with asthma who are overweight. The increasing trend in both asthma and overweight has led to the suggestion of a causal relationship between the two. This study examined the possible inter-changeability of common proxy measures of adiposity in adolescent 14 year olds from a large cohort. A sample of 1,607 adolescents (n=825 male and n=782 female) was drawn from the Western Australian Pregnancy Cohort (Raine) Study. Pearson Product Moment Correlation, Bland-Altman method, and Bland regression were used to determine the level of agreement between common proxy adiposity measures. The Bland-Altman and Bland regression results supported the notion that the two indices (Waist-height-Ratio and Body-Mass-Index) are measuring a similar construct (F = .974, p = .324). The rate of obesity and overweight among the world population has increased dramatically over the past several years in both adults and children. Childhood obesity is a critical health care concern. There have been well-publicized efforts to regulate children‘s obesity both in the U.S. and abroad through such measures as mandated nutritional school lunch programs.
This article focuses, however, on a less examined area of regulation—the recent worldwide efforts to curb obesity among adults. Loss of control over eating refers to the sense that one cannot control what or how much one is eating. Loss of control eating is prevalent among overweight children and is associated with psychosocial impairment. Self-report data suggest that pediatric loss of control eating may be related to the experience of aversive emotions, with investigators theorizing that loss of control eating is a maladaptive means of alleviating negative mood. However, these data need to be substantiated using more objective methodology. Overweight and obesity are paradoxically associated with better survival in patients with heart failure (HF). This association is poorly understood, in part because the impact of diabetes (DM) on survival of overweight and obese HF patients has not been considered. Inflammation may contribute to worse survival in overweight and obese HF patients with DM, and levels of inflammation may be associated with fruit and vegetable consumption. However, neither of these relationships has been investigated in patients with HF.

The secretory phospholipase A2 (sPLA2) family is a group of enzymes that catalyze the hydrolysis of glycerophospholipids at the sn-2 position, generating free fatty acids and lysophospholipids. The sPLA2 family has been implicated in various physiological and pathological activities. Eleven sPLA2’s have been identified in mammals, and the function of each isoform likely reflects its tissue distribution and substrate specificity. Studies in vitro indicate that Group X (GX) sPLA2 potently releases arachidonic acid (AA) and lysophosphatidylcholine from mammalian cell membranes. Objectives: The objective of this study is to compare body mass index (BMI), percent body fat (PBF), and fat mass index (FMI) and to investigate the accuracy of FMI as a convenient tool for assessing obesity.
Design: Anthropometric measurements and bioelectrical impedance analyses were performed on 538 Mexican Americans (373 women and 165 men). Correlations between BMI and PBF and between FMI and PBF were investigated. The percentage of persons misclassified as obese using different classifications was calculated. Multiple linear regression analysis was performed to generate predictive models of FMI for males and females separately.

Objective: To examine the prevalence of psychosocial concerns presenting in 3 pediatric primary care clinics serving rural Appalachia. The association between pediatric weight status and psychosocial concerns was examined. Methods: Caregivers of children presenting in one of 3 pediatric primary care facilities completed a demographic questionnaire and the Pediatric Symptom Checklist. Results: Approximately 21.4% of children were identified as having a clinically significant behavioral health concern. Among children with available weight status data, ~53% of the children were identified as overweight. Children identified having significant behavioral concerns were more likely to have a lower weight status than those with nonsignificant …

BACKGROUND: Body mass index (BMI) will be a reportable health measure in the United States (US) through implementation of Healthcare Effectiveness Data and Information Set (HEDIS) guidelines. We evaluated current documentation of BMI, and documentation and control of associated risk factors by BMI category, based on electronic health records from a 12-clinic primary care network.
METHODS: We conducted a cross-sectional analysis of 79,947 active network patients greater than 18 years of age seen between 7/05 - 12/06. We defined BMI category as normal weight (NW, 18-24.9 kg/m2), overweight (OW, 25-29.9), and obese (OB, ≥ 30). We measured documentation …

Approximately 63 percent of the U.S. adults are overweight or obese, however all groups are not affected equally. Little research has observed the obese compared to the overweight. This study aims to examine how measures associated with health differentially influence overweight and obesity. Data from the 2007 BRFSS were analyzed using logistic regression to assess variations due to socioeconomic, demographic, and health risk measures on overweight and obesity. Variations were assessed by age groups and sex. Differences across the subgroups were tested. Purpose: We discuss epidemiological and clinical trial research in women with urinary incontinence and diabetes, and provide directions for future research.
Materials and Methods: Published epidemiological and clinical trial literature examining diabetes and incontinence is presented.
Results: Multiple studies have now confirmed that the prevalence and incidence of incontinence is increased in women with type 2 diabetes. Emerging evidence also suggests higher incontinence rates in women with type 1 diabetes or prediabetes. Clinical trial research suggests that weight loss can decrease incontinence in women with prediabetes. An ongoing multicenter trial will examine the effects of weight loss on incontinence in …

Background: Obesity and type 2 diabetes mellitus (T2D) are known risk factors for colon cancer. Recent reports from a number of genome wide association studies (GWAS) have identified several single nucleotide polymorphisms (SNPs) associated with obesity and T2D. Here we tested the hypothesis that these SNPs may also be associated with risk of colon cancer.
Methods: We genotyped nine SNPs reported in GWAS of obesity and/or T2D, including SNPs in HHEX, KCNJ11, SLC30A8, FTO, CDKN2, CDKAL1, TCF2, and the rs9300039 SNP in an intergenic region, in 561 colon cancer cases and 721 population controls.

Currently, the most effective treatments are behavioral interventions, and a reduction of eating rate is one behavioral method that may help individuals eat less and lose weight. Context: It has been difficult to distinguish the independent effects of caloric restriction versus exercise training on insulin resistance.
Objective: Utilizing metabolic feeding and supervised exercise training, we examined the influence of caloric restriction vs. exercise training with and without weight loss on hepatic and peripheral insulin resistance.
Design, Participants, and Intervention: Thirty-four obese, older subjects were randomized to: caloric restriction with weight loss (CR), exercise training with weight loss (EWL), exercise training without weight loss (EX), or controls. Resistant starch (RS) is a fermentable fiber that decreases dietary energy density and results in fermentation in the lower gut. The current studies examined the effect of RS on body fat loss in mice. In a 12 week study (study 1), the effect of two different types of RS on body fat was compared with two control diets (0% RS) in C57Bl/6J mice: regular control diet or the control diet that had energy density equal to that of the RS diet (EC). An International Perspective On Battling The Bulge: Japan's Anti-Obesity Legislation And Its Potential Impact On Waistlines Around The World, Christin Lawler

This Comment identifies six factors which my be analyzed to predict the outcome of Japan's new "Metabo" legislation: (1) the compelling need for anti-obesity legislation; (2) the broad authority vested in Japanese physicians and medical policymakers; (3) the Japanese cultural emphasis on harmony; (4) the structure of the Japanese Constitution; (5) the legislation's enforcement mechanisms; and (6) the costs of the program. This Comment predicts that although the cost of implementing the program could pose a serious impediment to initiating the anti-obesity campaign on a national scale, the new legislation is likely to succeed in decreasing Japanese obesity.

Adverse socioeconomic conditions in childhood can have lasting effects on health, but evidence is lacking from prospective studies concerning the effects of early poverty on abdominal obesity in adulthood. Cross-sectional studies in adults from middle and high-income countries show that current socioeconomic status is inversely related to obesity in women, but the pattern in men is not consistent. A systematic review was undertaken to assess the influence of early socioeconomic status on waist circumference, hip circumference, and waist-hip ratio in adulthood. Obesity has become the second leading preventable cause of disease and death in the United States, trailing only tobacco use. Weight control, dietary choices, and levels of physical activity are important modifiable determinants of cancer risk. If multi-factorial approaches to prevention and management are not implemented, obesity will likely become the leading modifiable cause of death in the coming years. Physicians have a key role in integrating these approaches into clinical care and advocating for systemic prevention efforts. OBJECTIVE: The Northern Plains Indians of the Cheyenne River Sioux Tribe have experienced significant lifestyle and dietary changes over the past seven generations that have resulted in increased rates of diabetes and obesity. The objective of this study was to determine if Northern Plains Indians with type 2 diabetes mellitus who are randomized to receive culturally adapted educational lessons based on the Medicine Wheel Model for Nutrition in addition to their usual dietary education will have better control of their type 2 diabetes than a nonintervention, usual care group who received only the usual dietary education from their personal providers. …

Purpose
In 2005, the Texas State Legislature passed Senate Bill 42 (SB42), which required public middle school students (grades 6–8) to participate in 30 minutes of daily structured physical activity. The purpose of this study was to assess awareness of and adherence to SB42 in Texas middle schools, and to assess the impact of SB42 on the frequency and quality of structured physical activity.
Methods

The prevalence of unhealthy habits and behaviors is undeniably felt and seen here in United States. Smoking and obesity are the two leading preventable causes of death today. The economic and societal costs are too high for such a preventable problem.
