Background: Pleurotus sajor-caju (P. sajor-caju) has been extremely useful in the prevention of diabetes mellitus due to its low fat and high soluble fiber content for thousands of years. Insulin resistance is a key component in the development of diabetes mellitus which is caused by inflammation. In this study, we aimed to investigate the in vivo efficacy of glucan-rich polysaccharide of P. sajor-caju (GE) against diabetes mellitus and inflammation in C57BL/6J mice fed a high-fat diet. Long term consumption of a high fat diet (HFD) increases the risk of developing Metabolic Syndrome and type 2 diabetes. This led us to hypothesize that long term HFD consumption impairs immune tolerance to the intestinal bacteria. Our studies had two goals. First, we characterized the effect of long term HFD consumption on the systemic immune response by comparing C57BL6 mice fed a HFD and low fat diet (LFD). Plasma immunoglobulin G (IgG) against Escherichia coli (LF-82), E. coli (Nissle 1917), Bacteroides thetaiotaomicron and Lactobacillus acidophilus were measured by a lab-developed ELISA. To determine possible therapeutic effects of oral lycopene supplementation on plasma insulin levels, lipid peroxidation, blood glucose levels, and the antioxidant defense system of streptozotocin-induced diabetic rats. Materials and methods: Classical biochemical methods were used to determine plasma malondialdehyde (MDA), nitric oxide (NO), glutathione peroxidase (GSH-Px), catalase (CAT), superoxide dismutase (SOD), and glutathione (GSH) levels. Enzyme-linked immunosorbent assay was used to determine plasma insulin levels and reverse transcriptase-polymerase chain reaction was used to determine the levels of brain antioxidant enzymes (SOD, CAT, and GSH-Px). This applied dissertation was designed to conduct a preliminary examination of the effects of the provision of free glucose testing meters, and test-strips subsidies on blood glucose control among diabetics. The study examined whether there was any impact on the frequency of testing, and whether there was a correlation between the frequency of testing and blood sugar control, as measured by HbA1c tests.

The purpose of these studies was to investigate the impact of high-fat diet (HFD) exposure during pregnancy and the early post-natal period on fetal and post-natal development of the endocrine pancreas of the Japanese macaque. Specifically I hypothesized that the HFD would alter islet morphology and lead to disturbances in glucose homeostasis in these animals. Adult female Japanese macaques were placed on either a control (CTR) or HFD diet for 4 years. Obesity, especially at mid-life, is a major risk factor for atherosclerosis, insulin resistance and the metabolic syndrome, which in turn contrib- ute to coronary artery disease (CAD), Type 2 diabetes and Alzheimer’s disease (AD). The rise in overweight and obesity in all societies is prompting intense research into the causes and effects of the condition. Mutations in the insulin gene can impair proinsulin folding and cause diabetes mellitus. Although crystal structures of insulin dimers and hexamers are well established, proinsulin is refractory to crystallization. Although an NMR structure of an engineered proinsulin monomer has been reported, structures of the wild-type monomer and hexamer remain undetermined. We have utilized hydroxyl radical footprinting and molecular modeling to characterize these structures. Differences between the footprints of insulin and proinsulin, defining a "shadow" of the connecting (C) domain, were employed to refine the model. Advancements in medical therapy have augmented resources available to physicians to treat disease and, because of this, spending on prescription drugs has doubled in the past decade. Increasingly, clinical trials are demonstrating the benefits of aggressive disease management in reducing morbidity and mortality. Physicians treating patients with chronic conditions must balance the benefits of combination drug therapy against the risk of adverse drug events and drug interactions. Exercise is important for controlling hemoglobin A1c, and maintaining proper glycemic control in people with diabetes. Exercise also increases the diabetics overall insulin sensitivity, and decreases their dependency on diabetes medication. However, people with diabetes are faced with metabolic and endothelial impairment, which could result in a prolonged sensation of muscle soreness following exercise. This would make it difficult for these people to sustain exercise regimes. Delayed-onset muscle soreness (DOMS) is a common problem in healthy individuals and in people who have diabetes. Background
Appalachia is a region of the United States noted for the poverty and poor health outcomes of its residents. Residents of the poorest Appalachian counties have a high prevalence of diabetes and risk factors (obesity, low income, low education, etc.) for type 2 diabetes. However, diabetes prevalence exceeds what these risk factors alone explain. Based on this, the history of poor health outcomes in Appalachia, and personally observed high rates of childhood obesity and lack of concern about prediabetes, we speculated that people in Appalachia with diagnosed diabetes might tend to be diagnosed younger than their non-Appalachian counterparts.

Hemoglobin A1c (HbA1c) levels are indicators for managing Diabetes Mellitus. This study investigated the effectiveness of Diabetes Mellitus education classes based on participants’ HbA1c levels. The HbA1c and body mass index (BMI) values were analyzed using t-tests to determine significant (P ≤ 0.05) differences between baseline (N = 46) levels compared to three months, six months, and 12 months after class participation and reported as means ± standard deviations. Hypovitaminosis D may be associated with diabetes, hypertension and CHD. However, because studies examining the associations of all three chronic conditions with circulating 25-hydroxyvitamin D (25(0H)D) and 1,25-dihydroxyvitamin D (1,25(0H)2D) are limited, we examined these associations in the US Prostate, Lung, Colorectal and Ovarian (PLCO) Cancer Screening Trial (n 2465). Caucasian PLCO participants selected as controls in previous nested case-control studies of 25(0H)D and 1,25(0H)2D were included in this analysis. This dissertation presents two applications of discrete-event simulation (DES) to represent clinical processes: (1) a model to quantify the risk of the maternal obese and diabetic intrauterine environment influence on progression to adult obesity and diabetes, and (2) a model to evaluate health and economic outcomes of different smoking cessation strategies. Background and Aims
Obesity is increasingly a health problem and a risk factor for diabetes in young Mexican-American populations. Genetic association studies in older, mostly non-Hispanic populations have reported that polymorphisms in the candidate genes HSD11B1, CRP, ADIPOQ, PPARG, ANKK1, ABCC8 and SERPINF1 are associated with obesity or diabetes. We analyzed the polymorphisms rs846910, rs1205, rs1501299, rs1801282, rs1800497, rs757110 and rs1136287in these candidate genes, for association with obesity and metabolic traits in a young Mexican-American population from south Texas.

ix Objectives: To quantify polypharmacy and assess the socio-economic predictors of medication use and expenditure in diabetics. Methods: This study analyzed adult diabetes patients using a nationally representative sample in Medical Expenditure Panel Survey in 2006. Top ten most highly utilized drug classes were identified. Descriptive statistics were used to portray the patients’ medication utilization and spending. Generalized linear models were conducted to assess the socio-economic variants in drug use and spending. Background—Stroke, diabetes, and coronary heart disease (CHD) remain leading causes of death in the United States and are largely attributable to lifestyle behaviors. Integrative medicine can provide a supportive partnership that focuses on improving health by identifying and implementing lifestyle changes based upon personal values and goals.

The Juvenile Diabetes Research Foundation (JDRF) is a national organization that was started back in 1970 by two mothers of sons with type 1 diabetes. Since its founding, JDRF has awarded more than $1.5 billion to diabetes research around the world, including $9.9 million of research in San Diego. JDRF is a volunteer-driven organization with thousands of volunteers in locations worldwide. While JDRF is working hard to find a cure and improve the lives of those living with type 1 diabetes, it also has a goal of providing volunteer-led support programs to people with type 1 diabetes and their families. …

This article describes a model for developing diabetes coalitions in rural Appalachian counties and presents evidence of their sustainability. The rural Appalachian coalition model was developed through a partnership between two federal agencies and a regional university. Coalitions go through a competitive application process to apply for one-time $10,000 grants. The project has funded 7 to 9 coalitions annually since 2001, reaching 66 total coalitions in 2008. Sustainability of the coalitions is defined by the number of coalitions that voluntarily report on their programs and services. In 2008, 58 of 66 (87%) coalitions in the Appalachian region continue to function …

This project measured the effect of a clinical algorithm on the provision of care to medically uninsured adult patients with type 2 diabetes and a low socioeconomic status (SES). Primary providers often fail to implement established standards for diabetes care to their maximal benefit and do not achieve American Diabetes Association (ADA) treatment standards. Saydah, et al. (2004) reported that only 48% of patients with diabetes achieved the recommended HbA1c goal, and 33% reached blood pressure and LDL targets. BACKGROUND: Changes in maternal insulin sensitivity throughout pregnancy may help supply nutrients to a growing fetus; however, the changes may concomitantly increase the mother and child’s risk for diabetes and chronic diseases. Regular physical activity (PA) positively impacts precursors for diabetes, but the impact of PA throughout gestation is less studied. The purpose of this study was to examine the relationship between meeting PA recommendations and diabetes risk in pregnant women. Objective: The objectives of this study were to evaluate the demographic data of the study population, compare nutrition knowledge and retention in Hispanic pregnant women who were newly diagnosed with gestational diabetes (GDM) or with a history of GDM at time of GDM diagnosis and prior to delivery, and determine if the Sweet Success education program’s curriculum was effective in improving overall knowledge in pregnant women with GDM.
Design: A quasi-experimental design was used with a convenience sample.

Drugs that improve chronic hyperglycemia independently of insulin signaling or reduction of adiposity or dietary fat intake may be highly desirable. Ad36, a human adenovirus, promotes glucose uptake in vitro independently of adiposity or proximal insulin signaling. We tested the ability of Ad36 to improve glycemic control in vivo and determined if the natural Ad36 infection in humans is associated with better glycemic control. Clinical islet transplantation is a promising treatment for type 1 diabetes. It involves the transplantation of pancreatic islets, isolated from a donor, into the portal vein of a recipient in order to replace his/her dysfunctional islets. Though promising, islet implantation into the liver is greatly hindered by numerous problems, including mechanical stresses, inflammatory responses, exposure to high drug and toxin loads, as well as irretrievability. In order to address these concerns, investigation into alternative implant sites, such as the subcutaneous site, has intensified. The mid- to late nineteenth century was the beginning of the reservation era for most Native Americans, ushering in a series of unprecedented changes that affected the cultural traditions and physical health of groups across the United States. For the Pawnee, Omaha and Winnebago in Nebraska, changes in subsistence patterns not only destroyed traditions but influenced the spread of disease and malnutrition. Background: Women who develop gestational diabetes mellitus (GDM) have an increased risk for the development of type 2 diabetes. Despite this "window of opportunity," few intervention studies have targeted postpartum women with a history of GDM. We sought perspectives of women with a history of GDM to identify a) barriers and facilitators to healthy lifestyle changes postpartum, and b) specific intervention approaches that would facilitate participation in a postpartum lifestyle intervention program.

ABSTRACT This qualitative study focused on the enablers and deterrents of self-care behavior among Filipino adults with Type 2 Diabetes. Purposive sampling was used to recruit participants. Open-ended interviews with diabetic patients and their primary caregivers and self-care diaries were utilized as methods of data collection. Further, review of medical records was done.A “within-case analysis”, (triangulated data from diabetic patients and caregivers) and a “cross-case analysis” (comparing triangulated data among and across all cases) were undertaken, noting a specific pattern-match.
