SUMMARY OF THE ARGUMENT: The Sentencing Guidelines are, in practice, the most consequential legal text in federal criminal law. Yet this Court has abstained from interpreting the Guidelines for decades. That practice is anomalous in the federal judiciary and should end. District courts must correctly calculate every defendant’s guideline range at every sentencing hearing. Courts of Appeals routinely hear appeals asserting guideline mistakes. And differences of appellate opinion over purely legal, highly consequential questions often arise. BioMolViz is an educational community of practice (CoP) focused on creating resources to improve biomolecular visual literacy instruction and assessment. The community of life science instructors developed in three phases, beginning with a small Core Team that emerged from a conference session and established the domain of interest for the CoP. In-person workshops, supported by short-term funding, drove the initial expansion of the community and the development of shared activities in Phase 2. This CoP was sustained through intentional choices about how members interact, collaborate, and share work. As the community matured in Phase 3, it was supported by longer-term …

Background: Medullary Thyroid Carcinoma(MTC) had special entity in thyroid oncology because of unresponsiveness to Radioiodine and TSH suppression. The tumour originates from parafollicular C cells, not follicular epithelium, and that single histogenetic difference accounts for most of what makes this disease clinically difficult. Cure, when it happens, comes from surgery. When surgery is inadequate, the options that remain are limited. Most cases are sporadic. The minority with germline RET mutations; the hereditary forms; tend to present differently and demand a distinct pre-operative protocol, particularly regarding co-existing endocrine pathology.

IMPORTANCE: Blinatumomab, a novel immunotherapy administered as a 28-day continuous infusion, has fundamentally shifted the treatment paradigm for pediatric B-cell acute lymphoblastic leukemia (B-ALL) and is now considered a component of standard therapy for the most common childhood cancer. However, the care delivery challenges in transitioning from an experimental agent on a clinical trial to widespread clinical implementation are unknown.

OBJECTIVE: To characterize the clinical landscape of pediatric blinatumomab care-delivery practice and challenges in the US from the perspective of treating centers.

Purpose of Review Medetomidine is a veterinary α2-adrenergic agonist that has been an adulterant of illicitly manufactured opioids since 2022. We review its pharmacology, detection, intoxication, withdrawal, and management, and assess why severe withdrawal has emerged in some regions but not others. Recent Findings Medetomidine has spread through North American drug markets yet evades most toxicology testing. Roughly two hundred times more potent than xylazine, with greater α2 selectivity and imidazoline-1 activity, it causes intoxication resembling other α2 agonists: sedation and bradycardia. The dominant clinical problem is withdrawal: autonomic dysregulation, intractable vomiting, and encephalopathy frequently requiring intensive care. Treatment requires …

BACKGROUND: Despite growing attention to workforce diversity in orthopaedic surgery, limited data exist describing the demographic composition of academic sports medicine faculty. This study examines the diversity of sports medicine fellowship-trained orthopaedic surgery faculty across US orthopaedic surgery residency training programs.

METHODS: A cross-sectional analysis of all ACGME-accredited orthopaedic surgery sports medicine faculty was conducted. Faculty data included sex, race, level of professorship, residency attended, year of residency graduation, and h-index. Underrepresented in Orthopaedics (URiO) was defined as non-White and non-Asian by race and all women. Descriptive statistics were performed.

This study significantly advances previous attempts to model cold-water coral growth using Smoothed Particle Hydrodynamics (SPH) by incorporating the finite nature of food availability, an important parameter in coral development. Building on the Goldilocks Principle, which states that coral polyps must surpass an energetic survival threshold to prosper, this model integrates food-based growth dynamics. The underlying SPH framework simulated fluid-coral interactions in which growth was promoted in optimal flow conditions and food-rich zones. Key innovations include dynamic energy reserves, where the energetic reserves of coral particles depend on food obtained from the surrounding fluid particles. Simulations reveal that higher food …

A descriptive qualitative method with a musicological approach was employed. Data were collected through observations, interviews, and documentation from February to May 2026. This study aims to describe and compare DeepL and ChatGPT translations of the Brothers Grimm fairy tale Rotkäppchen from German into Indonesian based on Nida and Taber’s principle of dynamic equivalence. The study employs a qualitative descriptive approach with a comparative design. The data sources consist of the German text of Rotkäppchen from Kinder- und Hausmärchen (1857) and its translations produced by DeepL and ChatGPT. The analysis focuses on lexical, grammatical, semantic, pragmatic, and stylistic aspects while considering the preservation of meaning, context, target-language naturalness, and communicative effect.
