Tai Chi practice has been widely adopted to improve balance and prevent falls in older adults. However, the neural mechanisms underlying the benefits of Tai Chi are difficult to evaluate during traditional balance assessments. The goal of this study was to evaluate the effects of Tai Chi and healthy aging on corticomuscular coherence (CMC) while standing in virtual balance-demanding environments. We recorded neural, muscular, and behavioral data in an immersive virtual reality environment while implementing sensory and mechanical perturbations to introduce high postural threats. Changing fire regimes have important implications for biodiversity and challenge traditional conservation approaches that rely on historical conditions as proxies for ecological integrity. This historical-centric approach becomes increasingly tenuous under climate change, necessitating direct tests of environmental impacts on biodiversity. At the same time, widespread departures from historical fire regimes have limited the ability to sample diverse fire histories. We examined 2 areas in California's Sierra Nevada (USA) with active fire regimes to study the responses of bird, plant, and bat communities to a broad spectrum of temporal, spatial, and severity patterns of fire. Advancements in robotic systems aimed at improving mobility for individuals with disabilities have required more sophisticated control and navigation methods. Traditional control approaches often lack the complexity and adaptability needed for the high-dimensional nature of human activities. Consequently, reinforcement learning (RL) has emerged as a dynamic and effective framework for managing robotic actions in complex and unpredictable human environments. This article reviews the integration of RL in robotic systems for enhancing the mobility of individuals with disabilities, addressing the limitations of traditional control methods in complex and unpredictable environments. Reinforced soil walls are key earth retention features in the transportation infrastructure. They are used to support and retain soil in a wide variety of crucial structures, such as highways, bridges, and railways, to ensure stability. They also provide solutions for constructing embankments and slopes in constrained spaces, allowing for efficient land use and improved infrastructure planning. This study used advanced numerical modeling to improve the understanding of the behavior and long-term performance of the aging reinforced soil walls from the 1970s for asset management purposes. This report addresses the sensitivity and reliability of the sustainability rating systems by comparing each category’s verified scores with its respective submitted scores to evaluate how points are awarded for infrastructure projects, examine which categories present the most challenges for verification, and identify the best category for verification in the sustainability rating system. The authors conducted three analyses using credit score data obtained from fourteen actual civil infrastructure projects certified under Envision. First, the Natural World category had the highest average score from the submitted and verified data. Pairwise comparisons using t-tests indicate that the mean value of one category …

Dysphoric Milk Ejection Reflex (D-MER) is a distinct neurobiological condition characterized by negative alterations in mental state in response to milk letdown during lactation. Symptoms vary by patient and can include feelings of sadness, anxiety or agitation. Importantly, the symptoms are brief, typically lasting no more than 5 minutes. Prevalence has been found between 6 and 27% of lactating women, but studies show heterogeneity, due in part to inconsistent definition. D-MER is not currently classified in the Diagnostic and Statistical Manual of Mental Disorders (DSM) or the International Classification of Diseases (ICD), which presents a challenge for researchers of the …

Lowe syndrome is an X-linked disorder caused by mutations of the OCRL gene which encodes the enzyme inositol polyphosphate-5-phosphatase OCRL (Ocrl1) and is expressed in almost all body cells. Clinical characteristics involve kidney, brain, eye, muscle, bone, teeth, testes, skin and thrombocytes. Clinical phenotypes are heterogenous among families and even among affected boys in the same family. All have kidney disease varying from severe manifestations of Fanconi syndrome to only low molecular weight proteinuria, hypercalciuria and little kidney disease in the first decade of life. All develop chronic kidney disease (CKD) that typically progresses slowly and reaches stages 4–5 after …

This paper introduces a new pseudodifferential preconditioner for the Helmholtz equation in variable media with absorption. The pseudodifferential operator is associated with the multiplicative inverse to the symbol of the Helmholtz operator. This approach is well suited for the intermediate and high-frequency regimes. The main novel idea for the fast evaluation of the preconditioner is to interpolate its symbol, not as a function of the (high-dimensional) phase-space variables, but as a function of the wave speed itself. Introduction: Evidence-based recommendations for high-value care in pediatric critical care are needed. We sought to develop recommendations to identify unnecessary interventions, enhance outcomes, and promote efficient, patient-centered practices.
Methods: Using a modified Delphi process, a multi-institutional panel of pediatric intensivists from a national pediatric critical care quality improvement group identified 30 potential topics and selected 20 high- and medium-priority topics for further evaluation, and narrowed them to 10 by consensus voting. A structured scoping review of literature published between 2014 and 2023 identified 10 practices, which were critically assessed using the PICOST framework. Background: Type 2 diabetes (T2D) in adolescents is associated with an unfavorable lipid profile, but lipoprotein particle subspecies and branched-chain amino acid (BCAA) data are scarce.
Objective: To evaluate lipoprotein particle distributions, lipoprotein insulin resistance index (LP-IR), and BCAA levels longitudinally and their relationships with sex, race/ethnicity, treatment, and loss of glycemic control in adolescents with youth-onset T2D.
Methods: Participants from the TODAY study (n = 348) had samples analyzed yearly for glycated hemoglobin and nuclear magnetic resonance lipoprotein and BCAA assessments.

Objective: Autoimmune or inflammatory rheumatic diseases (AIRDs) increase the risk for poor COVID-19 outcomes. Although rurality is associated with higher post-COVID-19 mortality in the general population, whether rurality elevates this risk among people with AIRD is unknown. We assessed associations between rurality and post-COVID-19 all-cause mortality, up to two years post infection, among people with AIRD using a large nationally sampled US cohort.

Objective: Insulin resistance (IR) may be a risk factor for lung disease, but objective evidence is limited. We sought to define the relationship of longitudinal IR with radiographic imaging outcomes and examiner-identified incident lung disease in the Framingham Offspring Study.
Research design and methods: Participants without baseline lung disease underwent repeated measurements of fasting insulin and glucose levels over an average period of 13.6 years, from which time-weighted average HOMA-IR was calculated. Each participant then underwent a cardiac gated whole-lung computed tomography scan, which was analyzed for the presence of emphysema, interstitial lung abnormalities (ILAs), and quantitative airway features. Rationale: Shrimp (Litopenaeus vannamei) allergies (SA) can result in allergic reactions ranging from life threatening severe anaphylaxis to oral allergy syndrome. SA may pose lifestyle restrictions on daily life and interfere with social relationships and school performance, but this has not been thoroughly investigated. We examined the QoL in SA adults and caregivers.

The human microbiome consists of diverse microorganisms that inhabit various body sites. As these microbes are increasingly recognized as key determinants of health, there is significant interest in leveraging individual microbiome profiles for early disease detection, prevention, and drug efficacy prediction. However, the complexity of microbiome data, coupled with conflicting study outcomes, has hindered its integration into clinical practice. This challenge is partially due to demographic and technological biases that impede the development of reliable disease classifiers. Scholars and policymakers have expressed concern about the impact of high prescription drug costs on patients and healthcare budgets. This Article presents a new theoretical framework for evaluating both the problem to be addressed by drug pricing reforms as well as the efficacy of potential solutions. In seeking to solve “the drug pricing problem,” our legal system has given primacy to competition rather than regulation to drive down drug prices. The second Trump Administration has engaged in a vast swath of actions that press against the outer boundaries of legality. These actions provide a unique opportunity to identify potential vulnerabilities and expose how the rule of law can be undermined.
