Purpose of Review Medetomidine is a veterinary α2-adrenergic agonist that has been an adulterant of illicitly manufactured opioids since 2022. We review its pharmacology, detection, intoxication, withdrawal, and management, and assess why severe withdrawal has emerged in some regions but not others. Recent Findings Medetomidine has spread through North American drug markets yet evades most toxicology testing. Roughly two hundred times more potent than xylazine, with greater α2 selectivity and imidazoline-1 activity, it causes intoxication resembling other α2 agonists: sedation and bradycardia. The dominant clinical problem is withdrawal: autonomic dysregulation, intractable vomiting, and encephalopathy frequently requiring intensive care. Treatment requires …

"Within the pages of a Vice-President’s Register (1920–1984), held in the Old Library archives of his Oxford College, Lewis handwrote, in mock Shakespearean English, 'The Tragicomicall Briefe Reigne of Lewis the Bald.' As far as we know, this is the only complete play in his corpus. Mostly unpublished until the recent release of C. S. Lewis’s Oxford (2024) by Simon Horobin, this piece has yet to receive the scholarly attention it deserves. In what follows, I seek to address this gap through a close analysis of portions of the text and its broader implications for Lewis’s approach to writing."

This disclosure describes techniques that enable a computing device (e.g., a smartphone or other mobile computing device) to present a single smart button or predictive UI element that predicts and automatically completes a task on the user's behalf. This prediction may be based on contextual signals the computing device logs over time, such as accessibility events, notifications, location, time of day, and a recognized activity of the user (e.g., walking, driving, or sitting still). The growing number of cases of multidrug-resistant (MDR) urinary tract infections has made the clinical management of urinary tract diseases a significant challenge; therefore, the need for effective adjunctive therapies cannot be overemphasized. This study evaluated the bioactive components and antibacterial efficacy of cold-macerated garlic extracts obtained using distilled water, 70% ethanol and hexane solvents against antimicrobial-resistant urinary isolates. Out of 250 urine samples that were analyzed, 124 (49.6%) showed significant microbial growth, with Escherichia coli being the most frequently isolated organism. Neutrosophic Systems with Applications
Fuzzy set theory enriches classical sets by assigning to each element a graded membership in [0,1], thereby capturing partial inclusion and uncertainty. The notion of an Uncertain Set further abstracts this idea by allowing membership to take values in a general degree-domain, providing a unified language that subsumes fuzzy, intuitionistic fuzzy, neutrosophic, plithogenic, and related models. On the algebraic side, a hyperlattice replaces one lattice operation by a multivalued hyperoperation, enabling the representation of ambiguous or non-deterministic combinations, while a superhyperlattice iterates this structure through powerset lifting to obtain higher-order layers of interaction. This study develops a generalized neutrosophic ratio-type estimator for estimating the population mean by incorporating information from two auxiliary variables under Simple Random Sampling Without Replacement (SRSWOR). The proposed methodology extends the conventional single-auxiliary-variable approach by jointly incorporating bivariate auxiliary information within the neutrosophic framework, thereby accounting for uncertainty, indeterminacy, and inconsistency in the available information. The bias and mean squared error of the proposed estimator are derived using first-order approximations, and the corresponding efficiency conditions are established through theoretical comparisons with existing neutrosophic estimators. This article examines pelimau dapur, a Malay post-confinement ritual practised by some families in Kampung Alor Limbat, Terengganu. It investigates how ritual knowledge is transmitted, how natural and processed materials acquire cultural, spiritual, and perceived therapeutic meanings, and how the practice is reshaped by religious, social, healthcare, and material change. The study employed a focused ethnographic case study design involving semi-structured interviews with eight purposively selected participants (two traditional midwives and six women with direct experience of the ritual), together with participant observation, field notes, and visual documentation. This study examines how gender shapes psychological experience across five of Shakespeare’s major plays: Macbeth, Hamlet, Othello, King Lear, and Twelfth Night. Existing scholarship treats Shakespeare’s tragic women, tragic men, and comic heroines as separate problems; this article asks whether one framework connects them. Its starting point is the reversal in psychological positions between Macbeth and Lady Macbeth: he resists killing on moral grounds and then grows recklessly violent, while she plans the murder with cold resolve and is later overwhelmed by guilt. Pleural effusion (PE) is a life-threatening condition in cats that can cause severe respiratory compromise. Identification of the underlying etiology is essential for prognosis and treatment planning. This study investigated the prevalence and characteristics of diseases associated with feline pleural effusion in northeastern Thailand. Medical records of cats diagnosed with pleural effusion at the Veterinary Teaching Hospital, Faculty of Veterinary Medicine, Khon Kaen University, Thailand, between January 2024 and January 2025 were retrospectively reviewed. Cases lacking a complete diagnostic workup were excluded. Background: Chronic low back pain encompasses heterogeneous mechanical, neuropathic, nociplastic, and myofascial presentations that may reflect distinct neuroimmune mechanisms. We synthesized evidence linking these phenotypes to measurable biological features and clinically relevant treatment strategies.
Methods: We conducted a structured literature search of MEDLINE, Scopus, Web of Science, and the Cochrane Central Register of Controlled Trials for studies published from January 2005 through January 2025. Systematic reviews, randomized controlled trials, and mechanistic studies addressing neuroinflammation, glial activation, central sensitization, biomarkers, or imaging were organized according to four clinically relevant pain phenotypes and translational readiness.

Courts increasingly rely on cadaver dog evidence in criminal trials, treating canine alerts as a reliable form of forensic evidence. This naive acceptance of cadaver dog evidence, coupled with courts’ continued application of out-dated admissibility standards, threatens to perpetuate wrongful convictions in an era of rapidly advancing scientific understanding. This Article argues that courts must develop a new framework for evaluating cadaver dog evidence that incorporates modern scientific research on ca-nine olfaction and scent detection capabilities. The rise in use of herbal and dietary supplements, which are regulated as food rather than as drugs by the Food and Drug Association, poses the risk of unknown and potentially severe side effects. We describe the case of acute liver injury, characterized by significant elevation in AST and ALT, likely induced by the consumption of an over-the-counter supplement containing alpha lipoic acid (ALA). Our patient trusted in a “natural” alternative to Western medicine for management of neuropathy, which was ultimately detrimental to her liver. A system and method are provided for enhancing the operational intelligence of network-connected cameras through the establishment of camera-specific semantic context. A digital repository is associated with an individual monitoring device to store context metadata representing physical and logical relationships in the device's field of view. The context metadata can be defined via user input or automatically inferred using analytical models that identify historical activity patterns or process environmental feedback. McDonald's answer endeavors to stretch across time and becomes philosophical at times. To the outsider, it would appear that McDonald was answering an inquiry about the Piedmont plateau region within the Eastern United States, a relevant subject for a local historian who often described geography. Although brucellosis is one of the most common zoonotic infections worldwide, the development of a paradoxical reaction during treatment hasrarely been reported in the literature. We present a 17-year-old patient who developed acute sacroiliitis in the second week of treatment for olecranon bursitis brucellosis, despite her initial clinical findings showing signs of improvement. Objective: To investigate predictive factors for seizure recurrence in children with febrile seizures (FS) within a single febrile illness, providing a basis for early clinical identification and preventive measures. Methods: We conducted a retrospective analysis of children with FS admitted to the Department of Pediatrics, Suzhou Hospital Affiliated to Anhui Medical University, from April 2023 to December 2025. Purpose: Huatan Huoxue decoction (HTHXD) is effective in treating polycystic ovary syndrome (PCOS); however, its underlying mechanisms of action remain unclear. Methods: Gene targets were obtained through Mendelian randomization (MR) analysis using druggable genes from public databases and genome-wide association study data for PCOS from the FinnGen R12 database. Online pharmacies improve access to medicines by offering convenient ordering and home delivery, but their growth also creates important risks for medication safety and public health. This review examines the benefits and hazards associated with online medicine supply, including access without a valid prescription, falsified and substandard products, inappropriate medicine use, privacy breaches, and weak cross-border regulatory oversight. Cancer immunotherapy has emerged as a transformative therapeutic strategy that harnesses the host immune system to recognize and eliminate malignant cells. This review provides a comprehensive overview of current immunotherapeutic approaches, including immune checkpoint inhibitors (ICIs), monoclonal antibodies (mAbs), cancer vaccines, dendritic cell (DC)-based vaccines, chimeric antigen receptor (CAR)-T cell therapy, and oncolytic virus therapy. The mechanisms of action, therapeutic applications, and clinical benefits of each modality are discussed.
