The Dialogue
This paper examines the role of public administration in shaping and restricting comedic expression in the United States, specifically focusing on the FCC’s regulation of broadcast media, targeting stand-up comedians and late-night show hosts. By applying Weber's and Wilson's theories of bureaucracy the paper analyzes how administrative power influences the boundaries of acceptable speech, through legal frameworks and discourse theory. An important court case involving governmental regulation of comedic speech was the FCC v. Pacifica Foundation. This case sets the stage for administrative power to grow in terms of limiting freedom of speech and paving the way for new administrations, …

Journal of Social Work in the Global Community
This study evaluated a short-term individual development account for young workers (SIDAY) with low income in Gwangju Metropolitan City, South Korea. SIDAY used random sampling methods to form an experimental group (n = 191) and a control group (n = 106) out of 2,589 applications. The experimental group received a 100,000 Korean Won (KRW), which is the equivalent to an $85 USD monthly savings match for 10 months, as well as financial capability programs and services, such as online financial mentoring, spending plan consultations, and electronic financial newsletters. Infants born before 32 weeks of gestation are highly susceptible to late-onset sepsis because of immune immaturity, impaired intestinal barrier function, and frequent broad-spectrum antibiotic exposure. Whether probiotics should be continued or initiated during antibiotic therapy remains unresolved and is often conflated with routine prophylaxis in stable infants. This SANRA-guided structured narrative and translational review synthesized clinical, microbiome, resistome, mechanistic, and safety evidence identified in PubMed/MEDLINE, Scopus, and Web of Science through July 1, 2026, across three contexts: stable antibiotic overlap, suspected sepsis, and confirmed or unstable sepsis. Although brucellosis is one of the most common zoonotic infections worldwide, the development of a paradoxical reaction during treatment hasrarely been reported in the literature. We present a 17-year-old patient who developed acute sacroiliitis in the second week of treatment for olecranon bursitis brucellosis, despite her initial clinical findings showing signs of improvement. After excluding other potential etiologies for sacroiliitis, clinical and radiological recovery was achieved by adding anti-inflammatory therapy without modifying the existing antimicrobial regimen. Objective: To investigate predictive factors for seizure recurrence in children with febrile seizures (FS) within a single febrile illness, providing a basis for early clinical identification and preventive measures. Methods: We conducted a retrospective analysis of children with FS admitted to the Department of Pediatrics, Suzhou Hospital Affiliated to Anhui Medical University, from April 2023 to December 2025. A total of 337 children met the inclusion criteria. Mycoplasma pneumoniae is a well-recognized cause of respiratory infection in children, but its clinical spectrum extends beyond the lungs. Extrapulmonary manifestations may occasionally precede respiratory symptoms or dominate the presentation, making the diagnosis more difficult. This narrative clinical review focuses on nonspecific and extrapulmonary manifestations reported in pediatric patients, including neurological, mucocutaneous, hematological, cardiovascular, hepatic, gastrointestinal, and musculoskeletal complications. Relevant literature published between 2000 and 2026 was reviewed using PubMed/MEDLINE, Scopus, and Google Scholar. Most reported extrapulmonary manifestations appear to be immune-mediated, although direct microbial effects and vascular or prothrombotic mechanisms may contribute in selected cases. This study retrospectively analyzed the clinical data of two 19-month-old children diagnosed with Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis (EBV-HLH) who received treatment with a dexamethasone combined with etoposide (VP-16) regimen. Both children met the early remission criteria for body temperature, IL-10, and IFN-γ within 48-72 hours after treatment initiation, but their clinical outcomes were markedly different. In Case 1, the EBV-DNA load decreased significantly during the initial treatment phase, and platelet counts steadily increased after treatment; the patient had a favorable prognosis at the 2-year follow-up. Objective: This study investigated the age-dependent clinical characteristics of Kawasaki disease (KD) in pediatric patients and assessed the influence of age on disease presentation and outcomes. Methods: A retrospective analysis was conducted of 480 children diagnosed with KD who were admitted to Hangzhou First People's Hospital between 2013 and 2022. Subjects were stratified into two cohorts based on age: <2 years and 2-14 years). Demographic data, clinical manifestations, laboratory parameters, comorbidities, and coronary artery abnormalities were comprehensively evaluated across the age groups. Results: Time from symptom onset to diagnosis was significantly prolonged in the older cohort (z=-2.70, P=0.007). The incidence of cervical lymphadenopathy was markedly higher in older children (χ²=15.12, P<0.001), while peripheral extremity changes were significantly less frequent (χ²=5.79, P=0.016). The younger cohort exhibited significantly elevated platelet counts, alanine aminotransferase, and NT-proBNP levels compared to older patients (z=-3.57, P<0.001; z=-2.39, P=0.02; z=-3.59, P<0.001, respectively). Coronary artery dilatation and aneurysm formation occurred more frequently in younger patients (z=7.70, P=0.006; z=14.63, P<0.001). However, coronary artery recovery rates following intravenous immunoglobulin therapy showed no significant inter-group differences (P>0.05). Conclusions: The clinical presentation of KD demonstrates significant age-dependent variation. Clinicians should maintain a high index of suspicion for KD in older children presenting with fever and cervical lymphadenopathy. A Case Of Hemophagocytic Lymphohistiocytosis Caused By Human Herpesvirus 6b, Lihong Gao, Yang Wen

Hemophagocytic lymphohistiocytosis (HLH) is a severe inflammatory response syndrome. Common infectious triggers include Epstein Barr virus, cytomegalovirus, bacteria, and other pathogens. We report a case of HLH caused by human herpesvirus-6 (HHV-6) in a pediatric patient. Based on the HLH-2004 diagnostic criteria, the patient fulfilled 5 of 8 criteria, establishing a definitive diagnosis. HHV-6-associated HLH is rarely reported in the literature. The etiological diagnosis was confirmed through metagenomic next - generation sequencing, highlighting its utility in identifying underlying causes of HLH when the etiology remains unknown.

This study examines the impact of temporary housing policies on social stability in the context of slum redevelopment in Jeddah, focusing on the effectiveness of transitional housing mechanisms in mitigating displacement impacts. It adopts a descriptive-analytical approach within a qualitative case study, drawing on theoretical frameworks, international standards, expert interviews, and analysis of official documents. The findings show that housing security was organizationally achieved through rental support, but remained limited due to its temporary nature and unclear continuity. The study focuses on collecting and examining the narrations of Ḥumayd ibn ʿAbd al-Raḥmān al-Ḥumayrī that were identified as defective due to discrepancies in their transmission, as recorded in al-Dāraquṭnī’s al-ʿIlal. One of the reasons for selecting this topic is that the researcher did not find any previous study devoted specifically to this subject. Purpose: Huatan Huoxue decoction (HTHXD) is effective in treating polycystic ovary syndrome (PCOS); however, its underlying mechanisms of action remain unclear. Methods: Gene targets were obtained through Mendelian randomization (MR) analysis using druggable genes from public databases and genome-wide association study data for PCOS from the FinnGen R12 database. Online pharmacies improve access to medicines by offering convenient ordering and home delivery, but their growth also creates important risks for medication safety and public health. This review examines the benefits and hazards associated with online medicine supply, including access without a valid prescription, falsified and substandard products, inappropriate medicine use, privacy breaches, and weak cross-border regulatory oversight. The available evidence indicates that illicit online sellers can expose consumers to products of uncertain identity, quality, and dose, while vulnerable users may have limited ability to distinguish legitimate services from unlawful vendors. Background: Chronic kidney disease (CKD) is a progressive disorder frequently complicated by anemia and a persistent inflammatory state, both of which significantly contribute to morbidity and mortality. Reduced renal production of erythropoietin (EPO) is the primary cause of anemia in CKD. Emerging evidence suggests that EPO may exert anti-inflammatory and metabolic effects beyond its hematopoietic role. Cancer immunotherapy has emerged as a transformative therapeutic strategy that harnesses the host immune system to recognize and eliminate malignant cells. This review provides a comprehensive overview of current immunotherapeutic approaches, including immune checkpoint inhibitors (ICIs), monoclonal antibodies (mAbs), cancer vaccines, dendritic cell (DC)-based vaccines, chimeric antigen receptor (CAR)-T cell therapy, and oncolytic virus therapy. The mechanisms of action, therapeutic applications, and clinical benefits of each modality are discussed. Rheumatoid arthritis is an inflammatory autoimmune disease affecting the joints which eventually leads to systemic comorbidities. Its pathogenesis was attributed to disturbances in the Janus Kinase/Signal Transducer and Activator of Transcription signaling pathway. In this research, an innovative lead compound C6 was created and docked toward Janus Kinase1 (JAK1). The docking process was confirmed by re-docking of the co-crystallized ligand MI1 (Tofacitinib). C6 showed a predicted docking score of -9.17 kcal/mol, with enhancement of 1.26 kcal/mol over Tofacitinib. A system is proposed herein for detecting sudden turns, changes in turning behavior, and slow drift in the joint movement of multivariate telemetry counters. The system uses an overflow-bounded integer direction metric and an exact division-free threshold gate, requires work linear in the number of counters and fixed-size per-stream state, and produces exactly reproducible results without floating-point arithmetic or model training. Optional discrete direction keys support compact grouping, and peer-relative integer classification identifies outliers among comparable elements. The resulting signals can be generated close to the telemetry source, where independent per-counter thresholds may miss coordinated changes.
