The budget reconciliation bill signed on July 4, 2025 (“HR1”) weakened the Patient Protection and Affordable Care Act (ACA) and its core goal of near-universal health insurance coverage. To maintain 2017 tax cuts, Congress curtailed the ACA’s Medicaid eligibility expansion; simultaneously, HR1 limited the kinds of options states have been able to exercise within Medicaid while pushing the costs of deep cuts down to states. This essay argues that HR1 embodies an anti-federalism approach, and that anti-federalism is not the same as federalizing health reforms. In the past, Congress sometimes federalized health care or public health reforms to respond to …

How does this broader sense of Barnette’s proscription of government from imposing “what shall be orthodox” implicate governmental restrictions on reproductive autonomy? Battles over reproductive freedom centrally implicate objections that government is unconstitutionally imposing a gender orthodoxy. Arguments for a right of personal autonomy embodying an antitotalitarian principle of liberty are arguments for a right to resist governmental imposition of an orthodoxy— for example, an idea about the natural or traditional role of women as mothers together with a view about how best to respect the sanctity of life. Background: While the need for complete device removal is well-established for cardiovascular implantable electronic device (CIED) infections, there is insufficient evidence on the optimal duration of antimicrobial therapy.
Objectives: To examine how antimicrobial duration affects clinical outcomes of patients presenting with CIED-associated bacteremia and lead endocarditis.
Design: This is a retrospective cohort study conducted at a quaternary care hospital in Houston, TX, USA.
Methods: Patients who underwent device removal for a primary indication of bacteremia or lead endocarditis over a 10-year period were stratified by prescribed duration of antibiotics as ⩽2 weeks or >2 weeks post-extraction. Measured outcomes included all-cause …

Antigenic variation enables immune evasion of pathogens, yet the mechanisms for the rapid change in their antigens without loss of structural and functional features are not fully known. Tick-borne relapsing fever (TBRF) spirochetes of the genus Borrelia can be used as a model because their variable major proteins (Vmps) drive antigenic variation followed by recurrent waves of bacteremia. In this study we generated the genome of Borrelia persica LMU-C01 and evaluated Vmp structure across relapsing fever Borrelia species. Tourism is central to Belize's economy, yet the influx of international travelers may contribute to infectious disease introduction and transmission among local workers, including SARS-CoV-2. In June 2022, we conducted a cross-sectional seroepidemiologic study among tourism and government workers in the tropical island tourism center of Ambergris Caye, Belize. The goal of this study was to determine SARS-CoV-2 antibody prevalence for both vaccination (anti-spike protein) and natural infection (anti-nucleocapsid protein), vaccine uptake, history of COVID-like symptoms, and risks for infection two years into the pandemic. Background: Chronic Trypanosoma cruzi infection causes significant liver pathology, and current antiparasitic treatments often worsen hepatic damage. Hookworm-derived proteins have shown immunomodulatory effects in inflammatory diseases, including T. cruzi-induced myocarditis.
Objective: This study evaluates recombinant hookworm proteins AIP-1 and AIP-2 for treating liver inflammation in a murine model of chronic Chagas disease (CD).
Methods: Female BALB/c mice infected with T. cruzi were treated with AIP-1 or AIP-2 (1 mg/kg) for seven days. Controls were untreated or received aspirin (25 mg/kg) for 14 days. Background: Immune-mediated diarrhea and colitis (IMDC) due to checkpoint inhibition infrequently presents with normal stool biomarkers and no endoscopic or histologic evidence of inflammation. Little is known about the treatment needs and outcomes of this subset of patients. We aimed to describe this entity and clarify the role of immunosuppressive treatments in its management.
Method: This was a single-center, retrospective study of patients treated with immune checkpoint inhibitors who developed clinical symptoms of IMDC, with no evidence of inflammation based on fecal calprotectin or endoscopic/histologic evaluation, between January 2010 and February 2024.

Introduction: Radical cystectomy or trimodal therapy are treatment options for non-metastatic bladder cancer. However, there is limited evidence guiding post-treatment follow-up regimens, resulting in variations in care.
Objective: A Swiss consensus meeting aimed to identify gaps in follow-up strategies and subsequently tailoring follow-up protocols after curative treatment for bladder cancer.
Methods: A consensus meeting including participants from urology, oncology, radiation oncology, neuro-urology, international advisors, and patient representatives was held. 19 pre-formulated questions addressing follow-up duration, frequency, and modalities after radical cystectomy or trimodal therapy for bladder cancer were discussed and voted by participants.

Alzheimer's disease is a complex and multifactorial disease characterized by two key pathological hallmarks: amyloid-beta plaques and tau neurofibrillary tangles. Recent progress has led to the development and approval of disease-targeted therapies for Alzheimer's disease in the form of anti-amyloid-beta monoclonal antibodies. However, findings suggest that amelioration of multiple pathological drivers may be required to maximize clinical effect. An increasing body of evidence suggests that tau is a critical player in Alzheimer's disease pathophysiology, contributing significantly to neurodegeneration and cognitive decline. Deep brain stimulation (DBS) improves motor function in a growing list of movement diseases including Parkinson's disease, dystonia, and tremor. There is evidence that DBS may also be effective in ataxia. It is not known why DBS is effective, but modulating cell activity and conferring neuroprotection are hypothesized to underlie its benefits. Understanding the effects of DBS on neurons is paramount to extending its clinical use in the treatment of various motor and non-motor diseases. The Seventh-day Adventist Church emerged as a worldwide missionary movement committed to proclaiming the everlasting gospel to every nation, tribe, language, and people. Throughout much of its history, the Church demonstrated a strong commitment to cross-cultural mission and the establishment of an Adventist presence in territories with low or no access to the gospel message. However, as the Church expanded globally in membership, institutions, organizational structures, and administrative responsibilities, a gradual shift in mission priorities occurred. Increasing attention to institutional maintenance and regional demands often reduced emphasis on frontline mission territories and populations with no or low Adventist presence. …

Despite diverse approaches deployed worldwide to address harmful alcohol consumption, understanding binge drinking as a hazardous consumption behaviour remains a pressing issue in marketing and consumer behaviour research. Thus, this study examines the role of cognitive dissonance – and its absence – in facilitating transitions between different phases of binge drinking. Drawing on 38 biographical interviews conducted in Australia, the Netherlands, Poland, and the United States, we identify six instances where cognitive dissonance, or the absence thereof, contributes to the transition into, between, and out of phases of binge drinking. Land utilization is an important indicator of socioeconomic and environmental changes caused by both natural and man-made factors. Land use and land cover (LULC) simulation is a critical tool for monitoring and predicting LULC and is essential for sustainable development, land resource management and planning. Heavy metal pollution, particularly chromium (Cr) from electroplating industrial waste, has severely threatened environmental quality and human health. This study aims to develop a composite adsorbent material based on bentonite and god crown biomass capable of removing chromium ions from liquid waste through a combination of reduction and adsorption mechanisms. The god crown/bentonite (GC/Bt) composite was synthesized at a mass ratio of 2:1 and calcined at 900°C. FTIR characterization revealed active functional groups (–OH, C=O, Si–O, and Al–O–Si), whereas BET analysis revealed a mesoporous structure (surface area 31.12 m2 g-1, pore diameter 4.37 nm) suitable for ion diffusion. In recent decades, the decrease in rice cultivation areas in Vietnam, particularly in the Mekong River Delta, caused by climate change and soil salinization, has posed serious challenges for sustainable development. In this study, the accumulation of heavy metals (Cd, Cr, Cu, Fe, and Pb) in water, suspended sediments, sediments, and seagrass in Kalase Bay, Trang Province, Thailand, during the 2024 dry season was investigated. These findings indicate that the enrichment factor (EF) for all the metals was less than 1, suggesting that anthropogenic contamination is not a significant concern in the area. The translocation factor (TF) values were less than 1 for all the metals except Cu, whose TF was greater than 1; however, these values were not statistically significant, indicating limited phytoextraction capacity. This study presents a comprehensive multihazard risk assessment for Mueang Tak District, Thailand, that integrates hazard, vulnerability, and exposure (H-V-E) dimensions. By utilizing geographic information systems (GIS) and remote sensing, the analytic hierarchy process (AHP) was employed to prioritize critical risk factors. GIS analysis integrated multisource spatial data, including Landsat 9 and Sentinel-2 imagery, CHIRPS precipitation, and SRTM-derived topography, while AHP weights were established on the basis of expert judgment and socioeconomic indices such as the relative wealth index (RWI) and population statistics. In this study, the physicochemical properties of wastewater were characterized, and a quantitative microbial risk assessment (QMRA) of E. coli was conducted in the wastewater treatment system of the Faculty of Science and Technology, Valaya Alongkorn Rajabhat University, under the Royal Patronage, Pathum Thani, Thailand (VRU-SciTech). This study was conducted because the current water-quality assessment does not include the QMRA or evaluate the occupa-tional and incidental public risk of infection. Crude oil negatively affects soil physicochemical properties and microbial activity, thereby hindering plant growth and posing notable environmental and agricultural issues. The combined effects of cow dung, indole-3-acetic acid (IAA), and simulated microgravity on the recovery of soil degraded by crude oil were explored using Zea mays as the test plant. งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาความสามารถในการกำจัดไอออนตะกั่วของ ตะกอนจุลินทรีย์ที่ไม่มีชีวิต โดยการเปลี่ยนแปลงความเข้มข้นของสารละลายเริ่มต้นและปริมาณการใช้ตะกอนจุลินทรีย์ ตะกอน จุลินทรีย์ที่ใช้เป็นวัสดุดูดซับในการทดลองนี้มี 3 ชนิด โดย 2 ชนิดแรก คือ AS-P และ MUR-P ได้จากระบบบำบัดน้ำเสีย Activated Sludge และ Upflow Anaerobic Sludge Blanket ของอุตสาหกรรมน้ำอัดลม ส่วนชนิดที่ 3 คือ CASS-N ได้จากระบบบำบัดน้ำเสีย Cyclic Activated Sludge System ของโรงควบคุมคุณภาพน้ำ กรุงเทพมหานคร ตะกอนจุลินทรีย์ที่ได้จะนำมาล้างด้วยน้ำกลั่น อบแห้งในตู้อบที่อุณหภูมิ 105°C บดและแยกขนาดให้ได้อนุภาคที่มีขนาดเส้นผ่านศูนย์กลางประมาณ 106-150 ไมโครเมตร จากผลการทดลองพบว่าการดูดซับจะเข้าสู่สมดุลที่เวลา 15 นาที โดยมีเปอร์เซ็นต์การกำจัดไอออนตะกั่วสูงถึง 94-99 เปอร์เซ็นต์ ที่ความเข้มข้นของสารละลายเริ่มต้น 60 มิลลิกรัมต่อลิตร นอกจากนี้ ยังใช้สมการไอโซเทอร์มการดูดซับของ แลงเมียร์ (Langmuir adsorption isotherm) และไอโซเทอร์มการดูดซับของฟรุนด์ลิช (Freundlich adsorption isotherm) มาใช้ ทำนายผลการทดลองพบว่าสมการไอโซเทอร์มการดูดซับทั้ง 2 สมการ สามารถใช้ทำนายผลการทดลองได้ดี ค่า KF ที่สูง แสดงให้เห็นถึงความสามารถในการดูดซับไอออน ตะกั่ว ของตะกอนจุลินทรีย์ทั้ง 3 ชนิดสูง ส่วนปริมาณไอออนโลหะหนักตะกั่วที่ถูกดูดซับสูงสุด (qm) ของ AS-P, MUR-P และ CASS-N ที่ได้จากสมการไอโซเทอร์มการดูดซับของแลงเมียร์ มีค่าเป็น 253, 248 และ 247 มิลลิกรัมโลหะต่อกรัมวัสดุดูดซับ ตามลำดับ

งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาประสิทธิภาพของระบบบำบัดไร้อากาศแบบอีจีเอสบีในการบำบัดน้ำเสียชุมชน ประเภทอาคารเรียน จากอาคารเจริญวิศวกรรม คณะ วิศวกรรมศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย พบว่าเมื่อความเร็วไหลขึ้นคงที่เท่ากับ 4.5 ม./ชม. และเปลี่ยนแปลงสัดส่วนอัตราไหลเข้าต่ออัตราไหลเวียนกลับเป็น 1:3, 1:7, 1:1 และ 1:15 ระบบมีประสิทธิภาพในการกำจัดซีโอดีเฉลี่ยเท่ากับ 69.2, 72.6, 76.3 และ 74.6% ตามลำดับ ประสิทธิภาพในการกำจัดบีโอดีเฉลี่ยเท่ากับ 72.7, 73.5, 76.6 และ 75.6% ตามลำดับ และประสิทธิภาพในการกำจัดของแข็งแขวนลอยเฉลี่ยเท่ากับ 74.0, 77.4, 81.7 และ 73.3% ตามลำดับและคุณภาพน้ำทิ้งออกจากระบบอีจีเอสบีผ่านมาตรฐานน้ำทิ้งอาคาร

The present study investigates the use of lignite fly ash as a chemical to improve water quality in place of liming material. Two experiments were performed with freshwater and synthetic seawater. Improvement of water quality were measured by using pH and ammonia. The neutralizing value and the application rate of fly ash were studied. The results of the study indicate that the neutralizing value of fly ash is 70%, comparing to standard pure CaCO3 100%. The optimal dosage of fly ash to remove ammonia was 1 g/L. งานวิจัยนี้เป็นศึกษาพฤติกรรมการ เคลื่อนที่ของอาร์เซนิคในรูปของอาร์เซไนท์ และอาร์เซเนตในสภาวะน้ำใต้ดินโดยทำการ ทดลองกับดินตัวอย่าง 3 กลุ่มดิน คือ ดินร่วนเหนียวปนทราย ดินร่วนเหนียว และ ดินร่วนปนทราย ที่ pH 4, 7 และ 10 และ ในสภาวะที่มีไอออนลบฟอสเฟตอยู่ร่วมด้วย โดยทำการทดลองแบบต่อเนื่องโดยให้สาร ละลายอาร์เซนิคไหลผ่านคอลัมภ์ดินในทิศทางไหลขึ้นที่อัตราเร็ว 15 มิลลิลิตรต่อชั่วโมง จากการทดลองพบว่าอาร์เซไนท์สามารถเคลื่อนที่ได้เร็วที่สุดในดินทรายร่วน รองลงมาคือในดินร่วนเหนียวปนทราย และ ช้าที่สุดในดินร่วนเหนียว และเมื่อมีไอออนฟอสเฟตร่วมด้วยในสารละลาย พบว่า ไอออนฟอสเฟตทำให้การเคลื่อนที่ของอาร์เซไนท์ผ่านคอลัมภ์ดินเร็วขึ้นมาก สำหรับ การเคลื่อนที่ของอาร์เซเนต อาร์เซเนต สามารถเคลื่อนที่ได้ช้าที่สุดผ่านคอลัมน์ดินร่วนเหนียวปนทราย และคอลัมน์ที่มีไอออนลบพบว่าฟอสเฟตทำให้การเคลื่อนที่ ของอาร์เซเนตผ่านคอลัมภ์ดินเร็วขึ้นมากและเมื่อเปรียบเทียบระหว่างอารีเซไนท์และอาร์เซเนต พบว่าที่ pH 4 ความเร็วในการเคลื่อนที่ผ่านคอลัมภ์ดินร่วนเหนียวใกล้เคียง กัน แต่ที่พีเอช 7 และ ไ0 พบว่า อารีเซไนท์เคลื่อนที่ได้ช้ากว่าอารีเซเนต

Disruptions to the infant sensory environment can have lasting effects on neural response properties and behavior in both humans and animals. Recent work has begun to highlight an additional factor in infant sensory experience: differences in inhibitory signaling and sensory gating. Converging work from human and animal studies has begun to implicate a developmental cascade by which impaired sensory gating during a sensitive period of neonatal neurodevelopment promotes a phenotype of sensory over-responsivity, autistic traits, anxiety, and other psychiatric challenges. BACKGROUND: Steroid hormones influence breast morphology and cellular proliferation and are associated with breast carcinogenesis. However, their associations with mammographic breast density (MBD) are less studied, particularly in premenopausal women. We, therefore, investigated the associations of steroid hormone metabolites with MBD in premenopausal women.
METHODS: Our study included 700 premenopausal women scheduled for screening mammograms. We analyzed 54 steroid hormone metabolites (Metabolon®) and assessed volumetric measures of MBD including volumetric percent density (VPD), dense volume (DV), and non-dense volume (NDV) using Volpara.
