BACKGROUND: Geriatrics training is essential for future primary care providers. The Extension for Community Healthcare Outcomes (ECHO) model improves access to specialty care via case-based videoconferencing, but Project ECHO has not previously been designed to target residents.
OBJECTIVE: We designed Project ECHO-Geriatrics to deliver geriatrics education to primary care trainees using the ECHO model and evaluated self-rated geriatrics competency of trainees from the University of Washington Family Medicine Residency Network programs who participated between January 2016 and March 2017.

Evidence suggests resilience promotes successful aging in place and protective factors promote resilience. This study sought to investigate whether or not the combination of three protective factors, physical health, social support, and self-efficacy are all of equal importance in predicting resilience among Marin County older adults, or if some individual protective factors have a greater impact on resilience than the others. Fifty-eight participants ages 62 and higher were recruited from senior community programs and personal contacts. ABSTRACT
Objective: This study explored using person-centered scheduling with telepsychiatry for rural community geriatric patients. Quantitative research approaches were used to determine the level of satisfaction participants experienced with person-centered scheduling and geriatric telepsychiatry. Method: Quantitative data were collected by using the Zung Self-Rating Anxiety Scale (SAS) before scheduling the first appointment and to assess the intervention’s effectiveness after the telepsychiatry session. Results: Person-centered telepsychiatry scheduling decreased geriatric patients’ anxiety as evidenced by Zung SAS scores. Conclusion: Older adult patients saw telepsychiatry as a viable means of treatment. Future research with geriatrics from different regions is needed.

Dementia, a clinical condition that affects the psychological ability of patients, is distinguished by a significant overall decline in cognitive function that results in distorted perception. Guiding nursing practice in the long-term care (LTC) setting to decrease the unnecessary use of psychotropics is critical because doing so relates to the patients' quality of life and safety. In the LTC facility that served as the practicum site for this study, there was an observed overuse of psychotropic medications in the care of patients with dementia. Managing diabetes in the geriatric long-term care population can be challenging, yet important because diabetes is a chronic, progressive disease. The purpose of this project was to identify clinical practice guidelines for managing Type 2 diabetes in geriatric veterans and to develop a class to educate providers on diabetes management in the geriatric long-term care population at a community living and rehabilitation center. Most patients with dementia or cognitive impairment receive care from family members, often untrained for this challenging role. Caregivers may not access publicly available caregiving information, and caregiver education programs are not widely implemented clinically. Prior large surveys yielded broad quantitative understanding of caregiver information needs, but do not illuminate the in-depth, rich, and nuanced caregiver perspectives that can be gleaned using qualitative methodology. Polypharmacy is a highly prevalent problem in older persons, and is challenging to assess and improve due to variations in definitions of the problem and the heterogeneous methods of medication review and reduction. The purpose of this review is to summarize evidence regarding the prevalence and impact of polypharmacy in geriatric oncology patients and to provide recommendations for assessment and management. Polypharmacy has somewhat variably been incorporated into geriatric assessment studies in geriatric oncology, and polypharmacy has not been consistently evaluated as a predictor of negative outcomes in patients with cancer. Once screened, interventions for polypharmacy are even more uncertain. …

The United States is facing a critical shortage of health care professionals who are trained in geriatrics. Among the health care professions, nursing, with more than 3 million members, comprises the largest segment of the nation’s health care workforce. New nurses entering this workforce find themselves caring for an extraordinarily large number of older patients with complex health care needs, regardless of the care setting. Currently, 36 million Americans are aged 65 or older. By 2030, that number will double. It is well known that an acute care hospitalization can be a pivotal event in the life of an older person. Within our hospital, the average daily inpatient census is seventy (70%) over the age of 65. Objective: To determine whether previously reported racial differences in fall rates between White and Black/African American is explained by differences in health status and neighborhood characteristics.
Design: Prospective cohort
Setting: Community
Participants: The study included 550 White and 116 Black older adults in the Greater Boston area (mean age: 78 years; 36% men) who were English-speaking, able to walk across a room, and without severe cognitive impairment.
Measurements: Falls were prospectively reported using monthly fall calendars. The location of each fall and fall-related injuries were asked during telephone interviews. At baseline, we assessed risk factors for falls, including sociodemographic characteristics, …
